3. Experiences of adults who disclose self-harm to non-professionals

Background: Research suggests that self-harm is often a private behaviour and many individuals do not disclose their self-harm. People who feel unable to disclose their self-harm have less opportunity to seek support. There is a paucity of research on adult self-harm disclosure experiences, with most existing studies recruiting adolescents. Aims: This study aimed to understand the expectations and experiences of adults who self-harm when they disclose to non-professionals. Methods: Ten participants (aged 26-51) were recruited through the NHS and a third sector organisation. Semi-structured interviews were conducted and data was analysed using thematic analysis. Results: Four themes were identified; ‘The insular nature of self-harm’, ‘Imagined versus reality’, ‘Self-preservation’ and ‘New ways of relating’. Participants described self-harm as an insular behaviour, which created an inherent tension when considering disclosure; a social process. There were risks associated with self-harm disclosure, including stigma and losing control. Participants carefully considered the decision to disclose, experiencing high levels of ambivalence about disclosing and spending extended periods rehearsing potential disclosures. In reality, disclosure experiences were sometimes very different, with participants describing a range of positive and negative responses. Making such risky personal disclosures meant self-preservation was key, both during and after a disclosure. Participants found new ways of relating to their disclosure recipients and evaluated future potential disclosures depending on the response received. Most participants had experienced negative responses, which caused distress and curtailed future disclosures, highlighting a need for improved public awareness of supportive responses to disclosures of self-harm. Discussion: This study highlighted the personal risks and highly variable responses people experience when disclosing self-harm to a family member, friend or colleague. Helpful ways of responding to self-harm disclosures were highlighted by participants in this study and guidance could be published so that non-professionals can be informed and prepared for having conversations about self-harm.

4. Excited states in ¹⁶¹W and ¹⁶²W and the onset of collectivity in the neutron-deficient tungsten isotopes

Excited states have been established in the neutron-deficient odd-N nucleus 161W for the first time. The level scheme has been interpreted in terms of multi-particle excitations of independent nucleons outside of a closed shell. The excitation level scheme for the neutron-deficient nuclide 162W has been extended using recoil-decay correlations to a tentative spin of 17⁻. The collective nucleus has been interpreted in terms of rotationally aligned bands. Configuration assignments have been made on the basis of comparisons of the angular momentum of initial emitting states, as a function of transition energy, with the predictions of the cranked shell model. Comparisons made between these two nuclei and their neighbouring isotopes and isotones indicate rapid changes in nuclear structure dependent on the available valence space beyond the N = 82 shell closure.

7. The effect of text message reminders on compliance with Twin Block appliances: A randomised controlled trial.

Aims: The aim of this study was to evaluate whether text reminders influence patient compliance with Twin Block appliances. The null hypothesis was that there was no statistically significant differences in Twin Block compliance between those who receive text reminders and those that do not. Design: Single-blind parallel randomised controlled clinical trial. Setting: Health Service Executive (HSE) orthodontic outpatient clinic in Dublin, Ireland. Participants: A total of 59 patients aged 11–15 years with a 5a Index of Orthodontic Treatment Need (IOTN grade) starting treatment with Twin Block appliances. Methods: A computer-generated unstratified allocation sequence was used to randomise the participants into the control group (CG) or the text group (TG). Both groups were asked to wear their appliances full-time. In addition to the same verbal and written instruction received by the CG, the TG received text message reminders, every 3 days, to wear their appliances. The primary outcome measure was wear time reported by Theramon
® sensors embedded in the appliances. Data on wear time were uploaded from the Theramon® sensors onto cloud software. Participants in both groups were asked to fill out wear diaries and submit these at each visit. Treating clinicians and the primary investigator were blinded to the allocation group. Participants were followed up for 4 months. Participants were not blinded to their treatment group. Results: In total, 29 participants were allocated to the CG and 30 to the TG. The data for 53 participants were analysed, 24 from the CG and 29 from the TG. The median hours/day of wear recorded using the Theramon® sensors was 13.77 (interquartile range [IQR] = 10.19) for the CG and 17.72 (IQR = 5.62) for the TG. The difference in wear time recorded was not statistically significant (P = 0.16). Conclusion: The study concluded that text message reminders had no statistically significant influence on patient compliance with Twin Block appliances. [ABSTRACT FROM AUTHOR]- Published

