6. Multilocular sinonasal malignant melanoma: a poor prognostic subgroup?

Abstract

Clinical observations show that two subtypes of sinonasal malignant melanoma exist: uni- and multilocular melanoma. The aim of this retrospective study was to determine the prevalence and outcome of multilocular sinonasal malignant melanoma. All patients with sinonasal malignant melanoma treated at our institution between 1992 and 2011 were included. Survival and recurrence data were analyzed related to the distribution pattern of the tumors and other factors. Twenty-five patients were identified and included in the analysis. Seven patients (28%) suffered from multilocular, the remaining 18 patients (72%) from unilocular sinonasal malignant melanoma. The first group showed a significantly worse disease-free survival, whereas disease-specific and overall survival did not differ between the two subtypes. Multilocular sinonasal malignant melanoma is associated with an unfavorable disease-free survival compared to its unilocular counterpart.

Abstract

36th European Photovoltaic Solar Energy Conference and Exhibition; 1012-1014, Today there exists a growing market for biface modules. Compared to conventional modules, in which the solar cells are embedded between glass and a white back sheet, the cells are embedded normally between two glass panes. Due to the transparent backside reflected light can be used to increase energy yield. In order to stand out from the crowd and generate a competitive advantage for the company, various methods are already in use to optimize the energy yield. The aim of the current project is to increase the energy yield by using light guiding structured foils, glued to the back of a bifacial module. This paper first describes the concept of light management and the measurement concept for the study. The main part comprises the construction of one of the two used validation systems. Furthermore, reference measurements and bonding tests are presented.

16. Solitary fibrous tumor of the orbit—two cases and a review of the literature

Abstract

Solitary fibrous tumors of the orbit (SFT) are mesenchymal lesions that can develop either as malignant or benign neoplasias. We describe the histological features leading to the diagnosis in two females and review the current literature. Diagnosis of SFT only can be performed by histological examination, since clinical signs and radiological features are not specific enough. Even a malignant or benign course cannot be predicted, since clinical and radiological features do not correlate with histological signs of malignancy and vice versa. Radical resection is the treatment of choice, since no other treatment option has been proven to be efficient

Abstract

35th European Photovoltaic Solar Energy Conference and Exhibition; 1284-1286, Within this work miniature digital temperature sensors were utilized to monitor the moisture ingress in a specially prepared photovoltaic mini module during accelerated aging. This work was conducted within the Austrian flagship project INFINTIY, which deals with the development of climate-adapted PV solutions in order to achieve yield-optimized PV systems with extended system lifetimes. As a further step in conducting research related to moisture ingress in PV modules, within this work a mini PV module was manufactured, comprising eight digital temperature and humidity sensors in front and behind a solar cell (substituted by an aluminium sheet). The goal here was to determine the diffusion path of the humidity diffusing through the backsheet to the front side (sunny side) of the laminate. Here it was essential to gain knowledge about the duration of the moisture ingress to the front part of the test module during accelerated aging as elevated humidity causes corrosion effects on e.g. the grid fingers (reduced electrical conductivity).

Abstract

35th European Photovoltaic Solar Energy Conference and Exhibition; 1385-1389, Microclimatic conditions inside of the enclosures of photovoltaic (PV) inverters are suspected to have a tremendous impact on degradation and failure modes of the devices. To address this issue, a custom multi-channel data logger has been designed which is capable of continuously measuring temperature, relative humidity and wetness in confined space. The collected data is written to a flash memory and additionally sent to a server via GSM to enable remote real-time monitoring. Consequently, the instrument is designed to be integrated in a PV inverter and to be utilised for the characterisation of the respective microclimatic conditions and the identification of error sources, where required. Preliminary field test results showed that the ambient conditions inside of an inverter are rather dependent on its duty than on environmental situations. The findings can be useful to optimise the design of PV inverters and their enclosures and hence contribute to an increased durability and a prolonged life-time.

