Introducción: Las lesiones por pólvora generan altos costos socioeconómicos y daños ambientales. Se presenta un análisis, después de quince años de la emisión de una política nacional para evitar las lesiones por esta causa. Objetivo: Describir el comportamiento de las lesiones relacionadas con el uso de pólvora durante las festividades decembrinas en Santiago de Cali-Colombia durante el período 2002-2016. Métodos: Estudio descriptivo retrospectivo. Utilizando bases de datos secundarias del Observatorio Social y del SIVIGILA, se caracterizaron los casos de quemados por pólvora en la ciudad. Resultados: Se identificaron 655 lesionados por pólvora en época decembrina, el 83% de los lesionados son hombres, el 43% menores de edad, siendo el grupo de 10 a 14 años el más afectado, se alcanzó una importante disminución en el grupo de la primera infancia. Las partes más afectadas fueron miembros superiores y cabeza. Conclusión: La política ha tenido un impacto positivo para proteger a la primera infancia. Se recomienda una mayor difusión de la norma en el período pre decembrino y un fomento de la educación con enfoque de género, dirigida a la comunidad educativa (escolares, docentes y padres de familia) para incidir en costumbres patriarcales como el uso de pólvora para celebrar y en la disminución de quemados en otros menores de edad a fin de aumentar el impacto de la normatividad existente.

Introduction: The objective is to describe the utility of the Critical Administration Threshold (CAT) as a tool in hemostatic resuscitation in patients with severe trauma and REBOA. Methods: Retrospective review between January 2015 and June 2020 of adult patients with hemorrhage secondary to trauma with or without REBOA in a level I trauma center in Cali, Colombia. Demographic variables, trauma severity, clinical status, transfusion needs, time to CAT+ and number of CAT achieved (1, 2 or 3) were recorded. Results: Ninety-three patients were included, in which REBOA was used in 36 and traditional management in 57. The REBOA group had a higher bleeding volume (3000 ml), IQR: 1950-3625 ml vs the control group (1500 ml, IQR: 700-2975 ml) (p<0.001) and a higher rate of PRBC units transfused in the first 6 hours (median 5, IQR: 4-9); p=0.015 and in the first 24 hours (median 6, IQR: 4-11); p=0.005. There were no statistically significant differences in the number of CAT+ patients between groups or time to CAT+. However, CAT+ status during the first 30 minutes of surgery was higher in the REBOA Group (24/36, 66.7%) vs. the control group (17/57, 29.8%; p=0.001), having this group a higher in-hospital mortality rate vs. CAT-patients. Conclusion: CAT is a useful tool in the hemostatic resuscitation of patients with trauma and REBOA that could predict early mortality. [ABSTRACT FROM AUTHOR]

