2. What factors influence a Quality Improvement Collaborative in improving contraceptive services for foreign-born women? A qualitative study in Sweden

Abstract Background Improved contraceptive services could reduce the unmet need for contraception and unintended pregnancies globally. This is especially true among foreign-born women in high-income countries, as the health outcomes related to unmet need of contraception disproportionally affect this group. A widely used quality improvement approach to improve health care services is Quality Improvement Collaborative (QIC). However, evidence on to what extent, how and why it is effective and what factors influence a QIC in different healthcare contexts is limited. The purpose of this study was to analyse what factors have influenced a successful QIC intervention that is aimed to improve contraceptive service in postpartum care, mainly targeting foreign-born women in Sweden. Methods A qualitative, deductive design was used, guided by the Consolidated Framework for Implementation Research (CFIR). The study triangulated secondary data from four learning seminars as part of the QIC, with primary interview data with four QIC-facilitators. The QIC involved midwives at three maternal health clinics in Stockholm County, Sweden, 2018–2019. Results Factors from all five CFIR domains were identified, however, the majority of factors that influenced the QIC were found inside the QIC-setting, in three domains: intervention characteristics, inner setting and process. Outside factors and those related to individuals were less influential. A favourable learning climate, emphasizing co-creation and mutual learning, facilitated reflections among the participating midwives. The application of the QIC was facilitated by adaptability, trialability, and a motivated and skilled project team. Our study further suggests that the QIC was complex because it required a high level of engagement from the midwives and facilitators. Additionally, it was challenging due to unclear roles and objectives in the initial phases. Conclusions The application of the CFIR framework identified crucial factors influencing the success of a QIC in contraceptive services in a high-income setting. These factors highlight the importance of establishing a learning climate characterised by co-creation and mutual learning among the participating midwives as well as the facilitators. Furthermore, to invest in planning and formation of the project group during the QIC initiation; and to ensure adaptability and trialability of the improvement activities.

Abstract Background Sexual and reproductive health and rights (SRHR) is an important aspect for young people. In Sweden, young migrants often encounter barriers to accessing and using sexual and reproductive health (SRH) services, despite that these services are free of charge for young people (ages 15–25). Healthcare providers’ views and best practices are of great importance for improving the utilisation of existing SRH services, particularly for young people. This study aims to understand healthcare providers’ experiences and perspectives on barriers to SRHR among young migrants and their suggestions for strategies to improve the provision of SRH services to this group. Methods Midwives, counsellors and nurses with at least five years of professional experience within SRHR were reached through a purposeful sample at primary care clinics, specialised clinics and youth-friendly clinics, which provide SRH services to migrant youths in Stockholm. Twelve interviews were conducted from May 2018 to February 2020. Qualitative content analysis was performed. Results The analysis identified one theme: Improving the fragmentation in the SRH services, and four sub-themes: 1. Being unaware of SRHR; 2. Creating trust and responsive interactions; 3. Communicating in the same language; and 4. Collaborating to build bridges. The barriers included distrust in the healthcare system, socio-cultural norms surrounding SRHR, incomplete translations, and a need for long-lasting collaboration with SRH services and other range of services for migrants. The strategies for improvement as suggested by participants included involving existing cultural groups and organisations to enable trust, consistent and dependable interpreters, a streamline of SRH services with other healthcare staff and health facilities, and collaborations with homes designated for young migrants and language schools for a direct linkage to service providers. Conclusions Findings indicate that there are fragmentations in SRH services, and these include lack of knowledge about SRHR among migrant youth, language and communication barriers, and a lack of structure needed to build dependable services that go beyond one-time interventions. While initiatives and strategies from healthcare providers for improvement of SRH services exist, the implementation of some strategies may also require involving the regional and national decision-makers and multi-stakeholders like communities, civil society and young migrants themselves.

