Human cytogenetic biomonitoring (HCB) has long been used to evaluate the potential effects of work environments on the DNA integrity of workers. However, HCB studies on the genotoxic effects of occupational exposure to extremely low‐frequency electromagnetic fields (ELF‐MFs) were limited by the quality of the exposure assessment. More specifically, concerns were raised regarding the method of exposure assessment, the selection of exposure metrics, and the definition of exposure group. In this study, genotoxic effects of occupational exposure to ELF‐MFs were assessed on peripheral blood lymphocytes of 88 workers from the electrical sector using the comet and cytokinesis‐block micronucleus assay, considering workers' actual exposure over three consecutive days. Different methods were applied to define exposure groups. Overall, the summarized ELF‐MF data indicated a low exposure level in the whole study population. It also showed that relying solely on job titles might misclassify 12 workers into exposure groups. We proposed combining hierarchical agglomerative clustering on personal exposure data and job titles to define exposure groups. The final results showed that occupational MF exposure did not significantly induce more genetic damage. Potential health effects of extremely low-frequency (electro)magnetic fields (ELF-(E)MFs) have long been investigated, but the results are still inconclusive. With respect to genotoxicity, sound data related to the effects of long-term exposure to ELF-(E)MFs on the genetic material and the impact of long-term pre-exposure to ELF-(E)MFs on the sensitivity of cells to the damage induced by known mutagens are needed. In this manuscript, an optimized protocol for a combined in vitro comet/micronucleus study to investigate these effects in a human lymphoblastoid cell line (TK6) is provided including the description of a well-validated exposure system. Furthermore, the use of a shielding system to limit background ELF-MFs inside the incubator is described as well. • Optimized protocols for cytogenetic tests with ELF-MFs on TK6 cells ensure the reproducibility of test results. • Validation of exposure environment and exposure system are needed prior to performing tests with ELF-MFs. • A simple, but effective method to shield cells and reduce unintentional ELF-MF exposure consists of using the mu-metal cylinder. Par ce travail, nous avons voulu mieux cerner les paramètres cinématiques d’une manipulation cervicale, mais également les confronter au ressenti subjectif des sujets qui en ont bénéficié.Pour situer le contexte, nous avons débuté en relatant les données de la littérature quant à la définition, aux indications, à l’efficacité, aux risques, à la cinématique des manipulations vertébrales et à la place de cette technique dans le modèle bio-psycho-social des soins de santé.Nous avons réalisé un ensemble d’études aux Laboratoires d’Anatomie, de Biomécanique et d’Organogénèse (LABO) et d’Anatomie Fonctionnelle (LAF) de l’ULB. Elles portaient, d’une part, sur l’évaluation de la cinématique de la manipulation cervicale et ses répercussions sur le ressenti subjectif des individus en bénéficiant et, d’autre part, sur l’étude de l’axe hélicoïdal au moment de l’impulsion ainsi que l’analyse des effets de la manipulation sur l’amplitude active et l’erreur de repositionnement céphalique d’individus asymptomatiques.Au vu de nos résultats et de ceux de la littérature, une influence majeure du type de technique sur les paramètres cinématiques de la manipulation cervicale existe.Nous avons pu mettre en évidence que les amplitudes, vitesses et accélérations utilisées lors des manipulations cervicales HVBA sont liées à la technique évaluée et dépendent largement du praticien et de son expérience.De manière générale, nos résultats étaient en accord avec la littérature, avec des amplitudes de rotation axiale faibles. Ces résultats peuvent être liés au fait que certains praticiens étaient familiarisés à l’usage de la technique à composantes multiples.Notre évaluation de l’expérience subjective des individus a fait ressortir deux dimensions que nous avons appelées :facteur 1 et facteur 2. Ce travail de thèse investigue la cinématique des doigts longs par deux approches différentes et complémentaires ainsi que la faisabilité de la fusion entre celles-ci. La première approche comporte le développement complet d’un protocole d’évaluation fonctionnelle tridimensionnelle des doigts. Des mouvements continus sont étudiés de manière in vivo et la cinématique des cinq doigts est visualisée simultanément. La seconde approche consiste en l’application d’une méthodologie in vitro de modélisation des doigts afin de créer un modèle anatomiquement fidèle permettant d’apprécier la cinématique tridimensionnelle ainsi que différents paramètres biomécaniques des muscles mobilisant les doigts longs. Les deux approches sont finalement comparées et la faisabilité de leur couplage étudiée en vue d’une fusion des données issues de ces deux méthodologies. Les méthodes développées dans ce travail sont également considérées dans le cadre d’une utilisation clinique. La première partie de ce travail présente la méthodologie complète d’un protocole novateur d’évaluation de la cinématique des doigts in vivo. Plusieurs mouvements analytiques et fonctionnels des doigts ont été analysés sur un échantillon de vingt sujets sains. Les mouvements