Direct oral anticoagulants (DOACs) are widely used for the prevention of ischemic stroke and systemic embolism in patients with nonvalvular atrial fibrillation (NVAF). However, the differences in safety and effectiveness among four DOACs, dabigatran, rivaroxaban, apixaban, and edoxaban, in Japanese patients have not been clarified. Therefore, we conducted a retrospective cohort study to directly compare the safety and effectiveness among the four DOACs using the Japan Medical Data Center (JMDC) claims database. We identified 3823 patients with NVAF who started receiving a DOAC between March 2011 and June 2017. The safety outcome was major bleeding (a composite outcome of intracranial, gastrointestinal, respiratory, or renal/urinary tract bleeding) and the effectiveness outcome was the composite of ischemic stroke including transient ischemic attack (TIA) or systemic embolism. We constructed a Cox proportional hazard model to calculate the hazard ratio (HR) for all four DOAC combinations. The risk of major bleeding was significantly lower in the dabigatran group than in the apixaban group (HR, 0.55; 95% confidence interval (CI), 0.31-0.93; p = 0.03). In contrast, there was no significant difference in the risk of major bleeding among the other DOACs. In the composite risk of ischemic stroke including TIA or systemic embolism, there was no significant difference among the four DOACs. This study suggested that in the current use of DOACs in Japanese patients with NVAF, dabigatran had a significantly lower risk of major bleeding than apixaban, but there was no significant difference in effectiveness among the four DOACs.

A 75-year-old woman underwent sigmoid colon resection and transverse colostomy for perforation of the diverticulum of the sigmoid colon at 70 years of age at another hospital. She was referred to our hospital with complaints of abdominal discomfort 3 months prior to presentation. Abdominal computed tomography revealed a parastomal hernia (PSH). We performed laparoscopic repair using the Sugarbaker approach with a Symbotex Composite Mesh™ and laparoscopic adhesive intestinal repair. The patient’s post-operative course was unremarkable, and she was transferred to the Department of Internal Medicine after 10 days. There was no recurrence 6 months after surgery. Tension-free surgery using a mesh has been reported to be effective in preventing the recurrence of PSH. We performed a laparoscopic modified Sugarbaker mesh method using the Symbotex Composite Mesh™ with collagen film to repair an abdominal hernia.

In this multi-messenger astronomy era, all the observational probes are improving their sensitivities and overall performance. The Focusing on Relativistic universe and Cosmic Evolution (FORCE) mission, the product of a JAXA/NASA collaboration, will reach a 10 times higher sensitivity in the hard X-ray band ($E >$ 10~keV) in comparison with any previous hard X-ray missions, and provide simultaneous soft X-ray coverage. FORCE aims to be launched in the early 2030s, providing a perfect hard X-ray complement to the ESA flagship mission Athena. FORCE will be the most powerful X-ray probe for discovering obscured/hidden black holes and studying high energy particle acceleration in our Universe and will address how relativistic processes in the universe are realized and how these affect cosmic evolution. FORCE, which will operate over 1--79 keV, is equipped with two identical pairs of supermirrors and wideband X-ray imagers. The mirror and imager are connected by a high mechanical stiffness extensible optical bench with alignment monitor systems with a focal length of 12~m. The aims of the present study were to establish a population pharmacokinetic (PPK) model of cefazolin for adult patients undergoing cardiac surgery with cardiopulmonary bypass (CPB) and to assess the probability of target attainment (PTA) for the prophylaxis of surgical site infection (SSI) using cefazolin. Adult patients who underwent cardiac surgery with CPB were enrolled in the prospective study. Blood samples for plasma cefazolin assay were collected, and total and unbound drug concentrations were measured and analysed using the nonlinear mixed-effects modelling (NONMEM) software considering saturable plasma protein binding. Using the PPK model, plasma unbound cefazolin concentration-time courses with current prophylaxis protocols were simulated, and the PTA for common SSI pathogens was estimated. A total of 199 blood samples were obtained from 27 patients. A one-compartment model with first-order elimination plus an on/off CPB compartment best described the data. The population mean for systemic drug clearance (CL) was reduced and that for the volume of distribution (V) was increased during CPB compared with the pre-CPB values. CPB-induced hypoalbuminemia was associated with reduced maximum protein binding (Bmax). The simulation studies suggested that the current dosing protocols are insufficient for attaining PTA > 0.9 throughout surgery against pathogens with minimum inhibitory concentrations (MICs) >8 mg/L. A new dosing protocol that achieves a PTA > 0.9 for pathogens with a MIC of 16 mg/L was proposed. PPK modelling with simulation may be valuable for devising a cefazolin prophylaxis protocol for patients undergoing cardiac surgery with CPB.