Women worldwide are dealing with hot flashes (潮热), a perimenopausal feature that is often tiring, embarrassing, and energetically draining. Conventionally, Chinese medicine defines hot flashes as Kidney-yin deficiency and its clinical manifestations include red face, sweating, hot sensations in the palms and soles, mouth and nose dryness, constipation, insomnia, lumbar soreness, knee weakness, red tongue, etc. To obtain a broader perspective and understand the dynamics of hot flashes, we examined the mechanisms behind hot flashes based on both the knowledge from ancient Chinese medicinal texts, as well as novel research findings of Chinese and Western medicine. This perspective was the foundation for the acupuncture study of our traditional Chinese medicine conducted in 2018–2019 in Shanghai. This study, designed as a pragmatic randomized control trial with two parallel groups, focused on regulating and unblocking conception and governor vessels. The results confirmed that our acupuncture method could effectively reduce both the frequency and severity of hot flashes and improve life quality of middle-aged women.

Knappast någon organisation vill dock bli anklagad för att införa nya styrformer som tillfälligtvis råkar vara populära. Organisationer vill tvärtom markera att de är speciella. Ett skäl till att det har blivit populärt med processer kan vara att de – till skillnad från procedurer – tycks förekomma naturligt och därför inte kan hänföras till något managementmode. Det hindrar inte att sammansättningar med ordet process, som processorientering, processledning eller processtyrning, kan mötas med misstro av dem som blir föremål för sådana nya styrformer och i praktiken utsätts för omorganisation och andra förändringar. Kanske blir det lättare att processorientera en verksamhet om förändringarna genomförs under en unik beteckning. Den här artikeln handlar om Lotta och Esther, framför allt om Esther.

- Abstract
Background. Numerous studies have shown that baseline drug resistance patterns may influence the outcome of antiretroviral therapy. Therefore, guidelines recommend drug resistance testing to guide the choice of initial regimen. In addition to optimizing individual patient management, these baseline resistance data enable transmitted drug resistance (TDR) to be surveyed for public health purposes. The SPREAD program systematically collects data to gain insight into TDR occurring in Europe since 2001. Methods. Demographic, clinical, and virological data from 4140 antiretroviral-naive human immunodeficiency virus (HIV)-infected individuals from 26 countries who were newly diagnosed between 2008 and 2010 were analyzed. Evidence of TDR was defined using the WHO list for surveillance of drug resistance mutations. Prevalence of TDR was assessed over time by comparing the results to SPREAD data from 2002 to 2007. Baseline susceptibility to antiretroviral drugs was predicted using the Stanford HIVdb program version 7.0. Results. The overall prevalence of TDR did not change significantly over time and was 8.3% (95% confidence interval, 7.2%-9.5%) in 2008-2010. The most frequent indicators of TDR were nucleoside reverse transcriptase inhibitor (NRTI) mutations (4.5%), followed by nonnucleoside reverse transcriptase inhibitor (NNRTI) mutations (2.9%) and protease inhibitor mutations (2.0%). Baseline mutations were most predictive of reduced susceptibility to initial NNRTI-based regimens: 4.5% and 6.5% of patient isolates were predicted to have resistance to regimens containing efavirenz or rilpivirine, respectively, independent of current NRTI backbones. Conclusions. Although TDR was highest for NRTIs, the impact of baseline drug resistance patterns on susceptibility was largest for NNRTIs. The prevalence of TDR assessed by epidemiological surveys does not clearly indicate to what degree susceptibility to different drug classes is affected. © The Author 2015.

