Abstract

Background On festive days worldwide eyes are severely injured by fireworks. The data on the number and severity are to date not registered in Germany. Objective How frequent are firework-induced injuries in Germany, who are the affected, how serious are the injuries, which forms of treatment are necessary and how frequent are accompanying injuries? Method A German nationwide online-based survey was carried out in all inpatient eye departments and data over the last 3 consecutive years were descriptively analyzed. Results From New Years Eve 2016/2017, when 41 eye departments sent in data, the participation could be increased to 51 eye departments in 2018/2019. More than one third (33-39%) of all 1356 patients over 3 years were minors, 60% were younger than 25 years old and roughly 60% of patients were injured as bystanders or in an unclear situation. In total 25% of all eye injuries were considered severe and required inpatient treatment. Accompanying injuries of the other eye, the face and hands were more frequent in minors than in adults. Eyeball ruptures were reported in 10 minors and 38 adults over the 3 years. Conclusion Particularly minors and bystanders need better protection.

Abstract

Objectives: To characterize MDR Escherichia coli from bloodstream infections (BSIs) in Australia, New Zealand and Singapore. Method(s): We collected third-generation cephalosporin-resistant (3GC-R) E. coli from blood cultures in patients enrolled in a randomized controlled trial fromFebruary 2014 to August 2015. WGS was used to characterize antibiotic resistance genes, MLST, plasmids and phylogenetic relationships. Antibiotic susceptibility was determined using disc diffusion and Etest. Result(s): A total of 70 3GC-R E. coli were included, of which the majority were ST131 (61.4%). BSI was most frequently from a urinary source (69.6%), community associated (62.9%) and in older patients (median age 71 years). The median Pitt score was 1 and ICU admission was infrequent (3.1%). ST131 possessed more acquired resistance genes than non-ST131 (P=0.003). Clade C1/C2 ST131 predominated (30.2% and 53.5% of ST131, respectively) and these were all ciprofloxacin resistant. All clade A ST131 (n"6) were community associated. The predominant ESBL types were blaCTX-M (80.0%) and were strongly associated with ST131 (95% carried blaCTX-M), with the majority blaCTX-M-15. Clade C1 was associated with blaCTX-M-14 and blaCTX-M-27, whereas blaCTX-M-15 predominated in clade C2. Plasmid-mediated AmpC genes (mainly blaCMY-2) were frequent (17.1%) but were more common in non-ST131 (P<0.001) isolates from Singapore and Brisbane. Two strains carried both blaCMY-2 and blaCTX-M. The majority of plasmid replicon types were IncF. Conclusion(s): In a prospective collection of 3GC-R E. coli causing BSI, community-associated Clade C1/C2 ST131 predominate in associationwith blaCTX-M ESBLs, although a significant proportion of non-ST131 strains carried blaCMY-2.Copyright © The Author 2017. Abstract