M87 is a radio galaxy and it was detected in the TeV gamma-ray band, but its emission and particle acceleration region are still under debate. We searched for fast X-ray variability of M87 by using X-ray archive data with a long exposure than 10 ksec. As a result, we found an intraday variability during Suzaku/XIS data in 2006. Suzaku/XIS cannot resolve each component in the jet, but HST-1 knot standing at $\sim$100 pc from the core was the brightest component in the X-ray band in this period. Therefore, this variability is likely attributed to HST-1. A soft photon index of 2.38 in the X-ray band indicates that variability component is synchrotron emission by accelerated electrons in HST-1. Assuming the decay time scale as correspond to synchrotron cooling time, we estimated a magnetic field strength to $B\sim1.94\delta^{1/3}$ mG, which is enough to accelerate particles up to TeV, where $\delta$ is a Doppler factor. In addition, we also found a possible variability of the core during the Chandra/HRC observation in 2017. In this period, a NuStar X-ray spectrum was represented by a power law with a photon index of 1.96. Therefore we could not distinguish this fast variability is due to synchrotron emission or disk/corona emission. From these results, both of the core and HST-1 are could be the origin of the X-ray variability, and we found that knot HST-1 is a candidate of the location of particle acceleration up to TeV.

The Large-Sized Telescope (LST) prototype of the future Cherenkov Telescope Array (CTA) is located at the Northern site of CTA, on the Canary Island of La Palma. It is designed to provide optimal performance in the lowest part of the energy range covered by CTA, observing gamma rays down to energies of tens of GeV. The LST prototype started performing astronomical observations in November 2019 during the commissioning of the telescope and it has been taking data since then. In this contribution, we will present the tuning of the characteristics of the telescope in the Monte Carlo (MC) simulations to describe the data obtained, the estimation of its angular and energy resolution, and an evaluation of its sensitivity, both with simulations and with observations of the Crab Nebula.

XRISM is an X-ray astronomical mission by the JAXA, NASA, ESA and other international participants, that is planned for launch in 2022 (Japanese fiscal year), to quickly restore high-resolution X-ray spectroscopy of astrophysical objects. To enhance the scientific outputs of the mission, the Science Operations Team (SOT) is structured independently from the instrument teams and the Mission Operations Team. The responsibilities of the SOT are divided into four categories: 1) guest observer program and data distributions, 2) distribution of analysis software and the calibration database, 3) guest observer support activities, and 4) performance verification and optimization activities. As the first step, lessons on the science operations learned from past Japanese X-ray missions are reviewed, and 15 kinds of lessons are identified. Among them, a) the importance of early preparation of the operations from the ground stage, b) construction of an independent team for science operations separate from the instrument development, and c) operations with well-defined duties by appointed members are recognized as key lessons. Then, the team structure and the task division between the mission and science operations are defined; the tasks are shared among Japan, US, and Europe and are performed by three centers, the SOC, SDC, and ESAC, respectively. The SOC is designed to perform tasks close to the spacecraft operations, such as spacecraft planning, quick-look health checks, pre-pipeline processing, etc., and the SDC covers tasks regarding data calibration processing, maintenance of analysis tools, etc. The data-archive and user-support activities are covered both by the SOC and SDC. We are designing a fleet of nanosatellites to perform accurate position determinations of short-duration gamma-ray bursts by measuring arrival time differences. To achieve sufficient photon statistics to measure the arrival times precisely under the severe limitations of size, mass and power consumption, we propose the use of large-area CsI(Tl) scintillators which provide a high light output and the use of a small-sized multipixel photon counters (MPPC), a kind of SiPM, that have low power consumption. We plan to use one of the latest-model MPPCs provided by Hamamatsu Photonics, namely, S13360-6050CS, which has an active area of 6 x 6 mm(2). We have compared the performance of two scintillators of different sizes (150x75x5 mm(3) and 100x75x5 mm(3)); the bigger one is the maximum size that can be mounted on a three-unit satellite, according to CubeSat standards. We have found that the light yield of the bigger scintillator is lower than that of the smaller one, but the difference is only approximately 13%, and each has an energy threshold of similar to 10 keV at 25 degrees C. We have tested one-and two-MPPC readout and confirmed the same energy thresholds. The light yield for the two-MPPC readout is 1.4 times as high as that for the one-MPPC readout. We have also examined the position dependence of the light yield by using radiation from a Am-241 (59.5 keV) source, and confirmed that uniformity was improved, and energy resolution got better by similar to 7% for two-MPPC readout.

