We present measurements of single-photon ionization time delays between valence electrons of argon and neon using a coincidence detection technique that allows for the simultaneous measurement of both species under identical conditions. Taking into account the chirp of the ionizing single attosecond pulse (attochirp) ensures that the clock of our measurement technique is started at the same time for both types of electrons, revealing with high accuracy and resolution energy-dependent time delays of a few tens of attoseconds. By comparing our results with theoretical predictions, we confirm that the so-called Wigner delay correctly describes single-photon ionization delays as long as atomic resonances can be neglected. Our data, however, also reveal that such resonances can greatly affect the measured delays beyond the simple Wigner picture.

Objective: COVID-19 challenges curriculum managers worldwide to create digital substitutes for classroom teaching. Case-based teaching formats under expert supervision can be used as a substitute for practical bedside teaching, where the focus is on teaching clinical reasoning skills.Methods: For medical students of LMU and TU Munich, the interactive, case-based, and supervised teaching format of Clinical Case Discussion (CCD) was digitised and implemented as dCCD in their respective curricula. Case discussions were realised as videoconferences, led by a student moderator, and took place under the supervision of a board-certified clinician. To prevent passive participation, additional cognitive activations were implemented. Acceptance, usability, and subjective learning outcomes were assessed in dCCDs by means of a special evaluation concept.Results: With regard to acceptance, students were of the opinion that they had learned effectively by participating in dCCDs (M=4.31; SD=1.37). The majority of students also stated that they would recommend the course to others (M=4.23; SD=1.62). The technical implementation of the teaching format was judged positively overall, but findings for usability were heterogeneous. Students rated their clinical reasoning skills at the end of the dCCDs (M=4.43; SD=0.66) as being significantly higher than at the beginning (M=4.33; SD=0.69), with low effect size, t(181)=-2.352, p=.020, d=0.15.Conclusion: Our evaluation data shows that the dCCD format is well-accepted by students as a substitute for face-to-face teaching. In the next step, we plan to examine the extent to which participation in dCCDs leads to an increase in objectively measured clinical reasoning skills, analogous to a face-to-face CCD with on-site attendance. Zielsetzung: COVID-19 stellt Curriculums-Verantwortliche weltweit vor die Herausforderung, digitale Ersatzangebote für Präsenzlehre zu schaffen. Fallbasierte Formate unter Supervision bieten sich als Ersatz für klinisch-praktischen Unterricht am Krankenbett an, bei dem die Vermittlung klinischer Entscheidungsfindungskompetenzen im Vordergrund steht.Methodik: Für Medizinstudierende der LMU und der TU München wurde das interaktive, fallbasierte und supervidierte Lehrformat der Clinical Case Discussion (CCD) digitalisiert und als dCCD in die jeweiligen Curricula implementiert. Die Falldiskussionen wurden als Videokonferenz realisiert, von einem studentischen Moderator angeleitet und fanden unter Supervision eines Klinikers auf Facharztniveau statt. Um eine passive Teilnahme zu unterbinden, wurden kognitive Aktivierungen implementiert. Mit Hilfe eines speziellen Evaluationskonzeptes wurden Akzeptanz, Usability und subjektiver Lernerfolg bei der dCCD erfasst.Ergebnisse: Hinsichtlich der Akzeptanz waren die Studierenden der Meinung, durch die Teilnahme an den dCCDs effektiv gelernt zu haben (M=4.31; SD=1.37). Eine Mehrheit gab außerdem an, den Kurs weiterempfehlen zu wollen (M=4.23; SD=1.62). Die technische Umsetzung des Lehrformats wurde insgesamt positiv beurteilt, für die Usability ergab sich ein heterogenes Bild. Studierende schätzten ihre klinischen Entscheidungsfindungskompetenzen am Ende der dCCDs (M=4.43; SD=0.66) bei geringer Effektstärke signifikant höher ein als zu Beginn (M=4.33; SD=0.69), t(181)=-2.352, p=.020, d=0.15.Schlussfolgerung: Unsere Evaluationsdaten zeigen, dass die dCCD von den Studierenden als Ersatz für Präsenzlehre gut angenommen wird. In einem nächsten Schritt soll überprüft werden, inwiefern die Teilnahme an dCCDs analog zu einer CCD-Präsenzveranstaltung zu einem Zuwachs an objektiv gemessenen klinischen Entscheidungsfindungskompetenzen führt.

Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. Conclusion: The PREFER in VTE registry will provide valuable insights into the characteristics of patients with VTE and their acute and mid-term management, as well as into drug utilization and the use of health care resources in acute first-time and/or recurrent VTE across Europe in clinical practice. - Abstract
Background: Venous thromboembolism (VTE) is a major health problem, with over one million events every year in Europe. However, there is a paucity of data on the current management in real life, including factors influencing treatment pathways, patient satisfaction, quality of life (QoL), and utilization of health care resources and the corresponding costs. The PREFER in VTE registry has been designed to address this and to understand medical care and needs as well as potential gaps for improvement. Methods/design: The PREFER in VTE registry was a prospective, observational, multicenter study conducted in seven European countries including Austria, France Germany, Italy, Spain, Switzerland, and the UK to assess the characteristics and the management of patients with VTE, the use of health care resources, and to provide data to estimate the costs for 12 months treatment following a first-time and/or recurrent VTE diagnosed in hospitals or specialized or primary care centers. In addition, existing anticoagulant treatment patterns, patient pathways, clinical outcomes, treatment satisfaction, and health related QoL were documented. The centers were chosen to reflect the care environment in which patients with VTE are managed in each of the participating countries. Patients were eligible to be enrolled into the registry if they were at least 18 years old, had a symptomatic, objectively confirmed first time or recurrent acute VTE defined as either distal or proximal deep vein thrombosis, pulmonary embolism or both. After the baseline visit at the time of the acute VTE event, further follow-up documentations occurred at 1, 3, 6 and 12 months. Follow-up data was collected by either routinely scheduled visits or by telephone calls. Results: Overall, 381 centers participated, which enrolled 3,545 patients during an observational period of 1 year. SummaryVenous thromboembolism (VTE) is a significant cause of morbidity and mortality in Europe. Data from real-world registries are necessary, as clinical trials do not represent the full spectrum of VTE patients seen in clinical practice. We aimed to document the epidemiology, management and outcomes of VTE using data from a large, observational database. PREFER in VTE was an international, non-interventional disease registry conducted between January 2013 and July 2015 in primary and secondary care across seven European countries. Consecutive patients with acute VTE were documented and followed up over 12 months. PREFER in VTE included 3,455 patients with a mean age of 60.8 ± 17.0 years. Overall, 53.0% were male. The majority of patients were assessed in the hospital setting as inpatients or outpatients (78.5%). The diagnosis was deep-vein thrombosis (DVT) in 59.5% and pulmonary embolism (PE) in 40.5%. The most common comorbidities were the various types of cardiovascular disease (excluding hypertension; 45.5%), hypertension (42.3%) and dyslipidaemia (21.1%). Following the index VTE, a large proportion of patients received initial therapy with heparin (73.2%), almost half received a vitamin K antagonist (48.7%) and nearly a quarter received a DOAC (24.5%). Almost a quarter of all presentations were for recurrent VTE, with >80% of previous episodes having occurred more than 12 months prior to baseline. In conclusion, PREFER in VTE has provided contemporary insights into VTE patients and their real-world management, including their baseline characteristics, risk factors, disease history, symptoms and signs, initial therapy and outcomes.

Organ-transplant-recipients exhibit cancerization of the skin from which multiple human papillomavirus (HPV)-positive squamous cell carcinomas (SCCs) arise. However, the molecular basis for HPV-induced invasion of skin keratinocytes is not known. We generated a transgenic mouse model expressing the E7 oncoprotein of HPV8 in the murine epidermis under the control of the keratin-14 promoter and showed that E7 is carcinogenic in mice. We further showed that both, the E7-expressing keratinocyte and mesenchymal components of the extracellular matrix as critical in eliciting the invasive behavior. E7 expression in basal keratinocytes, grown on fibronectin, led to epithelial-mesenchymal transition mediated by a cadherin switch. E7-positive keratinocytes displayed enhanced EDA-fibronectin expression and secretion and stimulated dermal fibroblasts to express EDA-fibronectin. Deposition of fibronectin was also detected in the peritumoral stroma of HPV8-positive skin SCC. When grown on fibronectin, E7-positive keratinocytes, in particular stem cell-like cells, exhibited increased cell surface levels of the alpha 3-integrin chain. Functional blocking confirmed alpha 3 as a critical molecule sufficient to induce E7-mediated invasion. This mechanistic link is further supported by expression of an E7-mutant, impaired in targeting alpha 3 to the cell surface. These findings highlight the importance of epithelial-extracellular matrix interaction required for keratinocyte invasion and provide further mechanistic evidence for a role of HPV in skin carcinogenesis.

