3. Closing the Gap: Opportunities for Distance Education to Benefit Adult Learners in Higher Education

Abstract

Distance education in higher education is a fast-growing and widespread phenomenon. As many adults are unable to participate in on-campus education, distance education offers flexible learning paths that greatly enhance accessibility to higher education. Exploring distance education's potential to increase the participation of adult learners in higher education is an important objective of education policy in the European Union. Against this background, the UNESCO Institute for Lifelong Learning (UIL), the International Council for Open and Distance Education (ICDE) and StudyPortals (SP) collaborated on a research project entitled "The Impact of Distance Education on Adult Learning" (IDEAL). The IDEAL team has recently published the final report of the research project, "Closing the Gap: Opportunities for Distance Education to Benefit Adult Learners in Higher Education." The report sets out the findings of this research project, which was carried out between October 2013 and November 2015. It examines the interrelations of adult learning, higher education and distance education, and comes up with a set of recommendations for the development and implementation of programmes and national policies related to distance education. The main recommendations to policymakers and distance education providers refer to increasing the openness of higher education to adult learners by removing regulative barriers and developing standard procedures for recognition of prior learning across Europe. Higher education institutions should further facilitate learning for adults by offering more interactive possibilities, practical study guides and offer more, diverse options for financial support. The following are appended: (1) Summary of empirical results from the three independent studies; (2) An Overview of Distance Education; (3) The Distance Education Concept. A glossary of key terms and a bibliography are also included.

Abstract

Pan-STARRS1 has carried out a set of distinct synoptic imaging sky surveys including the $3\pi$ Steradian Survey and the Medium Deep Survey in 5 bands ($grizy_{P1}$). The mean 5$\sigma$ point source limiting sensitivities in the stacked 3$\pi$ Steradian Survey in $grizy_{P1}$ are (23.3, 23.2, 23.1, 22.3, 21.4) respectively. The upper bound on the systematic uncertainty in the photometric calibration across the sky is 7-12 millimag depending on the bandpass. The systematic uncertainty of the astrometric calibration using the Gaia frame comes from a comparison of the results with Gaia: the standard deviation of the mean and median residuals ($ \Delta ra, \Delta dec $) are (2.3, 1.7) milliarcsec, and (3.1, 4.8) milliarcsec respectively. The Pan-STARRS system and the design of the PS1 surveys are described and an overview of the resulting image and catalog data products and their basic characteristics are described together with a summary of important results. The catalog data products are available in the form of an SQL-based relational database from MAST, the Mikulski Archive for Space Telescopes at STScI. 7. Planning, implementation and revision of the longitudinal scientific curriculum at the Medical School Brandenburg

Abstract

Objectives: The aim of this paper is to present the development of a longitudinal curriculum for medical students that is rooted in the particularity of the medical sciences and that aims to build and strengthen medical students' scientific skills and use thereof in clinical practice. Methods: The curriculum development was initiated based on students' feedback on the initial curriculum. To improve and expand the curriculum appropriately, a needs assessment, a literature review to define science specific to the medical sciences and practice, and an analysis of national and international curricula were performed. The curriculum development followed the PDCA cycle (Plan-Do-Check-Act).Results: The curriculum extends across the entire medical study programme from semesters 1 to 10. It consists of the seminar series on basic conduct and the epistemological groundings of science, scientific methods in medical research and health sciences, statistics and the scientific internship. Up to the sixth semester, the focus is on the acquisition of skills and abilities to work on and carry out a concrete research project; starting in semester seven, the critical evaluation and application of research results in everyday clinical practice are introduced. The curriculum is taught by epidemiologists, anthropologists, statisticians and public health scholars. Starting in semester seven, seminars are generally taught together with clinicians as tandem teaching. The curriculum is regularly assessed and adjusted. Conclusions: The Brandenburg Scientific Curriculum can be seen as a model of a longitudinal curriculum to teach scientific thinking and acting. One that is at the same time highly integrated in the medical curriculum overall. A central coordination point seems to be necessary to coordinate the teaching content and to ensure that teachers are interconnected. Um das Curriculum entsprechend zu verbessern und zu erweitern, wurden eine Bedarfsanalyse, eine Literaturrecherche zur Definition von medizinischer Wissenschaft und Praxis sowie eine Analyse nationaler und internationaler Curricula durchgeführt. 34. A systematic mapping review identifying key features of restraint research in inpatient pediatric psychiatry: A human rights perspective.