Abstract

The aim this study was to evaluate imaging findings using position emission tomography (PET) in combination with computed tomography (CT) and 18F-fluorodeoxyglucose (18F-FDG) in sinonasal malignant melanoma (SNMM) of the head and neck in a retrospective analysis of a consecutive cohort of patients. 18F-FDG-PET/CT examinations were performed for initial staging and compared with CT or magnetic resonance tomography (MRI), and 18F-FDG-PET alone. Medical records were reviewed retrospectively with regard to the location and the size of the tumor. Furthermore, locoregional and distant metastases with a consecutive change in therapy detected by 18F-FDG-PET/CT were assessed. Ten patients suffering from sinonasal malignant melanoma were staged and followed by 18F-FDG-PET/CT imaging. A total of 34 examinations were obtained. 18F-FDG-PET/CT depicted all primary tumors adequately. Aside from one cerebral metastasis all regional and distant metastases were truly identified by using this method. In summary, if available, 18F-FDG-PET/CT is a valuable imaging modality for staging and re-staging sinonasal malignant melanoma to evaluate expansion of the primary tumor, locoregional disease, and distant metastases

29. Split-rib reconstruction of the frontal sinus: two cases and literature review

Abstract

Background: Large defects of the anterior wall of the frontal sinus require closure using either autologous or foreign material. In cases of osteomyelitis, the reconstruction must be resistant to bacterial infection. Split-rib osteoplasty can be used in different sites. Methods: Two patients with malignant sinonasal tumours underwent repeated treatment, and subsequently developed osteomyelitis of the frontal bone. After adequate therapy, a large defect of the anterior wall persisted. Reconstruction was performed using the split-rib method. The literature on this topic was reviewed. Results: Both patients' treatment were successful. No complications occurred. A PubMed search on the topic of rib reconstruction of the frontal sinus and skull was performed; 18 publications matched the inclusion criteria. From these sources, we noted that 182 reconstructions yielded good results with few complications. Conclusion: Large defects of the anterior wall of the frontal sinus can be closed successfully using autologous split-rib grafting. Aesthetic outcome is good and donor site morbidity is minimal

30. Isolated fungus ball mimicking mucocele or frontal sinus tumour: a diagnostic pitfall

Abstract

Objective: To demonstrate the clinical, radiological and diagnostic pitfalls of managing an isolated frontal sinus fungus ball, and to compare with the literature. Material and methods: Retrospective analysis of two cases and literature review. Results: Isolated frontal sinus fungus ball is a rare cause of frontal sinus disease. We present two cases of isolated frontal sinus fungus ball which pre-operatively were suspected to be either a tumour or a mucocele. In both cases, cheesy, clay-like material was found intra-operatively within the frontal sinus, suggesting a fungus ball. Effective treatment included surgical debridement via an exclusively endoscopic or an external approach, variously. Final histopathological and mycological analysis revealed Aspergillus fumigatus. A literature review revealed 20 reported cases of isolated frontal sinus fungus ball, confirming the low prevalence of the disease. Conclusions: Frontal sinus fungus ball should be considered in the differential diagnosis of chronic, nonspecific forehead symptoms. To evaluate the underlying disease, computed tomography scans should first be performed, followed by magnetic resonance imaging if malignancy is suspected. It is essential to be aware of the possibility of an atypical fungus ball appearance on computed tomography and magnetic resonance imaging scans. If bony destruction and calcification coexist on radiological images, then endoscopic biopsy is an indispensable part of the diagnostic procedure, and should be performed to collect material for both histological and mycological analysis, and to aid surgical planning. 32nd European Photovoltaic Solar Energy Conference and Exhibition; 1969-1972, During this work a portable solar cell characterization device as well as its application in a building integrated photovoltaic (BIPV) test system is shown. The solar cells are characterized by measuring their currentvoltage curve (I-V-curve) in user-defined intervals. A test setup was build that allows the application of various types of façade elements and solar cells. Up to four solar cells, oriented at the four cardinal directions, can be measured simultaneously over a whole day, which allows a comprehensive comparison of them. Besides additional energy yields also advantages of a more consistent/user adapted energy distribution over a day, resulting from façademounted solar cells/modules, can be evaluated. To enable a more detailed data analysis the local irradiance as well as the temperature of each cell and the specific façade panel is determined. Outdoor measurements under the influence of natural sunlight and various weather conditions were conducted with crystalline cells and thin film (CZTS) modules, mounted on exchangeable façade elements.