Transmitted human immunodeficiency virus drug resistance in Europe is stable at around 8%. The impact of baseline mutation patterns on susceptibility to antiretroviral drugs should be addressed using clinical guidelines. The impact on baseline susceptibility is largest for nonnucleoside reverse transcriptase inhibitors., Background. Numerous studies have shown that baseline drug resistance patterns may influence the outcome of antiretroviral therapy. Therefore, guidelines recommend drug resistance testing to guide the choice of initial regimen. In addition to optimizing individual patient management, these baseline resistance data enable transmitted drug resistance (TDR) to be surveyed for public health purposes. The SPREAD program systematically collects data to gain insight into TDR occurring in Europe since 2001. Methods. Demographic, clinical, and virological data from 4140 antiretroviral-naive human immunodeficiency virus (HIV)–infected individuals from 26 countries who were newly diagnosed between 2008 and 2010 were analyzed. Evidence of TDR was defined using the WHO list for surveillance of drug resistance mutations. Prevalence of TDR was assessed over time by comparing the results to SPREAD data from 2002 to 2007. Baseline susceptibility to antiretroviral drugs was predicted using the Stanford HIVdb program version 7.0. Results. The overall prevalence of TDR did not change significantly over time and was 8.3% (95% confidence interval, 7.2%–9.5%) in 2008–2010. The most frequent indicators of TDR were nucleoside reverse transcriptase inhibitor (NRTI) mutations (4.5%), followed by nonnucleoside reverse transcriptase inhibitor (NNRTI) mutations (2.9%) and protease inhibitor mutations (2.0%). Baseline mutations were most predictive of reduced susceptibility to initial NNRTI-based regimens: 4.5% and 6.5% of patient isolates were predicted to have resistance to regimens containing efavirenz or rilpivirine, respectively, independent of current NRTI backbones. Conclusions. Although TDR was highest for NRTIs, the impact of baseline drug resistance patterns on susceptibility was largest for NNRTIs. The prevalence of TDR assessed by epidemiological surveys does not clearly indicate to what degree susceptibility to different drug classes is affected.

Alloying Au into the outermost atomic layer of Ni(111) significantly alters the physical and chemical properties of the surface. The reactivity is investigated by the use of seeded supersonic molecular beams of CH4 and it is found that the nobleness of Au reduces the overall reactivity toward CH4. This is accounted for in an ensemble model resolving the sticking probability on Ni atoms having different nearest neighbor surroundings. Although a mean field description of site distributions is found to be a very good approximation it is improved by using experimentally determined ensemble statistics from STM images. The strong influence of the vibrational temperature on the sticking coefficients of CH4 vs translational energy on the pure Ni(111) is also demonstrated. Desorption energies of CO and D2 is observed to decrease approximately 25–30 kJ/mole as the coverage of Au is increased from 0.0 to 0.7 ML. In TPD spectra of deuterium saturated surface alloys a new clearly resolved desorption state is observed at 180–220 K with maximum intensity around θAu=0.3–0.4 ML. This state is clearly related to chemisorption sites involving both Au and Ni. A site model based on mean field statistics adequately accounts for the appearance of this state. The effect of Au is also evident in the TPD spectra of CO saturated Au/Ni(111) surface alloys where the saturation coverage decreases and new structure develops. © 1996 American Institute of Physics. This paper reports a detailed molecular beam study of the dissociative sticking of methane incident on clean Ni(100). It is demonstrated that the sticking coefficient depends strongly on the translational energy of the molecule. It is also observed that an increase in the vibrational energy of the methane leads to a dramatic enhancement of the sticking, emphasizing that the initial vibrational state is of crucial importance. These results are consistent with a mechanism of direct activated dissociative chemisorption where the dynamics is dominated by a barrier in the potential energy surface mainly located in the vibrational coordinates. Normal-energy scaling is only approximately observed. The effect of surface temperature is also investigated but is found to be smaller than activation by translational or vibrational energy. A simplified analysis in terms of state resolved sticking curves, Sν(Ei), considering only the vibrational C–H stretch modes of methane as relevant for the sticking, gives a good description of the data. These sticking curves enables a calculation of the thermal sticking coefficient which is found to be in excellent agreement with bulb experiments directly probing this. © 1995 American Institute of Physics. Background: One out of ten newly diagnosed patients in Europe was infected with a virus carrying a drug resistant mutation. We analysed the patterns over time for transmitted drug resistance mutations (TDRM) using data from the European Spread program.Methods: Clinical, epidemiological and virological data from 4317 patients newly diagnosed with HIV-1 infection between 2002 and 2007 were analysed. Patients were enrolled using a pre-defined sampling strategy.Results: The overall prevalence of TDRM in this period was 8.9% (95% CI: 8.1-9.8). Interestingly, significant changes over time in TDRM caused by the different drug classes were found. Whereas nucleoside resistance mutations remained constant at 5%, a significant decline in protease inhibitors resistance mutations was observed, from 3.9% in 2002 to 1.6% in 2007 (p = 0.001). In contrast, resistance to non-nucleoside reverse transcriptase inhibitors (NNRTIs) doubled from 2.0% in 2002 to 4.1% in 2007 (p = 0.004) with 58% of viral strains carrying a K103N mutation. Phylogenetic analysis showed that these temporal changes could not be explained by large clusters of TDRM.Conclusion: During the years 2002 to 2007 transmitted resistance to NNRTI has doubled to 4% in Europe. Background: International travel plays a role in the spread of HIV-1 across Europe. It is, however, not known whether international travel is more important for spread of the epidemic as compared to endogenous infections within single countries. In this study, phylogenetic associations among HIV of newly diagnosed patients were determined across Europe.Results: Data came from the SPREAD programme which collects samples of newly diagnosed patients that are representative for national HIV epidemics. 4260 pol sequences from 25 European countries and Israel collected in 2002-2007 were included.We identified 457 clusters including 1330 persons (31.2% of all patients). The cluster size ranged between 2 and 28. A number of 987 patients (74.2%) were part of a cluster that consisted only of patients originating from the same country. In addition, 135 patients (10.2%) were in a cluster including only individuals from neighboring countries. Finally, 208 patients (15.6%) clustered with individuals from countries without a common border. 28. A cell-surface superoxide dismutase is a binding protein for peroxinectin, a cell-adhesive peroxidase in crayfish.

