86歳の女性．動悸と胸部圧迫感と発熱，咳嗽があり食道裂孔ヘルニアに起因する症状と診断され当科に紹介入院となった．胸部X線写真で縦隔に拡大した胃泡と水平面を示す胃液の存在を認めた．巨大な食道裂孔ヘルニア（voluminous hiatus hernia）とバリウム検査で判明した．高齢であり腹腔鏡下手術を選択し，胸腔へ脱出していた横行結腸の一部，全胃，食道胃接合部を腹腔内へ整復し，Modified Nissen fundoplicationの手術を行った．術後4日目に食事開始して10日目に合併症もなく退院した．2年後他疾患での死亡まで再発は認めなかった．呼吸器外科医にとって横隔膜ヘルニア，横隔膜弛緩症，外傷性横隔膜ヘルニアなどに時々遭遇することがあり食道裂孔ヘルニアに通じておくことは重要と考える．

Bronchioloalveolar carcinoma of mixed mucinous and nonmucinous type fulfilling the 1999 WHO criteria is rare. Here, we report a case of this type of tumor determined entirely by histological examinations. A 57-year-old man was incidentally found to have a demarcated 3cm mass in his lower lobe of the right lung. The tumor was composed of tall columnar cells containing cytoplasmic mucins, cuboidal cells without mucins, and intermediate cell types with lepidic growth patterns. Tumor cells were distributed within a region of 2cm in diameter, and no stromal, vascular, or pleural invasion was observed. Immunohistochemically, both the mucinous and nonmucinous components were positive for cytokeratin 7, TTF-1, and E-cadherin, and negative for cytokeratin 20, consistent with the results for nonmucinous bronchioloalveolar carcinoma. No mutations were detected in exons 5-8 of the p53 gene. The present tumor was composed mainly of morphologically mucinous bronchioloalveolar carcinoma, but presented different immunohistochemical profiles of ordinary mucinous bronchioloalveolar carcinoma. This case suggests that the mucinous component in bronchioloalveolar carcinoma of mixed mucinous and nonmucinous type has characters dissimilar to conventional mucinous bronchioloalveolar carcinoma, and is probably derived from the nonmucinous component.

腹部動脈瘤の手術のさいに偶然指摘される小さな嚢胞も含めると,腎嚢胞を合併する腹部動脈瘤は希ではないと思われるが,本邦での腎嚢胞あるいは嚢胞腎を伴った腹部大動脈瘤手術の報告はきわめて少ない.大きな腎嚢胞は動脈瘤の手術中に腎嚢胞が破裂・出血の報告もあり,さらに手術操作に支障をきたすこともあり,なんらかの対策が必要である.今回,77歳,男性の腹腔を圧迫する両側巨大多発性腎嚢胞を合併した腹部動脈瘤に対して,術前に巨大な両側腎嚢胞を超音波ガイド下に経皮的に穿刺吸引を行い約1,550mlを除去した.3日後,5.2cmの大動脈瘤と3.0cmの左総腸骨動脈瘤,2.6cm右総腸骨動脈瘤に対して腹部正中切開,腹膜外到達法で動脈瘤切除+Y字型人工血管置換術を施行した.術後経過は順調で,巨大な嚢胞性腎疾患を合併する腹部動脈瘤に対し経皮的穿刺内容液吸引は有用な方法であった.

われわれは人工血管感染症例に対して自己下肢深部静脈である浅大腿静脈をグラフトとする非解剖学的再建を2例経験し良好な結果を得た.症例1は75歳,男性.間歇性跛行を主訴とする閉塞性動脈硬化症例に対し,人工血管による大腿-大腿動脈交叉型血行再建を行った.3ヵ月後に局所の発赤と腫脹をきたし,同部に細菌を認めた.症例2は72歳,男性.瘤径5cmの腹部大動脈瘤に対し,Y字人工血管置換術を施行,術後10日目より持続する高熱で発症し,菌血症を認めた.これら保存的加療に抵抗性の2例の人工血管感染に対し,自家の浅大腿静脈による非解剖学的血行再建を行った.ともに大腿深静脈は温存されており,浅大腿静脈採取による静脈鬱滞などの合併症は発症せず,感染は消失しその後経過良好のため退院となった.人工血管感染時の自家組織を用いた血行再建に浅大腿静脈が有用であることを経験し文献的に考察した.

