3. Determinants of referral network size for screening colonoscopies in the Veterans Health Administration after the implementation of the MISSION Act

Abstract

Objective: To measure key characteristics of the Veterans Health Administration's (VHA) Community Care (CC) referral network for screening colonoscopy and identify market and institutional factors associated with network size. Data Sources: VHA electronic health records, CC claim data, and National Plan and Provider Enumeration System. Study Design: In this retrospective cross-sectional study, we measure the size of the VHA's CC referral networks over time and by VHA parent facility (n = 137). We used a multivariable linear regression to identify factors associated with network size at the market-year level. Network size was measured as the number of physicians who performed at least one VHA-purchased screening colonoscopy per 1000 enrollees at baseline. Data Extraction: Data were extracted for all Veterans (n = 102,119) who underwent a screening colonoscopy purchased by the VHA from a non-VHA physician from 2018 to 2021. Principal Findings: From 2018 to 2021, median network volume of screening colonoscopies per 1000 enrollees grew from 1.6 (IQR: 0.6, 4.6) to 3.6 (IQR: 1.6, 6.6). The median network size grew from 0.63 (IQR: 0.30, 1.26) to 0.92 (IQR: 0.57, 1.63). Finally, the median procedures per physician increased from 2.5 (IQR: 1.6, 4.2) to 3.2 (IQR: 2.4, 4.7). After adjusting for baseline market characteristics, volume of screening colonoscopies was positively related to network size ([beta] = 0.15, 95% CI: [0.10, 0.20]), negatively related to procedures per physician ([beta] = -0.12, 95% CI: [-0.18, -0.05]), and positively associated with the percent of rural enrollees (0 = 0.01, 95% CI: [0.00, 0.01]). Conclusions: VHA facilities with a higher volume of VHA-purchased screening colonoscopies and more rural enrollees had more non-VHA physicians providing care. Geographic variation in referral networks may also explain differences in the effects of the MISSION Act on access to care and patient outcomes. Abstract

In a previous article the second author together with A. Pasquale determined the spectrum of the $Cos^\lambda$ transform on smooth functions on the Grassmann manifolds $G_{p,n+1}$. This article extends those results to line bundles over certain Grassmannians. In particular we define the $Cos^\lambda$ transform on smooth sections of homogeneous line bundles over$G_{p,n+1}$ and show that it is an intertwining operator between generalized ($\chi$-spherical) principal series representations induced from a maximal parabolic subgroup of $\mathrm{SL} (n+1, \mathbb{K})$. Then we use the spectrum generating method to determine the $K$-spectrum of the $Cos^\lambda$ transform.

Abstract

We study the action of hypergeometric shift operators on the Heckman-Opdam hypergeometric functions associated with the $BC_n$ type root system and some negative multiplicities. Those hypergeometric functions are connected to the $\chi$-spherical functions on Hermitian symmetric spaces $U/K$ where $\chi$ is a nontrivial character of $K$. We apply shift operators to the hypergeometric functions to move negative multiplicities to positive ones. This allows us to use many well-known results of the hypergeometric functions associated with positive multiplicities. In particular, we use this technique to achieve exponential estimates for the $\chi$-spherical functions. Abstract

Hospital CEO salaries have grown quickly over the past two decades. We investigate correlates of rising nonprofit hospital CEO pay between 2012 and 2019 by merging compensation data from Candid's IRS 990 forms with hospital data from the National Academy for State Health Policy Hospital Cost Tool. Almost half of the measured increase in CEO compensation (44.5%) accrued to a "base case" CEO, who was leading a non-teaching hospital system or independent hospital with fewer than 100 beds that earned 0 profits and provided no charity care. Another 28.5% of the measured salary increase resulted from changes in the generosity with which observable metrics were rewarded, particularly the reward for heading a system with 500 or more beds. The remaining 27% resulted mostly from hospital systems or single hospitals that increased their profits or bed size over time. The increase in CEO compensation associated with leading larger healthcare systems and earning greater profits may explain the increase in healthcare system consolidation which has occurred over the last several years. Abstract

The superconducting RF linac for LCLS-II calls for 1.3 GHz 9-cell cavities with an average intrinsic quality factor Q0 of 2.7x10^10 at 2 K and 16 MV/m accelerating gradient. Two niobium 9-cell cavities, prepared with nitrogen-doping at Fermilab, were assembled into the Cornell Horizontal Test Cryomodule (HTC) to test cavity performance in a cryomodule that is very similar to a full LCLS-II cryomodule. The cavities met LCLS-II specifications with an average quench field of 17 MV/m and an average Q0 of 3x10^10. The sensitivity of the cavities' residual resistance to ambient magnetic field was determined to be 0.5 nOhm/mG during fast cool down. In two cool downs, a heater attached to one of the cavity beam tubes was used to induce large horizontal temperature gradients. Abstract

Paley-Wiener type theorems describe the image of a given space of functions, often compactly supported functions, under an integral transform, usually a Fourier transform on a group or homogeneous space. Several authors have studied Paley-Wiener type theorems for Euclidean spaces, Riemannian symmetric spaces of compact or non-compact type as well as affine Riemannian symmetric spaces. In this article we prove a Paley-Wiener theorem for homogeneous line bundles over a compact symmetric space $U/K$. The Paley-Wiener theorem characterizes f with sufficiently small support in terms of holomorphic extendability and exponential growth of their Fourier transforms. An important tool is a generalization of Opdam's estimate for the hypergeometric functions for multiplicity functions that are not necessarily positive. Abstract

Purpose: Understanding rural‐urban disparities in patient satisfaction is critical to identify gaps for improvement in patient‐centered care and tailor interventions to specific patient needs, especially those in the Frontier and Remote areas (FAR). This study aimed to examine disparities in patient perceptions of care between urban, rural non‐FAR, and FAR hospitals between 2014 and 2019. Methods: This is a retrospective longitudinal study using 2014‐2019 Hospital Consumer Assessment of Healthcare Providers and Systems data linked to American Hospital Annual Survey data (3,524 hospitals in 2014 and 3,440 hospitals in 2019). Multivariable linear regression models were used to identify differential trends in patient perceptions of care by hospital rurality over 2014‐2019, adjusting hospital‐ and county‐level characteristics. Findings: In 2014, patients at rural non‐FAR and FAR hospitals had lower percentages of willingness to definitely recommend these hospitals than urban hospitals (average percentage difference, 95% CI: −4.0% [−4.5%, −3.5%]; −2.0% [−2.8%, −1.2%]); yet, over the study period, rural hospitals experienced steeper increases in patient willingness to recommend (0.2% [0.07%, 0.4%]; 0.4% [0.08%, 0.7%]). FAR hospitals also showed improvements in patient experience in a clean environment, communication with nurses, communication about medicines, and responsiveness of staff. Communication with doctors showed slight decreases across hospital locations. Conclusions: Patient perceptions of care were generally improved in all US hospitals from 2014 to 2019, except communications with doctors. These findings highlight the potential for enhancing patient satisfaction and experience in urban hospitals and suggest the need to improve patient willingness to recommend in rural FAR hospitals.
