Objectives: To assess the ability of the International Endometrial Tumor Analysis (IETA)-1 polynomial regression model to estimate the risk of endometrial cancer (EC) and other intracavitary uterine pathology in women without abnormal uterine bleeding. Methods: This was a retrospective study, in which we validated the IETA-1 model on the IETA-3 study cohort (n = 1745). The IETA-3 study is a prospective observational multicenter study. It includes women without vaginal bleeding who underwent a standardized transvaginal ultrasound examination in one of seven ultrasound centers between January 2011 and December 2018. The ultrasonography was performed either as part of a routine gynecological examination, during follow-up of non-endometrial pathology, in the work-up before fertility treatment or before treatment for uterine prolapse or ovarian pathology. Ultrasonographic findings were described using IETA terminology and were compared with histology, or with results of clinical and ultrasound follow-up of at least 1 year if endometrial sampling was not performed. The IETA-1 model, which was created using data from patients with abnormal uterine bleeding, predicts four histological outcomes: (1) EC or endometrial intraepithelial neoplasia (EIN); (2) endometrial polyp or intracavitary myoma; (3) proliferative or secretory endometrium, endometritis, or endometrial hyperplasia without atypia; and (4) endometrial atrophy. The predictors in the model are age, body mass index and seven ultrasound variables (visibility of the endometrium, endometrial thickness, color score, cysts in the endometrium, non-uniform echogenicity of the endometrium, presence of a bright edge, presence of a single dominant vessel). Results: The median age of the women in the IETA‐3 cohort was 51 (range, 20–85) years and 51% (887/1745) of the women were postmenopausal. Histology showed EC or EIN in 29 (2%) women, endometrial polyps or intracavitary myomas in 1094 (63%), proliferative or secretory endometrium, endometritis, or hyperplasia without atypia in 144 (8%) and endometrial atrophy in 265 (15%) women. The endometrial sample had insufficient material in five (0.3%) cases. In 208 (12%) women who did not undergo endometrial sampling but were followed up for at least 1 year without clinical or ultrasound signs of endometrial malignancy, the outcome was classified as benign. The IETA‐1 model had an AUC of 0.81 (95% CI, 0.73–0.89, n = 1745) for discrimination between malignant (EC or EIN) and benign endometrium, and the observed/expected ratio for EC or EIN was 0.51 (95% CI, 0.32–0.82). The model was able to categorize the four histological outcomes with considerable accuracy: the PDI of the model was 0.68 (95% CI, 0.62–0.73) (n = 1532). The IETA‐1 model discriminated very well between endometrial atrophy and all other intracavitary uterine conditions, with an AUC of 0.96 (95% CI, 0.95–0.98). Including only patients in whom the endometrium was measurable (n = 1689), the model's AUC was 0.83 (95% CI, 0.75–0.91), compared with 0.62 (95% CI, 0.52–0.73) when using endometrial thickness alone to predict malignancy (difference in AUC, 0.21; 95% CI, 0.08–0.32). In postmenopausal women with measurable endometrial thickness (n = 848), the IETA‐1 model gave an AUC of 0.81 (95% CI, 0.71–0.91), while endometrial thickness alone gave an AUC of 0.70 (95% CI, 0.60–0.81) (difference in AUC, 0.11; 95% CI, 0.01–0.20). Conclusion: The IETA‐1 model discriminates well between benign and malignant conditions in the uterine cavity in patients without abnormal