A 76-year-old man was admitted to our hospital for investigation of an apparent abdominal aortic aneurysm detected during treatment for epididymitis. A chest X-ray showed miliary shadows in the bilateral lung fields strongly suggestive of tuberculosis. The diameter of the aneurysm increased, and examinations showed impending rupture of a pseudoaneurysm. However, a definitive disease pathogenesis was not obtained before surgery. We performed a subemergency operation, which revealed an infrarenal abdominal pseudoaneurysm caused by tuberculosis. The pseudoaneurysm appeared to have resulted from direct extension of tuberculous lymphadenitis to the aortic wall, which ruptured. We review 24 other cases of tuberculous aortic aneurysms surgically treated in Japan before 2004.

われわれは造影剤アレルギーならびに腎機能障害をもつ症例に対する炭酸ガス造影法の有用性を報告してきた.さらに血管病変の診断法としての炭酸ガス造影に留まらず,一歩進めて炭酸ガス造影による血管内治療(ステント内挿術)を造影剤アレルギー2例,腎機能低下1例,糖尿病を合併した1例の計4例の閉塞性動脈硬化症に血管内治療を行った.すべての症例において血管内治療で血流は改善し,炭酸ガス造影に起因する直接ならびに間接の合併症は認めず,術後経過は順調で全例退院となった.現在定期的に外来での追跡を行っているが異常を認めていない.

症例は47歳,女性.胸部不快感認め,慢性心不全の診断にて循環器内科へ入院した.保存的加療に抵抗性の原因不明の心嚢液貯留で,精査加療目的,心タンポナーデ解除および原因検索のため,当科転科となり胸腔鏡下心膜開窓術と心膜生検を行った.心嚢水に悪性所見はなく,組織学的検討では細静脈から毛細血管レベルの血管炎を呈し,リンパ球浸潤が主体で内皮障害をきたし, small vesssel vasculitisと診断された.術後約1年以上経過しているが,心嚢液の再貯留ならびに心不全症状を認めていない.胸腔鏡下心膜開窓術は診断および治療的意義の両面を持ち合わせており,術前診断不可能な症例に対しても有用である.

A 60-year-old male underwent radical operation for esophageal cancer 45 days prior to complaining of several incidents of hemoptysis. The hemoptysis was found to be caused by infectious aneurysm of the descending thoracic aorta penetrating the lung. The aneurysm was resected and the aortic wall was sutured directly under percutaneous circulatory pulmonary support system. The sutured thoracic aorta was wrapped with the pedicle of an intercostal muscle flap to prevent reinfection. Forty-eight days after the aortic wall suture operation, however, the patient experienced massive hemoptysis and went into shock. Angiography was reveal no arterial lesions, so emergency left lower lobectomy was performed on suspicion of lung vessel rupture. Immediately after the lower lobectomy, recurrence of the aortic wall rupture caused uncontrollable bleeding. The patient died intraoperatively.

馬蹄腎を合併した腹部大動脈瘤の3症例を経験した.CTでは3例とも大動脈の腹側に騎乗するように馬蹄腎の峡部が認められた.また,MDCTやMRIで異所性腎動脈の分岐形態が明らかとなった.3例とも馬蹄腎峡部を切離することなく手術が可能で,症例1と2では異所性腎動脈の再建を行った.手術に際しては,馬蹄腎峡部の切離の是非や,異所性腎動脈の処置が問題となる.最終的には術中に判断を下さざるをえないことがまれではないので,術前に異所性腎動脈の分岐形態や,馬蹄腎峡部と動脈瘤の関係について,血管造影に加えて3D-CTを用いた十分な検索が必要である.