analytiques ont tout d’abord permis d’étudier le comportement physiologique des doigts mais également de constater des spécificités fonctionnelles de chacun. Les mouvements fonctionnels ont ensuite permis d’analyser la biomécanique des doigts lors de mouvements plus complexes. Les synergies intra- et inter-doigts ont été étudiées et les spécificités des muscles mobilisant les doigts sont discutées. Enfin, une base de données de sujets sains a été créée pour permettre de futures études incluant des sujets souffrant de pathologies de l’appareil neuro-musculo-squelettique de la main. Ces dernières décennies, les populations, civiles en général et professionnelles en particulier, ont été soumises à une exposition croissante aux champs électriques (EF) et magnétiques (MF) d'extrêmement basses fréquences (ELF). Différentes réponses biologiques ont été observées sur différents types de cellules et de tissus exposés à des courants et des champs ELF. Ces études permettent une meilleure connaissance de notre environnement électromagnétique mais les mécanismes biologiques précis et les implications des fréquences ELF sur la santé restent peu connus. Il est nécessaire de développer des protocoles de recherche pour répondre à ces questions. La revue exhaustive de la littérature met clairement en avant :- le manque de connaissance des mécanismes cellulaires activés après stimulation ELF;- l'importance de leur étude pour toutes recherches liées à l'utilisation de la stimulation ELF;- l'efficacité des techniques omiques qui permettent l'identification de ces mécanismes cellulaires;- l'importance du choix des modèles expérimentaux utilisés afin de faciliter l'extrapolation des résultats chez l'homme.Pour répondre à ces observations, nous avons recherché les gènes et les mécanismes cellulaires impliqués lors de la stimulation ELF sur un modèle de culture d'explants de kératinocytes humains mis en culture sur support dermique dévitalisé et stimulé par un courant électrique pulsé. We observed on different tissues and organisms a biological response after exposure to pulsed low frequency and low amplitude electric or electromagnetic fields but the precise mechanism of cell response remains unknown. The aim of this publication is to understand, using bioinformatics, the biological relevance of processes involved in the modification of gene expression. The list of genes analyzed was obtained after microarray protocol realized on cultures of human epidermal explants growing on deepidermized human skin exposed to a pulsed low frequency electric field. The directed acyclic graph on a WebGestalt Gene Ontology module shows six categories under the biological process root: “biological regulation”, “cellular process”, “cell proliferation”, “death”, “metabolic process” and “response to stimulus”. Enriched derived categories are coherent with the type of in vitro culture, the stimulation protocol or with the previous results showing a decrease of cell proliferation and an increase of differentiation. The Kegg module on WebGestalt has highlighted “cell cycle” and “p53 signaling pathway” as significantly involved. The Kegg website brings out interactions between FoxO, MAPK, JNK, p53, p38, PI3K/Akt, Wnt, mTor or NF-KappaB. Some genes expressed by the stimulation are known to have an exclusive function on these pathways. Analyses performed with Pathway Studio linked cell proliferation, cell differentiation, apoptosis, cell cycle, mitosis, cell death etc. with our microarrays results. Medline citation generated by the software and the fold change variation confirms a diminution of the proliferation, activation of the differentiation and a less well-defined role of apoptosis or wound healing. During recent decades the utilisation of growth factors, especially BMPs, has received an increasing interest in orthopaedic surgery. For clinical implantation the two main options are demineralised bone matrix (DBM) and recombinant bone morphogenetic proteins (rhBMP). Many clinical studies agree on an equivalent osteoinductive effect between DBM, BMPs and autologous bone graft; however, the different origins and processing of DBM and rhBMP may introduce some fluctuations. Their respective characteristics are reviewed and possible interactions with their effectiveness are analysed. The main difference concerns the concentration of BMPs, which varies to an order of magnitude of 10(6) between DBM and rhBMPs. This may explain the variability in efficiency of some products and the adverse effects. Currently, considering osteoinductive properties, safety and availability, the DBM seems to offer several advantages. Les soins pulpaires en denture lactéale sont les soins fréquemment réalisés en odontologie pédiatrique et la réaction pulpaire ainsi que les mécanismes de réparation tissulaire lors d’un traumatisme ne sont pas très bien connus ou investigués. En se référant à la littérature dentaire, on constate que les recherches sont souvent focalisées sur la denture définitive.Objectifs1) Etudier l’influence du traitement avec divers matériaux sur la formation dentinaire, la structure tissulaire et le degré d’inflammation.2) Acquérir des connaissances sur l’interaction à l’interface matériau/tissu.3) Etudier le rôle de la voie de signalisation Notch (1 et 2) dans le processus de remaniement et réparation du tissu pulpaire en cas de traumatisme.Matériels et méthodesDeux types de soins