bleeding, but it overestimates the risk of malignancy. OBJECTIVES: The primary aim is to describe the ultrasound features of various endometrial and other intracavitary pathologies in women without abnormal uterine bleeding (AUB) using the International Endometrial Tumor Analysis (IETA) terminology. The secondary aim is to compare our findings with published data on women with AUB.METHODS: Patients presenting in seven ultrasound centers from 2011 until 2018 for indications unrelated to abnormal vaginal bleeding underwent transvaginal ultrasound examination using the IETA examination technique and measurement technique. The ultrasonography was performed either as part of routine gynecological examination, follow-up of non-endometrial pathology, or in the workup before undergoing treatment for infertility, uterine prolapse or ovarian pathology. Ultrasound findings were described using IETA terminology. Findings were compared to those in a published cohort of women with AUB who were examined with transvaginal ultrasound between 2012 and 2015 using the same IETA examination technique and terminology.RESULTS: The IETA3 study includes 1745 women without vaginal bleeding who underwent a standardized transvaginal ultrasound examination followed by either endometrial sampling with histological diagnosis (n = 1537) or at least 1 year of clinical and ultrasound follow-up (n = 208). 858 women were premenopausal (49%), and 887 women were postmenopausal (51%). Histology showed endometrial cancer (EC) and/or endometrial intraepithelial neoplasia (EIN) in 29 (2%) women, endometrial polyps in 1028 (59%), intracavitary myomas in 66 (4%), proliferative or secretory changes or hyperplasia without atypia in 144 (8%), endometrial atrophy in 265 (15%) and insufficient tissue in 5 (0.3%). Most EC and EIN (25/29; 86%) were diagnosed after menopause. - Abstract
Objective: To investigate the association between personal history, anthropometric features and lifestyle characteristics and endometrial malignancy in women with abnormal vaginal bleeding. Methods: Prospective observational cohort assessed by descriptive and multivariable logistic regression analyses. Three features—age, body mass index (BMI; calculated as weight in kilograms divided by the square of height in meters), and nulliparity—were defined a priori for baseline risk assessment of endometrial malignancy. The following variables were tested for added value: intrauterine contraceptive device, bleeding pattern, age at menopause, coexisting diabetes/hypertension, physical exercise, fat distribution, bra size, waist circumference, smoking/drinking habits, family history, use of hormonal/anticoagulant therapy, and sonographic endometrial thickness. We calculated adjusted odds ratio, optimism-corrected area under the receiver operating characteristic curve (AUC), R2, and Akaike's information criterion. Results: Of 2417 women, 155 (6%) had endometrial malignancy or endometrial intraepithelial neoplasia. In women with endometrial cancer median age was 67 years (interquartile range [IQR] 56–75 years), median parity was 2 (IQR 0–10), and median BMI was 28 (IQR 25–32). Age, BMI, and parity produced an AUC of 0.82. Other variables marginally affected the AUC, adding endometrial thickness substantially increased the AUC in postmenopausal women. Conclusion: Age, parity, and BMI help in the assessment of endometrial cancer risk in women with abnormal uterine bleeding. Other patient information adds little, whereas sonographic endometrial thickness substantially improves assessment.

