Abstract

Die Klimakrise bedroht die Gesundheit heutiger und künftiger Generationen und stellt das Gesundheitssystem vor besondere Herausforderungen. Zur Anpassung an den anthropogene Klimawandel sind umfängliche Adaptationsstrategien und eine Mitigation des Klimawandels notwendig. In der Medizin sowie in der Augenheilkunde gibt es vielfältige Möglichkeiten zur Reduktion des CO2(Kohlendioxid)-Fußabdrucks, die es zu ergreifen gilt, die ordnungspolitisch gefördert und eingefordert werden sollten. Das aufkommende Feld der datengesteuerten Nachhaltigkeit kann Werkzeuge liefern, um den ökologischen Fußabdruck der eigenen Tätigkeit zu evaluieren sowie Optimierungen zu initiieren. Lebenszyklusanalysen können Instrumente für systematische Ökobilanzen sein und nachhaltige Produkt- und Praxisentscheidungen ermöglichen. Das deutsche Gesundheitssystem sollte eine quantifizierbare und holistische Strategie zur CO2-Reduktion entwickeln; Nachhaltigkeit könnte zukünftig ein Leistungsindikator sein. Dieser Artikel diskutiert mit augenärztlicher Perspektive Beispiele zur Adaptation an die Klimakrise und zur Mitigation; dies schließt kleine Maßnahmen, die jeder Einzelne umsetzen kann, als auch größere, strukturelle Ansätze ein.

Abstract

Regular treatment with intravitreal operative medication injections (IVOM) and the associated frequent check-ups are a major challenge for many patients, which can lead to treatment discontinuation. The aim of the modified treat and extend (TAE) in blocks scheme is to achieve stable retinal and visual outcomes with as few control visits as possible and thus to minimize the burden for the individual patient.This monocentric retrospective study examined the treatment courses of 387 patients with neovascular age-related macular degeneration, diabetic macular edema and retinal vein occlusion, who were treated by three blocks of intravitreal injections in the TAE regimen. The primary endpoint was achieving an injection interval of 12 weeks.Intravitreal injections in TAE blocks significantly reduced retinal thickness and improved visual acuity. On average, a treatment interval of 2 months was achieved across the various indications.Intravitreal injections in TAE blocks of three injections can reduce patients' burden and lead to stable retinal and visual results.Die regelmäßige Therapie mittels intravitrealer operativer Medikamentenapplikation (IVOM) und die damit verbundenen Kontrolluntersuchungen stellen für viele Patienten eine große Herausforderung dar, die bis zum Behandlungsabbruch führen kann. Das modifizierte blockweise Treat-and-Extend(TAE)-Schema verfolgt das Ziel, mit möglichst wenigen Kontrollvisiten stabile Netzhaut- und Visusbefunde zu erreichen und dadurch die Belastung der Patienten zu minimieren.Diese monozentrische retrospektive Studie untersuchte Behandlungsverläufe von insgesamt 387 Patienten mit neovaskulärer altersbedingter Makuladegeneration (AMD), diabetischem Makulaödem (DMÖ), Venenastverschluss (VAV) und Zentralvenenverschluss (ZVV), bei denen das TAE-Regime jeweils in 3er-Blöcken angewendet wurde. Primärer Endpunkt war das Erreichen eines Injektionsintervalls von 12 Wochen.Durch die blockweise applizierte IVOM konnte die Netzhautdicke signifikant reduziert und der Visus verbessert werden. Über die verschiedenen Indikationen konnte im Mittel ein Behandlungsintervall von 2 Monaten erreicht werden.Ein in 3er-Blöcken angepasstes TAE-Schema im Rahmen der IVOM kann bei reduzierter Patientenbelastung zu stabilen Netzhaut- und Visusbefunden führen.

Bleeding severity was graded on an ordinal scale ranging from 0 to 5. Immediately after surgery and one day later, the incidence and severity of bleeding events was documented on a standardized form. A grade ≥ 3 was defined as severe bleeding. The influence of known systemic disorders, the type of anesthesia, surgical procedure, intraoperative blood pressure, and the use of or change in AP or AC agents on intraoperative bleeding were analyzed.Results: Data from 89 eyes undergoing glaucoma procedures were included (age 71.3y ± 10.5). We observed severe intraoperative bleeding in 8 eyes (9%) and found that concomitant diseases such as the history of a deep vein thrombosis or peripheral arterial occlusive disease, and the type of surgical procedure (trabeculectomy and viscocanaloplasty) were significantly associated with severe bleeding events. Medical history, best corrected visual acuity (BCVA), best corrected high/low contrast visual acuity (BChcVA/BClcVA), mean sensitivity (MS) and mean defect (MD) in central visual field testing, and FAZ size in fluorescein-angiography (FAG) were recorded. Macular thickness (central point thickness CPT, central subfield thickness CST) and volume measurements (central subfield volume CSV, total macular volume) were taken from SD-OCT (6x6mm ETDRS-grid). Groups were categorised as presenting FAZ sizes smaller (G1) or larger (G2) than 0.35mm2. Smallest (Q1) and largest quartiles (Q3) were also compared.Results: Thirty-six of 40 patients were included. MS differed significantly between G1 (n = 6) and G2 (n = 30), and BChcVA/BClcVA as well as TMV correlated significantly with FAZ size in correlation analysis. Mean HbA1c tended to be lower in G1 than G2. Patients in G1 were slightly older than in G2. Treatment period with insulin was shorter in G1/Q1 than in G2/Q3.
