Abstract

A mathematical model of the process of carbon capture in a packed column by adsorption is developed and analysed. First a detailed study is made of the governing equations. Due to the complexity of the internal geometry it is standard practice to average these equations. Here the averaging process is revisited. This shows that there exists a number of errors and some confusion in the standard systems studied in the literature. These errors affect the parameter estimation, with consequences when the experimental set-up is modified or scaled-up. Assuming, as a first approximation, an isothermal model the gas concentration equation is solved numerically. Excellent agreement with data from a pressure swing adsorption experiment is demonstrated. A new analytical solution (valid away from the inlet) is obtained. This provides explicit relations for quantities such as the amount of adsorbed gas, time of first breakthrough, total process time and width and speed of the reaction zone, showing how these depend on the operating conditions and material parameters. The relations show clearly how to optimise the carbon capture process. By comparison with experimental data the analytical solution may also be used to calculate unknown system parameters.

Abstract

The heat flux across a nanowire is computed based on the Guyer-Krumhansl equation. Slip conditions with a slip length depending on both temperature and nanowire radius are introduced at the outer boundary. An explicit expression for the effective thermal conductivity is derived and compared to existing models across a given temperature range, providing excellent agreement with experimental data for Si nanowires.

Abstract

Objective: We analyzed the pooled case-control data from the International Head and Neck Cancer Epidemiology (INHANCE) consortium to compare cigarette smoking and alcohol consumption risk factors for head and neck cancer between less developed and more developed countries. Subjects and Methods: The location of each study was categorized as either a less developed or more developed country. We compared the risk of overall head and neck cancer and cancer of specific anatomic subsites associated with cigarette smoking and alcohol consumption. Additionally, age and sex distribution between categories was compared. Results: The odds ratios for head and neck cancer sites associated with smoking duration differed between less developed and more developed countries. Smoking greater than 20 years conferred a higher risk for oral cavity and laryngeal cancer in more developed countries, whereas the risk was greater for oropharynx and hypopharynx cancer in less developed countries. Alcohol consumed for more than 20 years conferred a higher risk for oropharynx, hypopharynx, and larynx cancer in less developed countries. The proportion of cases that were young (<45 years) or female differed by country type for some HNC subsites. Conclusion: These findings suggest the degree of industrialization and economic development affects the relationship between smoking and alcohol with head and neck cancer.

Abstract

We present direct upper limits on gravitational wave emission from the Crab pulsar using data from the first nine months of the fifth science run of the Laser Interferometer Gravitational-wave Observatory (LIGO). These limits are based on two searches. In the first we assume that the gravitational wave emission follows the observed radio timing, giving an upper limit on gravitational wave emission that beats indirect limits inferred from the spin-down and braking index of the pulsar and the energetics of the nebula. In the second we allow for a small mismatch between the gravitational and radio signal frequencies and interpret our results in the context of two possible gravitational wave emission mechanisms., Comment: Accepted for Ap. Abstract

Objectives: Ponatinib, a potent third-generation tyrosine kinase inhibitor (TKI), is approved for adults with chronic/accelerated/blast-phase chronic myeloid leukemia or Philadelphia chromosome positive (Ph+) acute lymphoblastic leukemia (ALL) whose disease is resistant/intolerant to other TKIs or has the T315I mutation. This study in pediatric patients with relapsed/resistant/intolerant Ph+ ALL or those with the T315I mutation aims to establish the recommended Phase 2 dose (RP2D) of ponatinib and assess the pharmacokinetics, safety, and efficacy of ponatinib in combination with chemotherapy. Methods: This Phase 1/2, single-arm, open-label, multicenter study is enrolling patients age ≥1–21 years with Ph+ ALL, Ph+ mixed-phenotype acute leukemia, or Ph-like ALL (United States only) who have a targetable kinase-activating lesion and whose disease has relapsed or is resistant/intolerant to ≥1 prior therapy with a second-generation BCR::ABL1–targeted TKI or has a BCR::ABL1 T315I mutation. Phase 1 uses a rolling 6 design; the first cohort will include up to 12 patients (≥30 kg and able to swallow tablets) for dose confirmation. Another cohort of 6 patients will be enrolled to assess the age-appropriate mini-tablet formulation at the RP2D. Phase 2 will enroll ≈68 patients, including those enrolled in Phase 1 at the RP2D. Patients must have an age-appropriate (Lansky or Karnofsky) Performance Scale score of ≥50%. In Phase 1, the primary endpoint is the RP2D of ponatinib in combination with chemotherapy. Secondary endpoints are complete response (CR) rate at the end of the reinduction block and characterization of BCR::ABL1 domain mutations before and after ponatinib treatment. In Phase 2, the primary endpoint is the CR rate at the end of the reinduction block. Secondary endpoints include the proportion of patients in continued CR or who achieve CR at the end of consolidation, event-free survival (EFS), progression-free survival, and overall survival. The proportion of cases that were young (<45 years) or female differed by country type for some HNC subsites. Conclusion These findings suggest the degree of industrialization and economic development affects the relationship between smoking and alcohol with head and neck cancer.

