Ambient polycyclic aromatic hydrocarbons (PAHs) originate predominantly from fuel combustion of motor vehicles and have the potential to affect human health. However, there is insufficient knowledge regarding serum PAHs health risks among the Malaysian population. This study aims to compare PAH concentrations, distributions, correlations, and health risks in 202 blood serum samples drawn from residents living in high-traffic volume areas (Kuala Lumpur) and low-traffic volume areas (Hulu Langat) in Malaysia. Solid phase extraction and gas chromatography-mass spectrometry (GC-MS) were employed to extract and analyze blood serum samples. Questionnaires were distributed to obtain sociodemographic and contributing factors of serum PAHs. The mean total PAHs concentration in serum of the Kuala Lumpur group was 54.44 ng g -1 lipids, double the Hulu Langat group's concentration (25.7 ng g -1 lipids). Indeno(1,2,3-cd)pyrene (IcP) and acenaphthene (ACP) feature the most and least abundant compounds in both study groups. The mean concentrations of IcP and ACP in the Kuala Lumpur and Hulu Langat groups were 26.8 vs 12.68 and 0.27 vs 0.14 ng g -1 lipids, respectively. High-molecular-weight PAHs (HMW-PAHs) composed 85% of serum total PAHs in both groups. Significant correlations were found (i) between the individual serum PAH congeners (p < 0.01) and (ii) between serum PAHs and total lipids (p < 0.01). According to the questionnaire data, high traffic volume and outdoor hobbies were the only contributory factors that confirmed significant relationships with serum PAHs (p < 0.001). Health risk assessment was computed using benzo(a)pyrene (BaP) equivalent (BaPeq) and demonstrated that the Kuala Lumpur group has twofold greater carcinogenic risk than the Hulu Langat group (16.11 vs 7.76 ng g -1 lipids). The interplay of physical, social, and economic factors during the pandemic adversely affected the mental health of healthy people and exacerbated pre-existing mental disorders. This study aimed to determine the impact of the COVID-19 pandemic on the mental health of the general population in Malaysia. A cross-sectional study involving 1246 participants was conducted. A validated questionnaire consisting of the level of knowledge and practice of precautionary behaviors, the Depression, Anxiety, and Stress Scales (DASS), and the World Health Organization Quality of Life-Brief Version (WHOQOL-BREF) was used as an instrument to assess the impacts of the COVID-19 pandemic. Results revealed that most participants possessed a high level of knowledge about COVID-19 and practiced wearing face masks daily as a precautionary measure. The average DASS scores were beyond the mild to moderate cut-off point for all three domains. The present study found that prolonged lockdowns had significantly impacted ( p < 0.05), the mental health of the general population in Malaysia, reducing quality of life during the pandemic. Employment status, financial instability, and low annual incomes appeared to be risk factors ( p < 0.05) contributing to mental distress, while older age played a protective role ( p < 0.05). This is the first large-scale study in Malaysia to assess the impacts of the COVID-19 pandemic on the general population.

Malaysia has been experiencing smoke-haze episodes almost annually for the past few decades. PM2.5 is the main component in haze and causes harmful impacts on health due to its small aerodynamic size. This study aimed to explore the implications of PM2.5 exposure on the dietary intake of working individuals. Two phased 13-weeks follow-up study was conducted involving 440 participants, consisting of two cohorts of outdoor and indoor workers. Ambient PM2.5 concentrations were monitored using DustTrakTM DRX Aerosol Monitor. Data on Simplified Nutritional Appetite Questionnaire (SNAQ) and 24 h diet recall were collected weekly. The highest PM2.5 concentration of 122.90 ± 2.07 µg/m3 was recorded in August, and it vastly exceeded the standard value stipulated by US EPA and WHO. SNAQ scores and calorie intake were found to be significantly (p < 0.05) associated with changes in PM2.5 exposure of outdoor workers. Several moderate and positive correlations (R-value ranged from 0.4 to 0.6) were established between SNAQ scores, calorie intake and PM2.5 exposure. Overall findings suggested that long hours of PM2.5 exposure affect personal dietary intake, potentially increasing the risk of metabolic syndromes and other undesired health conditions. The current policy should be strengthened to safeguard the well-being of outdoor workers.