El traumatismo cervical por arma cortopunzante es poco frecuente; sin embargo, por el gran interés clínico, puede dar lugar a complicaciones muy importantes, como asfixia, hemorragia masiva e incluso la muerte, sobre todo porque el 10 % de las lesiones traumáticas afectan a la región cervical (zona 1), la cual, por contener órganos y paquetes vasculares importantes, presenta una mortalidad del 1 al 7 %. De esta manera, se presenta el caso clínico de un paciente femenino de 90 años, con demencia senil, un cuadro clínico de 1 hora de evolución, con herida en región cervical derecha por objeto cortopunzante, con sangrado y dolor local, aparentemente por autolesión. La paciente fue sometida a cirugía para retiro de objeto cortopunzante y manejo en unidad de terapia intensiva (UTI) para monitorización, presentando una evolución optima y egresando al tercer día; posteriormente fue dada de alta a domicilio en buen estado general. The objective of this paper was to identify the main indicators used to measure the performance in emergency trauma care. A literature review was carried out in the electronic databases: PubMEd, LILACS and Epistemónikos, including publications between January 2011 and December 31, 2021, in Spanish, English and Portuguese. A total of 962 publications were identified. When reviewing the full text, 48 articles were included. The indicators were classified in the dimensions of process and results. 100 different indicators were identified to analyze the performance of emergency trauma care. 71% were process indicators, including service time and triage. In the results dimension 29 indicators were identified; mortality was the indicator most analyzed as well as length of stay. Six indicators on the disability of injured people and 14 indicators related to satisfaction were identified, the most frequent being complaints. Various indicators have been used to assess the performance of emergency trauma care. In the results dimension, the indicators related to satisfaction and disability after injuries have been little explored. Decision-makers and those responsible for emergency care must promote performance evaluation exercises to learn about their current situation using appropriate and sensitive indicators with the available data. The face for many centuries, has represented the focus of appearance. The middle facial third is its most important area and is therefore extremely vulnerable to facial deformities. Within it, the nose is the most important facial prominence. From years ago, his deformities were considered a curse, so many men were condemned to death. From this moment, the nasal reconstruction boom increases, and, currently, the evolution of techniques in nasal reconstruction is an extremely useful option to repair facial defects of any size. This review intends to update information on non-rhinoplastic techniques for the reconstruction of trans-surgical defects on the nasal dorsum. An evaluation of the available bibliography and previous research on the topic was carried out in the period from May 2018 to May 2023. 31 popular science magazines were considered. Articles published in English, Italian and Spanish were prioritized. The Pubmed, Ebsco, SciELO y Scopus databases were consulted. Of the 95 articles reviewed, the study was limited to 43 of them. Among the non-rhinoplasty techniques reviewed were healing by secondary intention, primary closure, grafts and flaps. There are factors that influence the selection of one or the other, such as: the diameter of the defect, its depth, location, texture and contour of the skin, mastery and professionalism of the surgeon, among others. There is no consensus on a technique of choice. With all of them, favorable results can be achieved by making a good selection and application of it. [ABSTRACT FROM AUTHOR]

Objetivo: crear y validar una aplicación que establezca el grado de riesgo del pie diabético asociándolo a sus condiciones de salud. Metodología: estudio metodológico realizado entre marzo de 2019 y diciembre de 2021, en Crato, CE, Brasil, dividido en tres fases: 1) Revisión de directrices nacionales e internacionales; 2) Creación de la tecnología educativa y 3) Validación del contenido y la apariencia de la tecnología. Este estudio siguió los lineamientos de la guía Revised Standards for Quality Improvement Reporting Excellence. Resultados: en la fase de revisión, se identificaron contenidos similares entre las directrices para el diseño y la organización del constructo. La tecnología creada permitió recolectar datos, evaluar y clasificar el riesgo del pie diabético. En la etapa de validación, 18 jueces evaluaron el instrumento, que contó con un IVC de 0,96. Conclusión: el estudio puede contribuir a mejorar los indicadores de hospitalización, amputación, movilidad reducida, dependencia, fragilidad y mortalidad derivados del pie diabético.

Objetivo: elaborar, validar el contenido y la apariencia de un instrumento para capacitar a los enfermeros en el cuidado de heridas neoplásicas. Método: investigación metodológica, con enfoque cuantitativo, realizada en tres etapas: revisión de alcance; elaboración del instrumento; validación de contenido; y apariencia. La recolección de datos se realizó entre octubre de 2021 y febrero de 2022, en Niterói, RJ, Brasil. Se utilizó el Índice de Validación de Contenido; Fleiss Generalized Kappa. Resultados: el instrumento final consta de las fases: definición de herida neoplásica; clasificación de heridas; estadio; escalas de evaluación del dolor y olor; cuidados de enfermería; y manejo de signos y síntomas. Se validó el contenido, alcanzó un índice de validación de contenido > 0,80 y un nivel de concordancia entre los jueces > 75%, según lo establecido. Conclusión: la validación del instrumento contribuirá a reducir la brecha que hay en la literatura sobre el cuidado de enfermería de las heridas neoplásicas, además se aplicará en la enseñanza y los servicios de enfermería.