- Abstract
OBJECTIVES: The aim of this study was to develop a model that can discriminate between different etiologies of abnormal uterine bleeding. DESIGN: The International Endometrial Tumor Analysis 1 study is a multicenter observational diagnostic study in 18 bleeding clinics in 9 countries. Consecutive women with abnormal vaginal bleeding presenting for ultrasound examination (n = 2,417) were recruited. The histology was obtained from endometrial sampling, D&C, hysteroscopic resection, hysterectomy, or ultrasound follow-up for >1 year. METHODS: A model was developed using multinomial regression based on age, body mass index, and ultrasound predictors to distinguish between: (1) endometrial atrophy, (2) endometrial polyp or intracavitary myoma, (3) endometrial malignancy or atypical hyperplasia, (4) proliferative/secretory changes, endometritis, or hyperplasia without atypia and validated using leave-center-out cross-validation and bootstrapping. The main outcomes are the model's ability to discriminate between the four outcomes and the calibration of risk estimates. RESULTS: The median age in 2,417 women was 50 (interquartile range 43-57). 414 (17%) women had endometrial atrophy; 996 (41%) had a polyp or myoma; 155 (6%) had an endometrial malignancy or atypical hyperplasia; and 852 (35%) had proliferative/secretory changes, endometritis, or hyperplasia without atypia. The model distinguished well between malignant and benign histology (c-statistic 0.88 95% CI: 0.85-0.91) and between all benign histologies. The probabilities for each of the four outcomes were over- or underestimated depending on the centers. LIMITATIONS: Not all patients had a diagnosis based on histology. The model over- or underestimated the risk for certain outcomes in some centers, indicating local recalibration is advisable. The histological endpoints were endometrial atrophy, proliferative or secretory endometrium, endometrial hyperplasia without atypia, endometrial polyp, intracavitary leiomyoma, endometrial intraepithelial neoplasia (EIN), endometrial cancer (EC) and insufficient tissue. The findings in our cohort of women without AUB were compared with those in a published cohort of women with AUB who were examined with transvaginal ultrasound between 2012 and 2015 using the same IETA examination technique and terminology.Results: In this study (IETA3), we included 1745 women without AUB who underwent a standardized transvaginal ultrasound examination followed by either endometrial sampling with histological diagnosis (n = 1537) or at least 1 year of clinical and ultrasound follow-up (n = 208). Of these, 858 (49.2%) women were premenopausal and 887 (50.8%) were postmenopausal. Histology showed the presence of EC and/or EIN in 29 (1.7%) women, endometrial polyps in 1028 (58.9%), intracavitary myomas in 66 (3.8%), proliferative or secretory changes or hyperplasia without atypia in 144 (8.3%), endometrial atrophy in 265 (15.2%) and insufficient tissue in five (0.3%). Most cases of EC or EIN (25/29 (86.2%)) were diagnosed after menopause. The mean endometrial thickness in women with EC or EIN was 11.2 mm (95% CI, 8.9-13.6 mm), being on average 2.4 mm (95% CI, 0.3-4.6 mm) thicker than their benign counterparts. Women with malignant endometrial pathology manifested more frequently non-uniform echogenicity (22/29 (75.9%)) than did those with benign endometrial pathology (929/1716 (54.1%)) (difference, +21.8% (95% CI, +4.2% to +39.2%)). Moderate to abundant vascularization (color score 3-4) was seen in 31.0% (9/29) of cases with EC or EIN compared with 12.8% (220/1716) of those with a benign outcome (difference, +18.2% (95% CI, -0.5% to +36.9%)). Multiple multifocal vessels were recorded in 24.1% (7/29) women with EC or EIN vs 4.0% (68/1716) of those with a benign outcome (difference, +20.2% (95% CI, +4.6% to +35.7%)). A regular endometrial-myometrial junction was seen less frequently in women with EC or EIN (19/29 (65.5%)) vs those with a benign outcome (1412/1716 (82.3%)) (difference, -16.8% (95% CI, -34.2% to +0.6%)). In women with endometrial polyps without AUB, a single dominant vessel was the most frequent vascular pattern (666/1028 (64.8%)). In women with EC, both in those with and those without AUB, the endometrium usually manifested heterogeneous echogenicity, but the endometrium was on average 8.6 mm (95% CI, 5.2-12.0 mm) thinner and less intensely vascularized (color score 3-4: difference, -26.8% (95% CI, -52.2% to -1.3%)) in women without compared to those with AUB. In both pre- and postmenopausal women, asymptomatic endometrial polyps were associated with a thinner endometrium, and they manifested more frequently a bright edge, a regular endometrial-myometrial junction and a single dominant vessel than did polyps in symptomatic women, and they were less intensely vascularized.Conclusions: We describe the typical ultrasound features of EC, polyps and other intracavitary histologies using IETA terminology in women without AUB. Our findings suggest that the presence of asymptomatic polyps or endometrial malignancy may be accompanied by thinner and less intensely vascularized endometria than their symptomatic counterparts. © 2022 International Society of Ultrasound in Obstetrics and Gynecology. Deep-inelastic scattering data in the range 150 < Q^2 < 35000 GeV^2 are used to investigate the minimum jet separation necessary to allow accurate description of the rate of dijet production using next-to-leading order perturbative QCD calculations. The required jet separation is found to be small, allowing about 1/3 of DIS data to be classified as dijet, as opposed to approximately 1/10 with more typical jet analyses. A number of precision measurements made using this dijet sample are well described by the calculations. The data are also described by the combination of leading order matrix elements and parton showers, as implemented in the QCD based Monte Carlo model RAPGAP., 18 pages, 4 figures, 4 tables