Background: Pesticides manage pests and diseases in agriculture, but they harm the health of agricultural workers. Concentrations of thirteen pesticides were determined in personal air and blood serum of 85 paddy farmers and 85 non-farmers, thereafter associated with health symptoms. Method: Samples were analyzed using ultra-high performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS). Results: The median concentration of pesticides in personal air samples ranged from 10.69 to 188.49 ng/m3 for farmers and from 5.79 to 73.66 ng/m3 for non-farmers. The median concentration of pesticides in blood serum was from 58.27 to 210.12 ng/mL for farmers and 47.83 to 62.74 ng/mL for non-farmers. Concentration of eleven pesticides in personal air and twelve pesticides in blood serum were significantly higher in farmers than non-farmers (p < 0.05). All pesticides detected in personal air correlated significantly with concentration in the blood serum of farmers (p < 0.05). Health symptoms reported by farmers were dizziness (49.4%), nausea (47.1%), cough (35.3%), chest pain (30.6%), breathing difficulty (23.5%), sore throat (22.4%), vomiting (18.8%), phlegm (16.5%), and wheezing (15.3%). Concentration of pesticides in personal air, blood serum, and health symptoms were not significantly associated. Conclusion: Occupational exposure to pesticides significantly contaminates blood serum of farmers compared to non-farmers.

Agricultural activities have been arising along with the use of pesticides. The use of pesticides can impact not only on vector or other pest but also able to harm human health. Pesticide may leach from the irrigation of plant into the groundwater and in surface water. These waters could be sources of drinking water in a pesticides polluted area. This study aims to determine the occurrence pesticides in surface water and pesticides removal efficiency in a conventional drinking water treatment plant (DWTP) and the potential health risk to consumers. The study was conducted in Tanjung Karang, Selangor, Malaysia. Thirty river water samples and eighteen water samples from DWTP were collected. The water samples were extracted using solid phase extraction (SPE) before injected to the ultra-high performance liquid chromatography tandem mass spectrometry (UHPLC-MS/MS). Five hundreds and ten respondents were interviewed using questionnaires to obtain information for health risk assessments. The results showed that propiconazole had the highest mean concentration (4493.1 ng/L) while pymetrozine had the lowest mean concentration (1.3 ng/L) in river water samples. The pesticides removal efficiencies in the conventional DWTP were 77% (imidacloprid), 86% (propiconazole and buprofezin), 88% (tebuconazole) and 100% (pymetrozine, tricyclazole, chlorantraniliprole, azoxystrobin and trifloxystrobin), respectively. The hazard quotients (HQs) and hazard index (HI) for all target pesticides were <1, indicating there was no significant chronic non-carcinogenic health risk due to consumption of the drinking water. Background: Little research exists in the teaching of evidence-based dentistry (EBD) to students in the fields of dental hygiene, dental nursing and orthodontic therapy. This study aims to analyse the effect of a 1-day EBD programme on knowledge and confidence whilst also gaining insight into students' experience of the intervention., Method: A mixed methods study was utilised with explanatory sequential design. The population consisted of dental hygiene (DH), dental nursing (DN) and orthodontic therapy (OT) students (N = 44). The intervention consisted of a 1-day active learning EBD programme, delivered via group projects and lectures. In the initial quantitative phase, a standardised questionnaire pre- and post -intervention measured changes in confidence for all participants, whilst change in knowledge was measured for DH and OT students only. Following