A 54-year-old man underwent laparoscopic distal gastrectomy with D2 lymph node dissection and ante-colic Roux-en-Y reconstruction for gastric cancer. The histopathological diagnosis was pT2N3aM0, pStage ⅢA, HER2 negative. After 8 courses of S-1 plus oxaliplatin as adjuvant chemotherapy, he was diagnosed as peritoneal dissemination and treated with ramucirumab(RAM)plus paclitaxel(PTX). On the 12th day of course 10, he visited to our hospital with abdominal pain. CT showed free air and massive ascites. Emergent surgery was performed under the diagnosis of gastrointestinal perforation. A small intestinal perforation in front of the jejunal limb near gastric-jejunal anastomosis was identified and there was no peritoneal dissemination. We performed partial resection of remnant stomach and jejunal limb by linear stapler and reconstruction by end to side gastric-jejunal anastomosis. Because the gastric and intestinal wall were quite fragile and RAM impaired wound healing as adverse event, we feared about leakage, but he had no major postoperative complications and discharged on the 33th day after surgery. After 24 courses of nivolumab as third-line chemotherapy, the peritoneal dissemination disappeared. He has been alive without recurrence for about 1 year since then.

A 77-year-old man was given a diagnosis of pT4aN0M1a(PUL2), stage Ⅳ, RAS mutant type, after the operation for advanced ascending colon cancer. He was administered mFOLFOX6 plus Bmab as first-line chemotherapy. He showed consciousness disturbance on the 2nd day during the 6 cycles. Because of head computed tomography and magnetic resonance imaging showing no abnormal findings, we diagnosed convulsive seizure. His consciousness level gradually improved after intravenous infusion. He showed consciousness disturbance on the 2nd day during the 7 cycles again. Because blood ammonia level were high at 400μg/dL, he was diagnosed as hyperammonemic encephalopathy. His consciousness level rapidly recovered after branched chain amino acid(BCAA)infusion. SOX plus Bmab therapy was started as a post-treatment, he developed hyperammonemia(NH3 288μg/dL)again, on the 4th day during the 3 cycles. After taking of oral administration of BCAA and lactulose, the recurrence of hyperammonemic encephalopathy was not found. Therefore, 3 cycles of SOX plus Bmab therapy and 12 cycles of IRIS plus Bmab therapy were administered.

A 63-year-old man was diagnosed with advanced sigmoid cancer of pT3, pN0, sM1c, sP3, fStage Ⅳ post-operation. After CAPOX plus Bmab as the first-line chemotherapy, he underwent IRIS plus Bmab as the second-line chemotherapy. After 1 course of IRIS plus Bmab, he was admitted to the hospital for fever, dyspnea, and general fatigue. The white blood cell count was 6.2×10 3/mL, and the C-reactive protein was elevated to 12.9 mg/dL. The PaO2 of the artery blood gas analysis in room air was 46.3 mmHg, suggesting respiratory failure. He was diagnosed with PCP based on the bilateral diffused ground-glass opacities on chest CT along with an elevated serum b-D-glucan. The treatment of trimethoprim-sulfamethoxazole and steroid was then initiated. After the patient's clinical condition improved, he was discharged on day 27 post-admission.

P21-activated kinase 5 (PAK5), also termed PAK7, is one of the six members of the PAK family of serine/threonine kinases, which are downstream effectors in several cancer signaling pathways. PAK5 promotes neural outgrowth, contributes to microtubule stability and induces resistance to apoptosis. However, the clinical importance of PAK5 in gastric cancer has not been comprehensively investigated. In the present study, PAK5 expression was evaluated in gastric cancer tissue samples. Furthermore, the associations between high expression of PAK5, and clinicopathological features and prognosis were examined. PAK5 expression in primary gastric cancer specimens resected from 279 patients who underwent gastrectomy at the Tokyo Medical and Dental University Hospital was evaluated using immunohistochemistry. A 66-year-old man was postoperatively diagnosed with pT4a, pN2, cM1a(H2), cP0, fStage Ⅳ, RAS wild type rectal cancer. He underwent SOX plus Bmab chemotherapy 4 weeks later. After 9 courses of SOX plus Bmab, he was admitted to the hospital for leg edema and proteinuria(4+). Because of severe proteinuria(14.7 g/day)and low protein(Alb 2.0 g/dL, TP 4.9 g/dL), he was diagnosed with nephrotic syndrome. His general condition improved on stopping chemotherapy and administration of conservative treatment, and he was discharged on day 20 after admission. The proteinuria improved 3 months later. He had been undergoing SOX chemotherapy for 4 months.

