4. Adjuvant Chemotherapy for Endometrial Cancer (ACE) trial: A randomized phase II study for advanced endometrial carcinoma.

Abstract

This study evaluated the feasibility and efficacy of three postoperative adjuvant chemotherapy regimens for endometrial cancer. Endometrioid cancer patients with intermediate‐risk stage I and II or high‐risk stage III and IV disease were randomly assigned to receive six cycles of either paclitaxel‐epirubicin‐carboplatin (TEC), paclitaxel‐anthracycline (doxorubicin)‐carboplatin (TAC), or dose‐dense paclitaxel‐carboplatin (ddTC). The primary end‐point was the completion rate (CRate) of six cycles of treatment. The secondary end‐points were progression‐free survival (PFS) and overall survival (OS). One hundred and one patients were treated as follows: 33 received TEC, 33 TAC, and 35 ddTC. The CRates for TEC, TAC, and ddTC were 94%, 64%, and 69%, respectively (P =.005). The TEC CRate was significantly higher than for the other two groups. However, the PFS and OS outcomes were not statistically different between the three groups. The 2‐year survival rates were 94%, 97%, and 97% for TEC, TAC, and ddTC, respectively. When compared to the current standard treatments for endometrial cancer, TEC is a promising candidate for a phase III trial based on its significantly superior CRate and equivalent PFS and OS. Abstract

There is some uncertainty surrounding the role of age in the outcome of cervical cancer. Some studies indicate that young women have a worse prognosis than older women with this cancer, while other studies have not found a relation between age and prognosis. Among the studies that have indicated a poorer prognosis for younger women, there is some difference in the age range over which this apparent effect seems to take place. Furthermore, the women in these studies are likely to have different stages of disease and to have received different methods of treatment. To clarify the role of age in the prognosis of cervical cancer, a study was conducted to determine which tumor characteristics might be different among younger women. A total of 380 patients who underwent radical hysterectomy for cervical cancer were studied. All the women had the specific form of cancer called squamous cell carcinoma, and all had disease that was either Stage IB or more advanced. These patients were divided into age groups covering the decades from 30 to 39, 40 to 49, 50 to 59, and 60 to 69 years. The microscopic features of the cancer were examined, including the cell types, the depth of cancer invasion of the underlying tissues, the invasion of the blood vessels by the cancer, and the spread of cancer to the local lymph nodes. All of these factors are known to affect the prognosis of the disease. However, of these factors, only the spread of cancer to the lymph nodes appeared to be influenced by age. The women in the 60 to 69 age group were significantly less likely to have metastatic cancer spread to the lymph nodes than were the women in the younger age groups. Although lymph node metastasis is an indicator of poor prognosis, it was also found that among the 60- to 69-year-old women with lymph node metastases, survival was superior to comparable patients in the younger groups. Abstract

The mechanism of the capping effect observed in magneto-optic (MO) recording in magnetic double-layer disks and magnetic super resolution (MSR) MO recording has been studied by means of an analytic calculation. Quantitative estimation of the field enhancement is presented as well as a comparison with experimental results. Implications of the present result are discussed.

32. Establishment and Characterization of a Subline Predisposed to Pulmonary Metastasis from a Human Gestational Choriocarcinoma Cell Line in Nude Mice

Abstract

A subline predisposed to pulmonary metastasis was successfully derived by repeating in vivo selection of GCH-1, a human gestational choriocarcinoma cell line, in nude mice. While the parent line GCH-1, transplanted subcutaneously to nude mice, induced pulmonary metastasis in a few of the host animals, the newly established subline GCH-l(m) successfully metastasized to the lungs in 100% of them. Compared with GCH-1, GCH-l(m) exhibited a higher degree of cell atypia, a lower capacity for cell growth and markedly higher productivity for human chorionic gonadotropin (hCG). Another characteristic of this subline was its enhanced growth after the addition of a pulmonary extract obtained from nude mice. The relationship of these findings to the mechanisms of metastasis of this cancer was discussed.

Abstract

Objective: This study was aimed to clarify the clinical profile and the proliferation activity of ovarian mucinous tumors of borderline malignancy. Materials and Methods: We reviewed 23 cases of ovarian mucinous tumor of borderline malignancy, all of which were FIGO stage I. Twenty-five cases of stage I mucinous adenocarcinoma were analyzed as a control. Results: The average age of cases of borderline malignancy (45.3 years) was significantly lower. The positive rate of tumor markers (CA125, CA19-9, CEA) and the mean tumor size was not different between two groups. Ten of 13 cases of borderline malignancy and 10 of 16 cases of malignancy were correctly diagnosed by frozen section examination and all other cases were underdiagnosed. Conservative operation was performed in half cases of borderline malignancy and no chemotherapy was added. All patients of borderline malignancy are free of disease, while three patients of malignant tumor died of disease in spite of intensive management. Proliferation activity was observed from a viewpoint of mitosis and positive rate in immunostaining using Ki-67 monoclonal antibody. Mitosis of borderline malignant tumors was not increased as compared to malignant tumors but the positive rate of borderline malignancy in Ki-67 immunostaining was significantly higher than adenoma and lower than adenocarcinoma. Conclusion: Ovarian mucinous tumor of borderline malignancy seems to be a poorer prognosis.