Introducción. Las membranas de la cáscara de huevo presentan propiedades beneficiosas para la regeneración de tejidos y sus aplicaciones biomédicas son importantes. Objetivo. Demostrar la efectividad de las membranas de la cáscara de huevo no fecundado de gallina en el tratamiento de heridas abiertas superficiales en ratones, en comparación con el procedimiento convencional. Materiales y métodos. Se hizo una herida superficial lineal de 15 mm en la espalda de 10 ratones albinos machos. Los ratones se dividieron en cuatro grupos, uno no recibió ningún tratamiento y los otros tres sí: uno, tratamiento convencional, otro, con membranas de huevo directamente aplicadas a la herida y, el otro, con membranas en forma de polvo. La evolución de las heridas se registró en fotografías y se calculó la tasa de reducción de la longitud de la herida, así como el tiempo y el porcentaje de curación. El deterioro de la piel asociado con adhesivos de uso médico en dispositivos de acceso intravascular es un evento adverso que puede contribuir a la interrupción de la terapia de infusión intravascular. La Sociedad de enfermeras de infusión recomienda usar tecnologías de protección como apósito estéril para aseguramiento del catéter y cinta quirúrgica para aseguramiento complementario. Objetivo: Analizar las causas y prevalencia del deterioro de la piel asociado con aseguramiento del dispositivo de acceso intravascular. Métodos: Estudio analítico, de corte transversal. Para recolectar datos se empleó un formato digital y observación directa a pacientes hospitalizados (n=813) con algún dispositivo de aseguramiento en acceso intravascular central o periférico, y aseguramiento complementario con cinta quirúrgica, dando seguimiento durante 7 días en febrero 2020. Resultados: El deterioro de la piel incluyó desprendimiento de epidermis, maceración, dermatitis irritativa por contacto y alérgica, con prevalencia de 2.2%. Se atribuyeron a catéter venoso central (f=8), catéter venoso periférico (f=7) y cinta quirúrgica para aseguramiento complementario del apósito transparente (f=3). Discusión: Las causas de deterioro de la piel se debieron al tipo de dispositivo y material implementado para aseguramiento; en la mayoría de casos las lesiones se encontraron en accesos venosos periféricos, seguida de accesos venosos centrales, siendo el aseguramiento complementario la menor causa de deterioro de la piel. Conclusiones: La técnica correcta de aplicación y retiro de insumos adhesivos de estabilización y aseguramiento del catéter intravascular evitará el desprendimiento de los mismos, lesiones de piel e interrupción de la terapia de infusión intravascular.

