Abstract

This article describes the process and results associated with the organizational-level recruitment of Black barbershops into Fitness in the Shop (FITShop), a 6-month barbershop-based intervention study designed to promote physical activity among Black men. Organizational-level recruitment activities included (1) a telephone call to prospective barbershop owners to assess their interest and eligibility for participation, (2) an organizational eligibility letter sent to all interested and eligible barbershops, (3) a visit to interested and eligible barbershops, where a culturally sensitive informational video was shown to barbershop owners to describe the study activities and share testimonies from trusted community stakeholders, and (4) a signed agreement with barbershop owners and barbers, which formalized the organizational partnership. Structured interviews were conducted with owners of a total of 14 enrolled barbershops, representing 30% of those determined to be eligible and interested. Most enrolled shops were located in urban settings and strip malls. Barbershop owners were motivated to enroll in the study based on commitment to their community, perceived client benefits, personal interest in physical activity, and a perception that the study had potential to make a positive impact on the barbershop and on reducing health disparities. Results offer important insights about recruiting barbershops into intervention trials.

17. Structural and Contextual Patterns in Family Health History Knowledge among African American Adults: A Mixed-Methods Social Network Analysis Study*

Abstract

Background:Family health history is a strong risk factor for many chronic diseases. Ethnic minorities have been found to have a low awareness of their family health history (FHH), which may pose a contributing factor to health disparities. Purpose:The purpose of this mixed-methods social network analysis study was to identify structural and contextual patterns in African American adults’ FHH knowledge based on interpersonal communication exchanges with their family members. Methods:African American adults completed individually administered family network interviews. Participants’ 3-generation family pedigree served as a visual aid to guide their interview. Our primary outcome of interest for this analysis was whether a family member was reported as someone who talks to the participant about their own (i.e., the family member’s) health, which we refer to as a “personal health informant.” To contextualize quantitative findings, participants were asked to describe howthey learned about the health history of the relatives they identified during their interview. Results:Participants (n=37) reported an average family network size of 29.4 relatives (SD = 15.5; Range = 10-67). Each participant, on average, named 17% of their familial network as personal health informants. Multivariate regression results showed that participants were more likely to name an alter as a personal health informant if the alter was female (OR = 2.14, p = 0.0519), from the maternal side of the participant’s family (OR = 1.12, p = 0.0006), had one or more chronic health conditions (OR = 2.41, p = 0.0041), was someone who has discussions with the participant about the participant’s health (OR = 16.28, p < 0.0001), was a source of family health information (OR = 3.46, p = 0.0072), and was someone whose health the participant helps to monitor or track (OR = 5.93, p = 0.0002). Complementary qualitative findings indicate that FHH knowledge is facilitated by open, direct communication among relatives. Personal health informants were described as disclosing information for the purposes of informing others for preventive purposes and for gaining social support. Participants also learned about FHH via other methods, including direct observation, during caretaking, and following a relative’s death. Conclusions:Communication and disclosure practices is an important determinant of African Americans’ FHH knowledge. More culturally and contextually meaningful public health efforts are needed to promote family health history sharing, especially regarding paternal family health history, siblings, and extended relatives.

23. Using the PRECEDE Planning Approach to Develop a Physical Activity Intervention for African American Men Who Visit Barbershops: Results From the FITShop Study.

Abstract

African American (AA) men have a higher prevalence of many chronic disease risk behaviors compared to Caucasian men, including physical inactivity. Innovative ways to reach AA men with interventions to increase physical activity (PA) and decrease other key risk factors are needed to reduce health disparities in this population. The barbershop is a natural but underutilized setting for reaching AA men. In the Fitness in the Shop (FITShop) study, shop owners, barbers, and customers were recruited from four local barbershops to complete structured interviews and customer focus groups. We assessed knowledge, perceived barriers, and interests/concerns about PA, as well as explored how to best intervene in the barbershop. Barbers and customers endorsed the idea of receiving health and PA information in the barbershop. These formative research results generated information and strategies for developing a multilevel barbershop-based health intervention to promote PA in the barbershop. This article describes the formative research results and how PRECEDE was used to develop a culturally and contextually appropriate, multilevel barbershop-based intervention designed to promote PA and to reduce chronic disease disparities among AA men. 24. The influence of social contextual conditions and basic psychological need fulfillment on self-management and emotional distress among urban African American adults with type-2 diabetes mellitus : a mixed methods study.

Abstract

Introduction: African Americans are disproportionally diagnosed with Type-2 diabetes, and have been observed to have poor self-management, which increases risk of complications. Social influences are commonly associated with diabetes-related outcomes. Purpose: The purpose of the study was to assess whether and how socio-contextual factors influence self-management and emotional distress in urban African American adults living with type-2 diabetes, as well as to assess whether social support facilitate relatedness need fulfillment within this population. The study also sought to gain an understanding of the psychosocial support needs and preferences of urban African American adults living with type-2 diabetes. Methods: The descriptive study employed a mixed-methods research design. African American adults were recruited to take a one-time written survey. Hypotheses one and two were tested using hierarchical linear regression, hypothesis three was assessed using VI a simple linear model, and hypotheses four and five were evaluated using path analysis. Four gender-stratified follow-up focus groups were also conducted, to confirm quantitative findings and gain insight into the support needs and preferences of urban African American adults living with type-2 diabetes. Study Variables: Independent study variables included social support, autonomy support, basic psychological needs (autonomy, competence, relatedness), and autonomous regulation. Primary dependent variables were diabetes self-management and diabetes-related emotional distress. Results: 155 eligible individuals (67 male, 88 female) were included in the quantitative analytic sample. 23 individuals (10 male, 13 female) participated in follow-up focus groups, yielding a total of 6 hours of audio-recorded focus group data. Autonomy and competence significantly predicted self-management and emotional distress in hypotheses one and two, respectively. Competence was the strongest predictor of selfmanagement and emotional distress among sample participants, followed by autonomy, but relatedness was not observed to be a significant variable in the prediction of study outcomes. Social support was observed to significantly predict relatedness need fulfillment in a simple linear model for hypothesis three. Data did not support hypothesis four and five. Qualitative findings contextualized the quantitative study findings. Conclusions: Future support programs should facilitate basic need fulfillment in urban African American adults living with type-2 diabetes. Relatedness is especially important in this collectivistic cultural group.
