1. Efficacy of Proactive Topical Antihistamine Use in Patients with Seasonal Allergic Conjunctivitis

Abstract

Topical antihistamines are often instilled symptomatically to control patients' eye allergy symptoms. The purpose of this study was to evaluate the effectiveness of proactive and as-needed use of antihistamine eye drops in controlling symptoms and to examine whether proactive use may improve quality of life (QOL).This was a prospective, multicenter, cohort study in Japan. We classified 418 patients who had developed certain symptoms and used antihistamine eye drops for 2 weeks into two groups: those who used the drops at the required frequency at a fixed time (proactive use) and those who used them as-needed. The Japanese Allergic and Conjunctival Diseases Quality of Life Questionnaire (JACQLQ) and Ten-Item Personality Inventory were used to evaluate QOL and personality. Participants' QOL was evaluated using JACQLQ scores after matching of baseline characteristics using propensity score analysis.After propensity score matching, 115 "proactive" and 115 "as-needed" patients were analyzed. After treatment, in "as-needed" patients, the overall QOL scale was 1.66 (95% CI 1.55-1.78); in "proactive" patients, the overall QOL scale was 1.34 (95% CI 1.23-1.46) and was significantly improved compared with the "as-needed" patients (analysis of covariance, P = 0.002). Furthermore, proactive use significantly alleviated depression (P = 0.03). This improvement of QOL was independent of improvement of the clinical sign scores.Proactive use of topical antihistamine may serve as an effective means for improving QOL of patients with seasonal allergic conjunctivitis.University Hospital Medical Information Network (UMIN) 000039554.

Abstract

Dry eye disease (DED) is commonly encountered in eye clinics and hospitals, and it is therefore very important to understand DED prevalence in outpatients. A multicenter, hospital-based cross-sectional study was conducted among outpatients in Japan to ascertain DED prevalence and relationships between DED and patient profiles, including eye disease, DED diagnosis history, and surgical history. DED was diagnosed according to diagnostic criteria of the Asia Dry Eye Society. Patient self-assessment of DED-related subjective symptoms was conducted using the 5-Item Dry Eye Questionnaire (DEQ-5). Tear break-up time was evaluated in subjective symptom-positive patients. The prevalence of DED was 55.7% in 990 patients (mean age 69.1 ± 13.4 years), DED was commonly experienced in combination with other ocular diseases. In revisiting patients, 15.2% had not previously been diagnosed as DED, and their total DEQ-5 scores were higher than those of patients who had undergone DED treatment. This study revealed that more than half of the outpatients had DED. Among revisiting patients, there were many “hidden” DED patients who had not been diagnosed with DED in the past. There is a high likelihood of finding DED comorbidity in patients with other eye diseases in eye clinics and hospitals. Santen Pharmaceutical Co., Ltd. Abstract

Titanium oxide powder and thin films were prepared from an (NH4)2TiF6 aqueous solution upon addition of boric acid in a process denoted as liquid phase deposition (LPD). In this process, titanium oxide powder and films were formed by the chemical equilibrium reaction between titanium fluoro-complex ions and metal oxide in the aqueous solution. The prepared powder and deposited films showed some features which differed from titanium oxide prepared from titanium alkoxide. The prepared powder and the deposited films included anatase (TiO2) and showed photocatalytic activity without prior calcination. The decomposition rate of CH3CHO reached a maximum when the powder or films were calcined at 300 °C. The LPD process enabled a uniform layer of TiO2 film to form homogeneously on the entire surface of glass wool. The prepared TiO2 contained fluorine and nitrogen. The amount of these impurities was reduced upon calcination.

Abstract

We report a rare case of herpes simplex virus (HSV) bronchopneumonia in an otherwise healthy middle-aged individual. Bronchoscopy indicated scattered white-coated lesions in the bronchial mucosa. The diagnosis was established following immunohistopathological staining for HSV of specimens obtained by bronchial biopsy. This case suggests that HSV could be a pathological agent of not only oral and genital mucosal lesions but also lower respiratory tract infection.

As an aid in the differential diagnosis of exudative pleural effusions, tumor markers were investigated. We measured immunosuppressive acidic protein (LAP), carbohydrate antigen 19-9 (CA 19-9), tissue polypeptide antigen (TPA), carcinoembryonic antigen (CEA), adenosine deaminase (ADA), and alpha, -acid glycoprotein (AGP) in the pleural fluid of 36 patients with carcinomatous pleural effusions and of 35 patients with tuberculous pleurisy because we have frequently found these diseases to be associated with exudative pleuritis. Abstract

The percentage of cells of different T-cell subsets and their functions in tuberculous pleural effusion were investigated. The percentage of OKT4-positive cells was 65 +/- 2% (mean +/- SEM, n = 8) and that of OKT8-positive cells was 19 +/- 3% (n = 8). Pleural T lymphocytes of patients with tuberculous pleurisy responded well to stimulation with purified protein derivative of tuberculin, and deoxyribonucleic acid synthesis was observed along with gamma interferon (IFN-gamma) production. When pleural T lymphocytes of patients with tuberculous pleurisy were treated with OKT4 monoclonal antibody and complement, a significant decrease in IFN-gamma production was observed in all eight patients (P less than 0.005), whereas no definite decrease in IFN-gamma production was found after treatment with OKT8 monoclonal antibody and complement. These results suggest that at least the OKT4+/OKT8- T-cell subset is responsible for the antigen-specific IFN-gamma production in pleural T lymphocytes of patients with tuberculous pleurisy.
