2. The Lothian Atopic Dermatitis Study

Abstract

The Lothian Atopic Dermatitis Study was designed to quantify the economic burden of atopic dermatitis and to measure the impact on the quality of life of patients with this common condition. The study population was a semi-rural general practice of 9,786. A sampling scheme was devised to identify as many as possible of those in the practice with atopic dermatitis. One-year period prevalences were calculated by standard methods for age groups < 2, 2-11, 12-15, 16-24, 25-40, 41+. Expense, estimated over a 2 month period, was attributed to the patient, health service or society according to the source. One unpublished quality of life measure was used for children and two well-validated measures, the Dermatology Life Quality Index and the Patient Generated Index, were competed by adults. The one year period prevalences were 9.8% for the under 2's, 8.1% age 2-11, 2.2% age 12-15, 2.1% age 16-24, 2.0% age 25-40 and 0.2% over 40's. This pattern shows clearly the sharp decline in the prevalence of atopic dermatitis during teenage and adult years. The largest source of cost to patients was clothing and laundry, and the mean annual cost to patients was £153. Treatments accounted for the largest source of expenditure by the health service giving a mean cost per patient of £97. There were 58 lost working days and 17 lost school days by all patients as a result of atopic dermatitis. If these results were extrapolated to the U.K. population they would represent a total annual expenditure of £172m by patients, £125m by the health service and £168m by society. This adds up to £465m per annum or a per capita cost of £7.38 per annum. The quality of life measures reflected the range of severity of atopic dermatitis in the community. The Dermatology Life Quality Index was not specific for atopic dermatitis but correlated significantly with the Patient Generated Index. Clinical studies are necessary to characterise adult atopic dermatitis and emphasise the pressure from the heavy economic burden and impaired life quality.

Abstract

This experiment was to evaluate a suite of biological traits likely to be associated with genetic variation in residual feed intake (RFI) in Angus cattle. Twenty nine steers and 30 heifers bred to be divergent in postweaning RFI (RFI p ) and that differed in midparent RFIp-EBV (RFI p -EBV mp ) by more than 2 kg DMI/d were used in this study. A 1-unit (1 kg DM/d) decrease in RFIp-EBVmp was accompanied by a 0.08 kg (SE = 0.03; P < 0.05) increase in ADG, a 0.58 kg/d (0.17; P < 0.01) decrease in DMI, a 0.89 kg/kg (0.22; P < 0.001) decrease in FCR, and a 0.62 kg/d (0.12; P < 0.001) decrease in feedlot RFI (RFI f ). Ultrasonically scanned depths of subcutaneous fat at the rib and rump sites, measured at the start and end of the RFI test, all had strong positive correlations with RFIp-EBVmp, DMI, and RFIf (all r values ≥0.5 and P < 0.001). Variation in RFIp-EBVmp was significantly correlated (P < 0.05) with flight speed (r = −0.32), number of visits to feed bins (r = 0.45), and visits to exhaled-emission monitors (r = −0.27), as well as the concentrations of propionate (r = −0.32) and valerate (r = −0.31) in rumen fluid, white blood cell (r = −0.51), lymphocyte (r = −0.43), and neutrophil (r = −0.31) counts in blood. RFIp-EBVmp was also correlated with the cellular immune response to vaccination (r = 0.25; P < 0.1) and heat production in fasted cattle (r = −0.46; P < 0.001). Traits that explained significant variation (P < 0.05) in DMI over the RFI test were midtest metabolic-BW (44.7%), rib fat depth at the end of test (an additional 18%), number of feeder visits (additional 5.7%), apparent digestibility of the ration by animals (additional 2.4%) and white blood-cell count (2.1%), and the cellular immune response to vaccine injection (additional 1.1%; P < 0.1), leaving ~23% of the variation in DMI unexplained. The same traits (BW excluded) explained 33%, 12%, 3.6%, 3.7%, and 3.1%, and together explained 57% of the variation in RFIf. This experiment showed that genetic variation in RFI was accompanied by variation in estimated body composition, behavior, rumen, fasted heat production, hematology, and immune competence traits, and that variation in feedlot DMI and RFIf was due to differences in BW, scanned fatness, and many other factors in these cattle fed ad libitum and able to display any innate differences in appetite, temperament, feeding behavior, and activity. Abstract