this, focus groups were scheduled for all members of each discipline 3 months post-intervention for DN and 2 months post-intervention for DH and OT students. Semi-structured focus group schedules were drawn up, and groups organised according to the outcomes of quantitative data analysis. Qualitative results were analysed using a deductive adaptation of Burnard's thematic content analysis., Results: Forty-two students took part (94.45%) in this study. Median knowledge scores increased from zero to two of five (P < .001), whilst median confidence score doubled from four to eight of eight (P < .001). Results of thematic content analysis were coincident with quantitative results; however, it also provided constructive feedback regarding design and content of the course., Conclusions: A 1-day bespoke programme in EBD increased students' confidence and knowledge in EBD skills. However, the findings further suggest that two successive half-day training sessions instead of one full-day training, tied in with coursework that requires the application of the acquired skills, may increase the learning experience further., (© 2017 John Wiley & Sons A/S. Tanjung Karang, Selangor, is widely known for its paddy cultivation activity and hosts the third largest paddy field in Malaysia. Pesticides contamination in agriculture fields has become an unavoidable problem, as pesticides are used to increase paddy productivity and reduce plant disease. Human exposure to agrichemicals is common and could results in both acute and chronic health effects, such as acute and chronic neurotoxicity. This study aims to determine the concentrations of commonly used pesticides (azoxystrobin, buprofezin, chlorantraniliprole, difenoconazole, fipronil, imidacloprid, isoprothiolane, pretilachlor, propiconazole, pymetrozine, tebuconazole, tricyclazole, and trifloxystrobin) in personal air samples and their associated health risks among paddy farmers. Eighty-three farmers from Tangjung Karang, Selangor were involved in this study. A solid sorbent tube was attached to the farmer's breathing zone with a clip, and an air pump was fastened to the belt to collect personal air samples. Pesticides collected in the XAD-2 resin were extracted with acetone, centrifuged, concentrated via nitrogen blowdown and reconstituted with 1mL of 3:1 ultrapure water/HPLC-grade methanol solution. The extract was analyzed using ultra-high-performance liquid chromatography tandem mass spectrometry (UHPLC-MS/MS). The target compounds were detected with a maximum concentration reaching up to 462.5ngm -3 (fipronil). The hazard quotient (HQ) was less than 1 and the hazard index (HI) value was 3.86×10 -3 , indicating that the risk of pesticides related diseases was not significant. The lifetime cancer risk (LCR) for pymetrozine was at an acceptable level (LCR<10 -6 ) with 4.10×10 -8 . There is a paucity of information about the occurrence of microplastics (MPs) in edible fish tissues. Here, we investigated the potential presence of MPs in the excised organs (viscera and gills) and eviscerated flesh (whole fish excluding the viscera and gills) of four commonly consumed dried fish species (n = 30 per species). The MP chemical composition was then determined using micro-Raman spectroscopy and elemental analysis with energy-dispersive X-ray spectroscopy (EDX). Out of 61 isolated particles, 59.0% were plastic polymers, 21.3% were pigment particles, 6.55% were non-plastic items (i.e. cellulose or actinolite), while 13.1% remained unidentified. The level of heavy metals on MPs or pigment particles were below the detection limit. Surprisingly, in two species, the eviscerated flesh contained higher MP loads than the excised organs, which highlights that evisceration does not necessarily eliminate the risk of MP intake by consumers. Future studies are encouraged to quantify anthropogenic particle loads in edible fish tissues.