IMPORTANCE Extended-spectrum beta-lactamases mediate resistance to third-generation cephalosporins (eg, ceftriaxone) in Escherichia coli and Klebsiella pneumoniae. Significant infections caused by these strains are usually treated with carbapenems, potentially selecting for carbapenem resistance. Piperacillin-tazobactam may be an effective "carbapenem-sparing" option to treat extended-spectrum beta-lactamase producers. OBJECTIVES To determine whether definitive therapy with piperacillin-tazobactam is noninferior to meropenem (a carbapenem) in patients with bloodstream infection caused by ceftriaxone-nonsusceptible E coli or K pneumoniae. DESIGN, SETTING, AND PARTICIPANTS Noninferiority, parallel group, randomized clinical trial included hospitalized patients enrolled from 26 sites in 9 countries from February 2014 to July 2017. Adult patients were eligible if they had at least 1 positive blood culture with E coli or Klebsiella spp testing nonsusceptible to ceftriaxone but susceptible to piperacillin-tazobactam. Of 1646 patients screened, 391 were included in the study. INTERVENTIONS Patients were randomly assigned 1:1 to intravenous piperacillin-tazobactam, 4.5 g, every 6 hours (n = 188 participants) or meropenem, 1 g, every 8 hours (n = 191 participants) for a minimum of 4 days, up to a maximum of 14 days, with the total duration determined by the treating clinician. MAIN OUTCOMES AND MEASURES The primary outcome was all-cause mortality at 30 days after randomization. A noninferiority margin of 5% was used. RESULTS Among 379 patients (mean age, 66.5 years; 47.8% women) who were randomized appropriately, received at least 1 dose of study drug, and were included in the primary analysis population, 378 (99.7%) completed the trial and were assessed for the primary outcome. 3,6-Bis(2H-tetrazol-5-yl)-1,2,4,5-tetrazine (3) (BTT) is an energy-rich compound and a strong dibasic acid. The bifunctional compound 3 is capable of [4+2] cycloadditions through its tetrazine unit as a 4 pi compound, and of acylating ring-opening reactions through its tetrazole rings, leading to the formation of linear oligoheterocycles 7, 12 with 1,2,4,5-tetrazine, pyridazine, 1,3,4-oxadiazole, thiophene, furan, and pyrrole units in sequences not easily available by other synthetic methods.

25. Far-infrared absorption of the superionic conductor Li3N. temperature and isotope effects

Abstract

The far-infrared absorption spectrum (20–250cm−1) of the superionic conductor Li3N —with6Li- and7Li-isotopes in the starting material—has been measured for different polarizations and temperatures (4–150 K). TheE ⊥c spectrum yields two lines near 80 and 140 cm−1, which are not observed forE ⊥c. From the isotopic frequency ratio (ω6/ω7≃1.07) and from the comparison to lattice dynamics it is concluded that these lines are due to resonant modes, weakly coupled to the lattice. Measurements of the isotope effects on the dielectric properties (10 Hz to 1 MHz) establish that the local diffusive type motion in shallow potentials and the low lying resonant modes originate from the same defect system (including Li-ions in non-regular positions).

31. [Monocentric, Retrospective, Comparative Study of the Safety and Effectiveness of Wound Modulation with Bevacizumab after Combined Cataract Surgery with Implantation of the CyPass Stent].

Abstract

Background: Because of the known positive effect of bevacizumab (Avastin ® ) on wound modulation in trabeculectomy, a retrospective study evaluated the safety and efficacy of wound modulation with Avastin following combined cataract surgery with CyPass stent., Method: Retrospectively, the pre-, intra- and postoperative data (after three and six months) were collected from 484 eyes after combined surgery (n = 187 without Avastin, n = 297 with Avastin). The safety criteria were intraoperative complications and postoperative slit-lamp microscopic findings. Efficacy criteria were intraocular pressure (IOP) and number of IOP-lowering drugs., Results: With respect to age and sex distribution, early postoperative fibrin reaction, intraocular pressure and reduction in medication, the two groups (with and without Avastin) did not differ statistically significantly. However, iris tissue reactions were statistically significantly more frequently observed three months postoperatively in the control group than in the Avastin group (chi square test: p = 0.02, n = 47/128); there is a slight trend after six months towards less iris tissue reaction in the Avastin group (chi square test: p = 0.15, n = 45/125). Other trends in favour of the Avastin group were identified within the failure rate (2% compared to 6% at three months (chi square test: p = 0.103, n = 50/130) and 4 to 10% at six months (chi square test: p = 0.106, n = 50/130) and within the proportion of eyes without supplemental hypotensive medication, with 86 vs. 74% and 68 vs. 56% (chi square test: three months: p = 0.053; six months: p = 0.12; n = 50/130). Abstract

