- Abstract
Among commonly used anesthetics, ketamine obviously occupies an unusual position. While most anesthetics exert a retarding effect on circulation and metabolism, ketamine clearly has the opposite effect. Figure 1 shows the synopsis of the most important circulatory characteristics after a representative individual test. The injection of ketamine in the animal test causes an increases in heart frequency (HF), HZV (lower curve), coronary blood supply (Vcor) and the myocardial 02 consumption, together with a reduction in coronary venous saturation (upper falling curve). The pressures and the rate of decrease are higher. Eight mongrel dogs weighing between 19 and 27 kg were pre-medicated with 1 mg of scopalamine and 30 minutes later received intravenously on average 20 mg/kg body weight of ketamine which produced in them a condition similar to narcosis. The behavior of PCO2, pH bases excess and the average arterial pressure, heart beat and the central venous pressure during ketamine is discussed. The change in heart beat and the plasma concentration of noradrenaline, adrenaline, glucose and free fatty acids during ketamine narcosis is reported. 11 páginas, 4 figuras, 1 tabla.-- CDF Collaboration: et al., We combine searches by the CDF and D0 Collaborations for a Higgs boson decaying to W+W-. The data correspond to an integrated total luminosity of 4.8 (CDF) and 5.4 (D0) fb(-1) of p (p) over bar collisions at root s = 1.96 TeV at the Fermilab Tevatron collider. No excess is observed above background expectation, and resulting limits on Higgs boson production exclude a standard model Higgs boson in the mass range 162-166 GeV at the 95% C.L., We thank the Fermilab staff and the technical staffs of the participating institutions for their vital contributions. - Abstract
Aim: The sodium/hydrogen exchanger 2 (NHE2) is an intestinal acid extruder with crypt-predominant localization and unresolved physiological significance. Our aim was to decipher its role in colonic epithelial cell proliferation, differentiation and electrolyte transport., Methods: Alterations induced by NHE2-deficiency were addressed in murine nhe2
-/- and nhe2+/+ colonic crypts and colonoids, and NHE2-knockdown and control Caco2Bbe cells using pH-fluorometry, gene expression analysis and immunofluorescence., Results: pHi -measurements along the colonic cryptal axis revealed significantly decreased intracellular pH (pHi ) in the middle segment of nhe2-/- compared to nhe2+/+ crypts. Increased Nhe2 mRNA expression was detected in murine colonoids in the transiently amplifying/progenitor cell stage (TA/PE). Lack of Nhe2 altered the differentiation programme of colonic epithelial cells with reduced expression of absorptive lineage markers alkaline phosphatase (iAlp), Slc26a3 and transcription factor hairy and enhancer-of-split 1 (Hes1), but increased expression of secretory lineage markers Mucin 2, trefoil factor 3 (Tff3), enteroendocrine marker chromogranin A and murine atonal homolog 1 (Math1). Enterocyte differentiation was found to be pHi dependent with acidic pHi reducing, and alkaline pHi stimulating the expression of enterocyte differentiation markers in Caco2Bbe cells. A thicker mucus layer, longer crypts and an expanded brush border membrane zone of sodium/hydrogen exchanger 3 (NHE3) abundance may explain the lack of inflammation and the normal fluid absorptive rate in nhe2-/- colon., Conclusions: The results suggest that NHE2 expression is activated when colonocytes emerge from the stem cell niche. Its activity increases progenitor cell pHi and thereby supports absorptive enterocyte differentiation., (© 2022 The Authors. - Abstract
Upon viral infection, natural killer (NK) cells expressing certain germline-encoded receptors are selected, expanded, and maintained in an adaptive-like manner. Currently, these are thought to differentiate along a common pathway. However, by fate mapping of single NK cells upon murine cytomegalovirus (MCMV) infection, we identified two distinct NK cell lineages that contributed to adaptive-like responses. One was equivalent to conventional NK (cNK) cells while the other was transcriptionally similar to type 1 innate lymphoid cells (ILC1s). ILC1-like NK cells showed splenic residency and strong cytokine production but also recognized and killed MCMV-infected cells, guided by activating receptor Ly49H. Moreover, they induced clustering of conventional type 1 dendritic cells and facilitated antigen-specific T cell priming early during MCMV infection, which depended on Ly49H and the NK cell-intrinsic expression of transcription factor Batf3. - Abstract
Chronic cytomegalovirus (CMV) infection leads to long-term maintenance of extraordinarily large CMV-specific T cell populations. The magnitude of this so-called 'memory inflation' is thought to mainly depend on antigenic stimulation during the chronic phase of infection. However, by mapping the long-term development of CD8
+ T cell families derived from single naive precursors, we find that fate decisions made during the acute phase of murine CMV infection can alter the level of memory inflation by more than 1,000-fold. Counterintuitively, a T cell family's capacity for memory inflation is not determined by its initial expansion. Instead, those rare T cell families that dominate the chronic phase of infection show an early transcriptomic signature akin to that of established T central memory cells. Accordingly, a T cell family's long-term dominance is best predicted by its early content of T central memory precursors, which later serve as a stem-cell-like source for memory inflation.- Published

- Abstract
A core feature of protective T cell responses to infection is the robust expansion and diversification of naïve antigen-specific T cell populations into short-lived effector and long-lived memory subsets. By means of in vivo fate mapping, we found a striking variability of immune responses derived from individual CD8(+) T cells and show that robust acute and recall immunity requires the initial recruitment of multiple precursors. Unbiased mathematical modeling identifies the random integration of multiple differentiation and division events as the driving force behind this variability. Within this probabilistic framework, cell fate is specified along a linear developmental path that progresses from slowly proliferating long-lived to rapidly expanding short-lived subsets. These data provide insights into how complex biological systems implement stochastic processes to guarantee robust outcomes.