32. Defective epithelial barrier in chronic rhinosinusitis: The regulation of tight junctions by IFN-? and IL-4

Abstract

BACKGROUND: Chronic rhinosinusitis (CRS) is a common disease with still unclear pathophysiologic mechanisms. Epithelial tight junctions (TJs) have been shown to be involved in different chronic disorders including bronchial asthma inflammatory bowel diseases and skin disorders. The regulation of epithelial barrier function and TJ expression has not been extensively studied in patients with CRS and in the paranasal sinus epithelium thus far. OBJECTIVE: We sought to elucidate the TJ expression pattern in the epithelium of the sinonasal mucosa and its regulation in patients with CRS. METHODS: Trans tissue resistance was measured in biopsy specimens from healthy control subjects and patients with CRS with and without nasal polyps. TJ protein expression was determined by using immunofluorescence Western blotting and real time PCR. Primary epithelial cell cultures from patients with CRS and control subjects were used in air liquid interface (ALI) cultures for the measurement of transepithelial resistance (TER) and TJ expression. The effect of IFN ? IL 4 and IL 17 on ALI cultures was assessed. RESULTS: A decreased trans tissue resistance was found in biopsy specimens from patients with CRS with nasal polyps along with an irregular patchy and decreased expression of the TJ molecules occludin and zonula occludens 1. TER was reduced in ALI cultures from patients with CRS with nasal polyps. The cytokines IFN ? and IL 4 decreased TER whereas IL 17 did not have any influence on epithelial integrity. CONCLUSION: A defective epithelial barrier was found in patients with CRS with nasal polyps along with a decreased expression of TJ proteins. The disruption of epithelial integrity by IFN ? and IL 4 in vitro indicates a possible role for these proinflammatory cytokines in the pathogenesis of patients with CRS.

40. Multilocular sinonasal malignant melanoma: a poor prognostic subgroup?

Abstract

Clinical observations show that two subtypes of sinonasal malignant melanoma exist: uni- and multilocular melanoma. The aim of this retrospective study was to determine the prevalence and outcome of multilocular sinonasal malignant melanoma. All patients with sinonasal malignant melanoma treated at our institution between 1992 and 2011 were included. Survival and recurrence data were analyzed related to the distribution pattern of the tumors and other factors. Twenty-five patients were identified and included in the analysis. Seven patients (28 %) suffered from multilocular, the remaining 18 patients (72 %) from unilocular sinonasal malignant melanoma. The first group showed a significantly worse disease-free survival, whereas disease-specific and overall survival did not differ between the two subtypes. Multilocular sinonasal malignant melanoma is associated with an unfavorable disease-free survival compared to its unilocular counterpart.

Abstract

Mucosal melanoma is a rare disease, which differs from its cutaneous counterpart genetically and for its clinical behaviour. Moreover this is a heterogeneous disease based on the tissue of origin. As CT7 and CT10 are highly expressed in cutaneous melanoma and are immunogenic in this disease, we analysed their expression throughout the different subtypes of mucosal melanoma and tumor development. We detected a frequent expression of CT7 in primaries and corresponding metastases (55%) as well as for CT10 (30%). This expression resulted to be heterogeneous in the same tumor specimen and moreover influenced by the tissue of origin. Our results support the role of these antigens in immunotherapy for mucosal melanoma.

Abstract

Bronchiolitis obliterans syndrome is the main limitation for long‐term survival after lung transplantation. Some specific B cell populations are associated with long‐term graft acceptance. We aimed to monitor the B cell profile during early development of bronchiolitis obliterans syndrome after lung transplantation. The B cell longitudinal profile was analyzed in peripheral blood mononuclear cells from patients with bronchiolitis obliterans syndrome and patients who remained stable over 3 years of follow‐up. CD24hiCD38hitransitional B cells were increased in stable patients only, and reached a peak 24 months after transplantation, whereas they remained unchanged in patients who developed a bronchiolitis obliterans syndrome. These CD24hiCD38hitransitional B cells specifically secrete IL‐10 and express CD9. Thus, patients with a total CD9+B cell frequency below 6.6% displayed significantly higher incidence of bronchiolitis obliterans syndrome (AUC = 0.836, PPV = 0.75, NPV = 1). These data are the first to associate IL‐10‐secreting CD24hiCD38hitransitional B cells expressing CD9 with better allograft outcome in lung transplant recipients. CD9‐expressing B cells appear as a contributor to a favorable environment essential for the maintenance of long‐term stable graft function and as a new predictive biomarker of bronchiolitis obliterans syndrome–free survival. In lung transplant patients with bronchiolitis obliterans syndrome and patients who remained stable over 3 years of follow‐up, IL‐10–secreting CD24hiCD38hi transitional B cells expressing CD9 are associated with better allograft outcome, suggesting CD9‐expressing B cells as a new predictive biomarker of bronchiolitis obliterans syndrome–free survival.
