24. TREATMENT FOR METASTATIC DISEASE OF THE COLON AND RECTUM.

Abstract

Metastatic disease from colorectal cancer (CRC) remains a major cause of death in the developed world despite advances in operative technique and adjuvant therapy. Complete eradication of the primary disease at the time of initial surgery remains the best, and in most cases the only, opportunity for cure of patients with colorectal cancer. Nonetheless, 25% of CRC patients will be found to have distant metastases at the time of diagnosis, and a further 25% will subsequently develop metastases. Up to 95% of CRC metastases occur in the absence of locally recurrent disease. Complete resection of metastases is compatible with long-term survival in 30 to 40% of patients, and surgical intervention must, therefore, always be considered when metastases are discovered preoperatively, intraoperatively, or during follow-up of patients with colorectal cancer.