Introducción. La fractura de pene es una emergencia urológica rara, se desconoce la incidencia real dado el subregistro causado por la reducida consulta de los pacientes, resultado del embarazoso contexto. En Estados Unidos alcanza un 38% donde la etiología de índole sexual predomina. En Colombia hay escasos reportes publicados. El diagnóstico de esta entidad es netamente clínico. El objetivo de este trabajo es reportar una evolución satisfactoria en términos de función miccional y eréctil en un paciente sometido a reconstrucción cavernosa y uretral temprana en rotura bilateral de origen traumático mediante una técnica quirúrgica poco convencional, de acuerdo con lo hallado en la literatura. Presentación del caso. Paciente masculino de 30 años, previamente sano, quien consultó al servicio de urgencias por uretrorragia, edema y dolor peneano secundario a trauma contuso del mismo durante relación sexual. El reporte ecográfico mostró disrupción en túnica albugínea del cuerpo esponjoso, hematoma y aparente transección uretral; se realizó exploración quirúrgica 6 horas después, con evidencia de fractura de cuerpos cavernosos, laceración del 40% de la circunferencia, transección completa de la uretra y cuerpo esponjoso. Se realizó rafia de cuerpos cavernosos y uretroplastia término terminal de uretra bulbar, con evolución clínica satisfactoria. Discusión. La reconstrucción quirúrgica en menos de 24 horas en fractura de pene disminuye significativamente la estancia hospitalaria y complicaciones precoces; asimismo, desciende el riesgo de disfunción eréctil, erecciones dolorosas y problemas miccionales. No hay publicaciones que comparen resultados a largo plazo entre los dos abordajes quirúrgicos. Conclusiones. La fractura de pene es una patología poco frecuente con diagnóstico clínico, la cual debe manejarse de manera temprana por cualquier urólogo y, en caso de ser posible, con experiencia reconstructiva y excelentes resultados en la función sexual y miccional. Introduction. Penile fracture is a rare urological emergency, the real incidence is unknown given the underreporting caused by the reduced consultation of patients, resulting from the embarrassing context. In the United States it reaches 38%, where sexual etiology predominates. In Colombia there are few published reports. The diagnosis of this entity is purely clinical. The aim of this work is to report a satisfactory evolution in terms of voiding and erectile function in a patient who underwent early cavernous and urethral reconstruction in bilateral rupture of traumatic origin by means of an unconventional surgical technique, in accordance with what has been found in the literature. Case Presentation. A 30-year-old male patient, previously healthy, consulted the emergency department for urethrorrhagia, edema and penile pain secondary to blunt trauma to the penis during sexual intercourse. The ultrasound report showed disruption in the tunica albuginea of the corpus spongiosum, hematoma and apparent urethral transection; surgical exploration was performed 6 hours later, with evidence of fracture of the corpora cavernosa, laceration of 40% of the circumference, complete transection of the urethra and corpus spongiosum. Sutures to the corpora cavernosa and end-to-end urethroplasty of the bulbar urethra were performed, with satisfactory clinical evolution. Discussion. Surgical reconstruction in less than 24 hours in penile fractures significantly reduces hospital stay and early complications; it also reduces the risk of erectile dysfunction, painful erections and voiding problems. There are no publications comparing long-term results between the two surgical approaches. Conclusions. Penile fracture is a rare pathology with clinical diagnosis, which should be managed early by any urologist and, if possible, with reconstructive experience and excellent results in sexual and voiding function. Introdução. A fratura peniana é uma emergência urológica rara, a real incidência é desconhecida dada a subnotificação causada pela reduzida consulta de pacientes, resultado do contexto constrangedor. Nos Estados Unidos chega a 38% onde predomina a etiologia de natureza sexual. Na Colômbia há poucos relatórios publicados. O diagnóstico desta entidade é puramente clínico. O objetivo deste trabalho é relatar uma evolução satisfatória da função miccional e erétil em um paciente submetido à reconstrução cavernosa e uretral precoce em ruptura bilateral de origem traumática por meio de técnica cirúrgica não convencional, de acordo com o que foi encontrado na literatura. Apresentação do caso. Paciente do sexo masculino, 30 anos, previamente saudável, procurou o pronto-socorro por uretrorragia, edema e dor peniana secundária a trauma contuso durante a relação sexual. O relatório do ultrassom mostrou ruptura na túnica albugínea do corpo esponjoso, hematoma e transecção uretral aparente; a exploração cirúrgica foi realizada 6 horas depois, com evidência de fratura dos corpos cavernosos, laceração de 40% da circunferência, transecção completa da uretra e corpo esponjoso. Foram realizadas ráfia dos corpos cavernosos e uretroplastia término-terminal da uretra bulbar, com evolução clínica satisfatória. Discussão. A reconstrução cirúrgica em menos de 24 horas nas fraturas penianas reduz significativamente o tempo de internação e as complicações precoces. Da mesma forma, diminui o risco de disfunção erétil, ereções dolorosas e problemas de micção. Não há publicações comparando os resultados a longo prazo entre as duas abordagens cirúrgicas. Conclusões. A fratura peniana é uma patologia rara com diagnóstico clínico, que deve ser tratada precocemente por qualquer urologista e, se possível, com experiência reconstrutiva e excelentes resultados na função sexual e miccional.

Introduction: in the rehabilitation service of the "Luis Augusto Turcios Lima" University Polyclinic in the city of Pinar del Río, gonarthrosis is one of the main reasons for consultation, but the effectiveness of this treatment in this disease is unknown. Objective: to evaluate the effectiveness of rehabilitation treatment of gonarthrosis. Methods: an observational, descriptive and longitudinal research was carried out in patients with gonarthrosis. The universe consisted of 156 patients and the study sample consisted of 112. Qualitative and quantitative variables were studied and their information was processed by means of descriptive statistics. Medical ethics were respected and the results were reflected in tables and statistical graphs. Results: the predominant age range was 70 to 79 years (35,7 %), as well as male sex (64,3 %). The occupation of retiree was higher in patients studied with 32,1 % presenting risk factors such as mechanical overload in the knee (30,4 %), obesity (25 %) and knee trauma (15,2 %). The prevailing symptoms were pain and difficulty in walking with 75 % and 36,6 % respectively, being higher the intensity of moderate pain (59,5 %); while the results of the rehabilitation treatment were good in 75,9 % of the intervened patients. Conclusions: the rehabilitation treatment of gonarthrosis was evaluated as good, being essential in the integral approach, health care and rehabilitation of patients treated at the "Luis Augusto Turcios Lima" University Polyclinic.