- Abstract
Time delays of electrons emitted from an isotropic initial state with the absorption of a single photon and leaving behind an isotropic ion are angle independent. Using an interferometric method involving XUV attosecond pulse trains and an IR-probe field in combination with a detection scheme, which allows for full three-dimensional momentum resolution, we show that measured time delays between electrons liberated from the 1s2 spherically symmetric ground state of helium depend on the emission direction of the electrons relative to the common linear polarization axis of the ionizing XUV light and the IR-probing field. Such time delay anisotropy, for which we measure values as large as 60 as, is caused by the interplay between final quantum states with different symmetry and arises naturally whenever the photoionization process involves the exchange of more than one photon. With the support of accurate theoretical models, the angular dependence of the time delay is attributed to small phase differences that are induced in the laser-driven continuum transitions to the final states. Since most measurement techniques tracing attosecond electron dynamics involve the exchange of at least two photons, this is a general and significant effect that must be taken into account in all measurements of time delays involving photoionization processes.

- Abstract
We present measurements of single-photon ionization time delays between the outermost valence electrons of argon and neon using a coincidence detection technique that allows for the simultaneous measurement of both species under identical conditions. The analysis of the measured traces reveals energy-dependent time delays of a few tens of attoseconds with high energy resolution. In contrast to photoelectrons ejected through tunneling, single-photon ionization can be well described in the framework of Wigner time delays. Accordingly, the overall trend of our data is reproduced by recent Wigner time delay calculations. However, besides the general trend we observe resonance features occurring at specific photon energies. These features have been qualitatively reproduced and identified by a calculation using the multiconfigurational Hartree-Fock method, including the influence of doubly excited states and ionization thresholds.

- Abstract
Background: According to the International Society on Thrombosis and Haemostasis (ISTH), intramuscular hematoma without other severity criteria is not considered a major bleeding.Objectives: In a large cohort of reversed vitamin K antagonist (VKA) patients admitted to the emergency unit for muscular hematoma, we assess frequency, severity, and anticoagulation management based on whether ISTH criteria were met or not.Materials and Methods: We performed a retrospective single-center study involving patients admitted to an emergency unit for VKA-induced intramuscular hematoma whose bleeding was reversed with prothrombin complex concentrates.Results: During the study period, 631 VKA-induced bleeding events occurred in our emergency unit, of which 73 (11.6%) were intramuscular hematomas and half met ISTH criteria. The mean age was 75.5years (95% CI=72.6-78.3). Admission blood tests showed that patients with ISTH criteria had higher international normalized ratio (7.0±4.6 vs. 4.1±3.0, p=0.002) and lower hemoglobin (8.1±1.8 vs. 11.9±2.2, p<0.001) than those without. Patients with ISTH criteria were more likely to have intramuscular hematoma in the iliopsoas, gluteal, and pectoral muscles than those without. Interestingly, two-thirds of rectus sheath hematomas involved patients without ISTH criteria. However, patients with or without ISTH criteria exhibited a similar hospitalization duration and rate of re-bleeding.Conclusion: We showed that half of the patients admitted with intramuscular hematoma could not be qualified as having ISTH-criteria major bleeding. Interestingly, these patients displayed a similar hospitalization duration and rate of re-bleeding to those with ISTH-criteria major bleeding. [ABSTRACT FROM AUTHOR]- Published