- Abstract
Peroxinectin, a cell-adhesive peroxidase (homologous to human myeloperoxidase), from the crayfish Pacifastacus leniusculus, was shown by immuno-fluorescence to bind to the surface of crayfish blood cells (haemocytes). In order to identify a cell surface receptor for peroxinectin, labelled peroxinectin was incubated with a blot of haemocyte membrane proteins. It was found to specifically bind two bands of 230 and 90 kDa; this binding was decreased in the presence of unlabelled peroxinectin. Purified 230/90 kDa complex also bound peroxinectin in the same assay. In addition, the 230 kDa band binds the crayfish beta-1,3-glucan-binding protein. The 230 kDa band could be reduced to 90 kDa, thus showing that the 230 kDa is a multimer of 90 kDa units. The peroxinectin-binding protein was cloned from a haemocyte cDNA library, using immuno-screening or polymerase chain reaction based on partial amino acid sequence of the purified protein. It has a signal sequence, a domain homologous to CuZn-containing superoxide dismutases, and a basic, proline-rich, C-terminal tail, but no membrane-spanning segment. In accordance, the 90 and 230 kDa bands had superoxide dismutase activity. Immuno-fluorescence of non-permeabilized haemocytes with affinity-purified antibodies confirmed that the crayfish CuZn-superoxide dismutase is localized at the cell surface; it could be released from the membrane with high salt. It was thus concluded that the peroxinectin-binding protein is an extracellular SOD (EC-SOD) and a peripheral membrane protein, presumably kept at the cell surface via ionic interaction with its C-terminal region. This interaction with a peroxidase seems to be a novel function for an SOD. The binding of the cell surface SOD to the cell-adhesive/opsonic peroxinectin may mediate, or regulate, cell adhesion and phagocytosis; it may also be important for efficient localized production of microbicidal substances.