Background The hedgehog pathway inhibitor sonidegib demonstrated meaningful tumor shrinkage in more than 90% of patients with locally advanced basal cell carcinoma (BCC) or metastatic BCC in the BCC Outcomes with LDE225 Treatment study. Objective This report provides long-term follow-up data collected up to 12 months after the last patient was randomized. Methods In this multicenter, randomized, double-blind phase II study, patients were randomized 1:2 to sonidegib 200 or 800 mg. The primary end point was objective response rate assessed by central review. Results Objective response rates in the 200- and 800-mg arms were 57.6% and 43.8% in locally advanced BCC and 7.7% and 17.4% in metastatic BCC, respectively. Among the 94 patients with locally advanced BCC who responded, only 18 progressed or died and more than 50% had responses lasting longer than 6 months. In addition, 4 of 5 responders with metastatic BCC maintained an objective response. Grade 3/4 adverse events and those leading to discontinuation were less frequent with sonidegib 200 versus 800 mg (38.0% vs 59.3%; 27.8% vs 37.3%, respectively). Limitations No placebo or comparator arms were used because sonidegib demonstrated efficacy in advanced BCC in a phase I study, and the hedgehog pathway inhibitor vismodegib was not yet approved. Conclusion With longer follow-up, sonidegib demonstrated sustained tumor responses in patients with advanced BCC

Abstract

Background: Patients with advanced basal cell carcinoma have limited treatment options. Hedgehog pathway signalling is aberrantly activated in around 95% of tumours. We assessed the antitumour activity of sonidegib, a Hedgehog signalling inhibitor, in patients with advanced basal cell carcinoma. Methods: BOLT is an ongoing multicentre, randomised, double-blind, phase 2 trial. Eligible patients had locally advanced basal cell carcinoma not amenable to curative surgery or radiation or metastatic basal cell carcinoma. Patients were randomised via an automated system in a 1:2 ratio to receive 200 mg or 800 mg oral sonidegib daily, stratified by disease, histological subtype, and geographical region. The primary endpoint was the proportion of patients who achieved an objective response, assessed in the primary efficacy analysis population (patients with fully assessable locally advanced disease and all those with metastatic disease) with data collected up to 6 months after randomisation of the last patient. This trial is registered with ClinicalTrials.gov, number NCT01327053. Findings: Between July 20, 2011, and Jan 10, 2013, we enrolled 230 patients, 79 in the 200 mg sonidegib group, and 151 in the 800 mg sonidegib group. Median follow-up was 13·9 months (IQR 10·1-17·3). In the primary efficacy analysis population, 20 (36%, 95% CI 24-50) of 55 patients receiving 200 mg sonidegib and 39 (34%, 25-43) of 116 receiving 800 mg sonidegib achieved an objective response. In the 200 mg sonidegib group, 18 (43%, 95% CI 28-59) patients who achieved an objective response, as assessed by central review, were noted among the 42 with locally advanced basal cell carcinoma and two (15%, 2-45) among the 13 with metastatic disease. 25. Baseline and greenhouse-gas emissions in extensive livestock enterprises, with a case study of feeding lipid to beef cattle

Abstract

For accurate calculation of reductions in greenhouse-gas (GHG) emissions, methodologies under the Australian Government's Carbon Farming Initiative (CFI) depend on a valid assessment of the baseline and project emissions. Life-cycle assessments (LCAs) clearly show that enteric methane emitted from the rumen of cattle and sheep is the major source of GHG emissions from livestock enterprises. Where a historic baseline for a CFI methodology for livestock is required, the use of simulated data for cow-calf enterprises at six sites in southern Australia demonstrated that a 5-year rolling emission average will provide an acceptable trade off in terms of accuracy and stability, but this is a much shorter time period than typically used for LCA. For many CFI livestock methodologies, comparative or pair-wise baselines are potentially more appropriate than historic baselines. A case study of lipid supplementation of beef cows over winter is presented. The case study of a control herd of 250 cows used a comparative baseline derived from simple data on livestock numbers and class of livestock to quantify the emission abatement. Compared with the control herd, lipid supplementation to cows over winter increased livestock productivity, total livestock production and enterprise GHG emissions from 990 t CO2-e to 1022 t CO2-e. Energy embodied in the supplement and extra diesel used in transporting the supplement diminished the enteric-methane abatement benefit of lipid supplementation. Reducing the cow herd to 238 cows maintained the level of livestock production of the control herd and reduced enterprise emissions to 938 t CO2-e, but was not cost effective under the assumptions of this case study.