Abstract

INTRODUCTION: Restraints, a highly regulated and contentious measure in pediatric psychiatry, have significant negative impacts on children. The application of international human rights standards, such as the Convention of the Rights of the Child (CRC) and the Convention of the Rights of Persons with Disabilities (CRPD), has spurred global efforts to reduce or eliminate the use of restraints. However, a lack of consensus on definitions and terminology, as well as quality indicators in this field, hinders the ability to compare studies and evaluate interventions consistently. AIM: To systematically map existing literature on restraints imposed upon children in inpatient pediatric psychiatry against a human rights perspective. Specifically, to identify and clarify gaps in literature in terms of publication trends, research approaches, study contexts, study participants, definitions and concepts being used, and legal aspects. These aspects are central to assess whether published research is contributing to achieve the CRPD and the CRC in terms of interpersonal, contextual, operational, and legal requirements of restraints. METHODS: A systematic mapping review based on PRISMA guidelines was conducted, adopting a descriptive-configurative approach to map the distribution of available research and gaps in the literature about restraints in inpatient pediatric psychiatry. Six databases were searched for literature reviews and empirical studies of all study designs published between each database's inception and March 24, 2021, manually updated on November 25, 2022. RESULTS: The search yielded 114 English-language publications, with a majority (76%) comprising quantitative studies that relied primarily on institutional records. Contextual information about the research setting was provided in less than half of the studies, and there was an unequal representation of the three main stakeholder groups: patients, family, and professionals. 36. Epidemiologie und operative Behandlung des Pankreaskarzinoms im Flächenland Brandenburg

Abstract

Hintergrund: Das Pankreaskarzinom ist die zweithäufigste Todesursache aller Krebserkrankungen in Deutschland mit mehr als 19.000 Todesfällen pro Jahr. Die deutschlandweit eingeführten klinischen Krebsregister sollen mit ihren Auswertungen die Versorgungsrealität darstellen und die Qualität der Versorgung durch gezielte Analysen zukünftig verbessern. Methode: Die Daten des Klinischen Krebsregisters Brandenburg-Berlin wurden für die Diagnosejahre 2001 bis 2017 hinsichtlich der Versorgung von Pankreaskarzinomen analysiert. Dabei wurden Daten von Patienten mit Wohnort im Land Brandenburg in Bezug auf epidemiologische und therapeutische Parameter ausgewertet. Ergebnisse: Insgesamt 5418 Patienten mit Pankreaskarzinom wurden im Register von 2001 bis 2017 dokumentiert. 49,6 % der Patienten wurden im Stadium UICC IV diagnostiziert. Eine Pankreasresektion erfolgte in 26,4 % der Fälle. Bei Pankreaskopfkarzinomen war die pyloruserhaltende Resektion mit 51,8 % die häufigste Prozedur. In 9,4 % erfolgte eine Pankreatektomie bei Pankreaskopfkarzinom. Die R0-Resektionsrate bei allen Pankreaskarzinomen lag in der Zeit von 2014 bis 2017 bei 61,9 %. Nach R0-Resektion lag das 5‑Jahres-Überleben bei 19 %. Relevante multivariate Überlebensfaktoren waren Alter, UICC-Stadium und R‑Klassifikation. Ein Einfluss der Fallzahl pro Klinik auf das absolute Überleben der im Bundesland Brandenburg operierten Patienten bestand nicht. Schlussfolgerung: Die Versorgungsrealität im Bundesland Brandenburg bei Patienten mit Pankreaskarzinom entspricht den Ergebnissen internationaler Publikationen bezogen auf die untersuchten Kennzahlen. Eine qualitativ international vergleichbare Versorgung dieser Patienten ist auch in Nonmetropolenregionen möglich.

Contains fulltext : 249015.pdf (Publisher’s version ) (Open Access) OBJECTIVE: The aim of this study was to analyse accredited study programmes of health services research in Germany at Master's degree level regarding their structural data and content. METHODS: Using a descriptive qualitative design, all included plans of study courses, module handbooks and descriptions of the Master's degree programmes were analysed. RESULTS: The Master's degree programmes were similar in their structural elements as well as in their content of education. They aim at enhancing competences related to the areas of health services research to identify health care needs and to develop, initiate, evaluate and critically assess innovative care strategies across disciplines. DISCUSSION: Although the content is similar, the study programmes focus on specific aspects that allow students to choose a certain programme depending on their preferences. A next step can be the development of a core curriculum that takes into consideration further findings and the discourse of different stakeholders.

