Multinodular and vacuolating neuronal tumor (MVNT) is a new pattern of neuronal tumour included in the recently revised WHO 2016 classification of tumors of the CNS. There are 15 reports in the literature to date. They are typically associated with late onset epilepsy and a neoplastic vs. malformative biology has been questioned. We present a series of ten cases and compare their pathological and genetic features to better characterized epilepsy‐associated malformations including focal cortical dysplasia type II (FCDII) and low‐grade epilepsy‐associated tumors (LEAT). Clinical and neuroradiology data were reviewed and a broad immunohistochemistry panel was applied to explore neuronal and glial differentiation, interneuronal populations, mTOR pathway activation and neurodegenerative changes. Next generation sequencing was performed for targeted multi‐gene analysis to identify mutations common to epilepsy lesions including FCDII and LEAT. All of the surgical cases in this series presented with seizures, and were located in the temporal lobe. There was a lack of any progressive changes on serial pre‐operative MRI and a mean age at surgery of 45 years. The vacuolated cells of the lesion expressed mature neuronal markers (neurofilament/SMI32, MAP2, synaptophysin). Prominent labelling of the lesional cells for developmentally regulated proteins (OTX1, TBR1, SOX2, MAP1b, CD34, GFAPδ) and oligodendroglial lineage markers (OLIG2, SMI94) was observed. No mutations were detected in the mTOR pathway genes, BRAF, FGFR1 or MYB. Clinical, pathological and genetic data could indicate that MVNT aligns more with a malformative lesion than a true neoplasm with origin from a progenitor neuro‐glial cell type showing aberrant maturation.

Purpose: Human Herpesvirus 6 (HHV-6) is a ubiquitous virus acquired mainly during the first 2 years of life. Mesial Temporal Lobe Epilepsy with Hippocampal Sclerosis (MTLE-HS) is the most frequent pharmacoresistant epilepsy. One of the most common antecedents of MTLE-HS is febrile seizures (FS). Although the aetiology of MTLE-HS remains unclear, evidences suggest that HHV-6 infection could be implicated. The objective of this study was to investigate the presence of HHV-6B DNA in the hippocampus and adjoining temporal cortex of MTLE-HS patients submitted to surgery. Methods: A total of 22 MTLE-HS (13 females and nine males) cases were studied. The mean age at surgery was 39 9 years and mean age at onset of seizures was 10 6 years. These study cohort was compared to a group of 10 epileptic patients without MTLE-HS (six females, four males; mean age = 26 15 years) and with autopsy material from nine individuals without neurological disease. HHV-6B DNA was identified by real-time PCR with specific TaqMan probes. Results: We detected HHV-6B DNA in only one hippocampus from a MTLE-HS patient. This patient had a disease duration of 36 years and a history of febrile seizures in childhood. None of the non-MTLE or controls specimens showed positivity for HHV-6B. Conclusions: Our findings do not support a relevant etiologic role for HHV-6B in MTLE-HS, at least in this population. However, the possible role of viral infection in MTLE-HS epileptogenic process, in individual cases, cannot be excluded. FCT Grant PIC/IC/83297/2007

Glioblastoma (GBM) is the most common and aggressive primary brain tumor. The identification of novel molecular prognostic markers of GBM has recently been an area of great interest in neuro-oncology. The methylation status of the MGMT gene promoter is currently a promising molecular prognostic marker, but some controversial data have precluded its clinical use. We analyzed MGMT methylation by methylation-specific PCR in 90 GBM patients from four Portuguese hospitals, uniformly treated with radiotherapy combined with concomitant and adjuvant temozolomide (Stupp protocol). The Kaplan-Meier method was used to construct survival curves, and the log-rank test and a Cox-regression model were used to analyze patient survival. The methylation status of MGMT was successfully determined in 89% (80/90) of the tumors. The frequency of tumoral MGMT promoter methylation was 47.5%. The median overall survivals (OSs) were 16 months (95% CI 12.2-19.8) and 13 months (95% CI 13.3-18.7) for patients whose tumors had a methylated or unmethylated MGMT, respectively. Univariate and multivariate analyses did not show any statistically significant association between MGMT methylation status and patient OS (P=0.583 by the log-rank test; P=0.617 by the Cox-regression test) or progression-free survival (P=0.775 by the log-rank test; P=0.691 by the Cox-regression test). None of the patient clinical features were significantly correlated with survival. This is the first study to report the frequency of MGMT methylation among Portuguese GBM patients. Our data did not show statistically significant associations between MGMT promoter methylation and the outcome of GBM patients treated with temozolomide. Additional robust prospective studies are warranted to clarify whether the MGMT status should be used in clinical decisions.

El término tumor fibroso solitario designa una entidad patológica bien definida. Inicialmente descrito como tumor pleural, el aparecimiento de estas neoformaciones en otras localizaciones ha comenzado a ser descrito con mayor frecuencia. Actualmente se acepta la posibilidad de que estas neoplasias se puedan desarrollar virtualmente en todos los departamentos anatómicos. La rareza de esta patología no ha permitido hasta el momento esclarecer su histogénesis, así como otros aspectos relacionados con el comportamiento clínico, aspectos de tratamiento y de definición de pronóstico. El diagnóstico diferencial es importante, particularmente cuando la localización es extratorácica, para evitar confundirlo con neoplasias mesenquimatosas notablemente más agresivas. Los autores presentan dos casos adicionales de este tipo de neoplasia. En el primer caso la neoplasia se localizaba en la pelvis y fue responsable por uropatía obstructiva, descubierta accidentalmente en el transcurso de un traumatismo renal. El segundo caso es referente a un tumor de este tipo histológico localizado a nivel del cordón espermático. A propósito de los mismos se realiza una revisión del tema, teniendo como base la literatura disponible. Solitary fibrous tumour is a well defined pathological entity originally described as a tumour of the pleura, but the occurrence of this neoplasm has increasingly been described at other sites. At present the development of these tumours is recognized as possible in virtually all anatomical sites. The rarity of this type of pathology has not allowed up to the present time clarification of the histogenesis, clinical behaviour, treatment and prognosis of these tumours. Differential diagnosis becomes important, especially in extra-thoracic sites so as not to mistake them for much more aggressive mesenquimal tumours. Two additional cases of this neoplasm are presented, one of them responsible for obstructive uropathy, accidentally found in a patient suffering renal trauma. The second tumour was located in the spermatic cord. A review of the theme is presented based on the available literature.