We report a polarimetric constraint on the hard X-ray synchrotron jet emission from the Cygnus X-1 black-hole binary system. The observational data were obtained using the PoGO+ hard X-ray polarimeter in July 2016, when Cygnus X-1 was in the hard state. We have previously reported that emission from an extended corona with a low polarization fraction is dominating, and that the polarization angle is perpendicular to the disk surface. In the soft gamma-ray regime, a highly-polarized synchrotron jet is reported with INTEGRAL observations. To constrain the polarization fraction and flux of such a jet component in the hard X-ray regime, we now extend analyses through vector calculations in the Stokes QU plane, where the dominant corona emission and the jet component are considered simultaneously. The presence of another emission component with different polarization angle could partly cancel out the net polarization. The 90% upper limit of the polarization fraction for the additional synchrotron jet component is estimated as, 6 pages, 3 figures, accepted for publication in Monthly Notices of the Royal Astronomical Society Letters

In recent years the number of CubeSats (U-class spacecrafts) launched into space has increased exponentially marking the dawn of the nanosatellite technology. In general these satellites have a much smaller mass budget compared to conventional scientific satellites which limits shielding of scientific instruments against direct and indirect radiation in space. In this paper we present a simulation framework to quantify the signal in large field-of-view gamma-ray scintillation detectors of satellites induced by X-ray/gamma-ray transients, by taking into account the response of the detector. Furthermore, we quantify the signal induced by X-ray and particle background sources at a Low-Earth Orbit outside South Atlantic Anomaly and polar regions. Finally, we calculate the signal-to-noise ratio taking into account different energy threshold levels. Our simulation can be used to optimize material composition and predict detectability of various astrophysical sources by CubeSats. We apply the developed simulation to a satellite belonging to a planned CAMELOT CubeSat constellation. This project mainly aims to detect short and long gamma-ray bursts (GRBs) and as a secondary science objective, to detect soft gamma-ray repeaters (SGRs) and terrestrial gamma-ray flashes (TGFs). The simulation includes a detailed computer-aided design (CAD) model of the satellite to take into account the interaction of particles with the material of the satellite as accurately as possible. Results of our simulations predict that CubeSats can complement the large space observatories in high-energy astrophysics for observations of GRBs, SGRs and TGFs. For the detectors planned to be on board of the CAMELOT CubeSats the simulations show that detections with signal-to-noise ratio of at least 9 for median GRB and SGR fluxes are achievable., 86 pages, 30 figures, 8 tables, accepted for publication in Journal of Astronomical Telescopes, Instruments, and Systems

This paper introduces a second-generation balloon-borne hard X-ray polarimetry mission, XL-Calibur. X-ray polarimetry promises to give qualitatively new information about high-energy astrophysical sources, such as pulsars and binary black hole systems. The XL-Calibur contains a grazing incidence X-ray telescope with a focal plane detector unit that is sensitive to linear polarization. The telescope is very similar in design to the ASTRO-H HXT telescopes that has the world’s largest effective area above ~10 keV. The detector unit combines a low atomic number Compton scatterer with a CdZnTe detector assembly to measure the polarization making use of the fact that polarized photons Compton scatter preferentially perpendicular to the electric field orientation. It also contains a CdZnTe imager at the bottom. The detector assembly is surrounded by the improved anti-coincidence shielding, giving a better sensitivity. The pointing system with arcsecond accuracy will be achieved.

IGR J00370+6122 is a high-mass X-ray binary, of which the primary is a B1 Ib star, whereas the companion is suggested to be a neutron star by the detection of 346-s pulsation in one-off 4-ks observation. To better understand the nature of the compact companion, the present work performs timing and spectral studies of the X-ray data of this object, taken with XMM-Newton, Swift, Suzaku, RXTE, and INTEGRAL. In the XMM-Newton data, a sign of coherent 674 s pulsation was detected, for which the previous 346-s period may be the 2nd harmonic. The spectra exhibited the "harder when brighter" trend in the 1$-$10 keV range, and a flat continuum without clear cutoff in the 10$-$80 keV range. These properties are both similar to those observed from several low-luminosity accreting pulsars, including X Persei in particular. Thus, the compact object in IGR J00370+6122 is considered to be a magnetized neutron star with a rather low luminosity. The orbital period was refined to $15.6649 \pm 0.0014$ d. Along the orbit, the luminosity changes by 3 orders of magnitude, involving a sudden drop from $\sim 4 \times 10^{33}$ to $\sim 1\times10^{32}$ erg s$^{-1}$ at an orbital phase of 0.3 (and probably vice verse at 0.95). Although these phenomena cannot be explained by a simple Hoyle-Lyttleton accretion from the primary's stellar winds, they can be explained when incorporating the propeller effect with a strong dipole magnetic field of $\sim 5 \times10^{13}$ G. Silicon photomultipliers (SiPMs) have become the baseline choice for cameras of the small-sized telescopes (SSTs) of the Cherenkov Telescope Array (CTA). On the other hand, SiPMs are relatively new to the field and covering large surfaces and operating at high data rates still are challenges to outperform photomultipliers (PMTs). The higher sensitivity in the near infra-red and longer signals compared to PMTs result in higher night sky background rate for SiPMs. However, the robustness of the SiPMs represents a unique opportunity to ensure long-term operation with low maintenance and better duty cycle than PMTs. The proposed camera for large size telescopes will feature 0.05 degree pixels, low power and fast front-end electronics and a fully digital readout. In this work, we present the status of dedicated simulations and data analysis for the performance estimation. The design features and the different strategies identified, so far, to tackle the demanding requirements and the improved performance are described.