The inclusive cross section for the photoproduction of neutral pions has been measured as a function of the transverse momentum, rapidity, and Feynman x of the pizero mesons at an average photon--proton centre-of-mass energy of 208 GeV and for photon virtualities below Q^2=0.01 GeV^2. The pizero measurement extends the range covered by previous charged particle measurements at HERA by two units of rapidity in the photon direction down to a value of -5.5 in the gamma p centre-of-mass frame. The pizero transverse momentum distribution is well described over the whole measured range by a power law ansatz, while an exponential fit falls below the data at transverse momentum values above 1.5 GeV/c. Good agreement with the predictions of the Monte Carlo models PYTHIA and PHOJET is found. In the context of the PYTHIA model the data are inconsistent with large intrinsic transverse momentum values in the photon., 20 pages, 6 figures and 3 tables

The triple-differential dijet cross-section, \({\rm d}^3 \sigma_{ep}/{\rm d} Q^2{\rm d} \overline{E_t}^2 {\rm d}x^{jets}_\gamma\), is measured with the H1 detector at HERA as a function of the photon virtuality \(Q^2\), the fraction of the photon's momentum carried by the parton entering the hard scattering, \(\x^{jets}_\gamma\), and the square of the mean transverse energy, \(\overline{E_t}^2\), of the two highest \(E_t\) jets. Jets are found using a longitudinal boost-invariant \(k_T\) clustering algorithm in the \(\gamma^* p\) center of mass frame. The measurements cover the ranges \(1.6 < Q^2 < 80\) GeV\(^2\) in virtuality and \(0.1 < y < 0.7\) in inelasticity y. The results are well described by leading order QCD models which include the effects of a resolved component to the virtual photon. Models which treat the photon as point-like fail to describe the data. An effective leading order parton density for the virtual photon is extracted as a function of the photon virtuality, the probing scale and the parton momentum fraction. The \(x_{\gamma}\) and probing scale dependences of the parton density show characteristic features of photon structure, and a suppression of this structure with increasing \(Q^2\) is seen.

Results are presented on the photoproduction of isolated prompt photons, inclusively and associated with jets, in the gamma p center of mass energy range 142 < W < 266 GeV. The cross sections are measured for the transverse momentum range of the photons 5 < E_T^gamma < 10 GeV and for associated jets with E_T^jet > 4.5 GeV. They are measured differentially as a function of E_T^gamma, E_T^jet, the pseudorapidities eta^gamma and eta^jet and estimators of the momentum fractions x_gamma and x_p of the incident photon and proton carried by the constituents participating in the hard process. In order to further investigate the underlying dynamics, the angular correlation between the prompt photon and the jet in the transverse plane is studied. Predictions by perturbative QCD calculations in next to leading order are about 30% below the inclusive prompt photon data after corrections for hadronisation and multiple interactions, but are in reasonable agreement with the results for prompt photons associated with jets. Comparisons with the predictions of the event generators PYTHIA and HERWIG are also presented., 21 pages, 6 figures, 3 tables

A search for single top quark production is performed in e±p collisions at HERA. The search exploits data corresponding to an integrated luminosity of 118.3 pb-1. A model for the anomalous production of top quarks in a flavour changing neutral current process involving a tuγ coupling is investigated. Decays of top quarks into a b quark and a W boson are considered in the leptonic and the hadronic decay channels of the W. Both a cut-based analysis and a multivariate likelihood analysis are performed to discriminate anomalous top quark production from Standard Model background processes. In the leptonic channel, 5 events are found while 1.31 ± 0.22 events are expected from the Standard Model background. In the hadronic channel, no excess above the expectation for Standard Model processes is found. These observations lead to a cross section σ(ep→etX)=0.29+0.15−0.14 pb at s√=319 GeV. Alternatively, assuming that the observed events are due to a statistical fluctuation, upper limits of 0.55 pb on the anomalous top production cross section and of 0.27 on the tuγ coupling κtuγ are established at the 95% confidence level., The European Physical Journal C, 33 (1), ISSN:1434-6044, ISSN:1434-6052

The proton structure function F_2(x,Q^2) is measured in inelastic QED Compton scattering using data collected with the H1 detector at HERA. QED Compton events are used to access the kinematic range of very low virtualities of the exchanged photon, Q^2, down to 0.5 GeV^2, and Bjorken x up to \sim 0.06, a region which has not been covered previously by inclusive measurements at HERA. Deep-inelastic positron-proton interactions at low values of Bjorken-x down to x \approx 4.10^-5 which give rise to high transverse momentum pi^0 mesons are studied with the H1 experiment at HERA. The inclusive cross section for pi^0 mesons produced at small angles with respect to the proton remnant (the forward region) is presented as a function of the transverse momentum and energy of the pi^0 and of the four-momentum transfer Q^2 and Bjorken-x. Measurements are also presented of the transverse energy flow in events containing a forward pi^0 meson. Hadronic final state calculations based on QCD models implementing different parton evolution schemes are confronted with the data., Comment: 27 pages, 8 figures and 3 tables