A 70-year-old man underwent endoscopic submucosal dissection(ESD)for 2 early-stage gastric cancers in 2009 and 2014, respectively. Both were pathologically diagnosed after curative resection. In 2015, we detected swollen lymph nodes in the lesser curvature of the stomach on computed tomography(CT). PET-CT revealed that the swollen lymph nodes showed an abnormal uptake of fluorodeoxyglucose; therefore, we considered the possibility of metastatic recurrence of the gastric can- cer. The patient's serum a-fetoprotein(AFP)level was elevated to 30.6 ng/mL. The intraoperative pathological diagnosis of the swollen lymph node was tub2. We diagnosed this case as metastatic recurrence of gastric cancer and performed distal gastrectomy with lymph node dissection. Metastases were found in 3 of the resected lymph nodes. Immunohistochemical staining for AFP was positive in the specimens obtained via ESD in 2014 and in the metastatic lymph nodes. We diagnosed the patient with AFP-producing gastric cancer. On additional trimming of the specimen obtained via ESD in 2014, we found slight lymphatic invasion.

Background Receptor tyrosine kinases promote tumor progression in many cancers, although oncologic activation differs between diffuse-type gastric cancer (DGC) and intestinal-type gastric cancer (IGC). Fibroblast growth factor receptor (FGFR) is one RTK, and we previously reported the clinical significance of FGFR1, 2, 3, and 4 in gastric cancer. The aim of the present study was to reevaluate the clinical significance of FGFR1–4 expression separately in DGC and IGC. Methods Tumor samples, including 109 DGCs and 100 IGCs, were obtained from patients who underwent gastrectomy between 2003 and 2007 in our institution. The expression levels of FGFR1, 2, 3, and 4 were measured in the tumors by immunohistochemical analysis. Results In DGC, high expression of FGFR1, FGFR2, or FGFR4 was significantly associated with the depth of invasion, lymph-node metastasis, pathological stage, and distant metastasis or recurrent disease. Patients with high expression of FGFR1, FGFR2, or FGFR4 had significantly poorer disease-specific survival (DSS) (p = 0.009, p = 0.001, and p = 0.023, respectively). In IGC, only FGFR4 expression was significantly associated with factors relative to tumor progression and with shorter DSS (p = 0.012). Conclusion In conclusion, high FGFR4 expression correlated with tumor progression and survival in both DGC and IGC, whereas high expression of FGFR1 and 2 correlated with tumor progression and survival in only DGC.

Proximal gastrectomy (PG) is widely performed in Japan as a function-preserving surgical approach. Since esophagogastrostomy (EG) was associated with increased reflux symptoms and anastomotic strictures, we have chosen double-tract reconstruction (DTR) as the standard reconstruction method since March 2013. In this study, we described a novel method of laparoscopic DTR using detachable ENDO-PSD and compared its 1-year outcome with EG performed formerly in our institution. Patients who underwent laparoscopic PG between May 2005 and July 2014 were retrospectively divided into two groups based on the type of reconstruction and were subsequently analyzed (19 patients in the DTR group and 22 in the EG group). All of them underwent a laparoscopic PG with regional lymph node dissection. In the DTR group, the lower left port site was extended to 4 cm, and an intracorporeal purse-string suture was performed using the detachable ENDO-PSD. The jejunogastrostomy was fashioned on the anterior side of the remnant stomach parallel to the transection line, 2 cm from the cut end. The EG group used the conventional purse-string suture instrument through the 6 cm upper midline mini-laparotomy incision. Patient characteristics, operative data, early operative complications and 1-year postoperative follow-up findings were compared between the two groups. The frequencies of reflux symptoms (10.5 vs. 54.5%, P = 0.003), usage of proton pump inhibitors (31.6 vs. 72.7%, P = 0.008), and anastomotic strictures (0 vs. 27%, P = 0.014) were significantly lower in the DTR group as compared to the EG group. There were no significant differences between the two groups with regard to operation time, blood loss, postoperative hospital stay, postoperative complications, average postoperative/preoperative weight loss ratio, and postoperative/preoperative ratio of biochemical markers (hemoglobin, total protein, albumin, cholesterol). Our results indicate that DTR is a useful reconstruction method after PG, especially in terms of preventing reflux esophagitis and anastomotic strictures.