By applying the methodology of fuzzy logic the operational experience of manual control can be used as the basis for implementing automatic control schemes. The paper describes the application of fuzzy logic to the computer control of a rotary cement kiln. A special language - Fuzzy Control Language - facilitating computer programming with the relevant control algorithms is outlined. Based on experience gained through fuzzy control on an actual cement kiln it is concluded that fuzzy control is a practicable and effective way of increasing the level of coordinative control on industrial processes

The rate of methanol synthesis over a Cu(100) single crystal from a 1 : 1 mixture of CO
2 and H2 has been measured at a total pressure of 2 bar and a temperature range of 483–563 K. At these conditions the apparent activation energy is determined to be 69 kJ mol-1 , and at 543 K the turnover rate is 2.7 × 10-4 (site s)-1 . A kinetic model for the methanol synthesis is presented. Predictions from this model are in good agreement with the rates of methanol synthesis observed on real catalysts at industrial conditions.- Published

32. Peroxinectin, a Novel Cell Adhesion Protein from Crayfish Blood

- Abstract
From blood cells of the crayfish Pacifastacus leniusculusa 76-kDa protein that mediated attachment and spreading of the crayfish blood cells was purified. The cDNA for this cell adhesion protein was isolated, cloned, and sequenced. The deduced protein sequence was significantly similar to one family of peroxidases, e.g.,myeloperoxidase. Consistently, the 76-kDa protein, for which we propose the name peroxinectin, had peroxidase activity. A synthetic peptide derived from the peroxinectin sequence containing Lys-Gly-Asp mimicked the cellular activity of the intact protein, implicating this sequence as the cell-binding site. Peroxinectin is the first cell adhesion molecule cloned from invertebrate blood and, to our knowledge, the first protein from any organism that combines being a cell adhesion ligand and a peroxidase.

- Abstract
We have cloned and sequenced a β subunit of integrin from a cDNA library of crayfish hemocytes. This β integrin shows great similarity to β integrin subunits from other animals; the highest is towards βpat-3 from Caenorhabditis elegans followed by βPS from Drosophila melanogaster. By immunoblotting with antibodies raised towards a synthetic peptide corresponding to a part of the cytoplasmic region of the deduced protein sequence, it was shown that the integrin is present in the membrane of the hemocytes. This is the first integrin found in hemocytes of an invertebrate animal and this finding opens the door for further investigations on integrins and their role in the invertebrate immune system. In 1915 one of us reported some work on this subject (G. W. H.),1 and in 1917 Hall, Schlegel and McNally made further examinations (Marsh's test) with the following results (unpublished): In thirteen cases the spinal fluid was withdrawn twelve hours after the intravenous administration of either novarsenobenzol or neodiarsenol brand of arsphenamin in 0.6 gm. doses and in four instances the presence of arsenic was noted. In twelve cases treated similarly and the spinal fluid withdrawn twenty-four hours after the intravenous medication, no arsenic was found.March 15, 1919, we reported to the Chicago Neurological Society a new procedure which we hoped would facilitate the passage of arsenic into the spinal fluid after its intravenous administration. Because it is axiomatic that the venereal diseases must be attacked wherever they exist, the provision of adequate control measures for these diseases in industry is important. Thus far, however, only scattered attempts at establishing such control have been made. Present control activities are restricted largely to a general program. They do not aim at specific population groups. Industry represents a great number of special population groups—a total labor force of fifty-three million men and women, 14 years of age or older, in communities throughout the country.A venereal disease control program carried directly to the public will provide for a large percentage of workers. But it will not reach them all, nor is there assurance that patients will remain under treatment. 36. TREATMENT OF INJURIES DURING THE ACUTE STAGE BY DIATHERMY

- Abstract
The work reported in this paper was prompted by the desire to give greater relief to minor injuries not requiring hospitalization. The early cases consisted principally of sprained wrists and ankles. Previous to November, 1923, these ankles and wrists were treated about as follows: Roentgen ray examination ruled out fractures; rest was given to the part; immobilization by casts or splints was employed where pain persisted; the patients were given three 1 grain (0.065 gm.) tablets of powdered opium to be taken at night; codein sulphate, one-half grain (0.03 gm.) was given if the patient was still losing sleep after the second day. The patient was advised to apply ice bags for from twenty-four to forty-eight hours, after which soaking the foot or wrist in hot water was recommended. It was noticed that several patients disobeyed, and stated that greater relief was obtained during the first twenty-four to forty-eight hours