症例は46歳,女性.平成13年3月,咳嗽,発熱を主訴として来院.胸部X線写真で右下葉に肺炎の所見を認めた.肺炎と診断して加療のため入院した.幼少時より感染を繰り返していたとのことで精査を進め,検査の結果Pryce I型肺内肺分画症を疑った. MRI, DSAで異常血管を同定し,更に肺動脈との相互関連を検討する目的で肺動脈と異常血管の同時造影を行い,分画部から正常肺静脈への還流を認めた.感染の沈静化後の平成13年6月手術目的に再入院した.分画部より肺静脈への還流の情報を得ていたため, VATSでの分葉は危険と判断して開胸した.約1cmの異常動脈を可及的に大動脈近くで刺入結紮切離した.その後右下葉切除を行った.経過良好で術後17日目に退院し,その後肺の感染症状をきたしていない.肺動脈と異常血管の同時造影が肺内分画部の切離時に有用であった症例を経験した.

症例は53歳, 男性.1998年2月, 前医で胸腔鏡下の肺生検で, 非特異性間質性肺炎II群と診断.ステロイド加療中に胃癌が指摘され, 1999年9月胃全摘術を受けた.2001年3月, 左肺上葉の異常陰影の気管支鏡下生検で悪性と判明した.腹部手術後の通過障害や腸閉塞など3回の手術を繰り返していたため手術を望まず.2001年10月当院消化器外科での腸閉塞症の加療の際に再度胸部異常陰影指摘.陰影の急速な増大を認めたため, 今回手術を希望した.2001年11月7日右肺上葉切除+ND2a (pT2N1M0, Stage II B) を施行した.切除組織より通常型間質性肺炎と診断した.術後経過は良好で問質性肺炎の増悪もなく退院した.血清KL-6とSP-Dを指標として経過観察中, 食欲低下のため消化器外科へ入院.肺癌の肝転移と判明, 動注療法と化学療法を行った.次第に呼吸困難が増悪し, 2002年10月11日死亡した.間質性肺炎に胃癌と肺癌が発症した興味ある症例を経験した.

慢性腎不全による透析患者に石灰沈着が著しい血管を認める事があるが, 非透析者でも全周性管状石灰化を伴った大動脈いわゆるporcelain aortaを経験する.Porcelain aortaは硬く脆弱なため, 心臓手術や大血管手術において, 大動脈遮断など大動脈自体の手術操作が困難な場合がある.たとえocclusion balloonなどにより血流が遮断できたとしても, 血管壁が硬くて吻合が困難な場合があり, porcelain (unclampable) aortaは心臓血管外科医にとって課題となっている.今回我々は57歳, 男性の透析患者のporcelain aortaに浸潤した左上葉の大細胞癌を経験した.Porcelain aortaのため大動脈の遮断, 側遮断, 浸潤部の切除, 人工血管置換が困難で結果的に非根治の左上葉切除術となった.このため術後60Gyの放射線療法を行い, 1年8ヵ月現在, 局所再発, 胸水, 肺内転移, 骨・脳などへの遠隔転移も認めず生存中である.

A 63 year-old female presented with dyspnea on exertion. Her chest X-ray showed cardiomegaly, and right ventricular overload and tricuspid regurgitation were detected. Her pulmonary ventilation and blood flow scintigraphy findings were suspicious of pulmonary vascular disease; the diagnosis was pulmonary hypertension and bilateral branch pulmonary artery stenosis. After the inflammation settled, the stenotic bilateral branch pulmonary artery was reconstructed with a prosthetic vessel and the pulmonary pressure normalized immediately. A resected specimen revealed that the stenotic changes were from Takayasu's disease. The patient's postoperative course was uneventful, and pulmonary ventilation and blood scintigraphy returned to an almost normal range. At follow-up 5 years and 6 months after the operation, there was no evidence of pulmonary artery disease (eg, stenosis and/or ischemia) or of any change in the central vessels of the retina, the so-called Takayasu's retinopathy.

心嚢液貯留による心タンポナーデに対して,剣状突起下心嚢穿刺や心膜切開が行われるが,再発も多く不確実である.心膜を切除する開窓術は確実性が高いが全身麻酔による開胸操作を要し,侵襲が大きい.われわれは利尿剤投与などの内科的治療に反応が乏しく,数回にわたる心嚢穿刺の施行によっても心嚢液貯留を認める高齢者の難治性心タンポナーデ症例に対して胸腔鏡下心膜開窓術を施行し良好な結果を得た.胸腔鏡下心膜開窓術は分離肺換気による全身麻酔を必要とするが低侵襲で患者の回復も早く,良性の心タンポナーデのみでなく,悪性心嚢液貯留による心タンポナーデの姑息的治療としても施行できる方法と思われた.

