Background: This study aims to evaluate the entire experience in heart-lung transplantation (HLTx) in a country of the European Union with 47 million inhabitants according to the etiologies that motivated the procedure. Methods: A retrospective study on 1,751 consecutive transplants (HLTx: 78) was performed from 1990 to 2020 in two centers. Overall survival, adjusted for clinical profile and etiological subgroups, was compared. 7 subgroups were considered: 1) Cardiomyopathy with pulmonary hypertension (CM + PH). 2) Eisenmenger syndrome. 3) Congenital heart disease (CHD). 4) Idiopathic pulmonary arterial hypertension (IPAH). 5) Cystic fibrosis. 6) Chronic obstructive pulmonary disease (COPD)/Emphysema. 7) Diffuse interstitial lung disease (ILD). Results: Early mortality was 44% and that of the rest of the follow-up was 31%. There were differences between HTLx and HTx in survival, also comparing groups with a similar clinical profile with propensity score (p= 0.04). Median survival was low in CM + PH (18 days), ILD (29 days) and CHD (114 days), intermediate in Eisenmenger syndrome (600 days), and longer in IPAH, COPD/Emphysema and cystic fibrosis. We present a method to identify ultrafaint dwarf galaxies (UFDGs) candidates in the halo of the Milky Way using the future Gaia catalogue and we explore its detection limits and completeness. The method is based on the Wavelet Transform and searches for overdensities in the combined space of sky coordinates and proper motions, using kinematics in the search for the first time. We test the method with a Gaia mock catalogue that has the Gaia Universe Model Snapshot as a background, and use a library of around 30 000 UFDGs simulated as Plummer spheres with a single stellar population. For the UFDGs, we use a wide range of structural and orbital parameters that go beyond the range spanned by real systems, where some UFDGs may remain undetected. We characterize the detection limits as function of the number of observable stars by Gaia in the UFDGs with respect to that of the background and their apparent sizes in the sky and proper motion planes. We find that the addition of proper motions in the search improves considerably the detections compared to a photometric survey at the same magnitude limit. Our experiments suggest that Gaia will be able to detect UFDGs that are similar to some of the known UFDGs even if the limit of Gaia is around 2 mag brighter than that of SDSS, with the advantage of having a full-sky catalogue. It is not known whether donor-transmitted coronary artery (DTCAD) disease after heart transplantation (HT) and its severity have prognostic implications. Our aim is to assess if patients with multivessel significant-DTCAD (s-DTCAD) and left anterior descendent coronary artery (LAD) s-DTCAD have worse prognosis at long-term follow-up. Retrospective study of HT patients > 18-year-old between 2008-2018 in 11 centers in Spain who underwent coronary angiogram within the first 3 months post HT. Significant DTCAD (s-DTCAD) was defined as stenosis ≥ 50% in main arteries. Multivessel s-DTCAD (multi s-DTCAD) was defined as s-DTCAD in more than one coronary territory simultaneously. Survival was analyzed by Kaplan Meier function and compared with log-rank test. Among 937 patients, s-DTCAD was found in 65 (6.9%) patients. The most frequent coronary affected was the LAD (37 patients). Multi-s-DTCAD was found in 18 patients. Percutaneous revascularization was performed in 15 patients with one-vessel s-DTCAD (32%) and in 9 with multi s-DTCAD (50%), (p= 0.144). Median follow-up was 6.3 years. Overall survival of patients with and without DTCAD was similar (p log-rank=0.143). No differences were found in survival when comparing patients with LAD s-DTCAD and patients with other arteries affected (p Log rank=0.748). When comparing patients with multi s-DTCAD and patients without s-DTCAD, there was a trend to a worse survival in patients with multi s-DTCAD. One-year, 5 years and 10 years survival of multi s-DTCAD patients was 88.9%, 38.9% and 11.1% respectively and 96.8%, 63.8% and 19.5% in patients with no s-DTCAD (p log-rank=0.053) (see figure). Survival after HT of patients with s-DTCAD was similar regardless of whether LAD was or wasn't affected. Background: A heterogeneous geographic distribution