Objective: To summarize current evidence regarding testosterone treatment for women with low sexual desire. Materials and methods: The Female Endocrinology and Andrology Department of the Brazilian Society of Endocrinology and Metabolism invited nine experts to review the physiology of testosterone secretion and the use, misuse, and side effects of exogenous testosterone therapy in women, based on the available literature and guidelines and statements from international societies. Results: Low sexual desire is a common complaint in clinical practice, especially in postmenopausal women, and may negatively interfere with quality of life. Testosterone seems to exert a positive effect on sexual desire in women with sexual dysfunction, despite a small magnitude of effect, a lack of long-term safety data, and insufficient evidence to make a broad recommendation for testosterone therapy. Furthermore, there are currently no testosterone formulations approved for women by the relevant regulatory agencies in the United States, Brazil, and most other countries, and testosterone formulations approved for men are not recommended for use by women. Conclusion: Therefore, testosterone therapy might be considered if other strategies fail, but the risks and benefits must be discussed with the patient before prescription.

A previously healthy 24 yo male presented with a two-month history of epigastric pain, nausea, vomiting, fatigue and malaise. He reported abuse of different substances, including an injectable veterinary vitamin compound, which contains high doses of vitamin A, D and E, and an oily vehicle that induces local edema and enhances muscle volume. Serum creatinine was 3.1 mg/dL, alanine transaminase 160 mg/dL, aspartate transaminase 11 mg/dL, total testosterone 23 ng/dL, 25-OH-vitamin D >150 ng/mL (toxicity >100), 1,25-OH-vitamin D 80 pg/mL, vitamin A 0.7 mg/dL, parathormone 150 ng/mL (toxicidade > 100), 1,25-OH-vitamina D 80 pg/mL, vitamina A 0,7 mg/dL, paratormônio < 3 pg/mL, cálcio total 13,6 mg/dL, cálcio urinário de 24h 635 mg/24h (VR 42-353). Uma ultrassonografia do trato urinário demonstrou sinais de nefropatia parenquimatosa. O diagnóstico foi hipercalcemia e insuficiência renal aguda secundária a intoxicação por vitamina D. Ele foi tratado inicialmente com hidratação intravenosa, furosemida e prednisona. No quinto dia de hospitalização uma dose de pamidronato dissódico foi adicionada. O paciente evoluiu com normalização do cálcio sérico e da função renal. Trinta dias depois ele apresentou testes clínicos e laboratoriais normais, exceto a 25-OH-vitamina D que estava persistentemente elevada (107 ng/mL), já que ela pode demorar vários meses para normalizar. Este relato de caso é um alerta aos riscos relacionados ao uso de substâncias veterinárias para fins estéticos.

Hirsutism is defined as excessive terminal hair growth in androgen-dependent areas of the body in women, which grows in a typical male distribution pattern. Hirsutism is a common clinical problem in women, and the treatment depends on the cause. The condition is often associated with a loss of self-esteem. Hirsutism reflects the interaction between circulating androgen concentrations, local androgen concentrations, and the sensitivity of the hair follicle to androgens. Polycystic ovary syndrome and idiopathic hirsutism are the most common causes of the condition. A woman’s history and, physical examination are particularly important in evaluating excess hair growth. The vast majority of women with hirsutism have the idiopathic variety, and the diagnosis is made by exclusion. Serum testosterone level > 200 ng/dL is highly suggestive of adrenal or ovarian tumor. Treatment of hirsutism should be based on the degree of excess hair growth presented by the patient and in the pathophysiology of the disorder. Treatment includes lifestyle therapies, androgen suppression, peripheral androgen blockage, and cosmetic treatments. The current review discusses definition, pathogenesis, physiopathology, differential diagnosis, diagnostic strategies, and treatment. O hirsutismo é definido como o excesso de crescimento terminal de pelos em áreas dependentes de andrógenos no corpo de mulheres, com crescimento em um padrão de distribuição tipicamente masculino. O hirsutismo é um problema clínico comum em mulheres, e o tratamento depende da causa. A condição está geralmente associada com a perda de autoestima. O hirsutismo reflete a interação entre as concentrações de andrógenos circulantes, concentrações locais de andrógenos e a sensibilidade do folículo capilar aos androgênios. A síndrome dos ovários policísticos e o hirsutismo idiopático são as causas mais comuns do transtorno. O histórico e o exame físico são particularmente importantes na avaliação do excesso de crescimento de pelos. A maioria das mulheres com hirsutismo possui a variedade idiopática, e o diagnóstico é feito por exclusão. Uma concentração sérica de testosterona > 200 ng/dL é altamente sugestiva de tumor ovariano ou de adrenal. O tratamento do hirsutismo deve ser baseado no nível de excesso de crescimento dos pelos e a fisiopatologia do transtorno. O tratamento inclui alterações no estilo de vida, supressão de andrógenos, bloqueio periférico de andrógenos e tratamentos cosméticos. A presente revisão discute definição, patogênese, fisiopatologia, diagnóstico diferencial e estratégias de diagnóstico e tratamento.