症例は肺結核の既往のある77歳,男性.胸部X線写真で右肺野の異常を指摘された.胸部CTで右胸腔内に横隔膜上腫瘤と他に右下葉に小結節を認めた.腫瘤は下葉とのつながりはなく完全に被包され,内容物はほとんど壊死に陥っていた.胸腔内壊死性腫瘤は胸腔内異所性胸腺腫の被包化壊死を疑ったが,壊死組織の中にCD3 (T細胞表面抗原)陽性の集簇巣を認めるに留まり,胸腺組織が同定できなかった.同時に行った右下葉小結節の肺部分切除で乳頭型腺癌と判明し,下葉切除を行った.

大動脈弁狭窄兼閉鎖不全症に対する大動脈弁置換術後1年6ヵ月の64歳, 男性.退院後の通院中に胸部異常陰影を指摘され, 経過観察中に陰影の増大を認めた.症状はなく, 血液検査に異常なく, 胸部CTで縦隔に7cmの腫瘤と67Gaシンチで同部に強い集積を認めた.抗凝固剤をワーファリンよりヘパリンに代えてVATSを行った.術中病理で悪性腫瘍を疑い, 胸骨正中切開に変更した.左無名静脈を一時的に切離, 腫瘤を完全切除した後再建した.組織病理・組織免疫学的検討で転移腫瘍は否定され, 原発性胚細胞腫 (精上皮腫) と判明し, 浸潤・転移はなかった.術後経過は順調で退院し, 約1ヵ月後にBEP療法 (Cisplatinum, Etoposide, Bleomycin) を施行した.白血球数400/μl (好中球20%) の骨髄抑制が生じたが, G-CSF投与により正常化して退院した.現在元気に通院中である.

We report a case of a 70-year-old male smoker with a single primary tumor 2.5 x 3.0 cm in size in the right lung lower lobe. A transbronchial lung biopsy revealed squamous cell carcinoma of the lung. We performed right lower lobectomy with lymph node dissection (ND2a). The resected specimen consisted of three different cell types; small cell carcinoma, adenocarcinoma and squamous cell carcinoma (in a ratio of 70: 20: 10). Each cancer cell types had metastasized to different lymph nodes. The final diagnosis was a combined small cell carcinoma in the lung. Combined small cell carcinoma is uncommon, but is nevertheless a well-described diagnostic category in lung cancers.

検診で胸部異常陰影を指摘された18歳,女性.精査の結果,縦隔の低位傍椎体部に約3椎体の長径,肝臓・横隔膜に接してはいるが亜鈴型ではない後縦隔腫瘍が疑われた.神経原性腫瘍を疑い血液生化学的検査を行った. 131I-MIBGの淡い集積は認めたが,カテーコーラミンやステロイドの分泌はなく,非内分泌性腫瘍と考えた.低位後縦隔の腫瘍に対しより広い術野の確保を目的に,経腹経横隔膜アプローチを選択した.同法にて腫瘤を完全に切除した.腫瘍は神経節細胞腫であり悪性所見は認めなかった.術後経過は順調で術後19病日に退院した.

A 62-year-old male had received a right upper and middle lobectomy for pseudolymphoma at another hospital 14 years before. During his post-operative follow up, a shaggy shadow was found in his lower left lung. This shadow progressed gradually, repeated chest CTs and transbronchial lung biopsies did not yield a definitive diagnosis. He was referred to our hospital for diagnosis and treatment.We performed VATS for biopsy and obtained several specimens from the left lung. We extracted the DNA and amplified with PCR these surgical specimens and the specimen resected and paraffined 14 years before. We analyzed the genes by Southern blot hybridization, and detected a rearrangement of the immunoglobulin (Ig H) gene but no T cell receptor (TCR γ) gene, leading to a diagnosis of bronchus-associated lymphoid tissue (BALToma). After obtaining a definitive diagnosis, we administered chemotherapy twice consisting of purine analogue, fludarabine (regimen for chronic leukemia) and achieved a marked effect.

