Abstract

Epilepsy is the fourth most common neurological disorder and affects people of all ages worldwide. Deep Brain Stimulation (DBS) has emerged as an alternative treatment option when anti-epileptic drugs or resective surgery cannot lead to satisfactory outcomes. To facilitate the planning of the procedure and for its standardization, it is desirable to develop an algorithm to automatically localize the DBS stimulation target, i.e., Anterior Nucleus of Thalamus (ANT), which is a challenging target to plan. In this work, we perform an extensive comparative study by benchmarking various localization methods for ANT-DBS. Specifically, the methods involved in this study include traditional registration method and deep-learning-based methods including heatmap matching and differentiable spatial to numerical transform (DSNT). Our experimental results show that the deep-learning (DL)-based localization methods that are trained with pseudo labels can achieve a performance that is comparable to the inter-rater and intra-rater variability and that they are orders of magnitude faster than traditional methods., Comment: Accepted by SPIE Medical Imaging 2022

Abstract

ObjectivesOur objective was to compare long-term outcomes of deep brain stimulation (DBS) of the globus pallidus interna (GPi) and subthalamic nucleus (STN) for patients with Parkinson disease (PD) in a multicenter randomized controlled trial.MethodsPatients randomly assigned to GPi (n = 89) or STN DBS (n = 70) were followed for 36 months. The primary outcome was motor function on stimulation/off medication using the Unified Parkinson's Disease Rating Scale motor subscale. Secondary outcomes included quality of life and neurocognitive function.ResultsMotor function improved between baseline and 36 months for GPi (41.1 to 27.1; 95% confidence interval [CI] -16.4 to -10.8; p < 0.001) and STN (42.5 to 29.7; 95% CI -15.8 to -9.4; p < 0.001); improvements were similar between targets and stable over time (p = 0.59). Health-related quality of life improved at 6 months on all subscales (all p values significant), but improvement diminished over time. Mattis Dementia Rating Scale scores declined faster for STN than GPi patients (p = 0.01); other neurocognitive measures showed gradual decline overall.ConclusionsThe beneficial effect of DBS on motor function was stable and comparable by target over 36 months. Slight declines in quality of life following initial gains and gradual decline in neurocognitive function likely reflect underlying disease progression and highlight the importance of nonmotor symptoms in determining quality of life.Classification of evidenceThis study provides Class III evidence that improvement of motor symptoms of PD by DBS remains stable over 3 years and does not differ by surgical target. 14. The effects of exercise training on cardiac and peripheral function in men and women

Abstract

Aerobic power (VOzma) and cardiac output decrease in both sexes with age. Endurance exercise is known to affect cardiac structures and function, and could therefore attenuate the effects of ageing. However, recent studies have suggested that men and women of similar ages adapt differently to exercise training, including adaptations in cardiac function. In younger men and women, training modality is also an important determinant of improvements in cardiac function, but the full effects of exercise training need to be determined both centrally and peripherally. Cardiac power output (CPO) incorporates measurements of both blood flow (Q) and mean blood pressure (MAP), and is the most comprehensive .method of measuring overall cardiac function. In preliminary studies we elucidated the reliability and reproducibility of the C02 rebreathing technique used to determine Q, and the potential effects of caffeine ingestion on CPO. Then older men and women participated in 30 weeks of training, with step-wise increments in exercise intensity. This programme increased aerobic p~wer and increased the extraction of oxygen in the peripheries, but with no discernable effects on the heart's maximum pumping or reserve capacities. Six week endurance training (interval and continuous) of young men produced similar results. However using proteomics, interval training induced greater expressions of some contractile proteins, creatine kinase-M and heat shock protein 70 kDa, in the vastus lateralis muscle, suggesting possible conversion towards a faster muscle phenotype, but only in men. We conclude that endurance training with exercise intensities < 75 % HRR induces increases in VOZmax and peripheral adaptations in older people, but intensities >75 % HRR are needed to induce changes in cardiac function. We also found no discemable cardiovascular sex-specific differences in either young or older people after either interval or continuous exercise training. However, skeletal muscle exhibited contractile and metabolic adaptations to these training regimes, and these were sex-specific.