of childhood acute lymphoblastic leukemia (ALL) cases has been described, possibly, related to the presence of different environmental factors. The aim of the present study was to explore the geographical distribution of childhood ALL cases in Greater Mexico City (GMC)., Methods: A population-based case-control study was conducted. Children <18 years old, newly diagnosed with ALL and residents of GMC were included. Controls were patients without leukemia recruited from second-level public hospitals, frequency-matched by sex, age, and health institution with the cases. The residence address where the patients lived during the last year before diagnosis (cases) or the interview (controls) was used for geolocation. Kulldorff's spatial scan statistic was used to detect spatial clusters (SCs). Relative risks (RR), associated p-value and number of cases included for each cluster were obtained., Results: A total of 1054 cases with ALL were analyzed. Of these, 408 (38.7%) were distributed across eight SCs detected. A relative risk of 1.61 (p<0.0001) was observed for the main cluster. Similar results were noted for the remaining seven ones. Introduction and Objectives: The Spanish heart transplant registry updates its data annually. The current update presents the data for the year 2022., Methods: We describe the main clinical characteristics, treatments received, and survival outcomes including procedures performed in 2022, along with their trends since 2013., Results: In 2022, 311 cardiac transplants were performed, representing a 3.0% increase compared with 2021. Compared with previous years, no significant changes in demographic and clinical characteristics were observed in 2022, confirming the trends identified in the last decade. These trends indicate a decrease in urgent procedures and the use of circulatory support, particularly ventricular assist devices. In the last decade, survival rates at 1 and 3 years were 81.4% and 73.4% respectively, with a slight, nonsignificant improvement., Conclusions: In the last decade, there has been a stabilization in the characteristics of heart transplant procedures and their outcomes. Background: The outcomes of heart-lung transplant (HLT) are worse than those of heart transplant (HT) and lung transplant alone; this and the availability of mechanical assistance have meant that the indications for HLT have been changing. This study aims to analyze the evolution of indications for HLT in a country of 47 million inhabitants., Methods: We performed a retrospective observational study of all HLTs performed in Spain (performed in 2 centers) from 1990 to 2020. The total number of patients included was 1751 (HT 1673 and HLT 78). After clinical adjustment, overall survival was compared between the 2 groups. Seven etiological subgroups were considered within the HLT group: (1) cardiomyopathy with pulmonary hypertension (CM + PH);, (2) Eisenmenger syndrome, (3) congenital heart disease without Eisenmenger syndrome, (4) idiopathic pulmonary arterial hypertension (IPAH), (5) cystic fibrosis, (6) chronic obstructive pulmonary disease (COPD) and/or emphysema), and (7) diffuse interstitial lung disease., Results: There were a large number of differences between patients with HLT vs HT. HLT had a 2.69-fold increased probability of death in the first year compared with HT. The indications for HLT have changed over the years. In the recent period the indications are mainly congenital heart disease and Eisenmenger syndrome, with some cases of CM + PH. Other indications for HLT have virtually disappeared, mainly lung diseases (IPAH, COPD, cystic fibrosis). Median survival was low in CM + PH (18 days), diffuse interstitial lung disease (29 days), and ischemic heart disease (114 days); intermediate in Eisenmenger syndrome (600 days); and longer in IPAH, COPD and/or emphysema, and cystic fibrosis., Conclusions: HLT is a procedure with high mortality. This and mechanical assists mean that the indications have changed over the years. Cardiogenic shock (CS) is a condition comprising multiple etiologies, which associates high mortality rates. Some scoring systems have been shown to be good predictors of hospital mortality in patients admitted to Critical Care Units (CCU). The main objective of this study is to analyze their usefulness