The timing-based localization, which utilize the triangulation principle with the different arrival time of gamma-ray photons, with a fleet of Cubesats is a unique and powerful solution for the future all-sky gamma-ray observation, which is a key for identification of the electromagnetic counterpart of the gravitational wave sources. The Cubesats Applied for MEasuring and Localising Transients (CAMELOT) mission is now being promoted by the Hungarian and Japanese collaboration with a basic concept of the nine Cubesats constellations in low earth orbit. The simulation framework for estimation of the localization capability has been developed including orbital parameters, an algorithm to estimate the expected observed profile of gamma-ray photons, finding the peak of the cross-correlation function, and a statistical method to find a best-fit position and its uncertainty. It is revealed that a degree-scale localization uncertainty can be achieved by the CAMELOT mission concept for bright short gamma-ray bursts, which could be covered by future large field of view ground-based telescopes. The new approach utilizing machine-learning approach is also investigated to make the procedure automated for the future large scale constellations. The trained neural network with 10$^6$ simulated light curves generated by the artificial short burst templates successfully predicts the time-delay of the real light curve and achieves a comparable performance to the cross-correlation algorithm with full automated procedures., Comment: 9 pages, 4 figures, Proceedings of SPIE "Astronomical Telescopes and Instrumentation" 2020

A.; Zech, A.; Zechlin, H.; Zhdanov, V., I; Zivec, M., The Cherenkov Telescope Array (CTA), the new-generation ground-based observatory for γ astronomy, provides unique capabilities to address significant open questions in astrophysics, cosmology, and fundamental physics. We study some of the salient areas of γ cosmology that can be explored as part of the Key Science Projects of CTA, through simulated observations of active galactic nuclei (AGN) and of their relativistic jets. Observations of AGN with CTA will enable a measurement of γ absorption on the extragalactic background light with a statistical uncertainty below 15% up to a redshift z=2 and to constrain or detect γ halos up to intergalactic-magnetic-field strengths of at least 0.3 pG . Extragalactic observations with CTA also show promising potential to probe physics beyond the Standard Model. The best limits on Lorentz invariance violation from γ astronomy will be improved by a factor of at least two to three. CTA will also probe the parameter space in which axion-like particles could constitute a significant fraction, if not all, of dark matter. XL-Calibur is a balloon-borne hard X-ray polarimetry mission, the first flight of which is currently foreseen for 2021. XL-Calibur carries an X-ray telescope consists of consists of 213 Wolter I grazing-incidence mirrors which are nested in a coaxial and cofocal configuration. The optics design is nearly identical to the Hard X-ray Telescope (HXT) on board the ASTRO-H satellite. The telescope was originally fabricated for the Formation Flying Astronomical Survey Telescope (FFAST) project. However, the telescope can be used for XL-Calibur, since the FFAST project was terminated before completion. The mirror surfaces are coated with Pt/C depth-graded multilayers to reflect hard X-rays above 10 keV by Bragg reflection. The effective area of the telescope is larger than 300 cm^2 at 30 keV. The mirrors are supported by alignment bars in the housing, and each of the bars has a series of 213 grooves to hold the mirrors. To obtain the best focus of the optics, the positions of the mirrors have to be adjusted by tuning the positions of the alignment bars. The tuning of the mirror positions is conducted using the X-ray beam at the synchrotron facility SPring-8 BL20B2, and this process is called optical tuning. First the positions of the second reflectors are tuned, and then those of the first reflectors are tuned. We did the first optical tuning in Jan 2020. The second tuning will be planned between April to July, 2020. This paper reports the current status of the hard X-ray telescope for XL-Calibur.