Introdução: O traumatismo crânio-encefálico (TCE) é a principal causa de morte e incapacidade em pessoas jovens, principalmente do sexo masculino. Estudos experimentais sugerem que os níveis de diferentes tipos de hormônios possam influenciar o prognóstico de lesões do Sistema Nervoso Central devido ao TCE. A aplicabilidade clínica das dosagens hormonais como biomarcadores do prognóstico do TCE é um ponto controverso na literatura.Objetivos: Determinar a existência ou não de associação entre os níveis plasmáticos de hormônios hipofisários e periféricos e a mortalidade hospitalar de homens vítimas de TCE grave. Avaliar os níveis plasmáticos de testosterona de homens na fase crônica após o TCE grave e sua associação com o desempenho cognitivo, sintomas psiquiátricos e qualidade de vida destes pacientes.Métodos: No estudo de fase aguda, analisamos através de regressão logística binária múltipla a associação independente entre a mortalidade e as variáveis demográficas, clínicas, radiológicas e neurocirúrgicas e medidas plasmáticas de hormônio luteinizante (LH), hormônio folículo-estimulante (FSH), hormônio de crescimento (GH), hormônio estimulante da tiroide (TSH), testosterona total, testosterona livre calculada, tiroxina (T4) livre e fator de crescimento semelhante à insulina tipo 1 (IGF-1) feitas em uma mediana de 10 e 30 horas após o traumatismo de 60 homens vítimas de TCE grave. Uma medida adicional dos níveis de hormônios do eixo gonadotrófico foi realizada em amostras coletadas 70 horas após o TCE. Desequilíbrios na distribuição de variáveis demográficas, neurocirúrgicas, clínicas e radiológicas foram corrigidos por meio da análise de regressão logística binária múltipla. No estudo de fase crônica avaliamos 20 homens com história de TCE grave, acompanhados ambulatorialmente e investigamos: 1) os níveis plasmáticos de gonadotrofinas 1 ano após a hospitalização; 2) a associação entre os níveis plasmáticos de testosterona total destes pacientes e seu desempenho cognitivo (linguagem, memória, atenção, concentração, controle mental, raciocínio abstrato, capacidade visuo-espacial e motora), sintomas psiquiátricos (depressão, ansiedade e apatia), e de qualidade de vida através de regressão linear múltipla.Resultados: A média de idade dos pacientes foi de 35 anos e a mortalidade hospitalar foi de 33%. Após a regressão logística múltipla binária, o odds ratio (OR) ajustado para óbito foi 4 vezes maior nos8pacientes com escore na Escala de Coma de Glasgow (ECG) de admissão menor que 5 (OR ajustado = 3,9, CI 95% 1,2 ? 12,0, p = 0,02) em comparação aos escores superiores. Os pacientes com pupilas anormais na admissão apresentaram mortalidade 4,5 vezes maior do que pacientes com pupilas normais (OR = 4,5, CI 95% 1,4 ? 14,5, p = 0,01). A incidência de pelo menos uma anormalidade hormonal foi observada em 3,6 a 73,1% dos pacientes. A testosterona total e a testosterona livre calculada diminuíram significativamente 10, 30 e 70 horas após o trauma em homens adultos vítimas de TCE grave, mas não foram associadas com a mortalidade hospitalar dos pacientes. A regressão logística múltipla mostrou uma tendência de associação entre os níveis elevados ou normais de LH e a mortalidade nas amostras de 10 horas (OR 3,7, CI 95% 0,8 ? 16,3, p = 0,08) e 30 horas (OR 3,9, CI 95% 0,9 ? 16,7, p = 0,06). Setenta horas após o TCE grave, níveis elevados ou normais de LH apresentaram uma associação independente com a mortalidade (OR 8,4, CI 95% 1,7 ? 41,2, p = 0,008). Pupilas anormais e escore baixo na ECG na admissão também estiveram independentemente associados com a mortalidade em ambiente hospitalar. Hipogonadismo foi diagnosticado em 20% dos pacientes avaliados na fase crônica. Não houve associação entre os níveis de testosterona total e sintomas psiquiátricos, cognitivos e de qualidade de vida nos pacientes avaliados 14 meses após o TCE grave.Conclusão: Não foi encontrada associação entre os níveis de T4 livre, TSH, cortisol, FSH, testosterona total, GH e IGF-1 na fase aguda do TCE grave e a mortalidade hospitalar em homens adultos. Níveis normais ou elevados de LH plasmático até 70 horas após o TCE grave foram um marcador independente de maior mortalidade