Abstract

An assessment of verbal memory functioning must address a number of issues. A thorough and comprehensive evaluation needs to account for new and important models of memory function and dysfunction, and the significant implications which these may have for diagnosis and rehabilitation. Behaviourally based cognitive information must be integrated with neuropsychological data which provide neuroanatomical correlates for particular memory processes. Memory functioning is a diverse and complex phenomenon which can be accurately and comprehensively assessed, only when cognitive and neuropsychological perspectives are combined. Cognitive theories have had a significant influence on the clinical assessment of verbal memory, and the development of specific neuropsychological procedures. However, often clinical evaluations have tended to lag behind theoretical and experimental advances. In the following review, current cognitive and neuropsychological models of memory function will be evaluated, with an emphasis on the most clinically useful developments dealing with the structure of short-term or working memory, the efficiency of acquisition, storage and retrieval processes, along with the fractionation of long-term memory. A number of commonly used neuropsychological procedures will be reviewed, and an evaluation made of their psychometric reliability and validity, including their ability to differentiate both the nature and severity of memory dysfunction. The tests to be considered are the National Adult Reading Test (NART), the verbal subtests of the Wechsler Memory Scale (WMS), the Rey Auditory Verbal Learning Test (RAVLT), and the Bushke Selective Reminding Test (BSRT).

Abstract

Objective: Retrograde open mesenteric stenting (ROMS) is an alternative to mesenteric bypass/antegrade angioplasty and stenting in patients with acute and chronic mesenteric ischemia. We present a case and literature review utilizing ROMS in a patient with severe vascular occlusive disease to treat her superior mesenteric artery (SMA) stenosis. Case report: A 63-year-old woman with a history of severe vascular occlusive disease and previous extensive surgical interventions required ROMS due to SMA stenosis for acute on chronic mesenteric ischemia. There were no in-hospital complications. We also performed a review of the literature to assess the technical success and clinical outcomes of ROMS. Results: A total of six articles with 210 patients (92 males, 118 females) were included. In the studies where the celiac artery and SMA were implicated, technical success was 92.9%, conversion to bypass was 4.3%, and 30-day mortality was 29.5%. In the studies where only the SMA was implicated, technical success was 90.7%, conversion to bypass was 7.2%, and 30-day mortality was 23.7%. In cases of acute mesenteric ischemia, the 30-day mortality rate was 34.2%. Conclusion: Though open surgical bypass and antegrade endovascular angioplasty and stenting have been the first-line treatment options for mesenteric ischemia, ROMS is an excellent alternative option with a high technical success rate that can be utilized when conventional treatment modalities are not technically feasible or in the setting of concomitant open abdominal exploration for the assessment of bowel ischemia.

Abstract

Background The complexity of oesophageal surgery and the significant risk of morbidity necessitates that oesophagectomy is predominantly performed by a consultant surgeon, or a senior trainee under their supervision. The aim of this study was to determine the impact of trainee involvement in oesophagectomy on postoperative outcomes in an international multicentre setting. Methods Data from the multicentre Oesophago-Gastric Anastomosis Study Group (OGAA) cohort study were analysed, which comprised prospectively collected data from patients undergoing oesophagectomy for oesophageal cancer between April 2018 and December 2018. Procedures were grouped by the level of trainee involvement, and univariable and multivariable analyses were performed to compare patient outcomes across groups. Results Of 2232 oesophagectomies from 137 centres in 41 countries, trainees were involved in 29.1 per cent of them (n = 650), performing only the abdominal phase in 230, only the chest and/or neck phases in 130, and all phases in 315 procedures. For procedures with a chest anastomosis, those with trainee involvement had similar 90-day mortality, complication and reoperation rates to consultant-performed oesophagectomies (P = 0.451, P = 0.318, and P = 0.382, respectively), while anastomotic leak rates were significantly lower in the trainee groups (P = 0.030). Procedures with a neck anastomosis had equivalent complication, anastomotic leak, and reoperation rates (P = 0.150, P = 0.430, and P = 0.632, respectively) in trainee-involved versus consultant-performed oesophagectomies, with significantly lower 90-day mortality in the trainee groups (P = 0.005). Conclusion Trainee involvement was not found to be associated with significantly inferior postoperative outcomes for selected patients undergoing oesophagectomy. The results support continued supervised trainee involvement in oesophageal cancer surgery., The aim of this study was to determine the impact of trainee involvement in oesophagectomy on perioperative outcomes in the international multicentre Oesophago-Gastric Anastomosis Audit (OGAA). Analysis of 2232 oesophagectomies has shown that trainee involvement did not negatively impact perioperative outcomes.