and validity in a cohort of CS patients., Methods: Observational unicentric study of a cohort of CS patients. SOFA, SAPS II and APACHE II scores were calculated in the first 24 h of CCU admission., Results: 130 patients with CS were included. SOFA, SAPS II and APACHE II scores revealed good discrimination for hospital mortality: (AUC) ROC values (AUC: 0.711, 0.752 and 0.742 respectively; P = .6). Calibration, estimated by the Hosmer-Lemeshow test, was adequate in all cases. Acute coronary syndrome, lactate serum values, SAPS II score and vasoactive inotropic score (VIS) were found to be independent predictors for mortality, upon ICU admission. With these variables, a specific prognostic indicator was developed (SAPS-2-LIVE), which improved predictive capability for mortality in our series (AUC) ROC, 0.825 (95% CI 0.752-0.89)., Conclusion: In this contemporary CS cohort, the aforementioned scores have been shown to have good predictive ability for hospital mortality. Background: The parental age at conception has been reported to be a risk factor for childhood acute leukaemia (AL); however, the relationship is controversial. The aim of the present study was to investigate the association between parental age at conception and the risk of AL in Mexican children, a population with a high incidence of the disease and a high prevalence of pregnancies in adolescents and young adults., Methods: A multicentre case-control study was conducted. Incident AL cases younger than 17 years of age diagnosed between 2010 and 2015 were included. Controls were matched with cases according to age, sex, and health institution. Using logistic regression analysis, adjusted odds ratios (aOR) and 95 % confidence intervals (95 % CI) were calculated for each maternal stratum after adjusting for paternal age at conception of index child. The maternal age between 25 and 29.99 years was selected as the reference category., Results: In most strata where maternal and paternal ages were assessed, no association was found with the risk of developing acute lymphoblastic leukaemia (ALL) and acute myeloid leukaemia (AML) in their offspring. An increased risk for AML was observed when the mother was between 20 and 24.99 years of age and the father aged 25-29.99 years (aOR, 1.94; 95 % CI, 1.03-3.67). In addition, there was a positive association for ALL when the mother´s age was between 20 and 24.99 years and the father was <20 years of age, however, a very wide confidence interval was noted (aOR, 12.26; 95 % CI, 1.41-106.83)., Conclusion: In the present study, maternal and paternal ages assessed in different strata showed little association with risk of developing ALL and AML in children. Background: In the configuration of the teacher profile, the way in which the teacher represents his professional function and the way and intensity in which he feels challenged by others plays a decisive role., Objective: To know the predominant type of profile in teachers who attend courses at the Educational Research and Teacher Training Center from the National Medical Center Siglo XXI (NMC SXXI)., Material and Methods: Cross-sectional study in professors who attended the aforementioned center, where the Questionnaire for the Assessment of Teaching Methodology was applied to know if the teaching profile was focused on teaching or learning., Results: 43 professors were included, with an average age of 40.84 ± 6.7 years. 65% came from general hospitals (GH). The majority disagreed with teacher-centered education, while learner-centered education had a high agreement; however, when comparing medians, no significant difference was found. Although there was a clear significant difference in the learner-centered profile in terms of those under 40-years-old, both genders as well as teaching background, especially within the Instituto Mexicano del Seguro Social (IMSS) (p = 0.000) and more specifically in the NMC SXXI Specialties Hospital (p = 0.001) and at the GH (p = 0.000)., Conclusions: There is a dominant trend in the learner-centered teaching profile; however, it was higher for teachers with teacher preparation within IMSS than outside this institute, as well as for professors from the Specialties Hospital and GH., (Copyright: © 2020 Revista Medica del Instituto Mexicano del Seguro Social.)