pulpaires, la pulpotomie et le coiffage direct, sont réalisés sur les dents lactéales porcines et les résultats sont étudiés après 3 périodes :7 jours, 21-28 jours et 90 jours.La pulpotomie :8 matériaux, 10 dents par matériau, 2 matériaux par cochon et par période pour un total de 240 dents.Le coiffage direct :10 matériaux, 10 dents par matériau, 2 matériaux par cochon et par période pour un total de 300 dents.Par la suite, différentes techniques ont été utilisées :1. The use of bone and connective tissue allografts has grown rapidly and surpassed the use of autografts in many countries. Being of human origin, bone and tendon allografts carry the risk of disease transmission and complications have been reported. As part of the Project NOTIFY led by the World Health Organisation, an effort to improve recognition, reporting, tracking and investigation of adverse outcomes of allografts was initiated, achieving a comprehensive review of associated disease transmission and failures. Those involving the use of musculoskeletal allografts are reported here. Ce travail de thèse porte sur l’étude de la cinématique 3D du rachis cervical supérieur ainsi quesur l’analyse des bras de levier des muscles sous-occipitaux postérieurs, par le développementde différentes méthodes contribuant à la modélisation anatomique du mouvement.La première partie de ce travail s’oriente vers l’étude des mouvements de flexion extension, derotation axiale et de flexion latérale ainsi que des axes qui y sont rapportés. Pour ce faire, deuxprotocoles ont été développés sur préparations anatomiques, l’un visant à analyser lesmouvements discrets à partir de différentes attitudes du rachis cervical supérieur et l’autres’intéressant aux mouvements instantanés. Ceux-ci ont permis de mettre en évidence uneorientation et une position de l’axe hélicoïdal dépendantes du mouvement, du segment étudiéet de sa position spatiale.En deuxième intention, nous nous sommes intéressés aux propriétés biomécaniques musculairesde cette région, et en particulier à l’étude des bras de levier. Deux méthodes de calculdistinctes ont été utilisées, montrant chacune des résultats différents d’un point de vuequantitatif mais relativement similaires au niveau qualitatif.Pour terminer, nous avons exploité ces méthodes afin d’apprécier leur faisabilité dans le cadred’une évaluation fonctionnelle in vivo. The discovery of the dynamic electrical properties of bone is at the origin of the therapeutical application of the electromagnetic fields in Orthopaedics and Traumatology. The first empirical treatment of non-union, fresh fractures and osteonecroses allowed the observation of several effects which, without justifying a systematic clinical application, encouraged further fundamental research. The results of this work realized during 35 years are summarized in the present article. After exposure to specific electromagnetic fields, we observed a modification of the DNA activity and an increased production of RNA. During enchondral ossification, the amount of acid GAGS increased faster and the ossification of the primary ossification point is accelerated. On fresh fractures, the rigidity of the callus increased faster. Finally, the microarrays analyses show an upregulation of mRNA involved in cellular differentiation and proliferation. The mRNA responsible of the production of BMP-2 is significantly increased, explaining the main results observed after the expense of experimental models of the bond tissues. The history of the Orthopaedics and Traumatology Research Laboratory (LROT) is summarized during the last thirty years. The approach, initially focused on bone biomechanics and strength of materials, underwent a rapid diversification as expressed by the list of research topics: monitoring of bone healing; bone strains measurements for different level of activities including microgravity and prevention of disuse osteoporosis; biological effects of electromagnetic fields; evolution of the viscoelastic properties of the callus during bone healing; improvement of the osteoinductive properties of bone substitutes produced by the Tissues Bank of the University Hospital Erasme; Kashin-Beck disease; SICOT telediagnostic, and biomechanics of threaded implants. Those topics, event the most fundamental ones, have immediate significant clinical applications allowing a decrease of the morbidity and an acceleration of the rehabilitation of the patients. On the occasion of the thirty years since its inauguration, the Department of Orthopaedics and Traumatology of the University Hospital Erasme reviews the milestones of its development. Various original new techniques have been implemented: monitoring of implants using strain gauges, external fixation of the limbs, external minifixation, miniinvasive anterior approach in hip arthroplasty, knee ligamentoplasties, orthopaedic microsurgery and composite tissue allotransplantation. The care of aged patients takes place in close collaboration with the Department of Geriatrics. Two new clinical units have been created, the Unit of Hand and Peripheral Nerve Surgery, and the Centre for Sports. The Department has organized an efficient Bone Bank. The new day care hospital improves the possibilities of ambulatory surgery. Through the technological evolution of the medical devices used for the treatment of bone fractures, two main concepts are in opposition: the