Free gas within the human body tends to increase in volume when a person ascends in altitude and to decrease in volume when the atmospheric pressure is increased. This is in accordance with Boyle's law, which states that the volume occupied by a given quantity of gas is inversely proportional to the absolute pressure exerted on it (figs. 1, 2 and 3). Gas enclosed within a body cavity is under pressure of the external atmosphere, but this pressure can be modified significantly by the elastic properties of the walls of the enclosing organ and by pressure of surrounding structures. Those body cavities which communicate with the external atmosphere, such as the paranasal sinuses, are capable of equalizing pressure changes, provided the ostiums are not obstructed, but such equilization is not possible within the cavity of a closed pneumothorax. No report has come to our attention concerning the effect of sulfapyridine (2-[paraaminobenzenesulfonamido]-pyridine) on postoperative pneumonia. We wish to record our experience with this drug in 21 cases of postoperative pneumonia and 6 cases of primary pneumonia. This includes the cases of all patients with uncomplicated pneumonia under our personal supervision to whom we have given the drug up to the time of writing this report. DOSE Patients usually were given 15 grains (1 Gm.) of sulfapyridine by mouth every four hours day and night (90 grains, or 6 Gm., per day). The first dose, and sometimes the second dose also, was doubled, making a total of either 105 or 120 grains (7 or 8 Gm.) during the first twenty-four hours. The duration of treatment is indicated in the charts. UNFAVORABLE EFFECTS No seriously bad results could be attributed to treatment with sulfapyridine. Significant leukopenia was not observed. This preliminary report on treatment of certain types of cutaneous tuberculosis and tuberculids is part of a study of various agents in the treatment of tuberculosis that was started several years ago by two of us (Feldman and Hinshaw).1 This report deals with the clinical application to cutaneous tuberculosis of promizole2 and streptomycin,3 which were found to be the least toxic and have demonstrated the greatest therapeutic effect of any of the substances which were used in treatment of tuberculous guinea pigs.Since Ehrlich developed the satisfactory arsenical for treatment of syphilis, a constant search has been made for some similarly effective agent for the treatment of tuberculosis.