The XRISM is the astronomical mission to perform the high-resolution X-ray spectroscopy of astrophysical objects using the micro-calorimeter array. In order to enhance the scientific outputs of the mission, the science operations team (SOT) is structured with responsibilities of the spacecraft planning, data processing and distributions, development and maintenance of analyses software and calibration database, and users’ supports. The operation concepts are established based on lessons learned from past X-ray missions. The SOT consists of the Science Operations Center at JAXA and the Science Data Center at NASA. Details of science operations plan and preparation status on SOC are summarized.

All-sky surveys are crucial to discover transient objects. In reality, however, it is impossible to achieve high sensitivity, high cadence, wide sky coverage, and broad wavelength range at the same time. This is where observations with small telescopes can come in significant, as small telescopes often can make high cadence monitoring and flexible operations, playing a complementary role to large observatories. We plan to launch a new 6U-size CubeSat X-ray observatory, NinjaSat, in 2022 to conduct a flexible X-ray observation program. The satellite is equipped with two identical non-imaging Gas Multiplier Counters (GMCs) sensitive to X-rays in the 2–50 keV band with a total effective area of 36 cm2 at 6 keV. Coupled with X-ray collimators of a 2.1° field-of-view, NinjaSat is suitable for flexible multi-wavelength coordinated observations of bright (g10 mCrab) X-ray sources with particular emphasis on their time variability. An example of our targets is one of the brightest celestial X-ray objects, Scorpius X-1, which hosts a fast-spinning neutron star and is a candidate source for coherent gravitational waves. The quasi-periodic oscillation (QPO) of neutron-star systems is considered to carry important information on the neutron star’s rotational frequency, which is useful for sensitive gravitational-wave searches. Scorpius X-1, being one of the brightest, provides the best opportunity to study the QPO. Combining with coordinated simultaneous monitoring observations with recently-developed fast optical photometry, the mechanism of the mass accretion of the disk can also be studied. We plan to use NinjaSat also for space science education, particularly X-ray astronomy, for students and the general public.

The X-Ray Imaging and Spectroscopy Mission, XRISM, is scheduled to launch in 2022, with the goal of building on the brief successes of the ASTRO-H (Hitomi) mission, and recovering the prime science objective to solve outstanding astrophysical questions using high resolution X-ray spectroscopy. The XRISM Science Operations Team (SOT), consists of the JAXA-led Science Operations Center (SOC) and NASA-led Science Data Center (SDC) that work together to optimize the scientific output from the Resolve high-resolution spectrometer and the Xtend wide-field imager through planning and scheduling observations, processing and distribution of data, development and distribution of software tools and the calibration database (CaldB), user support, and support of ground and in-flight calibration. Here, we summarize the roles and responsibilities of the SDC, and the current status and future plans, covering scheduling software, software and CalDB production and release, data transmission and processing pipeline, and simulation and other post-pipeline analysis tools. Resolve poses particular challenges due to its unprecedented combination of high spectral resolution and throughput, broad spectral coverage, and relatively small field-of-view and large pixel-size; and, we highlight those challenges.

CFRP is a composite material composed of carbon fiber and resin. CFRP is commonly applied to the aerospace industry which requires lightweight and intensity. Thanks to superior formability of CFRP, we can form shape of Wolter-1 optics, which consists of paraboloid and hyperboloid, to a monolithic substrate. Since the surface roughness of a CFRP substrate is a few µm, we have to make the smooth surface for reflecting X-rays on the CFRP substrate. We have developed a new method of shaping the reflective surface instead of the replica method used in lightweight X-ray mirrors such as Astro-H. In the new method, the reflective surface is formed by pasting thin sheet-glasses with 100 µm thick onto the CFRP substrate. The thin sheet-glass has a surface roughness about 0.4 nm as measured by Zygo. We fabricated a CFRP mirror pasting thin sheet-glasses, and then coated tungsten on the mirror in June 2020. The figure error (s) of the CFRP mirror was achieved to be about 1-2 μm by stacking the CFRP mirror on the housing module. X-ray imaging quality of the CFRP mirror was measured at Spring-8 in July 2020. The half-power diameter of the CFRP mirror was estimated to be about 150 arcsec, which was nearly equal to the prediction from a distribution of the slope error deduced from the surface profile. We describe a future plan to improve the image quality of the CFRP mirror.