41. Is the Use of Ozonated Oil Effective in the Treatment of Oral Lesions? Systematic Review of Clinical Studies

The aim of this study was to perform a systematic review (SR) of the therapeutic effect of ozonated oil for oral lesions treatment. A SR was conducted according to the PRISMA guidelines. The Medline (PubMed), Embase, Cochrane Library, Scielo and LILACS were investigated, together with manual searches, to extract all publications until December 2020, including randomized and non-randomized clinical trials reporting the effects of ozonated oils on the management of oral lesions when compared with other methods. The risk of bias (RoB) of the studies included were assessed by using the RoB 2 tool and ROBINS-I. After analyzing the titles and reading the abstracts, 1932 articles were excluded; the remaining 25 passed a full-text evaluation. Ultimately, 13 articles were included in this SR. There was heterogeneity of the results regarding healing times and intervals of ozonated oil application for the treatment of each type of oral lesion, but in general, there was a shorter healing time when ozonated oil was used as therapy, and no adverse effects were reported. Despite the limited information found and the lack of rigorous methodological standards for the use of ozonated oil on oral lesions, a positive effect was suggested. The findings indicated an advantage in terms of shorter healing times when compared with other conventional treatments. No adverse effects were reported, showing safety and reliability for patient’s treatment., El objetivo de este estudio fue realizar una revisión sistemática (RS) del efecto terapéutico del aceite ozonizado sobre las lesiones orales. Se realizó una RS siguiendo las directrices PRISMA. Se realizaron búsquedas en Medline (PubMed), Embase, Cochrane Library, Scielo y LILACS, y búsquedas manuales, que abarcaron hasta diciembre de 2020, de ensayos clínicos aleatorizados y no aleatorizados que informaran sobre el efecto de los aceites ozonizados en el tratamiento de las lesiones orales en comparación con cualquier otro método. El riesgo de sesgo (RoB) de los estudios incluidos se evaluó mediante la herramienta RoB 2 y ROBINS-I. Tras analizar los títulos y leer los resúmenes, se excluyeron 1932 artículos; los 25 restantes pasaron una evaluación exhaustiva del texto completo. Finalmente, se incluyeron 13 artículos en esta RS. Hubo heterogeneidad de resultados en cuanto a los tiempos de cicatrización y los intervalos de aplicación del aceite ozonizado para el tratamiento de cada tipo de lesión oral, pero en general, hubo un menor tiempo de cicatrización cuando se utilizó el aceite ozonizado como terapia, y no se comunicaron efectos adversos. A pesar de la limitada información encontrada y de la falta de normas metodológicas rigurosas sobre el uso de aceite ozonizado en lesiones orales, se sugirió un efecto positivo del uso de aceite ozonizado para el tratamiento de lesiones orales. Los resultados indicaron una ventaja en términos de menor tiempo de curación en comparación con otros tratamientos convencionales; además, no se notificaron efectos adversos, por lo que se demostró una opción de tratamiento segura y fiable para los pacientes.

42. Vibrio vulnificus, heridas bullosas y un paciente migrante de África: un caso clínico poco común en los cuidados de enfermería