This paper empirically investigates how ownership structure of Japanese firms affects the risk-return profiles of their stocks. We find significant relationships between ownership and firms’ operational performance, i.e., the ownership of financial institutions is associated with poor performance while the ownership of large shareholders is associated with the opposite. However, comparing the returns on portfolios sorted by ownership, we find no significant relationships between ownership and the rates of returns. Specifically, excess returns are insignificant after controlling for three risk factors (i.e., market, size, and value) while factor loadings are significantly different across portfolios, i.e., the ownership of financial institutions is associated with low-risk, low-return portfolios while the ownership of large shareholders is associated with the opposite

In this study, morphology control of ZnO was performed by a wet chemical processing without templates at room temperature. In special, the effect of aging time and pH of this wet processing on ZnO morphologies was in detail investigated. As a result, products composed of a single phase of ZnO with a unique morphology like a hollow tube were obtained. Then, it was found that an aging process played an important role on controlling morphology of ZnO during this wet chemical process. Furthermore, it was found that this hollow-tubular ZnO had a high transparency.

In this paper, the effect of high magnetic field on the development of property were investigated by calcination of MgFe-LDH (CO3 )a s a precursor of ferrite material under applied field of 10 T. Obtained products were identified by X-ray diffraction analysis (XRD) and their microstructures were observed using scanning electron microscopy (SEM). Furthermore, their magnetic properties were evaluated by a superconducting quantum interference device (SQUID). It was observed that the particle size of mixed oxide prepared by calcination under 10 T was homogeneous and small. Saturation magnetization of mixed oxide synthesized under 10 T was higher than that of zero field. Therefore, it was found that calcination due to imposition of high magnetic field affected phase transition to mixed oxide and provided enhancement of its property. [doi:10.2320/matertrans.MI200710]

In democracies, ideological politicians often win elections. This phenomenon contradicts the Median Voter Theorem. I propose a model of election to explain this puzzle. Ideology can be a signal of ability, and if the benefits of ability outweigh the costs of ideology the electorate will prefer ideological politicians. Meanwhile, politicians disguise their ideology even if such disguise is costly. Under certain conditions, the most extreme politician is the most likely to be elected. Further, stiffer electoral competition can harm the electorate. These results contrast sharply with the standard belief that politicians’ desire to gain office leads to their ideological convergence.

The overexpression of fibroblast growth factor receptor (FGFR) 2 is an established prognostic factor and treatment target in gastric cancer. However, the roles of other FGFRs have not been fully elucidated. In this study, we investigated the correlations of the expression of FGFR1-4 with clinicopathological characteristics and outcomes in gastric cancer. Tumor samples were obtained from 222 patients with gastric adenocarcinoma who underwent gastrectomy between 2003 and 2007. A 67-year-old woman with epigastralgia was admitted to our hospital and was diagnosed with type 3 advanced gastric cancer with lymph node metastases.The clinical diagnosis was Stage III A(cT3, N2, M0).Since curative surgery was not feasible, we administered preoperative combination chemotherapy with docetaxel, cisplatin(CDDP), and S-1.After 3 courses of chemotherapy, the lymph nodes became undetectable on computed tomography(CT).Distal gastrectomy was performed with curative intent, and the final diagnosis was Stage IIA(ypT3, N0, M0).There has been no recurrence for 1 year and 4 months after the operation.

The patient was a 68-year-old man who had undergone sigmoidectomy 1 year previously. Adjuvant therapy with tegafur- uraci(l UFT) and Leucovorin( UZEL) was administered. Seven months later, the carcinoembryonic antigen( CEA) level increased to 7.5 ng/mL. Enhanced computed tomography (CT) revealed a 4-cm mass in the mesorectum, and the patient was diagnosed as having local recurrence. Chemotherapy with oxaliplatin, 5-fluorouracil, and Leucovorin( mFOLFOX6) and radiation therapy( 60 Gy) were administered. As the tumor could not be detected after chemoradiotherapy, abdominoperineal resection was performed. Pathological examination showed no cancer and indicated a complete response. The patients CEA level has not increased and no recurrence has been detected on enhanced CT for 3.5 years. The tumor could be decreased in size by chemoradiotherapy and the operation could be safely performed without resecting other organs. Although surgery is generally performed for local recurrence, multidisciplinary therapy could be useful in performing such surgery safely.

This paper investigates the theory of land price formation, taking into consideration the fact that an “option” is implicitly attached to land. Using a theoretical model, it tries to explain the land price bubbles in Japan in the late 1980s as the result of investors' expectations of alternative uses of the land. The model is estimated and validated using data on the Tokyo metropolitan residential area. The modelling exercise also determines the extent to which the option contributes to the formation of land prices. It is shown that the separation of the land price from its fundamental value can be satisfactorily explained by the option property of land. JEL Classification Numbers: C53, G12, R14.

