Abstract

The measurement of quality indicators supports quality improvement initiatives. The German Interdisciplinary Society of Intensive Care Medicine (DIVI) has published quality indicators for intensive care medicine for the fourth time now. After a scheduled evaluation after three years, changes in several indicators were made. Other indicators were not changed or only minimally. The focus remained strongly on relevant treatment processes like management of analgesia and sedation, mechanical ventilation and weaning, and infections in the ICU. Another focus was communication inside the ICU. The number of 10 indicators remained the same. The development method was more structured and transparency was increased by adding new features like evidence levels or author contribution and potential conflicts of interest. These quality indicators should be used in the peer review in intensive care, a method endorsed by the DIVI. Other forms of measurement and evaluation are also reasonable, for example in quality management. This fourth edition of the quality indicators will be updated in the future to reflect the recently published recommendations on the structure of intensive care units by the DIVI. Die Messung relevanter Qualitätsindikatoren unterstützt Initiativen zur Qualitätsverbesserung. Die Deutsche Interdisziplinäre Vereinigung für Intensiv- und Notfallmedizin (DIVI) hat die intensivmedizinischen Qualitätsindikatoren nun zum vierten Mal publiziert. Sie wurden nach drei Jahren überarbeitet und einige Indikatoren wurden angepasst. Andere Indikatoren erfuhren keine oder nur minimale Änderungen. Der Fokus besteht weiter auf relevanten Behandlungsprozessen wie Management von Analgesie und Sedierung, Beatmung und Weaning sowie Infektionen auf der Intensivstation. Die Gesamtzahl von zehn Indikatoren blieb bestehen. Die Entwicklung der Indikatoren erfolgte in der neuen Version nach einem anderen Prozess. Die Transparenz wurde durch Erwähnung der Einzelbeiträge der Autoren sowie auch potenzieller Interessenkonflikte erhöht. Die Anwendung der Qualitätsindikatoren im Peer-Review-Prozess der DIVI ist weiter wesentlicher Schwerpunkt. Aber auch andere Anwendungen, z.B. im Qualitätsmanagement, sind sinnvoll. Diese vierte Auflage der Qualitätsindikatoren wird noch einmal aktualisiert, um die kürzlich veröffentlichten Empfehlungen der DIVI zur Struktur der Intensivstationen zu berücksichtigen.

8. Besonderheiten des kindlichen Asthmas

Abstract

Zusammenfassung: Asthma ist die häufigste chronische Erkrankung im Kindesalter mit Beginn meist schon vor dem 5. Lebensjahr. Pathophysiologisch gesehen findet sich eine Hyperreagibilität der Atemwege, die auf einer chronischen Entzündung der Bronchialschleimhaut, insbesondere im Bereich der kleinen Atemwege, beruht. Die Pathogenese ist durch eine komplexe Interaktion von endogenen Faktoren (z. B. genetische Disposition) und exogenen Auslösern, die Klinik durch eine überwiegend anfallsartige expiratorische Atembehinderung charakterisiert. Vor allem im Winterhalbjahr kommt es bei Kleinkindern gehäuft zum Auftreten viral ausgelöster obstruktiver Bronchitiden. Auch wenn es Hinweise für die Bedeutung früher viraler Luftwegsinfektionen für die spätere Entwicklung einer bronchialen Hyperreagibilität gibt, entwickelt die Mehrzahl der betroffenen Kinder im weiteren Leben kein Asthma. Eine exakte Anamnese (Atopie) und Differenzialdiagnostik (Mukoviszidose etc.) sind von entscheidender Bedeutung. Die Schulung der kleinen Patienten und ihrer Eltern (ggf. Lehrer) spielt für die Bewältigung dieser chronischen Erkrankung eine besondere Rolle. Die Therapie sollte so optimiert werden, dass die Kinder sich normal entwickeln und möglichst uneingeschränkt sowohl am Schulsport als auch am täglichen Leben teilnehmen können.

Abstract

BACKGROUND/OBJECTIVES: We analysed at what age parents start complementary food in very low birth weight infants, determined risk factors for early introduction of complementary food (post-term age) and analysed whether the age at introduction of complementary food influences height or weight at 2 years of age. SUBJECTS/METHODS: Parents of premature infants born in 2009-2011 answered questionnaires regarding introduction of complementary food in the first year of life (N =2262) and were followed up at a post-term age of 2 years (N =981). Length and weight were compared with full-term infants from the KiGGs study. Logistic and linear regression analyses were conducted to study predictors for early introduction of complementary food and the influence of age at introduction of complementary food on later height and weight. RESULTS: Average age at introduction of complementary food was 3.5 months post-term age. The lower the gestational age at birth, the earlier (post-term age) vegetables and meat were introduced. Age at introduction of complementary food was influenced by intrauterine growth restriction, gestational age at birth, maternal education and a developmental delay perceived by the parents. Length and weight at a post-term age of 2 years was not negatively influenced by early introduction of complementary food. CONCLUSIONS: VLBW infants are introduced to complementary food on average before a post-term age of 4 months. There was no negative effect of early introduction of complementary food on height and weight at 2 years of age. Abstract

The specialty of neonatology has undergone comprehensive changes with improved diagnostic tools and therapeutic measures, thereby improving the health care for vulnerable fetuses and newborns. Neonatologists are now involved in prenatal, perinatal, and postnatal care. Developments in fetal imaging have broadened the time period in which multidisciplinary counseling concerning prenatal and postnatal strategies can be performed. Lethal congenital defects have changed into treatable defects with improved neonatal outcomes, which lead to new ethical dilemmas for health care professionals and parents. These include considerations regarding termination of pregnancy, performing fetal in utero interventions, and withdrawal of postnatal care. Clinical trials have provided new insights regarding strategies in the delivery room, thereby adapting the practice in the "golden hour" of neonatal life. In newborns requiring support during the fetal-neonatal transition, optimal timing of cord clamping, steps securing the airways, and initial ventilation strategies should be based on individual characteristics. Moreover, many neonatal intensive care guidelines have changed to evidence-based recommendations as a result of numerous innovative trials conducted in the past decades. This chapter will discuss the most recent developments and improvements for neonatal conditions outside the scope of prematurity, illustrating the role of the neonatologist in each time period and highlighting aims for future clinical research projects.

