Abstract

Background: Mantle cell lymphoma (MCL) rarely presents as early-stage disease, but clinical observations suggest that patients who present with early-stage disease may have better outcomes than those with advanced-stage disease.Patients and methods: In this 13-institution study, we examined outcomes among 179 patients with early-stage (stage I or II) MCL in an attempt to identify prognostic factors that influence treatment selection and outcome. Variables examined included clinical characteristics, treatment modality, response to therapy, sites of failure, and survival.Results: Patients were predominantly male (78%) with head and neck being the most common presenting sites (75%). Most failures occurred outside the original disease site (79%). Although the administration of radiation therapy, either alone or with chemotherapy, reduced the risk of local failure, it did not translate into an improved freedom from progression or overall survival (OS). The treatment outcomes were independent of treatment modality. The 10-year OS for patients treated with chemotherapy alone, chemo-radiation therapy and radiation therapy alone were 69%, 62%, and 74% (P = 0.79), and the 10-year freedom from progression were 46%, 43%, and 31% (P = 0.64), respectively.Conclusion: Given the excellent OS rates regardless of initial therapy in patients with early-stage MCL, de-intensified therapy to limit treatment-related toxicity is a reasonable approach.

Abstract

To define further the characteristics of CD8+ cells in skin lesions of CD3+ CD4+ mycosis fungoides (MF), the authors used single- and double-label immunohistologic techniques and in situ hybridization to detect antigens and transcripts associated with certain types of cytotoxic or suppressor function. The cytotoxic markers included CD16, CD56, CD57, and an anti-sense probe for human Hanukah factor (HuHf) mRNA. Analysis of 23 cases demonstrated that lesional CD8+ cells were CD3+ T cells that generally lacked expression of any of the cytotoxic markers studied. Analysis of another 10 cases confirmed the CD3+ T-cell lineage of lesional CD8+ cells and demonstrated that these cells also lacked expression of the suppressor-associated marker, CD11b. In aggregate, these results indicate that most CD8+ cells in CD3+ CD4+ MF skin lesions are of T-cell rather than NK-cell differentiation. Their overall phenotype suggests that they may be major histocompatibility complex (MHC)-restricted cytotoxic T cells lacking appreciable levels of HuHF serine protease. Because the induction of CD8+ suppressor T cells is mediated by CD4+ T cells expressing the CD45RA+ RO- phenotype, CD45 epitope expression was studied in 15 MF cases. The vast majority (13/15) contained CD3+ CD4+ tumor cells that were CD45+ RA- RB+ RO+ 2B11+. This phenotype is consistent with memory T cells rather than suppressor-inducer T cells, and correlates with the paucity of phenotypically defined suppressor T cells in CD3+ CD4+ MF skin lesions.

Abstract

Capacitive current interruption with air-break disconnectors in a high-voltage network is an interactive event between the circuit and arc with a variety of interruptions and reignitions. In this contribution, first, a theoretical analysis related to this interaction is presented. The effect of capacitances at the source side (Cs) and load side (Cl) is investigated. Three distinct frequencies are identified as contributing to the voltage and current events in the circuit. Besides the power frequency quantities, a medium frequency transient arises related to the excursion of voltage across capacitances to the applied voltage, and a high-frequency transient arises due to charge redistribution between load- and source-side capacitance at reignition. Second, experimental results from an interruption measurement are studied in detail. Typical waveshapes of voltages across the capacitances, disconnector, and currents through the disconnector show that the transients during interrupted are in agreement with the theoretical analysis. Reignition voltage of the air gap and energy input to the arc on reignition are also studied. It is concluded that besides a higher interruption current and a higher power supply level, a lower (Cs/(Cl)ratio leads to more severe interruption and longer arc duration. Finally, the actual status of IEC recommendations on testing, that has taken into account this arc-circuit interaction, is summarized. [ABSTRACT FROM AUTHOR]