International audience; BACKGROUND: A fixed-dose regimen of rivaroxaban, an oral factor Xa inhibitor, has been shown to be as effective as standard anticoagulant therapy for the treatment of deep-vein thrombosis, without the need for laboratory monitoring. This approach may also simplify the treatment of pulmonary embolism. METHODS: In a randomized, open-label, event-driven, noninferiority trial involving 4832 patients who had acute symptomatic pulmonary embolism with or without deep-vein thrombosis, we compared rivaroxaban (15 mg twice daily for 3 weeks, followed by 20 mg once daily) with standard therapy with enoxaparin followed by an adjusted-dose vitamin K antagonist for 3, 6, or 12 months. The primary efficacy outcome was symptomatic recurrent venous thromboembolism. The principal safety outcome was major or clinically relevant nonmajor bleeding. RESULTS: Rivaroxaban was noninferior to standard therapy (noninferiority margin, 2.0; P=0.003) for the primary efficacy outcome, with 50 events in the rivaroxaban group (2.1%) versus 44 events in the standard-therapy group (1.8%) (hazard ratio, 1.12; 95% confidence interval [CI], 0.75 to 1.68). The principal safety outcome occurred in 10.3% of patients in the rivaroxaban group and 11.4% of those in the standard-therapy group (hazard ratio, 0.90; 95% CI, 0.76 to 1.07; P=0.23). Major bleeding was observed in 26 patients (1.1%) in the rivaroxaban group and 52 patients (2.2%) in the standard-therapy group (hazard ratio, 0.49; 95% CI, 0.31 to 0.79; P=0.003). Rates of other adverse events were similar in the two groups. Mit Hilfe des erweiterten GAL4/UAS-Transplantationssystems wurde die Herkunft und Differenzierung abdominaler Mesodermderivate von Drosophila melanogaster untersucht. Neben der Analyse der klonalen Zusammensetzung syncytialer Zellverbände ermöglichte diese Methode die Blastoderm-Anlagenkartierung larvaler und imaginaler Gewebe. Durch die Verwendung der in vivo-Marker DsRed und GFP konnte außerdem die gesamte cell-lineage zu verschiedenen Entwicklungszeitpunkten untersucht und Aussagen zur Persistenz einzelner Gewebe " wie der visceralen Muskulatur" getroffen werden. Für die imaginale Myogenese der abdominalen somatischen Muskulatur konnte gezeigt werden, dass neben der Fusion nicht-verwandter Myoblasten auch das Prinzip der genetischen Determination verschiedener Myoblasten-Populationen analog (und was die genetische Identität der beteiligten Myoblasten betrifft, sogar homolog) zur larvalen Myogenese erfolgt.

Fully transparent organic field effect transistors (OFET) in top- and bottom-gate layouts were prepared. Poly(3,4-ethylenedioxythiophene) polystyrene sulfonate (PEDOT:PSS) was structured photolithographically as either gate- or source- and drain-contact on top of poly(4-vinylphenol) (PVP). The dielectric material was fitted to the sequence of materials by combining water soluble polyvinyl alcohol (PVA) and PVP. N-type conducting hexadecafluorophthalocyaninato-copper (F 16 PcCu) was used as organic semiconductor sensitive to interface traps. Critical boundary surfaces for film growth were examined by microscopic methods, which revealed smooth polymer surfaces as a result of our preparation and lithography steps characterizing the presented concept as a viable alternative to existing approaches. Here we present a successful combination of an attosecond beamline with a COLTRIMS apparatus, which we refer to as AttoCOLTRIMS. The setup provides either single attosecond pulses or attosecond pulse trains for extreme ultraviolet-infrared pump-probe experiments. We achieve full attosecond stability by using an active interferometer stabilization. The capability of the setup is demonstrated by means of two measurements, which lie at the heart of the COLTRIMS detector: firstly, we resolve the rotating electric field vector of an elliptically polarized few-cycle infrared laser field by attosecond streaking exploiting the access to the 3D momentum space of the charged particles. Secondly, we show streaking measurements on different atomic species obtained simultaneously in a single measurement making use of the advantage of measuring ions and electrons in coincidence. Both of these studies demonstrate the potential of the AttoCOLTRIMS for attosecond science. [ABSTRACT FROM AUTHOR]