Background: Results of a retrospective study of perioperative changes in intraocular pressure (IOP) following radical prostatectomy (RPE) for prostate cancer., Patients and Methods: In 25 consecutive patients, PSA (prostate specific antigen) and IOP were measured with Goldmann tonometry before and after a mean of 10 days following RPE. The data of three patients with proven glaucoma were evaluated separately. The data of 22 patients without glaucoma were analysed with non-parametric statistical methods. The primary objective of the investigation was the statistical analysis of the changes in the IOP of the right eyes. The results for the left eyes are presented as secondary objective., Results: The mean IOP in the right eye decreased from 13.95 ± 2.95 mmHg (n = 22) preoperatively to 12.18 ± 1.99 mmHg postoperatively; the average reduction in pressure was 1.77 ± 2.44 mmHg. In the left eyes, the IOP of 14.45 ± 3.16 mmHg preoperatively decreased to 12.05 ± 2.21 mmHg postoperatively. The average reduction in pressure was 2.41 ± 3.53 mmHg. The PSA level decreased from 18.70 ± 21.66 ng/ml preoperatively (median: 11.3 ng/ml) to 2.21 ± 3.13 ng/ml postoperatively (median: 1.25 ng/ml). The decline in IOP and the difference in PSA are highly statistically significant (Wilcoxon test for paired samples, n = 22, p < 0.01). There was no linear relationship between the reductions in IOP and PSA (Pearson correlation coefficient: r = 0.137, p = 0.542 for right eyes, r = 0.062, p = 0.784 for left eyes)., Conclusion: Our case series shows that there is a statistically significant decrease in IOP following RPE, and to a clinically relevant extent. The changes in PSA and IOP might be linked by changes in prostaglandin metabolism in the prostate and/or the seminal vesicles. This theory remains to be confirmed by further studies. The increase in IOP during the development of prostate cancer might also be linked to changes in prostaglandin metabolism and composition. Abstract

Medical History: In February 2007, a 13-year old boy presented with a livid tumour in the lower conjunctival fornix of the left eye. OPHTHALMOLOGICAL FINDINGS: The tumor was salmon-coloured, bulging and elastic and filled the whole lower conjunctival fornix of the left eye. There was no other pathological finding in the left eye. Uncorrected visual acuity was 20/20. Intraocular pressure was 12 mmHg. The eye was fully motile., Treatment: Incisional biopsy was performed in February 2007. The tumor was histologically an extranodal MALT lymphoma. DNA testing for Chlamydophila trachomatis and Chlamydophila pneumonia was negative. Systemic treatment was started with doxycycline (200 mg daily). After six weeks, the tumour was slightly smaller. Azithromycin 500 mg once a week was added. 18 months after initiation of the treatment, the tumour had completely regressed. A second sample taking in the former tumor area showed tumor-free conjunctiva and subconjunctival tissue. As a precaution, the combined antibiotic therapy was continued for 10 months and the patient was followed for five more years. The lymphoma did not relapse in the conjunctiva and orbit or in the whole body., Conclusion: We showed that extranodal MALT lymphomas of the conjunctiva can be successfully treated with antibiotics alone. At the start of therapy, the child was 13 years old. To our knowledge, this patient is the first child in Germany and one of the first in the world with ocular adnexal lymphoma who could be successfully treated with combined antibiotic therapy and who could be followed up for 5 years without relapse. Thus, we could avoid radiotherapy or chemotherapy in childhood and eliminate the risk of a therapy-induced secondary malignancy., (Georg Thieme Verlag KG Stuttgart · New York.)

Abstract

A case of a false-negative first-trimester diagnosis following chorionic villus sampling is reported that ended with the birth of a child with Down syndrome. Chromosome analysis of 30 metaphases from 24 h-cultured chorionic villi obtained in week 12 of gestation showed a normal chromosome constitution. However, the newborn showed manifestations of Down syndrome, and 99 of 100 metaphases analysed from cultured lymphocytes showed 47, XY, + 21. The remaining metaphase was normal (46, XY).