24. Hybrid Transcranial Magnetic Stimulation and Deep Brain Stimulation in the Presence of an Implantable Pulse Generator

Abstract

Transcranial magnetic stimulation (TMS) has been shown to improve speech impairment in Parkinson’s disease (PD) patients. Most PD patients undergo deep brain stimulation (DBS) implantation to improve their motor function prior to the development of speech impairment. Therefore, the potential for TMS treatment must take into account the DBS probes’ high conductivity, potentially causing excess stimulation. Prior research regarding this phenomenon is highly conflicting and needs investigation. This study reports electromagnetic interference using a highly accurate finite element analysis approach to determine the electric field distribution in patients with DBS probes undergoing TMS therapy. Additionally, we tested various coil orientations and coil models to fully examine the extent that TMS affects DBS. We considered accurate DBS geometry by creating high-resolution models of Medtronic 3389 DBS, including an implantable pulse generator (IPG) and multiple loops at the exit point of the cranium, which previous studies lack. By studying the induced current in the DBS leads, we determined that the combination of TMS and DBS varies in safety relative to its coil orientation and coil type. Specifically, the quadruple butterfly coil (QBC) targeted at the lateral motor cortex did not induce unsafe levels of currents in the DBS leads even at 100% of the maximum stimulator output.

Abstract

With an aging population and increasing utilization of both cochlear implants (CI) to treat hearing loss and deep brain stimulation (DBS) to treat neurodegenerative movement disorders, more patients will be sequentially implanted with the two devices. As such, understanding both presurgical evaluations and surgical procedures will be of great importance to the treating cochlear implant surgeon and neurosurgeon in optimizing outcomes for both conditions. Two illustrative cases of sequential CI followed by DBS are reviewed. Relevant pre-, intra-, and postoperative issues are discussed for both DBS following CI surgery and CI following DBS surgery. Despite the potential for challenges, sequential CI and DBS (and vice versa) can provide substantial benefit to those individuals with severe hearing loss and movement disorders. Abstract

BackgroundMagnetic resonance-guided laser interstitial thermal therapy (MRgLITT) is a minimally invasive alternative to surgical resection for drug-resistant mesial temporal lobe epilepsy (mTLE). Reported rates of seizure freedom are variable and long-term durability is largely unproven. Anterior temporal lobectomy (ATL) remains an option for patients with MRgLITT treatment failure. However, the safety and efficacy of this staged strategy is unknown.MethodsThis multicentre, retrospective cohort study included 268 patients consecutively treated with mesial temporal MRgLITT at 11 centres between 2012 and 2018. Seizure outcomes and complications of MRgLITT and any subsequent surgery are reported. Predictive value of preoperative variables for seizure outcome was assessed.ResultsEngel I seizure freedom was achieved in 55.8% (149/267) at 1 year, 52.5% (126/240) at 2 years and 49.3% (132/268) at the last follow-up ≥1 year (median 47 months). Engel I or II outcomes were achieved in 74.2% (198/267) at 1 year, 75.0% (180/240) at 2 years and 66.0% (177/268) at the last follow-up. Preoperative focal to bilateral tonic-clonic seizures were independently associated with seizure recurrence. Among patients with seizure recurrence, 14/21 (66.7%) became seizure-free after subsequent ATL and 5/10 (50%) after repeat MRgLITT at last follow-up≥1 year.ConclusionsMRgLITT is a viable treatment with durable outcomes for patients with drug-resistant mTLE evaluated at a comprehensive epilepsy centre. Although seizure freedom rates were lower than reported with ATL, this series represents the early experience of each centre and a heterogeneous cohort. ATL remains a safe and effective treatment for well-selected patients who fail MRgLITT.