A search for squarks in R-parity violating supersymmetry is performed in e+/- p collisions at HERA using the H1 detector. The data were taken at a centre-of-mass energy of 319 GeV and correspond to an integrated luminosity of 64.3 pb-1 for e+p collisions and 13.5 pb-1 for e-p collisions. The resonant production of squarks via a Yukawa coupling lambda' is considered, taking into account direct and indirect R-parity violating decay modes. No evidence for squark production is found in the multi-lepton and multi-jet final state topologies investigated. Mass dependent limits on lambda' are obtained in the framework of the Minimal Supersymmetric Standard Model. In addition, the results are interpreted in terms of constraints on the parameters of the minimal Supergravity model. At the 95% confidence level squarks of all flavours with masses up to 275 GeV are excluded in a large part of the parameter space for a Yukawa coupling of electromagnetic strength. For a coupling strength 100 times smaller, masses up to 220 GeV can be ruled out., 28 pages, 12 figures and 3 tables

The inclusive e^+ p single and double differential cross sections for neutral and charged current processes are measured with the H1 detector at HERA. The data were taken in 1999 and 2000 at a centre-of-mass energy of \sqrt{s} = 319 GeV and correspond to an integrated luminosity of 65.2 pb^-1. The cross sections are measured in the range of four-momentum transfer squared Q^2 between 100 and 30000 GeV^2 and Bjorken x between 0.0013 and 0.65. The neutral current analysis for the new e^+ p data and the earlier e^- p data taken in 1998 and 1999 is extended to small energies of the scattered electron and therefore to higher values of inelasticity y, allowing a determination of the longitudinal structure function F_L at high Q^2 (110 - 700 GeV^2). A new measurement of the structure function x F_3 is obtained using the new e^+ p and previously published e^\pm p neutral current cross section data at high Q^2. These data together with H1 low Q^2 precision data are further used to perform new next-to-leading order QCD analyses in the framework of the Standard Model to extract flavour separated parton distributions in the proton., 54 pages, 19 figures and 15 tables

The diffractive photoproduction of J/psi mesons is measured with the H1 detector at the ep collider HERA using an integrated luminosity of 78 pb^-1. The differential cross section d sigma(gamma p -> J/psi Y) / d t is studied in the range 2 < |t| < 30 GeV^2, where t is the square of the four-momentum transferred at the proton vertex. The cross section is also presented as a function of the photon-proton centre-of-mass energy W in three t intervals, spanning the range 50 < W < 200 GeV. A fast rise of the cross section with W is observed for each t range and the slope for the effective linear Pomeron trajectory is measured to be alpha^\prime= -0.0135 \pm 0.0074 (stat.) \pm 0.0051 (syst.) GeV^-2. The measurements are compared with perturbative QCD models based on BFKL and DGLAP evolution. The data are found to be compatible with s-channel helicity conservation.

Deep-inelastic e^\pm p scattering at high squared momentum transfer Q^2 up to 30000 GeV^2 is used to search for eq contact interactions associated to scales far beyond the HERA centre of mass energy. The neutral current cross section measurements d sigma / d Q^2, corresponding to integrated luminosities of 16.4 pb^-1 of e^- p data and 100.8 pb^-1 of e^+ p data, are well described by the Standard Model and are analysed to set constraints on new phenomena. For conventional contact interactions lower limits are set on compositeness scales Lambda ranging between 1.6 - 5.5 TeV. Couplings and masses of leptoquarks and squarks in R-parity violating supersymmetry are constrained to M / lambda > 0.3 - 1.4 TeV. A search for low scale quantum gravity effects in models with large extra dimensions provides limits on the effective Planck scale of M_S > 0.8 TeV. A form factor analysis yields a bound on the radius of light quarks of R_q < 1.0 x 10^-18 m.

