Item does not contain fulltext, PURPOSE: This study assesses the repeatability of quantitative autofluorescence (qAF) in a multicenter setting and evaluates qAF as the end point for clinical trials in recessive Stargardt disease 1 (STGD1). METHODS: A total of 102 patients with STGD1 underwent qAF imaging as part of the Stargardt Remofuscin Treatment Trial (STARTT; EudraCT No. 2018-001496-20). For 166 eyes, we obtained qAF imaging at 2 visits, with 2 recordings per visit. The qAF8 values were independently determined by the study site and a central reading center. Intra- and inter-visit reproducibility, as well as interobserver (study site versus reading center) reproducibility were obtained using intraclass correlation (ICC), one-sample t-test, and Bland-Altman coefficient of repeatability. RESULTS: The qAF repeatability was ± 26.1% for intra-visit, ± 40.5% for inter-visit, and ± 20.2% for the interobserver reproducibility measures. Intra-visit repeatability was good to excellent for all sites (ICC of 0.88-0.96). Variability between visits was higher with an overall ICC of 0.76 (0.69-0.81). We observed no significant difference in qAF values across sites between visits (7.06 ± 93.33, P = 0.238). CONCLUSIONS: Real-life test-retest variability of qAF is higher in this set of data than previously reported in single center settings. With improved operator training and by selecting the better of two recordings for evaluation, qAF serves as a useful method for assessing changes in autofluorescence signal. TRANSLATIONAL RELEVANCE: The qAF can be adopted as a clinical trial end point, but steps to counterbalance variability should be considered.

Zusammenfassung: Der Einsatz von „patient reported outcomes“ (PROs), Beurteilungen der Auswirkungen von Krankheit oder Therapie in einer standardisierten Form durch den Patienten selbst, hat in den letzten Jahren zunehmend an Bedeutung gewonnen. Sowohl im Rahmen von Zulassungen neuer Therapien durch regulierende Behörden als auch im Bereich politischer Entscheidungen und Aktivitäten der Pharmaindustrie findet die durch PROs generierte Evidenz ihre Anwendung. Durch PROs lässt sich eindrücklich der Vorteil einer Therapie belegen, weil die Lebensqualität der Patienten, deren Verbesserung oder Erhalt Ziel jeglicher therapeutischer Intervention ist, als eine zentrale Variable erfasst werden kann. Dem Routineeinsatz von PROs stehen noch einige methodische Probleme im Weg. Jedoch sind durch die Implementierung von PROs deutliche Vorteile zu erwarten, die sowohl dem Arzt als auch dem Patienten nutzen, wie eine Verbesserung der Kommunikation und der gemeinsamen Entscheidungsfindung, die zu einer höheren Compliance führen sollte.

Abstract

Background/Aims: To further validate the Vision Impairment in Low Luminance (VILL) questionnaire, which captures visual functioning and vision-related quality of life under low luminance, low contrast conditions relevant to age-related macular degeneration (AMD).\ud \ud Methods: The VILL was translated from German into English (UK), Danish, Dutch, French, Italian and Portuguese. Rasch analysis was used to assess psychometric characteristics of 716 participants (65% female, mean age 72±7 years, 82% intermediate AMD) from the baseline visit of the MACUSTAR study. In a sub-set of participants (n=301), test-retest reliability (intra-class correlation coefficient, ICC; coefficient of repeatability, CoR) and construct validity were assessed.\ud \ud Results: Four items were removed from the VILL-37 due to misfit. The resulting VILL-33 has three subscales with no disordered thresholds and no misfitting items. No differential item functioning and no multidimensionality were observed. Person reliability and person separation index were 0.91 and 3.27 for the reading subscale (VILL-R), 0.87 and 2.58 for the mobility subscale (VILL-M) and 0.78 and 1.90 for the emotional subscale (VILL-E).