So far, several classes of digesting solutions have been employed to extract microplastics (MPs) from biological matrices. However, the performance of digesting solutions across different temperatures has never been systematically investigated. In the first phase of the present study, we measured the efficiency of different oxidative agents (NaClO or H 2 O 2 ), bases (NaOH or KOH), and acids [HCl or HNO 3 ; concentrated and diluted (5%)] in digesting fish tissues at room temperature (RT, 25°C), 40, 50, or 60°C. In the second phase, the treatments that were efficient in digesting the biological materials (>95%) were evaluated for their compatibility with eight major plastic polymers (assessed through recovery rate, Raman spectroscopy analysis, and morphological changes). Among the tested solutions, NaClO, NaOH, and diluted acids did not result in a satisfactory digestion efficiency at any of the temperatures. The H 2 O 2 treatment at 50°C efficiently digested the biological materials, although it decreased the recovery rate of nylon-6 (NY6) and nylon-66 (NY66) and altered the colour of polyethylene terephthalate (PET) fragments. Similarly, concentrated HCl and HNO 3 treatments at RT fully digested the fish tissues, but also fully dissolved NY6 and NY66, and reduced the recovery rate of most or all of the polymers, respectively. Potassium hydroxide solution fully eliminated the biological matrices at all temperatures. However, at 50 and 60°C, it degraded PET, reduced the recovery rate of PET and polyvinyl chloride (PVC), and changed the colour of NY66. According to our results, treating biological materials with a 10% KOH solution and incubating at 40°C was both time and cost-effective, efficient in digesting biological materials, and had no impact on the integrity of the plastic polymers. This study aimed to determine the heavy metals (lead, cadmium, and chromium) concentration in lipsticks of different price categories sold in the Malaysian market and evaluate the potential health risks due to daily ingestion of heavy metals in lipsticks. A total of 374 questionnaires were distributed to the female staff in a public university in Malaysia in order to obtain information such as brand and price of the lipsticks, body weight, and frequency and duration of wearing lipstick. This information was important for the calculation of hazard quotient (HQ) in health risk assessment. The samples were extracted using a microwave digester and analyzed using Inductively Coupled Plasma Optical Emission Spectrometry (ICP-OES). The concentrations of lead, cadmium, and chromium in lipsticks ranged from 0.77 to 15.44 mg kg(-1), 0.06-0.33 mg kg(-1), and 0.48-2.50 mg kg(-1), respectively. There was a significant difference of lead content in the lipsticks of different price categories. Repeated applications of animal manure as fertilizer are normal agricultural practices that may release veterinary antibiotics and hormones into the environment from treated animals. Broiler manure samples and their respective manure-amended agricultural soil samples were collected in selected locations in the states of Selangor, Negeri Sembilan and Melaka in Malaysia to identify and quantify veterinary antibiotic and hormone residues in the environment. The samples were analyzed using ultrasonic extraction followed by solid phase extraction (SPE) and liquid chromatography-tandem mass spectrometry (LC-MS/MS). The broiler manure samples were found to be contaminated with at least six target analytes, namely, doxycycline, enrofloxacin, flumequine, norfloxacin, trimethoprim and tylosin. These analytes were detected in broiler manure samples with maximum concentrations reaching up to 78,516 μg kg(-1) dry weight (DW) (doxycycline). For manure-amended agricultural soil samples, doxycycline and enrofloxacin residues were detected in every soil sample. The maximum concentration of antibiotic detected in soil was 1331 μg kg(-1) DW (flumequine). The occurrence of antibiotics and hormones in animal manure at high concentration poses a risk of contaminating agricultural soil via fertilization with animal manure. Some physico-chemical parameters such as pH, total organic carbon (TOC) and metal content played a considerable role in the fate of the target veterinary antibiotics and progesterone in the environment. The fate of nine veterinary antibiotics and one hormone in broiler manure during 40 days of composting was investigated. Results showed that composting can significantly reduce the concentration of veterinary antibiotics and hormone in broiler manure, making application of the post-compost manure safer for soil application. More than 99% of the nine antibiotics and one hormone involved