Abstract

Residual feed intake (RFI) is a measure of feed efficiency in beef cattle. It is a moderately heritable trait, and cattle with low RFI consume less feed than expected at the same level of growth relative to their high RFI contemporaries. Selection for RFI is a relatively new genetic improvement tool in beef cattle to reduce the cost of production, and currently has a low level of adoption in the industry. Selection for low RFI is associated with reduction in greenhouse gas (GHG) emissions, hence could play a role in any carbon trading scheme implemented in Australia. For any GHG reduction protocol to be acceptable in a carbon trading/offset environment, it needs to follow United Nations IPCC guidelines, be based on science, be quantifiable and be auditable. The beef industry already has quality assurance systems in place for RFI that can be fine-tuned to meet these criteria. Scientific information currently available is adequate for the development of GHG emission reduction protocols for cattle raised for slaughter. Selection for RFI also has an impact on GHG emissions from the breeding herd. However, information currently available lacks the degree of accuracy needed for protocol development. This experiment investigated the benefits of feeding steers bred from parents known to be of high genetic merit for feed efficiency (HE; midparent net feed intake-estimated breeding value (NFI-EBV) ≤-0.3kg/day), medium efficiency (ME: midparent NFI-EBV >-0.3 to 0.14 kg/day), or low efficiency (LE; midparent NFI-EBV ≥0.16kg/day). The steers were evaluated over a 251-day feeding period in a large commercial feedlot. Genetic superiority in NFI had a favourable impact on the commercial performance of Angus steers by reducing feed consumed with no adverse effect on final turnoff weight. Each HE steer consumed on average 2.60t of feed compared to 2.87t by the ME and LE steers, with no compromise in weight gain. The HE steers gained less subcutaneous ribfat during the feedlot period but marbling scores were not influenced by NFI, and dressing percentage was higher in the HE steers. The feed efficiency benefit was sustained for 251 days and showed that genetic improvement of feed efficiency will reduce feed costs in a large commercial feedlot.

Abstract

The CRC for Beef Genetic Technologies (the 'Beef CRC') has established the Maternal Efficiency Project to provide new knowledge to help better inform beef producers on the impacts estimated breeding values for carcase fatness/carcase muscularity and for feed efficiency have on maternal efficiency, especially under variable nutritional environments. The project is being conducted across southern Australia. It has an industry herd component and a research station component being run simultaneously. Early results from the research stations show that selection for traits affecting body composition holds across seasons and nutrition treatments, and that genetically leaner cows seem to be less fertile, especially under low nutrition. Results are peer-reviewed and compiled for industry dissemination in the Maternal Journal available through the Beef CRC website (www.beefcrc.com.au)., Special Edition Sheep and Cattle Production in New South Wales

Abstract

MicroRNAs (miRNAs) are short non-coding RNAs that post-transcriptionally regulate expression of mRNAs in many biological pathways. Liver plays an important role in the feed efficiency of animals and high and low efficient cattle demonstrated different gene expression profiles by microarray. Here we report comprehensive miRNAs profiles by next-gen deep sequencing in Angus cattle divergently selected for residual feed intake (RFI) and identify miRNAs related to feed efficiency in beef cattle. Two microRNA libraries were constructed from pooled RNA extracted from livers of low and high RFI cattle, and sequenced by Illumina genome analyser. In total, 23,628,103 high quality short sequence reads were obtained and more than half of these reads were matched to the bovine genome (UMD 3.1). We identified 305 known bovine miRNAs. Bta-miR-143, bta-miR-30, bta-miR-122, bta-miR-378, and btalet- 7 were the top five most abundant miRNAs families expressed in liver, representing more than 63% of expressed miRNAs. We also identified 52 homologous miRNAs and 10 novel putative bovine-specific miRNAs, based on precursor sequence and the secondary structure and utilizing the miRBase (v. 21). We compared the miRNAs profile between high and low RFI animals and ranked the most differentially expressed bovine known miRNAs. Bovine miR-143 was the most abundant miRNA in the bovine liver and comprised 20% of total expressed mapped miRNAs. The most highly expressed miRNA in liver of mice and humans, miR-122, was the third most abundant in our cattle liver samples. We also identified 10 putative novel bovine-specific miRNA candidates. Differentially expressed miRNAs between high and low RFI cattle were identified with 18 miRNAs being up-regulated and 7 other miRNAs down-regulated in low RFI cattle. Our study has identified comprehensive miRNAs expressed in bovine liver. Some of the expressed miRNAs are novel in cattle. The differentially expressed miRNAs between high and low RFI give some insights into liver miRNAs regulating physiological pathways underlying variation in this measure of feed efficiency in bovines. Abstract

Atopic dermatitis is a chronic, debilitating skin disease. Children have occupied the major part of investigators' time, but cross-sectional community studies, encompassing the whole population, are lacking. This thesis describes an epidemiological study, based on a general practice of 9,786, and assesses, at all ages, the prevalence, the economic burden to patients and the health service, and the quality of life of patients with atopic dermatitis.