- Abstract
Background: The World Health Organization (WHO) has recommended guidelines for a HIV drug resistance (HIVDR) survey for resource-limited countries. Eligibility criteria for patients include age below 25 years in order to focus on the prevalence of transmitted HIVDR (tHIVDR) in newly-infected individuals. Most of the participating sites across Africa have so far reported tHIVDR prevalences of below 5%. In this study we investigated whether the rate of HIVDR in patients <25 years is representative for HIVDR in the rest of the therapy-naïve population., Methods and Findings: HIVDR was determined in 88 sequentially enrolled ART-naïve patients from Mwanza, Tanzania (mean age 35.4 years). Twenty patients were aged <25 years and 68 patients were aged 25-63 years. The frequency of HIVDR in the study population was 14.8% (95%; CI 0.072-0.223) and independent of NVP-resistance induced by prevention of mother-to-child transmission programs. Patients >25 years had a significantly higher HIVDR frequency than younger patients (19.1%; 95% CI 0.095-0.28) versus 0%, P = 0.0344). In 2 out of the 16 patients with HIVDR we found traces of antiretrovirals (ARVs) in plasma., Conclusions: ART-naïve patients aged over 25 years exhibited significantly higher HIVDR than younger patients. Detection of traces of ARVs in individuals with HIVDR suggests that besides transmission, undisclosed misuse of ARVs may constitute a significant factor in the generation of the observed high HIVDR rate. The current WHO tHIVDR survey that is solely focused on the transmission of HIVDR and that excludes patients over 25 years of age may therefore result in substantial underestimation of the prevalence of HIVDR in the therapy-naïve population. Similar studies should be performed also in other areas to test whether the so far reported optimistic picture of low HIVDR prevalence in young individuals is really representative for the rest of the ART-naïve HIV-infected population.

- Abstract
A simple method for determination of the functional residual capacity (FRC) during artificial ventilation by inert gas dilution using a rebreathing technique is described. The measurements can be made during various techniques of mechanical ventilation (eg. PEEP, IMV etc.). Myorelaxation is not required. This technique was used to assess the volume of a lung model (actual volume: 3670 ml) with the result of repeated measures being 3686 +/- 29 ml. Duplicate measurements in artificially ventilated pigs showed a coefficient of variation of +/- 3.2%. In artificially ventilated patients FRC was determined both with this technique and with a reference method, giving a difference of 111 +/- 55 ml (p less than 0.001). The errors in measurements of FRC by this method are small, and as the measurement requires a minimum of time and the apparatus is simple, the method is accurate and yet simple enough to be used on a routine basis in the intensive care unit. Even high risk patients are not disturbed by this technique. The method has the advantage that it can easily be learned by nurses and paramedical personal.

- Abstract
Pulmonary diffusing capacity may be determined by rebreathing techniques using various test gases. The errors caused by functional inhomogenities are smaller with the rebreathing method than those found in the single-breath technique. However, until now there have been some disadvantages to the clinical application of the rebreathing technique. In the present paper, a modification of the rebreathing technique suggested by Piiper, Scheid, Meyer and Adaro (1975 and 1976), in which pulmonary diffusing capacity for carbon monoxide, DCO(RB), is measured, has been applied to intubated and anesthetised patients. Measurements were performed in 39 surgical patients with healthy lungs during artificial ventilation and narcosis. Under these conditions all patients had a DCO(RB) greater than or equal to 75 ml/min . kPa (10 ml/min.torr). The coefficient of variation for duplicate measurements was +/- 4.2%. In the same patients DCO-single-breath-measurements, DCO(SB), were also made. There was a significant (p less than 0.0001) correlation between DCO(RB) in artificially ventilated, anaesthetised patients in a supine position and DCO(SB) for spontaneously breathing, awake patients in a sitting position. A linear regression analysis of the data showed the DCO(SB) was related to DCO(RB) in the following manner: DCO(SB) = 1.62DCO(RB) = 4.44 (ml/min.torr) (r = 0.71, n = 39). The mean value of DCO(RB) was 53.4% of the mean value of DCO(SB). This difference may be due to the combination of different techniques used and different conditions during the measurements (reduced FRC, difference in F1O2 etc.). The histogramm of the distribution of DCO(RB) had a similar form to that of DCO(SB). The modified method has a high reproducibility and can easily be performed as a bed-side test without disturbance to patients with an endotracheal tube.