We came in with high expectations, yet the Arthroscopy Association of North America (AANA) Traveling Fellowship far exceeded them. The 4 traveling fellows came from different backgrounds, different parts of North America, and different practice settings, including an independent private practice, a hybrid private-academic practice, the military, and academia. We were lucky to have been ushered along the way by our godfather, the distinguished John Richmond, M.D., Past-President of AANA and Associate Editor Emeritus of Arthroscopy, who was gracious enough to give his time to the expedition. Over the course of the journey, this gang came together quickly and forged relationships that will last a lifetime. Aims: Techniques for identifying specific microcirculatory structural changes are desirable. As such, capillary rarefaction constitutes one of the earliest changes of cardiac allograft vasculopathy (CAV) in cardiac allograft recipients, but its identification with coronary flow reserve (CFR) or intracoronary resistance measurements is hampered because of non-selective interrogation of the capillary bed. We therefore investigated the potential of wave intensity analysis (WIA) to assess capillary rarefaction and thereby predict CAV., Methods and Results: Fifty-two allograft patients with unobstructed coronary arteries and normal left ventricular (LV) function were assessed. Adequate aortic pressure and left anterior descending artery flow measurements at rest and with intracoronary adenosine were obtained in 46 of which 2 were lost to follow-up. In a subgroup of 15 patients, simultaneous RV biopsies were obtained and analysed for capillary density. Patients were followed up with 1-3 yearly screening angiography. A significant relationship with capillary density was noted with CFR (r = 0.52, P = 0.048) and the backward decompression wave (BDW) (r = -0.65, P < 0.01). Over a mean follow-up of 9.3 ± 5.2 years patients with a smaller BDW had an increased risk of developing angiographic CAV (hazard ratio 2.89, 95% CI 1.12-7.39; P = 0.03). Additionally, the index BDW was lower in those who went on to have a clinical CAV-events (P = 0.04) as well as more severe disease (P = 0.01)., Conclusions: Within cardiac transplant patients, WIA is able to quantify the earliest histological changes of CAV and can predict clinical and angiographic outcomes. This proof-of-concept for WIA also lends weight to its use in the assessment of other disease processes in which capillary rarefaction is involved.

In Mexico, due to the high rates of diabetes, overweight, and obesity, there has also been noted an increased newborn weight, which may be contributing to the elevated incidence rate of childhood acute leukemia (AL). We conducted a case-control study in public hospitals of Mexico City aimed to know whether a greater weight at birth is associated with a higher risk of developing leukemia. We included incident cases with acute lymphoblastic leukemia (ALL) and acute myeloid leukemia (AML) diagnosed between 2010 and 2015. Controls were frequency-matched to the cases by age, sex, and health institution. Logistic regression analysis was performed adjusting risks by child's sex, overcrowding index, birth order, and mother's age at the time of pregnancy. Adjusted odds ratios (aORs) and 95% confidence intervals were calculated. A total of 1455 cases and 1455 controls were included. An evident association between ALL and child's birthweight ≥2500 g was found (aOR 2.06; 95% CI: 1.59, 2.66) and also, in those with birthweight ≥3500 g (aOR 1.19; 95% CI: 1.00, 1.41). In AML patients with birthweight ≥2500 g and ≥3500 g, an aOR of 1.77 (95% CI: 1.07, 2.94) and 1.42 (95% CI: 1.03-1.95) was observed, respectively. No association was noticed with either type of AL and a birthweight ≥4000 g. To sum up, we found a moderate association between not having a low birthweight and an increased risk of acute leukemias. Birthweight ≥3500 g was also a risk factor for both types of leukemia. This suggests that a greater birthweight may increase the risk of acute leukemias in Mexican children., (© 2018 The Authors. Aims: Our aim was to assess the safety and efficacy of paclitaxel-eluting balloon (PTX-B) treatment after bare metal stent (BMS) implantation in patients undergoing primary angioplasty., Methods and Results: After BMS implantation, patients were randomised (1:1) to treatment with a PTX-B or no PTX-B treatment (BMS group). The primary endpoint was in-stent late luminal loss (LLL) at nine-month follow-up. OCT was carried out on the first 20% of consecutive patients included in the study. Two hundred and twenty-three patients were randomised (BMS: 112, PTX-B: 111). At nine months, median LLL was 0.80 mm (interquartile range [IQR] 0.36-1.26) in the BMS group vs. 0.31 mm (IQR 0.00-0.58) in the PTX-B group, p<0.0001. Binary restenosis was significantly lower in the PTX-B group: 29.8% vs. 2.2%, p<0.0001, 95% confidence interval (CI): 3.2-54.2. Nine-month OCT showed good strut coverage in both groups but greater in the BMS group (100±0.0% vs. 99.52±1.11%, p=0.03) with very low rates of malapposed struts per lesion. One-year MACE was significantly lower in the PTX-B group (12.5% vs. 3.6%, p=0.016)., Conclusions: PTX-B after successful BMS implantation resulted in less LLL and better clinical outcomes as compared with a BMS-only strategy. This was associated with good stent strut coverage and very low rates of malapposed struts.