Abstract

Background Textbook outcome has been proposed as a tool for the assessment of oncological surgical care. However, an international assessment in patients undergoing oesophagectomy for oesophageal cancer has not been reported. This study aimed to assess textbook outcome in an international setting. Methods Patients undergoing curative resection for oesophageal cancer were identified from the international Oesophagogastric Anastomosis Audit (OGAA) from April 2018 to December 2018. Textbook outcome was defined as the percentage of patients who underwent a complete tumour resection with at least 15 lymph nodes in the resected specimen and an uneventful postoperative course, without hospital readmission. A multivariable binary logistic regression model was used to identify factors independently associated with textbook outcome, and results are presented as odds ratio (OR) and 95 per cent confidence intervals (95 per cent c.i.). Results Of 2159 patients with oesophageal cancer, 39.7 per cent achieved a textbook outcome. The outcome parameter ‘no major postoperative complication’ had the greatest negative impact on a textbook outcome for patients with oesophageal cancer, compared to other textbook outcome parameters. Multivariable analysis identified male gender and increasing Charlson comorbidity index with a significantly lower likelihood of textbook outcome. Presence of 24-hour on-call rota for oesophageal surgeons (OR 2.05, 95 per cent c.i. 1.30 to 3.22; P = 0.002) and radiology (OR 1.54, 95 per cent c.i. 1.05 to 2.24; P = 0.027), total minimally invasive oesophagectomies (OR 1.63, 95 per cent c.i. 1.27 to 2.08; P < 0.001), and chest anastomosis above azygous (OR 2.17, 95 per cent c.i. 1.58 to 2.98; P < 0.001) were independently associated with a significantly increased likelihood of textbook outcome. Conclusion Textbook outcome is achieved in less than 40 per cent of patients having oesophagectomy for cancer. Improvements in centralization, hospital resources, access to minimal access surgery, and adoption of newer techniques for improving lymph node yield could improve textbook outcome.

Abstract

Objective: We analyzed the pooled case-control data from the International Head and Neck Cancer Epidemiology (INHANCE) consortium to compare cigarette smoking and alcohol consumption risk factors for head and neck cancer between less developed and more developed countries. Subjects and Methods: The location of each study was categorized as either a less developed or more developed country. We compared the risk of overall head and neck cancer and cancer of specific anatomic subsites associated with cigarette smoking and alcohol consumption. Additionally, age and sex distribution between categories was compared. Results: The odds ratios for head and neck cancer sites associated with smoking duration differed between less developed and more developed countries. Smoking greater than 20 years conferred a higher risk for oral cavity and laryngeal cancer in more developed countries, whereas the risk was greater for oropharynx and hypopharynx cancer in less developed countries. Abstract

The exotic internal structure of polar topologies in multi-ferroic materials offers a rich landscape for materials science research. As the spatial scale of these entities are often sub-atomic in nature, aberration corrected transmission electron microscopy (TEM) is the ideal characterisation technique. Software to quantify and visualise the slight shifts in atomic placement within unit cells is of paramount importance due to the now routine acquisition of images at such resolution. In the previous ~decade since the commercialisation of aberration corrected TEM, many research groups have written their own code to visualise these polar entities. More recently, open access Python packages have been developed for the purpose of TEM atomic position quantification. Building on these packages, we introduce the TEMUL Toolkit: a Python package for analysis and visualisation of atomic resolution images. Here, we focus specifically on the TopoTEM module of the toolkit where we show an easy to follow, streamlined version of calculating the atomic displacements relative to the surrounding lattice and thus polarisation plotting. We hope this toolkit will benefit the rapidly expanding field of topology based nano-electronic and quantum materials research, and we invite the electron microscopy community to contribute to this open access project.