16. Mapping the Color Space of Saccadic Selectivity in Visual Search

- Abstract
Color coding is used to guide attention in computer displays for such critical tasks as baggage screening or air traffic control. It has been shown that a display object attracts more attention if its color is more similar to the color for which one is searching. However, what does "similar" precisely mean? Can we predict the amount of attention that a display color will receive during a search for a given target color? To tackle this question, two color-search experiments measuring the selectivity of saccadic eye movements and mapping out its underlying color space were conducted. A variety of mathematical models, predicting saccadic selectivity for given target and display colors, were devised and evaluated. The results suggest that applying a Gaussian function to a weighted Euclidean distance in a slightly modified HSI color space is the best predictor of saccadic selectivity in the chosen paradigm. Hue and intensity information by itself provides a basis for useful predictors, spanning a possibly spherical color space of saccadic selectivity. Although the current models cannot predict saccadic selectivity values for a wide variety of visual search tasks, they reveal some characteristics of color search that are of both theoretical and applied interest, such as for the design of human-computer interfaces. 17. Context Pre-Exposure Obscures Amygdala Modulation of Contextual-Fear Conditioning

- Abstract
We report that post-training inactivation of basolateral amygdala region (BLA) with muscimol impaired memory for contextual-fear conditioning (as measured by freezing) and intra-BLA norepinephrine enhanced this memory. However, pre-exposure to the context eliminated both of these effects. These findings provide a likely explanation of why an earlier study failed to observe that the BLA modulates contextual fear conditioning--they pre-exposed their rats to the context. These results also suggest that the amygdala modulates the storage of the context fear memory and may do so by influencing the storage of the representation of the context in which the shock occurred. - Abstract
Semantic priming has been studied for nearly 50 years across various experimental manipulations and theoretical frameworks. These studies provide insight into the cognitive underpinnings of semantic representations in both healthy and clinical populations; however, they have suffered from several issues including generally low sample sizes and a lack of diversity in linguistic implementations. Here, we will test the size and the variability of the semantic priming effect across ten languages by creating a large database of semantic priming values, based on an adaptive sampling procedure. Differences in response latencies between related word-pair conditions and unrelated word-pair conditions (i.e., difference score confidence interval is greater than zero) will allow quantifying evidence for semantic priming, whereas improvements in model fit with the addition of a random intercept for language will provide support for variability in semantic priming across languages.

In 2017, preliminary data show that gay, bisexual, and other men who have sex with men (MSM) accounted for 67% of new diagnoses of human immunodeficiency virus (HIV) infection, that MSM who inject drugs accounted for an additional 3%, and that African American/black (black) and Hispanic/Latino (Hispanic) MSM were disproportionately affected (1). During 2010-2015, racial/ethnic disparities in HIV incidence increased among MSM; in 2015, rates among black and Hispanic MSM were 10.5 and 4.9 times as high, respectively, as the rate among white MSM (compared with 9.2 and 3.8 times as high, respectively, in 2010) (2). Increased use of preexposure prophylaxis (PrEP), which reduces the risk for sexual acquisition of HIV infection by approximately 99% when taken daily as prescribed,* would help to reduce these disparities and support the Ending the HIV Epidemic: A Plan for America initiative† (3). Although PrEP use has increased among all MSM since 2014 (4), racial/ethnic disparities in PrEP use could increase existing disparities in HIV incidence among MSM (5). To understand racial/ethnic disparities in PrEP awareness, discussion with a health care provider, and use (steps in the HIV PrEP continuum of care) (6), CDC analyzed 2017 National HIV Behavioral Surveillance (NHBS) data. Black and Hispanic MSM were significantly less likely than were white MSM to be aware of PrEP, to have discussed PrEP with a health care provider, or to have used PrEP within the past year. Among those who had discussed PrEP with a health care provider within the past year, 68% of white MSM, 62% of Hispanic MSM, and 55% of black MSM, reported PrEP use. Prevention efforts need to increase PrEP use among all MSM and target eliminating racial/ethnic disparities in PrEP use.§.