Introduction: Primary Care emergency devices are the gateway to a wide variety of cases, sometimes encountering situations that may surprise and generate doubts in the healthcare professional such as the case at hand. This clinical case aims to raise awareness of Vibrio Vulnificus infection, an infection that will probably become a frequent case to be treated in health services given the current migration crisis. Case report: A male who arrived in Spain from Senegal in a cayuco 24 hours ago arrives at the emergency department, being accompanied by a social worker given the obvious language barriers. They consulted due to various bullous wounds and small circular lesions with a necrotic center surrounded by a perilesional erythematous area on both lower limbs. Suspecting their infectious origin, it was decided to apply a treatment regimen in a humid environment and start antibiotic therapy and refer to Primary Care, where a favorable evolution of the case was achieved by finding the agent causing the problem: Vibrio Vulnificus. For three weeks, the same regimen of dressings and antibiotic treatment is continued, combining Doxycycline and Ceftriaxone, which allows the wounds to completely resolve. Discussion: V. vulnificus infection is associated with the consumption of raw or undercooked seafood as well as the long exposure of wounds to contaminated seawater in warm environments, a condition that makes migrants in cayuco users at special risk. The evolution of the case is exceptional given that this infection has a mortality rate of 33% and complications such as septicemia or necrotizing fasciitis. The lack of personal history of interest, the age of the user and the speed of diagnosis and adequate treatment made it possible to present a case with an optimal evolution that is expected to help those health professionals who, with increasing probability, may care. in emergency services similar situations., Introducción: Los dispositivos de urgencias de Atención Primaria son la puerta de entrada a una gran variedad de casos, encontrando a veces situaciones que pueden extrañar y generar dudas en el profesional sanitario como la del caso que nos ocupa. Este caso clínico pretende dar a conocer la infección por Vibrio Vulnificus, una infección que probablemente se convierta en un caso frecuente a atender en los servicios sanitarios dada la crisis migratoria actual. Reporte de caso: Llega a dispositivo de urgencias varón llegado a España desde Senegal en cayuco hace 24 horas, siendo acompañado por asistente social dadas las barreras lingüísticas evidentes. Consultan por presentar en ambos miembros inferiores diversas heridas bullosas y pequeñas lesiones circulares con centro necrótico rodeado por zona eritematosa perilesional. Sospechando origen infeccioso de las mismas, se decide aplicar pauta de cura en ambiente húmedo e iniciar antibioterapia y derivar a Atención Primaria, donde se consigue una evolución favorable del caso al dar con el agente causante del problema: Vibrio Vulnificus. Durante tres semanas, se continúa con misma pauta de curas y tratamiento antibiótico combinando Doxiciclina y Ceftriaxona, lo que permite la resolución total de las heridas. Discusión: La infección por Vibrio vulnificus se asocia al consumo de marisco crudo o poco cocinado así como por la larga exposición de heridas a agua marina contaminada en ambientes cálidos, condición que hace a los migrantes en cayuco usuarios de especial riesgo. Resulta excepcional la evolución del caso dado que esta infección cuenta con una tasa de mortalidad del 33% y complicaciones como septicemia o fascitis necrotizante. La falta de antecedentes personales de interés, la edad del usuario y la rapidez en su diagnóstico y adecuado tratamiento hicieron posible presentar un caso con una evolución óptima que se espera pueda ayudar a esos profesionales sanitarios que, cada vez con más probabilidad, puede que atiendan e