Background: Prevalence of oral candidiasis in diabetic patients is 13.7-64%. Candida albicans was the most frequently isolated species (75-86.5%)., Objective: To obtain the prevalence of Candida carriers among patients with type 2 diabetes mellitus to identify the species of the yeast., Study Design: It is an open, observational, descriptive, cross-sectional, and prospective study., Methods: We included voluntary patients from the National Diabetes Marathon and performed a blood glucose measurement, sialometry test, Gram-stained exfoliative cytology, and culture on Sabouraud dextrose agar and CHROMagar Candida TM. Results were analyzed using descriptive statistics., Results: We examined 141 patients (mean age 57 years): 103 women (73%) and 38 men (26.9%). Exfoliative cytology was positive in 32 cases (23 with oral lesions); 78 had oral lesions but no Candida (93.9%). Candida was isolated in 58 patients (41.1%), 21 (45.6 %) had blood glucose greater than 126 mg/dl, and 37 (38.9%) had less than 126 mg/dl. The most frequent species was C. albicans (82.7%). Forty-two Candida carriers had salivary flow greater than 20 mm (72.4%), and 16 (27.5%) had hyposalivation. Candida was isolated in 25 of 79 patients with dental prosthesis (31.6%), 9 of 15 were smokers (60%), and 22 of 71 had symptoms (30.9%)., Conclusions: Prevalence of oral Candida carriers in patients with type 2 diabetes mellitus in Mexico was similar to that found in other countries; exfoliative cytology was effective in finding Candida; salivary flow rate, use of prosthesis, and presence of oral lesions and symptoms were similar in oral Candida carriers and negative patients. Most smokers were Candida carriers.

Objective: to compare the effectiveness, safety and tolerability of Hibiscus sabadariffa extract (SHE) versus pravastatin in hypercholesterolemia., Methods: double blind and controlled clinical trial. Men and women with hypercholesterolemia ≥ 220 mg/dL, and over 30 years without previous treatment were included., Treatments: Experimental I, SHE standardized 10 mg of anthocyanins, control, 20 mg of pravastatin; Experimental II, extract of H. sabdariffa, standardized to 20 mg of anthocyanins for 12 weeks. SHE Data analysis with Anova and χ(2) tests were used., Results: we included 104 patients, at baseline there were no significant differences in age, weight, BMI, total cholesterol, HDL, LDL, VLDL or triglycerides. At the end of the study, the experimental I group reduced triglycerides (p ≤0.0001), the control treatment reduced triglycerides, total cholesterol and its fractions (p = 0.02), and finally, the experimental treatment II reduced triglycerides (p = 0.0001). The three treatments showed high percentages (> 96) of safety and tolerability, without showing significant differences (p ≥ 0.57)., Conclusions: the treatments, with 10 and 20 mg of anthocyanins reduce triglycerides. The control treatment showed the greatest effect on lowering total cholesterol and fractions compared to experimental treatments.