Abstract

Non-coding genetic variants play an important role in driving susceptibility to complex diseases but their characterization remains challenging. Here, we employed a novel approach to interrogate the genetic risk of such polymorphisms in a more systematic way by targeting specific regulatory regions relevant for the phenotype studied. We applied this method to meningococcal disease susceptibility, using the DNA binding pattern of RELA - a NF-kB subunit, master regulator of the response to infection - under bacterial stimuli in nasopharyngeal epithelial cells. We designed a custom panel to cover these RELA binding sites and used it for targeted sequencing in cases and controls. Variant calling and association analysis were performed followed by validation of candidate polymorphisms by genotyping in three independent cohorts. We identified two new polymorphisms, rs4823231 and rs11913168, showing signs of association with meningococcal disease susceptibility. In addition, using our genomic data as well as publicly available resources, we found evidences for these SNPs to have potential regulatory effects on ATXN10 and LIF genes respectively. The variants and related candidate genes are relevant for infectious diseases and may have important contribution for meningococcal disease pathology. Finally, we described a novel genetic association approach that could be applied to other phenotypes.

Abstract

It was the aim of this study to assess whether very-low-birth-weight (VLBW) infants born small for gestational age (SGA; birth weight less than 10th percentile) are at increased risk for late-onset sepsis.This was a prospective, multicenter study of the German Neonatal Network including VLBW infants from 23 to32 weeks post menstrual age born 2009-2011. Outcomes were compared between VLBW infants born SGA (birth weight less than tenth percentile according to gestational age and gender) and non-SGA infants. The main outcome measure was at least 1 episode of late-onset sepsis defined as blood-culture-confirmed clinical sepsis occurring at ≥ 72 hours of age.5886 VLBW infants were included. In SGA infants (n = 692), an increased incidence of late-onset sepsis was noted compared with non-SGA infants (20.1% vs. 14.3 %, P0.001). This difference was only observed among infants with a gestational age of 27 to32 weeks and attributed to sepsis episodes with coagulase-negative staphylococci (12.8% vs. 8.3%, P0.001). Different treatment modalities (eg more frequent use of central venous lines) and longer duration of invasive therapies (parenteral nutrition, mechanical ventilation, hospitalization) may account for the increased sepsis risk with coagulase-negative staphylococci in our SGA cohort. In a multivariate logistic regression analysis, higher gestational age [per week; odds ratio (OR): 0.75, 95% confidence interval (CI): 0.72-0.78, P0.0001], treatment with antenatal steroids (OR: 0.7, 95% CI: 0.53-0.92, P = 0.01), German descendance (OR: 0.76, 95% CI: 0.63-0.91, P = 0.003) and prophylaxis with glycopeptide antibiotics (OR: 0.64, 95% CI: 0.47-0.87, P = 0.005) were shown to be protective against late-onset sepsis. In contrast, longer duration of parenteral nutrition (per day; OR: 1.016, 95% CI: 1.011-1.021, P0.0001) and SGA were found to be risk factors (OR: 1.31, 95% CI: 1.02-1.68, P= 0.03).SGA contributes to the risk of late-onset sepsis in VLBW infants. Future studies are needed to investigate the underlying pathophysiology to guide individualized preventive measures in this vulnerable subgroup.

Abstract

Background Gut dysbiosis has been suggested as a major risk factor for the development of late-onset sepsis (LOS), a main cause of mortality and morbidity in preterm infants. We aimed to assess specific signatures of the gut microbiome, including metabolic profiles, in preterm infants <34 weeks of gestation preceding LOS. Methods In a single-center cohort, fecal samples from preterm infants were prospectively collected during the period of highest vulnerability for LOS (days 7, 14, and 21 of life). Following 16S rRNA gene profiling, we assessed microbial community function using microbial metabolic network modeling. Data were adjusted for gestational age and use of probiotics. Results We studied stool samples from 71 preterm infants with LOS and 164 unaffected controls (no LOS/necrotizing enterocolitis). In most cases, the bacteria isolated in diagnostic blood culture corresponded to the genera in the gut microbiome. LOS cases had a decelerated development of microbial diversity. Before onset of disease, LOS cases had specific gut microbiome signatures with higher abundance of Bacilli (specifically coagulase-negative Staphylococci) and a lack of anaerobic bacteria. In silico modeling of bacterial community metabolism suggested accumulation of the fermentation products ethanol and formic acid in LOS cases before the onset of disease. Conclusions Intestinal dysbiosis preceding LOS is characterized by an accumulation of Bacilli and their fermentation products and a paucity of anaerobic bacteria. Early microbiome and metabolic patterns may become a valuable biomarker to guide individualized prevention strategies of LOS in highly vulnerable populations. [ABSTRACT FROM AUTHOR]