hospitalar em homens adultos. Em homens adultos sobreviventes de TCE grave, os níveis de testosterona plasmática 14 meses após a hospitalização não foram associados ao desempenho cognitivo, sintomas psiquiátricos e de qualidade de vida em homens adultos vítimas de TCE grave na fase crônica. Abstract : Introduction: Traumatic brain injury (TBI) is the leading cause of death and disability in young male people. Experimental studies indicate that hormones may influence the prognosis of TBI. The clinical use of hormonal measures as biomarkers of TBI prognosis remain controverse.Objectives: To investigate the independent association among hospital mortality and the plasma levels of pituitary and peripherals hormones in male patients with severe TBI. To determine the plasma levels of LH and testosterone (total and calculated free) in the chronic phase (14 months after hospitalization) of male patients with severe TBI and the independent association among the total testosterone levels and their long-term cognitive performance, psychiatric manifestations and health-related quality of life (HRQL).Methods: In the acute phase study we investigated the independent association among the hospital mortality and the plasma levels of LH, FSH, GH, TSH, total testosterone, calculated free testosterone, free T4 and IGF-1 10 and 30 hours after trauma in 60 consecutive male victims of severe TBI. Additional measurement of the gonadotropic axis was done 70 hours after the TBI. Imbalances among the demographic, clinical, radiological and neurosurgical were corrected by the multiple binary logistic regression analysis. In the chronic phase study including 20 male chronic patients with severe TBI followed in our outpatient clinic we investigated: 1) the plasma levels of LH and testosterone (total and calculated free) in the chronic phase (14 months after hospitalization); 2) the independent association among the total plasma levels of testosterone and the cognitive performance (language, memory, attention, concentration, mental control, abstract verbal reasoning, visuospatial and motor skills), psychiatric symptoms (anxiety, depression and apathy) and HRQOL by multiple linear regression analysis.Results: The mean age of patients were 35 years and 33% of patients died during hospitalization. After the binary logistic regression analysis the adjusted odds ratio (OR) for the hospital mortality was 4 times higher in patients with Glasgow Coma Scale (GCS) score of less than 5 (adjusted OR = 3.9, 95 % CI 1.2-12.0, p = 0.02) in comparison to the higher GCS scores. Abnormal pupils examination on admission were 4.5 more associated to mortality than normal pupils (adjusted OR = 4.5, 95% CI 1.4-14.5, p = 0.01). At least one hormonal abnormality was 3.610to 73.1 % during the acute TBI phase. Total testosterone and calculated free testosterone decreased significantly in 10, 30 and 70 hours after injury in adult male victims of severe TBI. Multiple logistic regression analysis showed a trend for independent association among high or normal LH levels and hospital mortality in samples analyzed 10 hours (OR 3.7, 95% CI 0.8-16.3, p = 0.08) and 30 hours (OR 3.9, 95% CI 0.9-16.7, p = 0.06). Seventy hours after severe TBI, high or normal LH levels showed an independent and significant association with the hospital mortality (adjusted OR 8.4, 95% CI 1.7-41.2, p = 0.008). Abnormal pupils and lower GCS scores also were associated with the hospital mortality. Twenty percent of the evaluated chronic male patients showed hypogonadism 14 months after severe TBI. Plasma levels of total testosterone were not associated with their psychiatric, cognitive symptoms and quality of life 14 months after severe TBI.Conclusions: No associations were found among free T4, TSH, cortisol, FSH, total testosterone, GH and IGF-1 levels during acute phase of severe TBI in male patients and hospital mortality. There was a significant and independent association among LH levels up to 70 hours after the severe TBI and the hospital mortality. Plasma testosterone levels were not associated with cognitive performance, psychiatric symptoms and quality of life in adult male victims of severe TBI in the chronic phase.