稀な孤立性気管支乳頭腫の1例を経験した.症例は19歳, 男性.検診にて右肺の異常陰影を指摘されたが気管支鏡生検では確定診断は得られなかった.陰影の増大と末梢の閉塞性肺炎を呈していたため確定診断の目的もあり手術を行った.分葉不全と気管支内の腫瘤の位置確認のため, 胸腔鏡下手術に前胸壁の小開胸を追加し, 中葉切除を行った.切除標本ではB4, B5入口部に乳頭腫が存在した.末梢は粘液で充満されており, その中に乳頭腫細胞塊が浮遊し, 一部に肺胞上皮へ生着しており, 移植の可能性が示唆された.中葉切除術式の妥当性を確認した.さらに腫瘍細胞のDNAをPCR法で増幅しhuman papilloma virusの関与を検索したが関与は否定された.

症例は21歳,男性.検診の胸部写真で異常を指摘された.次第に呼吸困難出現,酸素吸入,仰臥位不能となった.強い胸背部疼痛のため麻薬系鎮痛剤を要した.経胸壁的針生検で胸腺腫を疑った.化学療法予定であったが,急速に腫瘤が拡大,症状が増悪し,緊急手術となった.胸腔・心嚢内に多量の褐色胸水を認めた.腫瘍は右胸腔全体を占め,縦隔・心嚢に浸潤しており,病期はIIIと診断された.腫瘍の一塊切除は困難で心膜合併切除を伴う分割切除を行った.腫瘍は総重量2480gで肉眼的に完全切除した.腫瘍の主体はtype B2, 浸潤部はtype B3の胸腺腫であった.この胸腺腫gelatinase活性は極めて高く急速に拡大浸潤した原因と推察した.術前の心タンポナーデ症状は術直後より消失した.術後経過良好で,呼吸困難・疼痛の消失,食欲の出現,体重増加を得た.後療法のため術後約1カ月で呼吸器内科へ転科した.

症例は54歳男性.53歳時の受傷直後の軽症の血気胸に保存的治療 (穿刺吸引) を行い紹介元へ転医した.4日後に突然大量の血胸が生じたため, 持続吸引ドレナージを行い約1ヵ月で退院となった.受傷1年後に横行結腸と大網が嵌頓する遅発性外傷性横隔膜ヘルニア発症を経験した.手術所見より肺の一部がヘルニア栓となって腹腔臓器の滑脱を防いでいたが, ヘルニア栓が外れて遅発性外傷性横隔膜ヘルニアが発症したものと考察した.受傷時, 4日後の横隔膜裂傷時, ヘルニア栓による無症状時, 横隔膜ヘルニア閉塞絞扼時のそれぞれ時期の状態を画像上比較検討し, 横隔膜損傷の手術適応と遅発性外傷性横隔膜ヘルニアの発症機序に関し考察した.

59歳男性, 未分化型腺癌, stage IBに対して左下葉切除+R2a郭清を施行した.術後10日目に腸閉塞症状を呈したが, 13年前の腹部手術後の癒着性イレウスと考え, 保存的療法を行った.腹部CTでは腫瘤は不明で, 軽快再発を繰り返し, 術後約50日目に緊急手術を施行した.癒着は軽度で, Treitz靱帯より7cm肛門側に管腔内の腫瘤と, 回腸端より20cm口側の腫瘤による小腸腸重積であった.小腸転移による腸重積症で気付かれた肺癌の早期遠隔転移症例を経験した.