Study Objective: Telemedicine (TM) for continuous positive airway pressure (CPAP) treated patients may save health-care resources without compromising treatment effectiveness. We assessed the effect of TM (AirView Online System, ResMed) during the CPAP habituation phase on 3-month and 1-year treatment adherence and efficacy in patients with moderate-to-severe obstructive sleep apnea (OSA).Methods: At CPAP initiation, 120 patients diagnosed with OSA were randomized to either usual care (UC) or TM during the habituation phase (clinical registration: ISRCTN12865936). Both groups received a first face-to-face appointment with a sleep care giver at CPAP initiation. Within the following month, 2 other physical visits were scheduled in the UC group whereas two phone consultations were planned in the TM group, in which CPAP parameters were remotely adapted. Additional physical visits were programmed at the patient's request. Face-to-face consultations were scheduled at 3 and 12 months after CPAP initiation. The primary outcome was the mean CPAP daily use over the course of 12 months.Results: Twenty of 60 patients stopped CPAP therapy in the UC group vs. 14 of 60 in the TM group (p = 0.24). In per protocol analysis, mean [95% CI] daily CPAP use among 86 patients still using CPAP at 12 months was 279 [237; 321] min in the 38 patients on UC and 279 [247; 311] min in the 43 patients on TM, mean difference [95% CI]: 0 [−52; 52] min, P = 0.99. Total consultation time per patient was not different between groups, TM: 163 [147; 178] min, UC: 178 [159; 197] min, difference: −15 [−39; 9] min, p = 0.22.Conclusions: Telemedicine during the CPAP habituation phase did not alter daily CPAP use or treatment adherence and did not require more healthcare time. Telemedicine may support clinic attendance for CPAP titration.Clinical Trial Registration: [ISRCTN], identifier [ISRCTN12865936].

BACKGROUND Both tacrolimus (TAC) and fentanyl are frequently used in patients receiving allogeneic hematopoietic stem-cell transplantation. A recently published report demonstrated that fentanyl can reduce the total body clearance of TAC; however, most patients in this study were administered concomitantly with azole antifungal agents, which are known to be strong inhibitors of CYP3A. Hence, the exact effect of fentanyl on TAC pharmacokinetics was unclear. In the current study, the authors retrospectively investigated the pharmacokinetic interaction between TAC and fentanyl in patients who were not concomitantly administered drugs that affect TAC metabolism. METHODS Patients with continuous infusion of TAC and fentanyl after hematopoietic stem-cell transplantation at the Tokyo Medical and Dental University between January 2014 and December 2018 were enrolled. The total body clearance of TAC was compared before and after the initiation or discontinuation of fentanyl. RESULTS Thirty patients (24 men and 6 women; median age, 11 years) were screened for their eligibility. Twenty-eight patients were enrolled for evaluating the effects of the fentanyl initiation on TAC pharmacokinetics; 2 patients were excluded because of the absence of data related to the TAC blood concentrations or the concomitant use of azole antifungals. Twenty patients were enrolled for investigating the effects of fentanyl discontinuation on TAC pharmacokinetics, whereas 10 patients were excluded because of the absence of data related to the blood concentration of TAC or the additional administration of azole antifungals. Furthermore, the total body clearance of TAC was not significantly affected by the initiation or discontinuation of fentanyl, although there were large interindividual variations. In addition, the results remained the same even when the analysis was performed independently for adults and children. CONCLUSIONS Intravenous infusion of fentanyl does not affect the pharmacokinetics of TAC.

The evolved, massive highly eccentric binary system, η Car, underwent a periastron passage in the summer of 2014. We obtained two coordinated X-ray observations with XMM-Newton and NuSTAR during the elevated X-ray flux state and just before the X-ray minimum flux state around this passage. These NuSTAR observations clearly detected X-ray emission associated with η Car extending up to ~50 keV for the first time. The NuSTAR spectrum above 10 keV can be fit with the bremsstrahlung tail from a kT ~6 keV plasma. This temperature is ΔkT ~2 keV higher than those measured from the iron K emission line complex, if the shocked gas is in collisional ionization equilibrium. This result may suggest that the companion star’s pre-shock wind velocity is underestimated. The NuSTAR observation near the X-ray minimum state showed a gradual decline in the X-ray emission by 40% at energies above 5 keV in a day, the largest rate of change of the X-ray flux yet observed in individual η Car observations. The column density to the hardest emission component, N(H) ~10(24) H cm(−2), marked one of the highest values ever observed for η Car, strongly suggesting the increased obscuration of the wind-wind colliding X-ray emission by the thick primary stellar wind prior to superior conjunction. Neither observation detected the power-law component in the extremely hard band that INTEGRAL and Suzaku observed prior to 2011. The power-law source might have faded before these observations.

