3. Impact of luteoma during pregnancy on fetal development and its long-term effects into adulthood.

Abstract

A woman in her 30s presents to the Differences in Sexual Development Programme at a tertiary care academic medical centre with vaginal stenosis and scarring. Her medical history is significant for virilisation in utero due to a maternal luteoma of pregnancy. Laboratory investigations at the time of birth showed elevated androgens in both mother and daughter. During infancy, she underwent clitoroplasty and vaginoplasty for correction of posterior vaginal fusion. She represented as an adult with vaginal stenosis, with associated physical and psychosocial implications. She was not able to insert a tampon or have penetrative intercourse. After examination and shared decisionmaking, the patient underwent cystoscopy, vaginoscopy and posterior vaginoplasty with the goal to create a normal calibre vagina. Postoperative dilator use was recommended to prevent restenosis of the introitus. In clinic follow-up, the patient was observed to have a normal calibre vagina. [ABSTRACT FROM AUTHOR]

Abstract

We report two cases of acute bladder injury with bladder neck necrosis identified during the initial operative evaluation and within the early postprocedural period in patients with significant pelvic trauma requiring pelvic vascular embolization. To our knowledge, this is the first report of bladder neck necrosis found during the initial intraoperative surgical evaluation or early postoperative setting.

Abstract

ObjectiveTo quantitatively assess the effect of radical prostatectomy (RP) on the specific domains that comprise overall sexual function (SF), focusing on the relationships among these domains and overall SF, and to identify predictors for recovery of SF over time, as a decline in SF and sexual bother (SB) are known potential complications of treatment for prostate cancer.Patients and methodsWithin the Cancer of the Prostate Strategic Urologic Research Endeavor database, we identified men diagnosed between 1995 and 2001 with localized prostate cancer treated with RP. SF and SB outcomes, measured using the University of California Los Angeles Prostate Cancer Index, were assessed at 6-month intervals for 4 years after RP.ResultsIn all, 620 men met the study criteria; at 6 months after RP, overall and all the specific domains of SF declined, with improvement in most specific domains by 2 years after RP. The greatest declines were in the ability to achieve erections, high-quality erections, and frequent erections; these domains were also most strongly correlated with overall SF. Sexual desire was relatively preserved, and there was a weak correlation between overall SF and sexual desire after RP, when there was the greatest discrepancy between sexual desire and other domains of function. SB showed continued improvement over time to 4 years but was not well correlated with any measurements of SF assessed. Younger age, college education, sexual aid and medication use, the absence of comorbid conditions, and nerve-sparing surgery were predictive of significant recovery of function in several specific domains of SF.ConclusionsRP affects specific domains of SF to differing degrees. Compromised erectile function is most commonly reported among these specific domains and seems to play a more dominant role in determining overall SF, but notably none of the domains of function were closely linked to SB. Because education is protective in the perception of bother, appropriate counselling and the setting of expectations for outcomes in overall and specific domains of SF might lead to improved quality of life after treatment for prostate cancer.

Abstract

The hormone-activated glucocorticoid receptor (GR), through its N- and C-terminal transcriptional activation functions AF-1 and AF-2, controls the transcription of target genes presumably through interaction(s) with transcriptional regulatory factors. Utilizing a modified yeast two-hybrid approach, we have identified the tumor susceptibility gene 101 (TSG101) and the vitamin D receptor-interacting protein 150 (DRI P150) as proteins that interact specifically with a functional GR AF-1 surface. In yeast and mammalian cells, TSG101 redresses whereas DRIP150 enhances GR AF-1-mediated transactivation. Thus, GR AF-1 is capable of recruiting both positive and negative regulatory factors that differentially regulate GR transcriptional enhancement. In addition, we show that another member of the DRIP complex, DRI P205, interacts with the GR ligand binding domain in a hormone-dependent manner and facilitates GR transactivation in concert with DRIP150. These results suggest that DRIP150 and DRIP205 functionally link GR AF-1 and AF-2, and represent important mediators of GR transcriptional enhancement. [ABSTRACT FROM AUTHOR]

Abstract

Introducción: En la literatura se ha descrito la estrecha relación que existe entre gónadas disgenéticas con presencia del cromosoma Y y el riesgo de desarrollar gonadoblastoma. Sin embargo, se sabe poco y es incierta la relación que existe con tumores estromales de cordones sexuales. En este trabajo se presenta el caso de una paciente de 14 años años con síndrome de Turner, 45X (8)/46XY(92) con tumor de cordones sexuales con patrón anular. Materiales y métodos: Se realizó una búsqueda en las bases de datos de Embase, Ovid, Ebsco y PubMed con los términos «(Turner Syndrome) AND Sex cord stromal tumor». Sin embargo, solo en PubMed se encontró un artículo que cumpliera con los criterios de búsqueda, y este se considera el primer reporte de caso en la literatura que hace referencia a la relación entre el síndrome de Turner y tumor estromal de cordones sexuales. Conclusiones: Es poca la evidencia que existe para casos como este, por lo cual el manejo de estos pacientes es aún incierto y controvertido, en especial por las diferentes perspectivas de las especialidades tratantes. Específicamente con esta paciente tenemos la dificultad de definir el riesgo-beneficio del manejo con hormona de crecimiento como parte de su tratamiento luego de tener el resultado neoplásico.