2-[Fluorine-18]fluoro-2-deoxy-d-glucose (FDG) uptake within the primary lesion correlates with survival on positron emission tomography (PET) studies of patients with non-small cell lung cancer. The more metabolically active the tumour, the worse the outcome. The aim of this study was to determine whether a correlation exists between aggressiveness as determined by pathology and the findings of FDG PET in pulmonary adenocarcinoma. Thirty-five patients with 38 adenocarcinomas of the lung were studied. All patients underwent thoracotomy within 4 weeks of the FDG PET study. Background . This clinical study evaluated changes in motor evoked potentials (MEP) elicited by direct cerebral cortical stimulation and evoked spinal cord potentials (ESCPs) elicited by direct spinal cord stimulation during selective intercostal arterial perfusion for thoracoabdominal aortic aneurysm (TAAA) repair. We also determined the efficacy of this perfusion method for prevention of paraplegia. Methods . Two kinds of ESCPs and MEPs were monitored during the prosthetic replacement step for TAAA surgeries. We performed selective intercostal arterial perfusion from the T7 intercostal artery to the L1 intercostal artery through a small piece of Dacron graft while monitoring spinal cord potentials in five cases of TAAA. Results . The MEP amplitude decreased after clamping the aorta but quickly recovered after selective perfusion of intercostal arteries. Other spinal cord potentials did not change during the reconstruction of intercostal arteries. Postoperative paraplegia or parapalesis did not occur in any of the patients. Conclusions . Monitoring of MEPs during selective intercostal arterial perfusion was a useful adjunct to prevent postoperative paraplegia in TAAA surgery.

サルコイドーシスは炎症性肉芽腫性病変を主病変とし, 悪性新生物の発生頻度が高いとされる.症例は66歳の男性で, 心房細動に対するMaze手術を4年前に受け, サルコイドーシスと診断されていた.術後4年目に右肺上葉に睡瘤が指摘され, TBLBで扁平上皮癌と診断された.CTで縦隔・肺門リンパ節腫大を認め, サルコイドーシスによる変化も否定出来ないが, 転移が疑われた.咳嗽・発熱などの症状強く, 手術を施行したが, 術前病期はstage IIIBであった.術後病理病期はstage IBで, 腫大したリンパ節はすべてサルコイドーシスによるものであった.炎症性肉芽腫病変に生じた悪性腫瘍の病期診断には画像診断に加えてリンパ節生検が必要と考える.

症例は72歳の男性.63歳時と64歳時の度重なる脳梗塞の進行により, 歩行が困難となっていた.次第に仰臥位困難となり, 近医にて縦隔・両側胸腔に広がる巨大腫瘤を指摘された.心肺圧迫症状を来たし, 緊急手術を行った.仰臥位での気管内挿管が不能で, 右側半坐位での挿管, 直ちに仰臥位, 胸骨正中切開を行った.血圧は一時的に低下したが, 圧迫が解除となり血圧は回復した.腫瘤は32×27×6cm大で, 縦隔・左右胸腔に連続的に広がり, 充実性腫瘍と結合組織・脂肪組織から成り, 一部出血壊死を伴っていた.腫瘤内には最大径10×8×6cm大の比較的軟らかな球状結節を十数個認めた.CD34, Desmin, Vimentin, NSE, S-100, AE1/AE3, α-smooth muscle actinなどの免疫組織化学的染色でSolitary Fibrous Tumorと診断された.皮膚のcafe-au-lait spot部と皮下結節の病理組織的検索を行い術前に疑われたvon Recklinghausen病と合致した.術後経過は良好で術後24病日に転院となった.転院後1ヵ月で退院したが, 1年後脳梗塞発生時にSFTの胸腔内再発が指摘され術後1年4ヵ月で死亡した.

A case of thoracoscopic resection of a esophageal duplication cyst is reported. An 18-year-old woman was seen at the hospital because of epigastralgia, Chest roentgenogram and CT scan revealed a mediastinal tumor. Chest CT showed a 3cm in diameter cystic tumor at the left side of the lower esophagus. Endoscopic ultrasonography was suggestive of esophageal cyst, therwfore, thoracoscopic resection of the tumor was performed. To prevent recurrence, the residual mucosa was cauterized as well. The tumor contained viscosous and mucinous mucus. The postoperative pathologic diagnosis was esophageal duplication cyst. Thoracoscopic resection is thought to be necessary for differential diagnosis of cysts of unknown origins, and that because the esophageal duplication cyst is a benign tumor, it is a good indication for thoracoscopic resection.