Las personas con heridas crónicas frecuentemente experimentan dolor, lo que tienen un impacto negativo en su calidad de vida y puede provocar un retraso en el proceso de cicatrización de las lesiones. Nuestro objetivo es identificar la mayor evidencia sobre el uso de tratamiento farmacológico frente al tratamiento no farmacológico en el abordaje del dolor en heridas crónicas. Se realizó una revisión sistemática de artículos publicados en los últimos 5 años en las bases de datos PubMed, CINAHL, Scielo y en los repositorios de las revistas de la editorial Wiley y Elsevier, en inglés y español. Los criterios de inclusión fueron: pacientes adultos que presentan heridas crónicas como lesiones por presión, pie diabético, lesiones por humedad y úlceras en la extremidad inferior. La valoración de la calidad metodológica de los estudios seleccionados, se realizó de forma independiente utilizando el Clinical Appraisal Skills Programme en su versión española. De 834 artículos se seleccionaron 8 artículos: 3 ensayos clínicos, 3 estudios de cohortes y 2 revisiones sistemáticas. Estudiaron el manejo y abordaje del dolor con intervenciones farmacológicas sistémicas, intervenciones farmacológicas tópicas e intervenciones no farmacológicas en pacientes con heridas crónicas. La primera elección para el manejo y abordaje del dolor es el tratamiento farmacológico sistémico, usando como primera línea los AINES, seguidos de los opioides. El tratamiento farmacológico tópico es una alternativa analgésica eficaz. Complex wounds represent a threat to the viability of a limb or to life as they affect large areas of the body, requiring special methods for their resolution. The objective was to identify the effectiveness of negative pressure therapy in the treatment of complex lesions. This is an integrative review carried out in April 2021, in the LILACS, Web of Science, Medline and SciELO databases. The MeSH descriptors "Wounds and injuries" and Therapeutics and "Negative-Pressure Wound Therapy" were crossed and after applying the eligibility criteria, 11 articles were selected. It was found that the potentialities related to the use of negative pressure therapy stand out from the existing weaknesses, which is considered effective in the treatment of complex injuries, however, little used by professionals in the public health network, requiring its appropriation by health professionals in order to provide alternative and more effective treatments for those with complex wounds. - Abstract
Introduction: Eggshell membranes offer beneficial properties for tissue regeneration and their biomedical applications are important. Objective: To demonstrate the effectiveness of hen unfertilized eggshell membranes in the treatment of superficial open wounds in mice compared to the conventional procedure. Materials and methods: 15-mm linear wounds were made on the back of 10 male CF-1 mice. The mice were divided into four groups. One group did not receive any treatment and the other three were given conventional treatment, eggshell membranes dressing, and eggshell membranes powder. The evolution of the wounds was registered by photographs. We measured wounds length and healing time and calculated the length reduction rate and the percentage of healing. The healing percentages were analyzed by ANOVA with a post hoc Dunnett test (p<0.05). Results: The treatments with eggshell membranes and membrane powder had a woundlength reduction rate of 1.009 and 1.020 mm/day, greater than the conventional treatment, of 0.852 mm/day, and a healing time of 12 days, i.e., less than the 16-day conventional treatment. The statistical analysis showed significant differences between eggshell membrane treatments and the conventional treatment. Conclusions: Eggshell membrane dressing and eggshell membrane powder applied to open superficial wounds in mice were more effective than the conventional treatment in regenerating injured tissue in mice. RESUMEN Introducción: Las lesiones neoplásicas cutáneas generan un elevado impacto en salud por la complejidad de su abordaje sintomatológico y las complicaciones asociadas, entre las que destaca el impacto emocional que provocan tanto en el paciente como en la familia. Es muy importante detectar las necesidades del paciente y, en ausencia de un horizonte curativo, brindar bienestar y confortabilidad, mejorando así la calidad de vida. Desarrollo del plan de cuidados: Se desarrolla un plan de cuidados estandarizado basado en la mejor evidencia científica disponible para atender las necesidades de una mujer de 65 años que presenta una herida neoplásica de 6 meses de evolución en la zona pectoral. Se proponen diagnósticos enfermeros basados en la taxonomía NANDA I, criterios de resultados (NOC) y las intervenciones enfermeras (NIC), así como las actividades correspondientes basadas en la evidencia científica. Se formulan los problemas de colaboración y complicaciones potenciales. Discusión y conclusiones: La valoración holística del paciente con enfermedad neoplásica y el abordaje integral mediante un equipo multidisciplinar son factores clave para brindar al paciente y a su familia, en ausencia de objetivos curativos, la mejor calidad de vida posible.

Objetivo: medir los costos directos medios correspondientes a curaciones de úlceras vasculogénicas realizadas por profesionales de Enfermería en pacientes atendidos por una Unidad de Tratamiento Integral de Heridas. Método: investigación cuantitativa y exploratoria-descriptiva realizada en marzo, agosto y septiembre de 2020 en una Unidad de Tratamiento Integral de Heridas situada en Vitória da Conquista, Bahía, Brasil. Los costos se calcularon multiplicando el tiempo empleado (cronometrado) por los profesionales por el costo de la mano de obra directa, sumando los costos de los insumos. Los datos se analizaron por medio de estadística descriptiva. Resultados: El costo directo medio total fue de US$ 11,90 (DE±10,79) para las curaciones ambulatorias (n=42), US$ 7,22 (DE±8,69) para las domiciliarias (n=22) y US$ 3,10 (DE±2,57) para las hospitalarias (n=11), con una significativa contribución de los costos de las terapias tópicas: US$ 9,82 (DE±10,55), US$ 5,60 (DE±8,43) y US$ 1,30 (DE±0,22), respectivamente. Conclusión: los resultados pueden sustentar una revisión de los insumos necesarios para las curaciones de úlceras vasculogénicas, en particular, las terapias tópicas y los materiales/soluciones.