Cu, Zn superoxide dismutase (SOD1) is one of the genes implicated in the devastating neurodegenerative disorder amyotrophic lateral sclerosis (ALS). Although the precise mechanisms of SOD1 mutant (SOD1(mut))-induced motoneuron toxicity are still unclear, defects in SOD1 proteostasis are known to have a critical role in ALS pathogenesis. We previously reported that the SOD1(mut) adopts a conformation that exposes a Derlin-1–binding region (DBR) and that DBR-exposed SOD1 interacts with Derlin-1, leading to motoneuron death. We also found that an environmental change, i.e. zinc depletion, induces a conformational change in WT SOD1 (SOD1(WT)) to the DBR-exposed conformation, suggesting the presence of an equilibrium state between the DBR-masked and DBR-exposed states even with SOD1(WT). Here, we conducted a high-throughput screening based on time-resolved FRET to further investigate the SOD1(WT) conformational change, and we used a genome-wide siRNA screen to search for regulators of SOD1 proteostasis. This screen yielded 30 candidate genes that maintained an absence of the DBR-exposed SOD1(WT) conformation. Among these genes was one encoding DDB1- and CUL4-associated factor 4 (DCAF4), a substrate receptor of the E3 ubiquitin–protein ligase complex. Of note, we found that DCAF4 mediates the ubiquitination of an ALS-associated protein and autophagy receptor, optineurin (OPTN), and facilitates autophagic degradation of DBR-exposed SOD1. In summary, our screen identifies DCAF4 as being required for proper proteostasis of DBR-exposed SOD1, which may have potential relevance for the development of therapies for managing ALS.

The accretion-powered X-ray pulsar GX 301-2 was observed with the balloon-borne X-Calibur hard X-ray polarimeter during late December 2018, with contiguous observations by the NICER X-ray telescope, the Swift X-ray Telescope and Burst Alert Telescope, and the Fermi Gamma-ray Burst Monitor spanning several months. The observations detected the pulsar in a rare apastron flaring state coinciding with a significant spin-up of the pulsar discovered with the Fermi GBM. The X-Calibur, NICER, and Swift observations reveal a pulse profile strongly dominated by one main peak, and the NICER and Swift data show strong variation of the profile from pulse to pulse. The X-Calibur observations constrain for the first time the linear polarization of the 15-35 keV emission from a highly magnetized accreting neutron star, indicating a polarization degree of (27+38-27)% (90% confidence limit) averaged over all pulse phases. We discuss the spin-up and the X-ray spectral and polarimetric results in the context of theoretical predictions. The X-Ray Imaging and Spectroscopy Mission (XRISM) is the successor to the 2016 Hitomi mission that ended prematurely. Like Hitomi, the primary science goals are to examine astrophysical problems with precise highresolution X-ray spectroscopy. XRISM promises to discover new horizons in X-ray astronomy. XRISM carries a 6 x 6 pixelized X-ray micro-calorimeter on the focal plane of an X-ray mirror assembly and a co-aligned X-ray CCD camera that covers the same energy band over a large field of view. XRISM utilizes Hitomi heritage, but all designs were reviewed. The attitude and orbit control system were improved in hardware and software. The number of star sensors were increased from two to three to improve coverage and robustness in onboard attitude determination and to obtain a wider field of view sun sensor. The fault detection, isolation, and reconfiguration (FDIR) system was carefully examined and reconfigured. Together with a planned increase of ground support stations, the survivability of the spacecraft is significantly improved.

