Abstract

Despite extensive research on severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) vaccination responses in healthy individuals, there is comparatively little known beyond antibody titers and T‐cell responses in the vulnerable cohort of patients after allogeneic hematopoietic stem cell transplantation (ASCT). In this study, we assessed the serological response and performed longitudinal multimodal analyses including T‐cell functionality and single‐cell RNA sequencing combined with T cell receptor (TCR)/B cell receptor (BCR) profiling in the context of BNT162b2 vaccination in ASCT patients. In addition, these data were compared to publicly available data sets of healthy vaccinees. Protective antibody titers were achieved in 40% of patients. We identified a distorted B‐ and T‐cell distribution, a reduced TCR diversity, and increased levels of exhaustion marker expression as possible causes for the poorer vaccine response rates in ASCT patients. Immunoglobulin heavy chain gene rearrangement after vaccination proved to be highly variable in ASCT patients. Changes in TCRα and TCRβ gene rearrangement after vaccination differed from patterns observed in healthy vaccinees. Crucially, ASCT patients elicited comparable proportions of SARS‐CoV‐2 vaccine‐induced (VI) CD8+ T‐cells, characterized by a distinct gene expression pattern that is associated with SARS‐CoV‐2 specificity in healthy individuals. Our study underlines the impaired immune system and thus the lower vaccine response rates in ASCT patients. However, since protective vaccine responses and VI CD8+ T‐cells can be induced in part of ASCT patients, our data advocate early posttransplant vaccination due to the high risk of infection in this vulnerable group. Abstract

The european shore crab Carcinus maenas of the Wadden Sea lives in a diverse range of habitats. It is highly adaptable to different environments where it chooses structures that offer protection from predators. Its choice of habitat is also influenced by the age of the specimen. Habitats that contain mussels and rocks function as a refuge and are chosen more often by juvenile crabs. This has been confirmed under laboratory conditions. Die Europäische Küstenkrabbe Carcinus maenas des Wattenmeeres lebt in einer Vielzahl von Lebensräumen. Es ist sehr anpassungsfähig an verschiedene Umgebungen, in denen es Strukturen wählt, die Schutz vor Raubtieren bieten. Die Wahl des Lebensraums wird auch vom Alter des Exemplars beeinflusst. Lebensräume, die Muscheln und Felsen enthalten, fungieren als Zufluchtsort und werden häufiger von jungen Krebsen gewählt. Dies wurde unter Laborbedingungen bestätigt.

Abstract

Tumors arising in the context of Lynch Syndrome or constitutional mismatch repair deficiency are hypermutated and have a good response towards immune-checkpoint inhibitors (ICIs), including α-PD-L1 antibodies. However, in most cases, resistance mechanisms evolve. To improve outcomes and prevent resistance development, combination approaches are warranted. Herein, we applied a combined regimen with an α-PD-L1 antibody and gemcitabine in a preclinical tumor model to activate endogenous antitumor immune responses. Mlh1−/− mice with established gastrointestinal tumors received the α-PD-L1 antibody (clone 6E11, 2.5 mg/kg bw, i.v., q2wx3) and gemcitabine (100 mg/kg bw, i.p., q4wx3) in mono- or combination therapy. Survival and tumor growth were recorded. Immunological changes in the blood were routinely examined via multi-color flow cytometry and complemented by ex vivo frameshift mutation analysis to identify alterations in Mlh1−/−-tumor-associated target genes. The combined therapy of α-PD-L1 and gemcitabine prolonged median overall survival of Mlh1−/− mice from four weeks in the untreated control group to 12 weeks, accompanied by therapy-induced tumor growth inhibition, as measured by [18F]-FDG PET/CT. Plasma cytokine levels of IL13, TNFα, and MIP1β were increased and also higher than in mice receiving either monotherapy. Circulating splenic and intratumoral myeloid-derived suppressor cells (MDSCs), as well as M2 macrophages, were markedly reduced. Besides, residual tumor specimens from combi-treated mice had increased numbers of infiltrating cytotoxic T-cells. Frameshift mutations in APC, Tmem60, and Casc3 were no longer detectable upon treatment, likely because of the successful eradication of single mutated cell clones. By contrast, novel mutations appeared. Collectively, we herein confirm the safe application of combined chemo-immunotherapy by long-term tumor growth control to prevent the development of resistance mechanisms.