- Abstract
Alternative methods for the extraction of unchanged carbon tetrachloride residues from fumigated whole and ground wheat and maize were examined and compared. Amounts of carbon tetrachloride extracted were determined by gas-liquid chromatography using two separate columns and an electron-capture detector. A portion of any carbon tetrachloride residue was found to be converted to chloroform by a steam distillation extraction method but not when a cold solvent extraction process was used. In addition, the effectiveness of removal of carbon tetrachloride from wheat and maize during a 3.5 h steam distillation was progressively lessened, in comparison with the cold extraction process, as the length of time that residual fumigant had been associated with the grain increased. The rate of elimination of carbon tetrachloride from wheat and maize during airing at two temperatures was determined and though partially dependent on the temperature of fumigation, airing was consistently more rapid at 25° than at 10°C. Initial residues of 200 to 400 parts/million in whole grains fell to 1 to 10 parts/million when aired for six months in thin layers at 25°C and to 5 to 50 parts/million at WC. Residues in wheat disappeared more rapidly than those in maize. Grinding initially caused a sharp reduction in carbon tetrachloride content but subsequent airing rates were little faster than those of the whole grains. It is concluded that complete elimination of trace amounts of carbon tetrachloride from products of treated grain is unlikely even after milling but the toxicological significance of such residues is uncertain. The effects of dosage, temperature, moisture content and conditions of storage on the retention of ethylene oxide by a range of commodities after exposure to the vapour were studied in laboratory tests at treatment levels normal for sterilisation and for insect control. The extent of reaction of ethylene oxide with inorganic halides and water to form halohydrins and glycols was investigated with reference to amounts of inorganic chloride and bromide present, moisture content and acidity of the commodity, and storage temperature. Residual compounds were determined by gas-liquid chromatography of acetone-water extracts during a period of up to a year after treatment. When commodities were kept at 25°c, either under air-tight storage conditions or freely aired, residual ethylene oxide usually fell to below 1 ppm within 14 days, but in flour kept under air-tight conditions after treatment at sterilisation level, 50-100 ppm remained at this time and traces were found after 90 days. At lower temperatures ethylene oxide disappeared more slowly. Levels of ethylene chlorohydrin produced during and after exposure to ethylene oxide ranged from zero in groundnuts and cocoa beans at fumigation dosage to thousands of parts per million in sterilised curry powder and turmeric. Ethylene chlorohydrin, ethylene bromohydrin, ethylene glycol and diethylene glycol persisted in some commodities after treatment but the halohydrins disappeared from finely divided products under the influence of air movement and ethylene bromohydrin slowly decomposed under sealed conditions. In the preparation of baked and steamed products from flour containing ethylene chlorohydrin and ethylene bromohydrin, from 20 to 100% of the original residue was lost, depending on the alkalinity of the conditions used. It is concluded that residual amounts of ethylene oxide and glycols produced in foodstuffs by such treatments are unlikely to constitute a toxic hazard, and that in ethylene chlorohydrin formed otherwise significant amounts may partly disappear during storage and cooking. The presence of glycols or halohydrins in treated commodities could be interpreted as an indication that other changes in chemical composition may have taken place, the overall effects of which might best be determined by long-term animal feeding trials. A number of methods in common use for determining amounts of bromine-containing compounds occurring as residues in foodstuffs, primarily as a result of commodity or soil fumigation, do not distinguish between bromide ion and organically bound bromine. Although such compounds may differ widely in their toxicity, national and international schedules have included tolerances for total bromide determined as 'inorganic bromide'. A method is described in which ionised bromide in foodstuffs is quantitatively converted to 2-brompethanol (ethylene bromohydrin) by reaction with ethylene oxide in an acid medium. The 2-bromoethanol produced is determined in processed extracts by gas-liquid chromatography. Residual methyl bromide and ethylene dibromide (1,2-dibromoethane), neither of which react with ethylene oxide under the conditions prescribed, are separated and determined concurrently. With foodstuffs which did not contain organo-bromine compounds, good agreement was observed between results obtained by the gas chromatographic method and by total bromide determination involving alkali digestion, controlled ashing, extraction of ash and wet oxidation. In the light of the more specific methods of determination now available, some clarification in tolerance schedules for residual bromine compounds in foodstuffs is suggested. [ABSTRACT FROM AUTHOR]

49. Residual free methyl bromide in fumigated commodities.

- Abstract
The extent to which methyl bromide was retained by fumigated material after treatment was followed in laboratory studies on a range of commodities exposed to the vapour at atmospheric pressure. Amounts of methyl bromide, recovered by solvent extraction and determined using gas-liquid chromatography, were related to the temperature, moisture content and manner of post-treatment storage. Immediately after exposure, the initial amount of free methyl bromide present was more dependent on the gas concentration used than on the time of exposure. Under the experimental conditions of exposure, the residual free methyl bromide in all commodities fumigated at 25°, except cocoa beans and groundnuts, fell to below 1 ppm within a few days when they were held at that temperature, whether spread in thin layers on trays or kept sealed in glass bottles. At lower temperatures, the rate of loss was slower, small amounts of methyl bromide being extracted from several commodities one month after treatment. The disappearance of fumigant from wheat and sultanas was more rapid from samples with higher moisture contents. A mathematical treatment of the data is presented, to assist in prediction of the behaviour of residual fumigant under storage conditions before processing. It is concluded that the risk of ingestion of harmful quantities of free methyl bromide by the consumer is small and that the occasions when relatively high residues might occur can be predicted.