48. Prevalencia de lesiones de piel asociadas a la dependencia física en geriátricos e instituciones de salud de Tunja

RESUMEN Objetivos: Determinar la prevalencia puntual de lesiones de piel asociadas a la dependencia física, realizar caracterización sociodemográfica de las personas afectadas, caracterizar las lesiones con base en disposiciones del Grupo Nacional para el Estudio y Asesoramiento en Úlceras por Presión y Heridas Crónicas (GNEAUPP), el National Pressure Ulcer Advisory Panel (NPUAP) y el European Pressure Ulcer Advisory Panel (EPUAP) y establecer la habilidad del personal de salud para el reconocimiento y clasificación de las lesiones de piel asociadas a dependencia. Metodología: Estudio multicéntrico descriptivo de prevalencia puntual en instituciones de servicios de salud de tercer nivel y centros gerontológicos de Tunja, Colombia. Resultados: Participaron cuatro instituciones. La muestra fue de 409 personas, 24 de las cuales presentaron lesiones de piel asociadas a la dependencia para una prevalencia del 5,86%. De los afectados, el 41,6% eran hombres y el 58,4% mujeres; el 79,1% con 50 años o más; en cuanto a la procedencia, el 53,8% pertenecía al área urbana y el 46,2% a la rural; un 56,6% de los pacientes pertenecían al régimen subsidiado, el 30% al contributivo y 13,4% al especial. La etiología de las lesiones estuvo discriminada de la siguiente manera: por presión, 67,3%; LESCAH, 13,4%; MARSI, 11,5% y desgarros cutáneos, 7,7%. Conclusiones: la prevalencia encontrada se relaciona con resultados de estudios colombianos previos, la afectación es mayor en mujeres y la mayor frecuencia de ocurrencia de lesiones corresponde a las ocasionadas por la presión y cizalla; las lesiones de piel asociadas a la dependencia se presentaron con mayor frecuencia en personas de procedencia urbana. El 75% de las personas que diligenciaron la encuesta reportaron no haber recibido capacitación institucional para identificación y clasificación de lesiones de piel asociadas a la dependencia.

Introduction. Penile fracture is a rare urological emergency, the real incidence is unknown given the underreporting caused by the reduced consultation of patients, resulting from the embarrassing context. In the United States it reaches 38%, where sexual etiology predominates. In Colombia there are few published reports. The diagnosis of this entity is purely clinical. The aim of this work is to report a satisfactory evolution in terms of voiding and erectile function in a patient who underwent early cavernous and urethral reconstruction in bilateral rupture of traumatic origin by means of an unconventional surgical technique, in accordance with what has been found in the literature. Case Presentation. A 30-year-old male patient, previously healthy, consulted the emergency department for urethrorrhagia, edema and penile pain secondary to blunt trauma to the penis during sexual intercourse. The ultrasound report showed disruption in the tunica albuginea of the corpus spongiosum, hematoma and apparent urethral transection; surgical exploration was performed 6 hours later, with evidence of fracture of the corpora cavernosa, laceration of 40% of the circumference, complete transection of the urethra and corpus spongiosum. Sutures to the corpora cavernosa and end-to-end urethroplasty of the bulbar urethra were performed, with satisfactory clinical evolution. Discussion. Surgical reconstruction in less than 24 hours in penile fractures significantly reduces hospital stay and early complications; it also reduces the risk of erectile dysfunction, painful erections and voiding problems. There are no publications comparing long-term results between the two surgical approaches. Conclusions. Penile fracture is a rare pathology with clinical diagnosis, which should be managed early by any urologist and, if possible, with reconstructive experience and excellent results in sexual and voiding function.