In February 2019, the U.S. Department of Health and Human Services proposed a strategic initiative to end the human immunodeficiency (HIV) epidemic in the United States by reducing new HIV infections by 90% during 2020-2030* (1). Phase 1 of the Ending the HIV Epidemic initiative focuses on Washington, DC; San Juan, Puerto Rico; and 48 counties where the majority of new diagnoses of HIV infection in 2016 and 2017 were concentrated and on seven states with a disproportionate occurrence of HIV in rural areas relative to other states.† One of the four pillars in the initiative is protecting persons at risk for HIV infection using proven, comprehensive prevention approaches and treatments, such as HIV preexposure prophylaxis (PrEP), which is the use of antiretroviral medications that have proven effective at preventing infection among persons at risk for acquiring HIV. In 2014, CDC released clinical PrEP guidelines to health care providers (2) and intensified efforts to raise awareness and increase the use of PrEP among persons at risk for infection, including gay, bisexual, and other men who have sex with men (MSM), a group that accounted for an estimated 68% of new HIV infections in 2016 (3). Data from CDC's National HIV Behavioral Surveillance (NHBS) were collected in 20 U.S. urban areas in 2014 and 2017, covering 26 of the geographic areas included in Phase I of the Ending the HIV Epidemic initiative, and were compared to assess changes in PrEP awareness and use among MSM. From 2014 to 2017, PrEP awareness increased by 50% overall, with >80% of MSM in 17 of the 20 urban areas reporting PrEP awareness in 2017. Among MSM with likely indications for PrEP (e.g., sexual risk behaviors or recent bacterial sexually transmitted infection [STI]), use of PrEP increased by approximately 500% from 6% to 35%, with significant increases observed in all urban areas and in almost all demographic subgroups. Despite this progress, PrEP use among MSM, especially among black and Hispanic MSM, remains low. Continued efforts to improve coverage are needed to reach the goal of 90% reduction in HIV incidence by 2030. In addition to developing new ways of connecting black and Hispanic MSM to health care providers through demonstration projects, CDC has developed resources and tools such as the Prescribe HIV Prevention program to enable health care providers to integrate PrEP into their clinical care.§ By routinely testing their patients for HIV, assessing HIV-negative patients for risk behaviors, and prescribing PrEP as needed, health care providers can play a critical role in this effort.

Men who have sex with men (MSM) in the United States are stigmatized for their same-sex practices, which can lead to risky sexual behavior, potentiating risk for human immunodeficiency virus (HIV) infection. Improved measurement is necessary for accurately reporting and mitigating sexual behavior stigma. We added 13 sexual behavior stigma items to local surveys administered in 2017 at 9 sites in the Centers for Disease Control and Prevention's National HIV Behavioral Surveillance system, which uses venue-based, time-sampling procedures to survey cisgender MSM in US Census Metropolitan Statistical Areas. We performed exploratory factor analytical procedures on site-specific (Baltimore, Maryland; Denver, Colorado; Detroit, Michigan; Houston, Texas; Nassau-Suffolk, New York; Portland, Oregon; Los Angeles, California; San Diego, California; and Virginia Beach-Norfolk, Virginia) and pooled responses to the survey items. A 3-factor solution—"stigma from family" (α = 0.70), "anticipated health-care stigma" (α = 0.75), and "general social stigma" (α = 0.66)—best fitted the pooled data and was the best-fitting solution across sites. Findings demonstrate that MSM across the United States experience sexual behavior stigma similarly. The purpose of this study was to research and evaluate trends in sports scandals and illegal activity amongst college athletes. Through the method of studying court case outcomes, and through observing news reports and social media interactions, the goal was intended to discover why these activities occurred. The study also intended to find out what factors play a role into these athletes and coaches being involved. Throughout sports history, there has been many cases of sports scandals whether it be regarding money bribes, gambling, sexual assault, or other issues. The study aimed to find information on these cases and the key impact of these scandals happening with the athletes and organizations. In the end the study found most of the cases if not all lead to negative publicity, coaches and players being fined or fired and a threat to the termination of the programs. Punishments varied from case to case, depending on the severity of the illegal acts committed by both the athletes and coaches.

Purpose: Rock climbing has become popular as a recreational activity. Overuse injuries of fingers and hands are common due to uniquely high demands placed on these structures. Climbers adapt hand positions to match types of holds on rock climbing routes, with open-hand and half-crimp positions being most used. The primary purpose of this study was to explore differences in climbing-specific grip strength between 2 hand positions and 2 shoulder positions. Methods: Participants' maximum isometric pull was tested on a 20mm edge climbing hold attached to a force transducer in each of 4 hand/shoulder position combinations bilaterally. 46 participants (20 female) across skill levels were included for analysis. Peak force was extracted and normalized to participants' body weight. Mixed model ANOVAs were used to explore effects and interactions between shoulder position, hand position and skill level. Paired t-tests were used explore asymmetry between dominant and non-dominant hands. Results: Half-crimp position was stronger than open-hand position and shoulder position did not impact force production. Climbers of higher skill level had higher force production in both hand positions. Greater asymmetry was observed in climbers of higher skill in the half-crimp position only. Conclusion: Findings support using a single shoulder position for testing finger strength versus multiple positions. Climbers of all levels should emphasize both open-hand and half-crimp training for performance and injury prevention. Applications in Sport: Shoulder position did not impact force in open-hand or half-crimp grip. Higher skill climbers produced greater force. Force was higher in half-crimp versus open-hand positions independent of skill. Climbers use open-hand and half-crimp positions and should train both for performance and injury prevention.
