1. Web-based Training an deutschen Universitäts-Augenkliniken – Lehre 2.0? [Web-based Training in German University Eye Hospitals – Education 2.0?]

Abstract

[english] Purpose: To analyse web-based training in ophthalmology offered by German university eye hospitals. Methods: In January 2010 the websites of all 36 German university hospitals were searched for information provided for visitors, students and doctors alike. We evaluated the offer in terms of quantity and quality. Results: All websites could be accessed at the time of the study. 28 pages provided information for students and doctors, one page only for students, three exclusively for doctors. Four pages didn’t offer any information for these target groups. The websites offered information on events like congresses or students curricular education, there were also material for download for these events or for other purposes. We found complex e-learning-platforms on 9 pages. These dealt with special ophthalmological topics in a didactic arrangement. In spite of the extensive possibilities offered by the technology of Web 2.0, many conceivable tools were only rarely made available. It was not always possible to determine if the information provided was up-to-date, very often the last actualization of the content was long ago. On one page the date for the last change was stated as 2004. Conclusion: Currently there are 9 functional e-learning-applications offered by German university eye hospitals. Two additional hospitals present links to a project of the German Ophthalmological Society. There was a considerable variation in quantity and quality. No website made use of crediting successful studying, e.g. with CME-points or OSCE-credits. All German university eye hospitals present themselves in the World Wide Web. However, the lack of modern, technical as well as didactical state-of-the-art learning applications is alarming as it leaves an essential medium of today’s communication unused. [german] Zielsetzung: Analyse der webbasierten ophthalmologischen Lernprogramme, welche von den Internetseiten der Universitäts-Augenkliniken in Deutschland angeboten werden. Methodik: Im Januar 2010 wurden in dieser prospektiven Studie alle 36 Internetseiten deutscher Universitäts-Augenkliniken besucht. Dabei wurde das angebotene Aus-, Weiter- und Fortbildungsangebot für Studierende und Ärzte quantitativ und qualitativ evaluiert. Ergebnisse: Die Homepages aller Kliniken waren zum Zeitpunkt der Besuche erreichbar. 28 Internetseiten hielten Informationen zur Aus-, Weiter- oder Fortbildung für Studierende und Ärzte vor. Eine Seite beinhaltete nur Angebote für Studenten, 3 ausschließlich für Ärzte. Auf 4 Seiten wurde für beide Zielgruppen kein Angebot gemacht. Das Angebot umfasste Informationen zu aktuellen Veranstaltungen wie ärztlichen Fortbildungen oder Lehrveranstaltungen für Studierende, weiter wurden Skripte oder ähnliches Material zum download bereitgestellt. Auf 9 Seiten wurden komplexe e-learning-Plattformen aufgefunden, welche sich einem bestimmten Thema im Rahmen einer logisch-didaktischen Gliederung widmen. Trotz der umfangreichen Möglichkeiten, welches das Web 2.0 bietet, wurden viele denkbare Hilfsmittel nur selten bereitgestellt. Die Aktualität des Angebots war nicht immer zu ermitteln, oft lag die letzte Aktualisierung lange zurück. Auf einer Seite war als letzte Aktualisierung der Fallsammlung das Jahr 2004 angegeben. Schlussfolgerung: Von den Homepages der Augenabteilungen deutscher Universitätskliniken werden 9 funktionsfähige e-learning-Applikationen zur Verfügung gestellt. Zwei weitere Kliniken verweisen auf ein Projekt der Deutschen Ophthalmologischen Gesellschaft. Das Angebot zeigt eine erhebliche Variation in Quantität und Qualität. In keinem Fall wird die Möglichkeit der Anrechnung des Besuchs dieser Programme auf die studentische Ausbildung (z.B. OSCE-Credits) oder die ärztliche Weiter- oder Fortbildung, z.B. in Form von CME-Punkten, genutzt.Der Mangel an modernen, technischen wie didaktischen Anforderungen genügenden Lernapplikationen ist alarmierend, wird doch ein wesentliches Kommunikations- und damit Lehr und Lernmedium von deutschen Universitäts-Augenkliniken nur zu einem kleinen Teil genutzt.