in this study were removed from the manure during 40 days of composting. The target antibiotics and hormone showed short half-life in broiler manure composting, ranging from 1.3 to 3.8 days. The relationship between the physico-chemical properties of soil, manure and manure compost and its veterinary antibiotic and hormone concentration was statistically evaluated by Pearson correlation matrix. The concentration of veterinary antibiotics and hormone in manure compost was suggested to be affected by physico-chemical properties such as pH, temperature, total organic carbon (TOC), total nitrogen (TN), total phosphorus (TP) and metal contents., (Copyright © 2013 Elsevier Ltd. A multi-residue analytical method was developed to quantify nine antibiotics and one hormone in soil, broiler manure and manure compost. The developed method was based on ultrasonic extraction with MeOH:ACN:EDTA:McIlvaine buffer, solid phase extraction (SPE) using HLB (3 cc/60 mg) cartridge, followed by instrumental analysis using liquid chromatography-tandem mass spectrometry (LC-MS/MS) with 25 min total run time. It was validated and tested on soil, broiler manure and manure compost samples and showed that the method is able to simultaneously detect and quantify the target analytes with good selectivity and sensitivity. The developed method was linear in a concentration range from its instrumental quantification limit (IQL) to 500 ng/mL, with correlation coefficients higher than 0.999. The overall method performance was good for the majority of the analytes, with recoveries range from 63% to 121% in all the sample matrices. The method quantification limit (MQL) for the 10 target analytes in the soil, broiler manure and manure compost samples were 2-10, 3-16 and 5-15 μg/kg dry weight (DW), respectively. The method has also included tilmicosin, an antibiotic known to be reported in the environment for the first time. The developed method was then applied on broiler manure samples and its relative manure amended agricultural soil samples to identify and quantify veterinary antibiotic and hormone residues in the environment. These analytes were detected in broiler manure and soil samples, with maximum concentrations reaching up to 78516.1 μg/kg DW (doxycycline) and 1331.4 μg/kg DW (flumequine), respectively. Background: The purpose of the present study was to determine the effects of massage therapy on motor development, weight gain, and hospital discharge in preterm very low-birthweight (VLBW) newborns., Methods: Twenty-four preterm VLBW newborns (<34 weeks and <1500 g) were enrolled in this randomized controlled pilot study. The intervention group (n = 12) received massage therapy starting at 34 weeks post-conceptional age (15 min daily, 5 days/week for 4 weeks). The infants in the sham treatment group (n = 12) received similar duration of light still touch. Test of Infant Motor Performance (TIMP) score gain, weight gain, and post-conceptional age at discharge were compared between the two groups after intervention using Mann-Whitney U-test., Results: No significant between-group difference in TIMP score gain and weight gain was identified when all subjects were analyzed. In subgroup analysis, among those with below-average pre-treatment TIMP score (<35), the intervention group (n = 6) achieved significantly higher TIMP score gain (P = 0.043) and earlier hospital discharge (P = 0.045) than the sham treatment group (n = 5). These same infants, however, also had significantly shorter duration of total parenteral nutrition than their counterparts in the sham treatment group (P = 0.044)., Conclusions: Massage therapy might be a viable intervention to promote motor outcomes in a subgroup of VLBW newborns with poor motor performance. A larger randomized controlled trial is required to further explore the effects of massage therapy in this high-risk group.

We investigated whether premature, very low-birthweight (VLBW) Asian children without major handicap had poor motor skills compared with their normal birthweight counterparts. We compared a cohort of 42 preterm babies with birthweights <1500 g who participated in a developmental stimulation programme with 69 children of normal birthweight matched for age, gender and paternal education. VLBW children participated in the programme for 3 years and were followed to the age of 5-7 years. The VLBW cohort had significantly lower scores in B- and C-rated skills of the Peabody Developmental Motor Scales. Their total score was also significantly lower. This is in agreement with studies in other populations that found that VLBW children had lower motor scores and that early interventions failed to ameliorate this motor disadvantage.