In these wartimes, among the many problems confronting us in the practice of industrial medicine and surgery, is the necessity of doing more work in a shorter period of time with less help. The very rapid expansion of some of our industries has placed tremendous demands on the medical profession. Not only has the actual enlistment of trained industrial medical personnel depleted our forces, but one of our sources of available medical personnel is almost completely gone. These are the young physicians and surgeons who have completed their internships and residencies and normally would be part time medical members in industry from two to five years or more. Practically all of these young men are taken into military service immediately on completion of their internships. Those available because they have been rejected from military service are very few, and even then more interesting or lucrative opportunities attract their services.The

The accurate diagnosis of pathologic conditions of the knee joint is at its best a difficult task before operation. X-ray study has been a great aid, but in some cases the pathologic condition cannot be demonstrated. In quite a few cases the clinical history is strongly suggestive of "joint mice," which are not demonstrated on the usual x-ray views. The overlapping shadows of the patella hidden behind the lower portion of the femur and the knee joint ramifications on the anterior surface of the femur are difficult to visualize on postero-anterior views or anteroposterior views with the leg extended. Small or faint shadow-casting foreign bodies, "joint mice" or osteophytes are frequently not visualized. The joint surfaces are not clear, sharp or well separated. - Abstract
Background: Numerous studies have shown that baseline drug resistance patterns may influence the outcome of antiretroviral therapy. Therefore, guidelines recommend drug resistance testing to guide the choice of initial regimen. In addition to optimizing individual patient management, these baseline resistance data enable transmitted drug resistance (TDR) to be surveyed for public health purposes. The SPREAD program systematically collects data to gain insight into TDR occurring in Europe since 2001., Methods: Demographic, clinical, and virological data from 4140 antiretroviral-naive human immunodeficiency virus (HIV)-infected individuals from 26 countries who were newly diagnosed between 2008 and 2010 were analyzed. Evidence of TDR was defined using the WHO list for surveillance of drug resistance mutations. Prevalence of TDR was assessed over time by comparing the results to SPREAD data from 2002 to 2007. Baseline susceptibility to antiretroviral drugs was predicted using the Stanford HIVdb program version 7.0., Results: The overall prevalence of TDR did not change significantly over time and was 8.3% (95% confidence interval, 7.2%-9.5%) in 2008-2010. The most frequent indicators of TDR were nucleoside reverse transcriptase inhibitor (NRTI) mutations (4.5%), followed by nonnucleoside reverse transcriptase inhibitor (NNRTI) mutations (2.9%) and protease inhibitor mutations (2.0%). Baseline mutations were most predictive of reduced susceptibility to initial NNRTI-based regimens: 4.5% and 6.5% of patient isolates were predicted to have resistance to regimens containing efavirenz or rilpivirine, respectively, independent of current NRTI backbones., Conclusions: Although TDR was highest for NRTIs, the impact of baseline drug resistance patterns on susceptibility was largest for NNRTIs. The prevalence of TDR assessed by epidemiological surveys does not clearly indicate to what degree susceptibility to different drug classes is affected., (© The Author 2015. - Abstract
In the absence of egg excretion, laboratory diagnosis of recently acquired schistosomiasis is dependent on serology. 42 of 83 Swedish adventure tourists to sub-Saharan Africa had serum anti-schistosome antibodies indicating recent infection. There is little doubt regarding the specificity and sensitivity of serology for the demonstration of infection, but there is a need for alternative serological methods which could be more widely used than the standard immunofluorescence assay (IFA) for antibodies against gut-derived antigens (anti-GAA). We present results suggesting that 40/42 anti-GAA positive cases also react with keyhole limpet haemocyanin (KLH), a readily available commercial antigen. High anti-GAA titres were seen for more than 2 y despite treatment with praziquantel. Thus we are faced with several questions. How likely is it that positive serology means treatment failure? What is the risk involved in chronic infection? What is the prospect for monitoring treatment outcome by serology? We conclude that there is a need for better information on the risk of becoming infected, for improved methods for testing and for monitoring the therapeutic effects in adventure tourists.