Abstract

The receptor for advanced glycation end-products (RAGE) has been implicated in the pathophysiology of chronic obstructive pulmonary disease (COPD). However, it is still unknown whether RAGE directly contributes to alveolar epithelial damage and abnormal repair responses. We hypothesize that RAGE activation not only induces lung tissue damage but also hampers alveolar epithelial repair responses. The effects of the RAGE ligands LL-37 and HMGB1 were examined on airway inflammation and alveolar tissue damage in wild-type and RAGE-deficient mice and on lung damage and repair responses using murine precision cut lung slices (PCLS) and organoids. In addition, their effects were studied on the repair response of human alveolar epithelial A549 cells, using siRNA knockdown of RAGE and treatment with the RAGE inhibitor FPS-ZM1. We observed that intranasal installation of LL-37 and HMGB1 induces RAGE-dependent inflammation and severe alveolar tissue damage in mice within 6 h, with stronger effects in a mouse strain susceptible for emphysema compared with a nonsusceptible strain. In PCLS, RAGE inhibition reduced the recovery from elastase-induced alveolar tissue damage. In organoids, RAGE ligands reduced the organoid-forming efficiency and epithelial differentiation into pneumocyte-organoids. Finally, in A549 cells, we confirmed the role of RAGE in impaired repair responses upon exposure to LL-37. Together, our data indicate that activation of RAGE by its ligands LL-37 and HMGB1 induces acute lung tissue damage and that this impedes alveolar epithelial repair, illustrating the therapeutic potential of RAGE inhibitors for lung tissue repair in emphysema.

Abstract

Background On festive days worldwide eyes are severely injured by fireworks. The data on the number and severity are to date not registered in Germany. Objective How frequent are firework-induced injuries in Germany, who are the affected, how serious are the injuries, which forms of treatment are necessary and how frequent are accompanying injuries? Method A German nationwide online-based survey was carried out in all inpatient eye departments and data over the last 3 consecutive years were descriptively analyzed. Results From New Years Eve 2016/2017, when 41 eye departments sent in data, the participation could be increased to 51 eye departments in 2018/2019. More than one third (33-39%) of all 1356 patients over 3 years were minors, 60% were younger than 25 years old and roughly 60% of patients were injured as bystanders or in an unclear situation. In total 25% of all eye injuries were considered severe and required inpatient treatment. Accompanying injuries of the other eye, the face and hands were more frequent in minors than in adults. Eyeball ruptures were reported in 10 minors and 38 adults over the 3 years. Conclusion Particularly minors and bystanders need better protection.

Abstract

A study of the hydrogen atom transfer in indole(NH3)(sub n) clusters excited at 263 nm to the S(sub 1)(pi pi*) state using pump-probe experiments with femtosecond laser pulses is presented. The results show that for small clusters, the reaction is characterized by two successive processes on distinct time scales, the first is the time constant of the primary process which is in the sub-ps region whereas the secondary decay time growing with the cluster size reflects a relaxation process within 25 to 150 ps.

Abstract

The airway epithelium regulates responses to aeroallergens, acting as a physical and immunological barrier. In asthma, epithelial barrier function and the expression of adherens junction protein E-cadherin is compromised, but it is unknown whether this is cause or consequence of the disease. We hypothesized that airway epithelial loss of E-cadherin is a critical step in the development of manifestations of asthma. We generated a transgenic mouse model with conditional loss of E-cadherin in lung epithelial cells at birth and onwards. We observed normal lung development at the time of birth in mice lacking E-cadherin in the lung epithelium. However, E-cadherin deficiency led to progressive epithelial damage in mice growing into adulthood, as evidenced by airway epithelial denudation, decreased zonula occludens (ZO)-1 expression, loss of ciliated cells, and enlarged alveolar spaces. In addition, spontaneous goblet cell metaplasia with mucus production was observed. These epithelial changes were accompanied by elevated levels of the epithelial-derived chemokine CCL17, infiltration of eosinophils and dendritic cells, and mucus production. In conclusion, loss of E-cadherin induces features in the lung reminiscent of those observed in asthma, indicating that the disruption of E-cadherin-mediated cell-cell contacts may play a key role in the development of asthma manifestations.