Syntheses of LDH (layered double hydroxides) substituted with Fe3+ and Ni2+ were attempted by a co-precipitation method and their products were characterized by X-ray diffraction (XRD), Fourier transform infrared (FT-IR), and X-ray absorption fine structure (XAFS). XRD and FT-IR results indicated that obtained products were composed of a single phase of LDH with CO3 2- anions at interlayer, suggesting the successful syntheses of various modified LDH, Mg-Fe/LDH and Ni-Fe/LDH. In addition, XAFS measurements of Mg-Fe/LDH indicated that substituted Fe3+ located at octahedral site in LDH structure. For the application to water purification, these modified-LDH substituted with Fe3+ and Ni2+ as an adsorbent material were used for the magnetic separation system. As a results, these modified-LDH possessed the higher removal ability for As ions than ordinary Mg-Al/LDH and FeOOH, and furthermore were able to collect with high gradient magnetic separations. In addition, after As removal the existence conditions of As ions in LDH were examined by XANES.

This paper empirically analyzes the relationship between ownership structure and the determination of managers' bonuses in Japanese firms. In the analysis, we obtain two notable findings. First, equity ownership by non-financial corporations increases the sensitivity of managers' bonuses to firms' profits. Second, equity ownership by financial institutions increases the magnitude of bonus cuts in times of firms' distress. These findings suggest that cross shareholdings effectively transfer residual claims from shareholders to managers. They also suggest that, when firms are performing badly, financial institutions intervene in management and deprive managers of residual claims. Our empirical results imply the existence of two complementary managerial incentive mechanisms in Japan, which may substitute for external takeover threats.

Resistive RAM (ReRAM) has been recently developed for applications that require higher speed and lower voltage than Flash memory is able to provide. One of the applications is micro-controller units (MCUs) or SoCs with several megabits of embedded ReRAM. Another is solid-state drives (SSDs) where a combination of higher-density ReRAM and NAND flash memory would achieve high-performance and high-reliability storage [1], suitable for server applications for future cloud computing. ReRAM is attractive for several reasons. First, it operates at high speed and low voltage. Second, it enables high density due to the simple structure of the resistive element (RE) [2]. Third, it is immune to external environment such as magnetic fields or radiation, since the resistive switching is based on the redox reaction [3].

This chapter develops a formal, game—theoretic model to analyze the importance of monetary policy regimes and central bank commitments in international policy games in a world economy of two interdependent countries. In particular, it examines the welfare implications of the choice of monetary policy instruments, the ability of monetary authorities to commit themselves to policy targets, private—sector responses and initiatives in international policy games, and international monetary policy cooperation. Important elements of the model are the strategic interactions between the monetary authority and the private sector and the policy interactions between the two countries’ monetary authorities.

We have prepared the solid solution thin films with 0.4Fe3O40.6Fe2TiO4 composition (molar ratio) on MgO(100) substrates by a pulsed laser deposition technique and examined their magnetic and electrical properties. The thin films are epitaxially grown on the MgO substrate cube-on-cube relationship and have a flat and smooth surface. The epitaxial thin films show ferrimagnetism with Curie temperature beyond room temperature and exhibit n-type semiconducting behavior.

In this study, the synthesis of Fe substituted layered double hydroxide (LDH) clays with unique layered structures, Mg∕Fe-LDH, was attempted by a coprecipitation method, and subsequent decomposition behaviors on in situ heat treatments under a high magnetic field were investigated for these LDH clays. Specifically, the evaluations of microstructure of Mg∕Fe-LDH were in detail carried out after in situ heat treatments with a high magnetic field of 10T. As a result, it was found that the thermal decomposition behaviors of Mg∕Fe-LDH on in situ heat treatments under the exposure of 10T were significantly different from the ones under 0T and the formation of magnesioferrite (MgFe2O4) with a spinel-type structure was enhanced by the application of a high magnetic field. Furthermore, there were some distinct differences between the morphologies of MgFe2O4 prepared between under 0 and 10T that were caused by the anisotropic magnetic susceptibility of MgFe2O4. The demand for precision in measurement of dielectric loss (tan δ), which is important to clarify the electrical characteristics of dielectrics of capacitors, etc., is increasingly becoming raised in level these days. The authors have published a direct-reading system of tan δ using the phase comparison method and developed a digital tan-δ meter based on the above. The next experiment was construction of a more compact sized all-electronic-type automatic digital tan-δ meter by using a MOSFET as the resistance element for the direct reading of tan δ. As the result of repeated investigations on the main circuits, an automatic digital tan-δ meter was successfully produced having a precision of 10-3- 10-4 at several 10 Hz-several 100 kHz.