Abstract

BACKGROUND: Increasing evidence suggests that participation proportions in longitudinal health research vary according to sex/gender, age, social class, or migration status. Intersectionality scholarship purports that such social categories cannot be understood in isolation and makes visible the co-dependent nature of the social determinants of health and illness. This paper uses an intersectionality-informed approach in order to expand the understanding of why people participate in health research, and the impact of intersecting social structures and experiences on these attitudes. METHODS: A sample of 80 respondents who had previously either accepted or declined an invitation to participate in the German National Cohort (NAKO) participated in our interview study. Interviews were semi-structured and contained both narrative elements and more structured probes. Data analysis proceeded in two steps: first, the entire data set was analysed thematically (separately for participants and non-participants); second, key themes were compared across self-reported sex/gender, age group and migration status to identify differences and commonalities. RESULTS: Respondents' attitudes towards study participation can be categorised into four themes: wanting to make a contribution, seeking personalised health information, excitement and feeling chosen, and seeking social recognition. Besides citing logistical challenges, non-participants narrated adverse experiences with or attitudes towards science and the healthcare system that deterred them from participating. A range of social experiences and cultural value systems shaped such attitudes; in particular, this includes the cultural authority of science as an arbiter of social questions, transgressing social categories and experiences of marginalisation. Care responsibilities, predominantly borne by female respondents, also impacted upon the decision to take part in NAKO. DISCUSSION: Our findings suggest that for participants, health research constitutes a site of distinction in the sense of making a difference and being distinct or distinguishable, whereas non-participants inhabited an orientation towards science that reflected their subjective marginalisation through science. No clear relationship can thereby be presumed between social location and a particular attitude towards study participation; rather, such attitudes transgress and challenge categorical boundaries. This challenges the understanding of particular populations as more or less disadvantaged, or as more or less inclined to participate in health research.

The corporate culture of both merger partners has an important influence on the integration.Purpose: The aim of this project was to transfer the concept of corporate culture in mergers to the merging of two medical systems.Methods: A two-step approach (literature analyses and expert consensus procedure) was used to develop practical guidance for the development of a cultural basis for integrative medicine, based on the framework of corporate culture in “mergers,” which could be used to build an integrative medicine department or integrative medicine service.Results: Results include recommendations for general strategic dimensions (definition of the medical model, motivation for integration, clarification of the available resources, development of the integration team, and development of a communication strategy), and recommendations to overcome cultural differences (the clinic environment, the professional language, the professional image, and the implementation of evidence-based medicine).Conclusion: The framework of mergers in corporate culture provides an understanding of the difficulties involved in integrative medicine projects. Abstract

Background: Coronavirus disease 2019 (COVID-19) was declared a pandemic by the WHO on 11 March 2020 and global surgical practice was compromised. This Commission aimed to document and reflect on the changes seen in the surgical environment during the pandemic, by reviewing colleagues experiences and published evidence. Methods: In late 2020, BJS contacted colleagues across the global surgical community and asked them to describe how severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) had affected their practice. In addition to this, the Commission undertook a literature review on the impact of COVID-19 on surgery and perioperative care. A thematic analysis was performed to identify the issues most frequently encountered by the correspondents, as well as the solutions and ideas suggested to address them. Results: BJS received communications for this Commission from leading clinicians and academics across a variety of surgical specialties in every inhabited continent. The responses from all over the world provided insights into multiple facets of surgical practice from a governmental level to individual clinical practice and training. Conclusion: The COVID-19 pandemic has uncovered a variety of problems in healthcare systems, including negative impacts on surgical practice. Global surgical multidisciplinary teams are working collaboratively to address research questions about the future of surgery in the post-COVID-19 era. The COVID-19 pandemic is severely damaging surgical training. The establishment of a multidisciplinary ethics committee should be encouraged at all surgical oncology centres. Innovative leadership and collaboration is vital in the post-COVID-19 era.

The goals of this study were 1) to understand the results of a randomized clinical trial – Qigong and Exercise Therapy for Elderly Patients with Chronic Neck Pain (QIBANE) – that showed no difference between qigong, exercise therapy, and no-treatment on quality of life, and 2) to understand how elderly individuals with chronic pain experience interventions of qigong and exercise therapy. A qualitative interview study was conducted with 20 QIBANE participants. Interviews asked about motivation for and expectations of trial participation, experiences with the exercise classes (qigong or exercise therapy), and changes in pain experience. Interviews were transcribed, entered into the software program ATLAS.ti, and coded thematically by two coders. Content analysis was performed. All interviewees reflected positively on their QIBANE experience and described their participation in QIBANE as helpful. However, what was discussed in both groups when they talked about “positive experiences” in the study differed between the two groups. For example, themes that emerged in the exercise-therapy group related to difficulties associated with aging and staying physically active. In the interviews with qigong group members, emergent themes related to qigong as a method that improved bodily experiences and influenced daily activities. The effects that exercise therapy and qigong have on an elderly population cannot be captured by health-related quality-of-life measurements, such as the Short Form (36) Health Survey. Broader concepts of quality of life that include the concepts of self-efficacy and positive affect may be more appropriate. The results presented in this study suggest that for this population group, the approach of patient-centered outcomes is especially pertinent in order to design meaningful intervention studies in the elderly. This means that research questions, interventions, and outcome measurements need to take into account the special situation of elderly people.