Multi-electron production is studied at high electron transverse momentum in positron- and electron-proton collisions using the H1 detector at HERA. The data correspond to an integrated luminosity of 115 pb-1. Di-electron and tri-electron event yields are measured. Cross sections are derived in a restricted phase space region dominated by photon-photon collisions. In general good agreement is found with the Standard Model predictions. However, for electron pair invariant masses above 100 GeV, three di-electron events and three tri-electron events are observed, compared to Standard Model expectations of 0.30 \pm 0.04 and 0.23 \pm 0.04, respectively.

Dijet events in photon-proton collisions in which there is a large pseudorapidity separation Delta eta > 2.5 between the two highest E_T jets are studied with the H1 detector at HERA. The inclusive dijet cross sections are measured as functions of the longitudinal momentum fractions of the proton and photon which participate in the production of the jets, x_pjet and x_gjet respectively, Delta eta, the pseudorapidity separation between the two highest E_T jets, and E_T^gap, the total summed transverse energy between the jets. Rapidity gap events are defined as events in which E_T^gap is less than E_T^cut, for E_T^cut varied between 0.5 and 2.0 GeV. The fraction of dijet events with a rapidity gap is measured differentially in Delta eta, x_pjet and x_gjet. An excess of events with rapidity gaps at low values of E_T^cut is observed above the expectation from standard photoproduction processes. This excess can be explained by the exchange of a strongly interacting colour singlet object between the jets., 22 pages, 5 tables, 6 figures

Dijet cross sections as functions of several jet observables are measured in photoproduction using the H1 detector at HERA. The data sample comprises e^+p data with an integrated luminosity of 34.9 pb^(-1). Jets are selected using the inclusive k_T algorithm with a minimum transverse energy of 25 GeV for the leading jet. The phase space covers longitudinal proton momentum fraction x_p and photon longitudinal momentum fraction x_gamma in the ranges 0.05, Comment: 25 pages, 8 figures, 7 tables

The helicity structure of the diffractive electroproduction of rho mesons, e + p -> e + rho + Y, is studied in a previously unexplored region of large four-momentum transfer squared at the proton vertex, t: 0 < t' < 3 GeV^2, where t' = |t| - |t|_min. The data used are collected with the H1 detector at HERA in the kinematic domain 2.5 < Q^2 < 60 GeV^2, 40 < W < 120 GeV. No t dependence of the r^04_00 spin density matrix element is found. A significant t dependent helicity non-conservation from the virtual photon to the rho meson is observed for the spin density matrix element combinations r^5_00+2r^5_11 and r^1_00+2r^1_11. These t dependences are consistently described by a perturbative QCD model based on the exchange of two gluons., 20 pages, 4 figures, 1 table

The inclusive e^-p single and double differential cross sections for neutral and charged current processes are measured with the H1 detector at HERA, in the range of four-momentum transfer squared Q^2 between 150 and 30000 GeV^2, and Bjorken x between 0.002 and 0.65. The data were taken in 1998 and 1999 with a centre-of-mass energy of 320 GeV and correspond to an integrated luminosity of 16.4 pb^(-1). The data are compared with recent measurements of the inclusive neutral and charged current e^+p cross sections. For Q^2 1000 GeV^2 clear evidence is observed for an asymmetry between e^+p and e^-p neutral current scattering and the generalised structure function xF_3 is extracted for the first time at HERA. A fit to the charged current data is used to extract a value for the W boson propagator mass. The data are found to be in good agreement with Standard Model predictions.

Jet production in charged and neutral current events in the kinematic range of Q^2 from 640 to 35000 GeV^2 is studied in deep-inelastic positron-proton scattering at HERA. The measured rate of multi-jet events and distributions of jet polar angle, transverse energy, dijet mass, and other dijet variables are presented. Using parton densities derived from inclusive DIS cross sections, perturbative QCD calculations in NLO are found to give a consistent description of both the neutral and charged current dijet production. A direct, model independent comparison of the jet distributions in charged and neutral current events confirms that the QCD dynamics of the hadronic final state is independent of the underlying electroweak scattering process., Comment: 28 pages, 10 figures, 4 tables