Abstract

Introduction/Background Multidisciplinary tumour boards (MDTBs) are universally recommended. Nevertheless access to MDTBs, especially in low-income countries and rural areas, is limited. In order to gain insight in its efficiency and in its impact on quality of cancer care this study has been performed on the international multidisciplinary (video-) online tumour board (iMDTB) established by Camfomedics e.V. and its partners Mephida e.V. and Global Health Catalyst Summit @ Harvard with a cancer centre in northwest region of Cameroon, the Mbingo Baptist Hospital. Methodology Patient’s data of all cases of 2019 of the Camfomedics-iMDTB have been collected and evaluated in regard of disease, age, sex, stage, recommendation and level of available care. Furthermore an online survey among participants of the Camfomedics-iMDTB on their practises, experiences and satisfaction with the iMDTB has been undertaken. Results International multidisciplinary tumour board was scheduled monthly with online video meeting times of 60 to 90 mins. In 2019 during 12 meetings 95 tumour cases had been discussed. The majority of patients (75%) were female. 24% of all tumour cases were breast cancer followed by cervical cancer with 10%. Remarkably anorectal carcinomas and sarcomas occurred with a percentage of 7–8% each. Furthermore three women out of 72 suffered from high risk trophoblastic tumours. 66% of cases could be presented with a proper TNM-classification. More than half of these patients were already in a late stage of their disease (extended, metastatic or high risk). Pathology results were limited to microscopy for most cases. Additional diagnostics (such as hormone receptor status, HER2neu status) were available only in a minority of the cases. Treatment plans had been changed in up to 50% of cases. The tumour board members describe their experience with the online conferences, data and documentation quality as satisfactory. Conclusion The iMDTB of Camfomedics is a helpful and effective way to improve cancer care in low income countries and rural areas such as the northwest region of Cameroon. The tumour board’s success very much depends on the charitable attendance of its specialists and the local (human) resources for time consuming preparation. Main challenges remain foremost the patients’ ability to afford expensive cancer diagnostics and therapies, local availability of medical, surgical and radiological treatment as well as a stable online video connection. The iMDTB has a significant impact not only on multidisciplinarity of cancer management in the cooperating institutions but also on social values and education of the participants. Disclosures None.

Congenital factor VII (FVII) deficiency is a rare bleeding disorder (RBD) with phenotypes ranging from asymptomatic state to life threatening bleeding episodes. There is no established recommendation for the perioperative management of patients scheduled for cardiac surgery. We have described the perioperative management of a patient with FVII deficiency treated for aortic valve stenosis, coronary artery disease, and atrial fibrillation. Abstract

Despite extensive research on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination responses in healthy individuals, there is comparatively little known beyond antibody titers and T-cell responses in the vulnerable cohort of patients after allogeneic hematopoietic stem cell transplantation (ASCT). In this study, we assessed the serological response and performed longitudinal multimodal analyses including T-cell functionality and single-cell RNA sequencing combined with T cell receptor (TCR)/B cell receptor (BCR) profiling in the context of BNT162b2 vaccination in ASCT patients. In addition, these data were compared to publicly available data sets of healthy vaccinees. Protective antibody titers were achieved in 40% of patients. We identified a distorted B- and T-cell distribution, a reduced TCR diversity, and increased levels of exhaustion marker expression as possible causes for the poorer vaccine response rates in ASCT patients. Immunoglobulin heavy chain gene rearrangement after vaccination proved to be highly variable in ASCT patients. Changes in TCRα and TCRβ gene rearrangement after vaccination differed from patterns observed in healthy vaccinees. Crucially, ASCT patients elicited comparable proportions of SARS-CoV-2 vaccine-induced (VI) CD8 +  T-cells, characterized by a distinct gene expression pattern that is associated with SARS-CoV-2 specificity in healthy individuals. Our study underlines the impaired immune system and thus the lower vaccine response rates in ASCT patients. However, since protective vaccine responses and VI CD8 +  T-cells can be induced in part of ASCT patients, our data advocate early posttransplant vaccination due to the high risk of infection in this vulnerable group., (© 2024 The Authors. Abstract

Advanced age is a main risk factor for severe COVID-19. However, low vaccination efficacy and accelerated waning immunity have been reported in this age group. To elucidate age-related differences in immunogenicity, we analyzed human cellular, serological, and salivary SARS-CoV-2 spike glycoprotein-specific immune responses to the BNT162b2 COVID-19 vaccine in old (69-92 y) and middle-aged (24-57 y) vaccinees compared with natural infection (COVID-19 convalescents, 21-55 y of age). Serological humoral responses to vaccination excee-ded those of convalescents, but salivary anti-spike subunit 1 (S1) IgA and neutralizing capacity were less durable in vaccinees. In old vaccinees, we observed that pre-existing spike-specific CD4 + T cells are associated with efficient induction of anti-S1 IgG and neutralizing capacity in serum but not saliva. Our results suggest pre-existing SARS-CoV-2 cross-reactive CD4 + T cells as a predictor of an efficient COVID-19 vaccine-induced humoral immune response in old individuals., (Copyright © 2022 by The American Association of Immunologists, Inc.)