Aim: To determine the prevalence of oral lesions (OL) in patients with psoriasis, and compare these findings with the ones found in patients without this condition., Materials and Methods: In the present observational and comparative study, we evaluated 207 patients, with and without psoriasis, attending the dermatological consulting service of a concentration hospital in Mexico City. The possible association between OL and psoriasis was analyzed through a logistic regression model; the Odds Ratio (OR) and its Confidence Interval (CI) was calculated to be 95%., Results: Two hundred and seven cases were examined (80 with psoriasis and 127 without psoriasis). Of these, 75 (36.2%) were men and 132 (63.7%) women. Oral lesions were found in 54 (67.5%) psoriatic patients and in 63 (49.6%) of the comparative group (p <0.012). Fissured tongue (FT) was present in 47.5 % of the patients with psoriasis and in 20.4 % of the group without psoriasis, (OR=3.46, 95% CI [1.14, 10.5], p=0.001). Geographic tongue (GT) was present in 12.5 % of the group with psoriasis and 4.7 % in the group without this disease (OR=3.54 95 % CI [1.97, 6.79], p=0.028). Likewise, six patients (7.5 %) with psoriasis and 3 (2.36 %) from the comparative group presented simultaneously GT and FT (p = 0.0776). The most frequent type of psoriasis was the vulgar psoriasis (90 %), in which a higher prevalence of FT (p <0.05) was present. There were no differences between both groups (p>0.05) regarding the use of tobacco. Alcohol consumption was greater (55.0%) among patients with psoriasis than among those without psoriasis (26.7%) (p<0.05), but when the association with GT and FT was analyzed, no significant differences were found among consumers and not consumers of tobacco and alcohol (p> 0.05)., Conclusions: The high prevalence of GT and FT in patients with psoriasis suggests that these lesions should be taken into account in new studies as possible predictors or markers of the severity of this dermatosis, in order to confirm the association of these entities.

Objective: to know the mortality rate after six months in patients with unstable angina., Material and Methods: A representative sample of patients who were admitted at the emergency room with unstable angina diagnosis during a period of two years was selected. The presence of angina pectoris, heart attack and death was registered during their stay at the emergency room. A six-month follow-up took place to evaluate the frequency of angina, heart attack and death related to ischemic acute coronary syndrome, as well as the determination of the relative risk of factors associated to mortality., Results: From the 110 admitted patients, we lost 4; 106 conformed the total of the cohort. There was a mortality rate within the emergency room of 1.8% and of 5.7% in six months. Heart attack was present in 6.6% and angina pectoris in 27.1% during their stay at the emergency room. None of the analyzed risk factors showed any relatively significant risk associated to mortality; the relative risk for age, sex, presence of angina during the stay at the emergency room, acute changes in EKG, history of diabetes mellitus, hypertension, current smoking and ischemic cardiac disease, analyzed with the Mantel-Haenszel test, did not show any significant differences by the Mann-Whitney U among the values of the laboratory markers, arterial tension when admitted, pain intensity, evolution time of diabetes, hypertension, hypercholesterolemia and previous ischemic heart disease., Conclusions: A low frequency of angina, heart attack and mortality was found during their stay at the emergency room, as well as a low mortality rate in six months; none of the risk factors was significantly associated to death.

Objectives: To determine the incidence of asymptomatic bacteriuria in women consulting at our hospital and its relationship with other chronic diseases., Methods: A cross-sectional study was conducted on 735 women who had consulted at our hospital from February to September, 1996. Urinary sediment, serial urine culture, serum urea and creatinine values were analyzed. Patient records were reviewed to obtain pertinent data for the study., Results: The incidence of asymptomatic bacteriuria was 8%; 47.4% of the cases were women over 60 years of age. E. coli was identified as the causative organism in 77.9% of the urine cultures. Diabetes mellitus was the most frequent associated chronic disease (27.1%). The incidence of renal damage was not statistically significant and was not directly related with bacteriuria., Conclusions: We emphasize the importance of performing a urine culture when leukocyturia is detected. The incidence of asymptomatic bacteriuria increases with age. E. coli was the most frequent causative organism. Diabetes mellitus was the most frequent associated chronic disease. The incidence of renal damage was not statistically significant.
