15. Inflammatory sacroiliitis in childhood

Abstract

A 27-month-old boy and a 10-month-old girl with unilateral inflammatory sacroiliitis are described. Both presented with refusal to walk or to stand. Increased erythrocyte sedimentation rate and negative HLA-B27 were found in both cases. There were no laboratory findings to suggest an underlying rheumatic disease. Radiograms of the sacroiliac (SI) joints, lumbosacral spines and the hip joints were normal. Joint/bone scan revealed increased radionuclide activity over the involved SI joints in both cases. Computerized tomograms of the SI joints were abnormal in one patient. Both patients improved with aspirin, recovered and had no sequelae. Inflammatory sacroiliitis appears to be transient and benign. It is a rare event in young children. Possibly some of them are being treated as though they had septic arthritis.

Abstract

Tendinous xanthomata with maximum expression on Achilles tendons are a characteristic feature of cerebrotendinous xanthomatosis, a rare autosomal recessive lipid storage disease. Twenty patients are under review; three of them, at different stages of the disease, underwent computed tomographic (CT) examination. CT demonstrates with a high degree of accuracy the increased size of the tendon and its heterogeneous structure resulting from cholesterol and cholestanol crystal deposits. [ABSTRACT FROM AUTHOR]

Abstract

Acute pancreatitis may present with a wide variety of clinical findings and may mimic other medical or surgical conditions. Three patients with acute pancreatitis are presented with clinical features of intestinal obstruction, perinephric abscess, and severe abdominal pain, respectively. The diagnosis and mode of spread of the inflammatory process in these patients was made by computed tomography. 29. Haemangiopericytoma of the larynx

Abstract

Haemangiopericytoma was first described in 1942 by Stout and Murray as a vascular tumour derived from Zimmermann's pericytes. Ultrastructural study supports the concept of a pericyte origin (Battifora, 1973; Nunnery et al, 1981).Haemangiopericytomas may arise in any part of the body with subcutaneous and muscular tissue predominating, but the larynx is an exceptionally rare location. The CT scan, angiographic examination and pre-operative embolisation in this situation have not previously been reported.A 34-year-old man was admitted with a six-month history of hoarseness getting progressively worse. Dyspnoea was present only two weeks prior to admission. There was no associated pain.Physical examination with special reference to the neck revealed no abnormality. Indirect laryngoscopy showed a large supraglottic cyst-like mass, vascular in appearance, which produced significant stridor. Direct laryngoscopy confirmed these findings.The plain lateral radiograph of the neck revealed a large well-defined supraglottic soft tissue mass extending up to the hyoid bone. No calcification or bony destruction was noted (Fig. 1). The CT scans before and after contrast medium, showed the presence of a large enhancing vascular mass (Fig. 2a, b). A preoperative angiogram demonstrated the hypervascular mass supplied predominantly by the superior thyroid artery (Fig. 3a). A persistent tumour blush without early venous filling was noted. A Gianturco 5 mm wool coil standard type manufactured by the Cook Co. was introduced into the superior thyroid artery. A control picture after 10 minutes showed arterial occlusion (Fig. 3b).