internal rigid and the external elastic fixations. His definition, given in 1908, is still valid today: "Osteo-synthesis is the artificial contention of the bone fragments of fractures, by special devices acting directly on bones, exposed or not, with the aim to strongly fix them in their original position". The authors review the methods of contention before the invention of osteosynthesis and later the developments of bone fixation techniques. An acceleration of differentiation, at the expense of proliferation, is observed after exposure of various biological models to low frequency and low amplitude electric and electromagnetic fields. Following these results showing significant modifications, we try to identify the biological mechanism involved at the cell level through microarray screening. For this study, we use epidermis cultures harvested from human abdominoplasty. Two platinum electrodes are used to apply the electric signal. The gene expressions of 38,500 well-characterized human genes are analyzed using Affymetrix(®) microarray U133 Plus 2.0 chips. The protocol is repeated on three different patients. After three periods of exposure, a total of 24 chips have been processed. After the application of ELF electric fields, the microarray analysis confirms a modification of the gene expression of epidermis cells. Particularly, four up-regulated genes (DKK1, TXNRD1, ATF3, and MME) and one down-regulated gene (MACF1) are involved in the regulation of proliferation and differentiation. Expression of these five genes was also confirmed by real-time rtPCR in all samples used for microarray analysis. La drépanocytose est la maladie moléculaire et héréditaire (transmission mendélienne récessive et autosomique) la plus répandue au monde. Elle est un problème de santé publique par sa gravité et ses implications socio-économiques dans de nombreux pays. Seuls les sujets homozygotes (SS) ou hétérozygotes composites (SC) sont malades, les hétérozygotes (AS) ne sont que des transmetteurs du gène S. Elle est la première cause d’OstéoNécrose de la Tête Fémorale (ONTF), douloureuse évoluant vers l’arthrose, en l’absence de traitement chez un patient jeune.La Guadeloupe compte 450.000 habitants, dont 12% sont porteurs de l’hémoglobine S. Le nombre des drépanocytaires est estimé à 1.200 dont les 3/4 sont suivis au Centre Caribéen de la Drépanocytose (CCD), créé en 1990. Le centre assure la prise en charge médicale des enfants dès leur naissance et des adultes malades. Nos activités au CHU de Pointe-à-Pitre, au CCD et à l’Unité INSERM-UMR S458 depuis juillet 1992 nous ont permis d’étudier:- le diagnostic de l’ONTF;- l’évaluation de l’hyperpression osseuse dans l’ONTF et l’évaluation du traitement par forage simple;- l’étude de l’impact de la prise en charge orthopédique précoce sur la survenue et l’évolution de l’ONTF.Notre étude concerne les patients drépanocytaires adultes homozygotes (SS) et double hétérozygotes (SC):- une série rétrospective de 1993-1994 [E-1994] portant sur 115 patients (58 SS, 57 SC) identifiés en 1984,sans suivi médical ni orthopédique;- une série prospective de 1995 à 2008 [E-2008] portant sur 215 patients (94 SS, 121 SC) avec prise encharge médicale et orthopédique.L’IRM est l’examen de référence pour le diagnostic de l’ONTF comme dans la nécrose idiopathique. Many devices are available on the market for the evaluation and rehabilitation of patients suffering from muscular disorders. Most of them are small, low-cost, passive devices based on the use of springs and resistive elements and exhibit very limited (or even not any) evaluation capabilities; extended muscular force evaluation is only possible on stationary, expensive, multi-purpose devices, available only in hospitals, which offer many exercise modes (e.g. isokinetic mode) that are not available on other devices.The objective of this thesis is to make the functionalities currently only implemented on bulky multi-purpose devices available at a lower cost and in a portable fashion, enabling their use by a large number of independent practitioners and patients, even at home (tele-medecine applications).In order to achieve this goal, a portable rehabilitation device, using a magneto-rheological fluid brake as actuator, has been designed. This particular technology was selected for its high level of compactness, simple mechanical design, high controllability, smooth and safe operation. In previous studies we observed a proximo-distal gradient of lesion frequencies along the limb, with the distal joints being the most often affected. This suggests an associated effect of environmental factors on the most exposed joints. On a population of 820 children (mean age 13 years) of endemic areas distributed in groups of healthy and severity stages I to III of KBD (Kashin-Beck disease), the effects of different working activities were studied. Heavy work like that of a ploughman were compared to light physical work, e.g. school children, and exposure to cold and history of frostbite were also considered. The most severe stages, II and III, were present in 72% of the ploughman vs. 29% of the schoolchildren, 70% of the shepherds vs. 30% (p < 0.001) of the schoolchildren, and in 65% of the shepherds working in winter vs. 40% of those working in the other seasons (p < 0.001). In the group with history of frostbite, 58% present the severest stages vs. 40% without (p < 0.001).