Understanding and reducing the in-orbit instrumental backgrounds are essential to achieving high sensitivity in hard X-ray astronomical observations. The observational data of the Hard X-ray Imager (HXI) on board the Hitomi satellite provides useful information on the background components, owing to its multi-layer configuration with different atomic numbers: the HXI consists of a stack of four layers of Si (Z = 14) detectors and one layer of CdTe (Z = 48, 52) detector surrounded by well-type BGO (Bi4Ge3O12) active shields. Based on the observational data, the backgrounds of top Si layer, the three underlying Si layers, and the CdTe layer are inferred to be dominated by different components, namely, low-energy electrons, albedo neutrons, and proton-induced radioactivation, respectively. XL-Calibur is a hard X-ray (15-80 keV) polarimetry mission operating from a stabilised balloon-borne platform in the stratosphere. It builds on heritage from the X-Calibur mission, which observed the accreting neutron star GX 301-2 from Antarctica, between December 29th 2018 and January 1st 2019. The XL-Calibur design incorporates an X-ray mirror, which focusses X-rays onto a polarimeter comprising a beryllium rod surrounded by Cadmium Zinc Telluride (CZT) detectors. The polarimeter is housed in an anticoincidence shield to mitigate background from particles present in the stratosphere. The mirror and polarimeter-shield assembly are mounted at opposite ends of a 12 m long lightweight truss, which is pointed with arcsecond precision by WASP - the Wallops Arc Second Pointer. The XL-Calibur mission will achieve a substantially improved sensitivity over X-Calibur by using a larger effective area X-ray mirror, reducing background through thinner CZT detectors, and improved anticoincidence shielding. When observing a 1 Crab source for $t_{\rm day}$ days, the Minimum Detectable Polarisation (at 99% confidence level) is $\sim$2$\%\cdot t_{\rm day}^{-1/2}$. The energy resolution at 40 keV is $\sim$5.9 keV. Black-hole binary (BHB) systems comprise a stellar-mass black hole and a closely orbiting companion star. Matter is transferred from the companion to the black hole, forming an accretion disk, corona and jet structures. The resulting release of gravitational energy leads to emission of X-rays. The radiation is affected by special/general relativistic effects, and can serve as a probe of the properties of the black hole and surrounding environment, if the accretion geometry is properly identified. Two competing models describe the disk-corona geometry for the hard spectral state of BHBs, based on spectral and timing measurements. Measuring the polarization of hard X-rays reflected from the disk allows the geometry to be determined. The extent of the corona differs between the two models, affecting the strength of relativistic effects (e.g., enhancement of polarization fraction and rotation of polarization angle). Here, we report observational results on linear polarization of hard X-ray (19-181 keV) emission from a BHB, Cygnus X-1, in the hard state. We searched for a short-term X-ray variability of the M87 core and jet from archival X-ray data with long exposure data taken by the Suzaku, Chandra, and NuSTAR telescopes. We found the intraday variability for the Suzaku data obtained in 2006, and for the Chandra core obtained in 2017. The intraday variability suggested a minute emission region of about the size of Schwartzshild radius of the M87 supermassive black hole. Suzaku could not resolve a core and HST-1; however, in 2006, HST-1 was much brighter than the core, and thus, the variability is likely due to the HST-1. Since the photon index in 2006 was 2.38, the emission was possibly synchrotron emission from the local shock region in the HST-1, indicating that the particle acceleration of TeV electrons occurred far away (~100 pc) from the core. Assuming the fading time to be equal to the synchrotron cooling time, the magnetic field is constrained to be B ~1.94 ${\delta}^{1/3}$ mG. Moreover, the photon index of the core in 2017 was approximately 1.96; thus, the possible emission was from the radiative inefficiency accretion flow of the core or inverse Compton scattering in the jet. Background: Telemonitoring might enhance continuous positive airway pressure (CPAP) adherence and save coaching time at the begining of CPAP therapy. We assessed telemonitoring (AirView Online System, ResMed) during the habituation phase of the CPAP therapy and after one year treatment in obstructive sleep apnea syndrome (OSAS). Method: 104 of 120 OSAS patients were randomised at the initiation of the treatment. The first visit was physical for the Usual care (UC, N=53) and telemonitoring groups (TM, N=50), while for TM all visits were unphysical, except at the request of the patient. Results: TM and UC groups did not differ in terms of patient characteristics (Age, gender, BMI, HAI, Epworth sleep scale and Pichot (depressive) scale). CPAP treatment did not differ (TM vs. UC) in terms of pressure treatment (11.4±2.4 vs. 11.1±2.3 cmh2o), leaks (25.5±16.1 vs 24.1±12.4 l/min), quality of sleep (ESS) (5.9±3.6 vs. 4.2±4) and depressive scale (Pichot) (2.1±3.4 vs. 2±3.7). 18 patients stopped treatment in UC and 14 in TM arm with no significant difference P=0.51, and CPAP adherence was equal in both groups p=0.57. Number of minutes coaching required per patient in the TM vs UC groups was not different 70±4min vs. 79±3min p=0.09 Conclusion: Implementation and follow-up at 1 year of CPAP for OSAS patients by TM do not modify the adherence of patients to treatment.

This study presents a background estimation for the Cubesats Applied for MEasuring and LOcalising Transients (CAMELOT), which is a proposed fleet of nanosatellites for the all-sky monitoring and timing based localization of gamma-ray transients with precise localization capability at low Earth orbits. CAMELOT will allow to observe and precisely localize short gamma-ray bursts (GRBs) associated with kilonovae, long GRBs associated with core-collapse massive stars, magnetar outbursts, terrestrial gamma-ray flashes, and gamma-ray counterparts to gravitational wave sources. The fleet of at least nine 3U CubeSats is proposed to be equipped with large and thin CsI(Tl) scintillators read out by multi-pixel photon counters (MPPC).