Background and Aims: The pancreatic cystic neoplasms, including solid pseudopapillary tumour (SPT), mucinous cystic neoplasm (MCN), and intraductal papillary mucin producing tumour (IPMT), have their characteristic clinicopathological features. A systematic investigation of oestrogen receptor (OR), progesterone receptor (PR), trefoil factor 1(TFF1), and epidermal growth factor and its receptor (EGF and EGFR) expressed in pancreatic cystic neoplasms and pancreatic ductal adenocarcinoma was determined to elucidate their corresponding sex and age predilection, cell origin, and pathway of malignant transformation., Methods: Surgical specimens of SPT (n=10), MCN (n=12), IPMT (n=10), and ductal adenocarcinoma (n=20) were studied. The expression of OR, PR, TFF1, EGF, and EGFR were each determined in each disease entity using monoclonal antibodies by immunohistochemical method. The results were correlated with the clinicopathological data., Results: PR was expressed in all 10 SPT, whereas OR was expressed in none of 10 SPT. TFF1 was not or weakly expressed in SPT. Although EGF was strongly expressed in seven of 10 SPT, synchronous expression of EGF and its receptor was expressed in none of 10 SPT. Of the 12 MCN, six had PR expression in the stroma cells but not in the neoplastic epithelium, seven had a moderate or strong expression of TFF1, and 10 had no or weak EGFR expression, irrespective of their benigneity or malignancy. Synchronous expression of EGF and EGFR was observed in only one of 12 MCN. Among 10 IPMT, TFF1 and EGFR were moderately or strongly expressed in all six malignancies, whereas TFF1 and EGFR were not or weakly expressed in three of four benigneity. Of 20 ductal adenocarcinomas, TFF1 and EGFR were moderately or strongly expressed in 16 and 12, respectively. Synchronous expression of EGF and EGFR was observed in six of 10 IPMT and nine of 20 ductal adenocarcinoma, respectively., Conclusion: PR was uniquely expressed in SPT, and OR and PR were expressed in stroma of MCN, reflecting their sex and age predilection. TFF1 expression was related to EGFR such as in IPMT and ductal adenocarcinoma, not related to EGFR such as in MCN, and not related to hormonal receptors such as in SPT. EGF and its receptor might play a part in the malignant transformation of IPMT and ductal adenocarcinoma, but not of SPT and MCN.

Before 1981, Pb in petrol in Hong Kong averaged about 0.84 g/l. Then the Pb level was stepwisely reduced and by early 1987, it was at 0.25 g/l. The effect of Pb reduction in petrol on the roadside environment was assessed by comparing the Pb content in kerbside dust samples (extracted by hot 3.5 M NO3 and analysed by flame AAS) taken in Nov 77-Jan 78 and in Jan 1987 from 37 sites which covered a wide range of traffic density. The arithmetic mean Pb levels for the 1977-78 and 1987 samples were 3280 and 1080 micrograms/g respectively and their difference was highly significant (P = 0.0005, paired t-test). Parallel studies on the levels of Cu, Zn and Mn in the two batches of samples showed no significant changes. Thus Pb reduction in petrol has drastically lowered the metal's contamination in the roadside environment in Hong Kong.

The Pb, Cu, Zn, Fe, Mn and Cd contents in roadside soils and grass from 36 sites on the Island of Hong Kong were determined by atomic absorption spectrometry. Results showed that both soil and grass contained elevated levels of the metals studied. Linear regression analysis between the logarithmic concentration of the metals in the samples and the logarithmic traffic volume of the sites revealed that, apart from Cd in soil, the two were strongly related (variance ratios' P values <0.001), thus indicating that the motor vehicle forms a major source of these metals in the roadside. The regional distribution of Pb and Cu in roadside soil and grass (presented in maps herein) shows high levels of contamination occurring mainly in the highly urbanised northern part of the Island where most of the traffic is concentrated. Apparently, both soil and grass could be utilised to reflect the extent of aerial deposition of metals in the roadside.

Lead levels were generally found to be high in road-side dust samples collected from different sites throughout the Island of Hong Kong during winter 1977-78. Considerable differences in the concentration of lead amongst these sites were detected but such differences bore no direct relationship to the traffic volumes of the sites from which the samples were collected. Different districts within the city varied widely in the mean concentration of lead in their road-side dust samples.
