Abstract

Background: Coronavirus 2019 (COVID-19) is a global pandemic with various clinical manifestations and is affected by multifactor. Epidemiological data of COVID-19 in Indonesia, especially in Surabaya have not been well established yet. Purpose: This study aims to provide the COVID-19 patients profile in Surabaya City, Indonesia. Method: The study data were retrospectively collected from electrical medical records in Primasatya Husada Citra (PHC) Hospital of Surabaya, one of the referral hospitals for COVID-19 in Surabaya. Descriptive and Spearman correlation statistics were done for data analysis. Results: Between 1 July – 31 August 2020, 456 subjects were diagnosed positive for COVID-19 by real-time polymerase chain reaction and enrolled in the study. Study subjects were dominated by the group age of 21-30 (26.75%), male (60.30%), living in East Surabaya (16.22%), and mild to moderate disease severity (47.59%) according to the patient's clinical manifestation, respiration rate, and peripheral oxygen saturation. Subjects with symptoms were majorly having cough (37.93%), fever (29.38%), malaise (28.07%), and dyspnoea (25.00%). Positive correlation was found between disease severity and group of age (r=0.35; p=0.01), gender (r = 0.13; p=0.01), comorbidities (diabetes mellitus, hypertension, and cardiovascular disease), and X-ray findings (r=0.14; p=0.02). A negative correlation was found between disease severity and the region of domicile (r=-0.15; p=0.01). Conclusion: This epidemiological data of COVID-19 patients in Surabaya city, Indonesia, may help in diagnosing COVID-19 patients and improving health management strategies during a pandemic. Patients need to be aware of COVID-19 and understand disease transmission, especially with symptoms and risk factors.

13. Immunotolerance regeneration by T regulatory cells in autologous haematopoietic stem cell transplantation for multiple sclerosis

Abstract

The breakdown of immunological self-tolerance manifests clinically as autoimmune diseases, such as Multiple sclerosis (MS), an autoimmune disease of the central nervous system (CNS). Autologous haematopoietic stem cell transplantation (AHSCT) has received increasing attention in recent years as a therapeutic option for treatment refractory MS. AHSCT has been proposed to induce long-term remission by initiating an immune system “reboot” via the chemotherapy-driven ablation of autoreactive immune cells and the regeneration of immunoregulatory lymphocytes, such as CD4+ T regulatory cells (Tregs). Tregs expressing the ectonucleotidase enzyme CD39 exert immunoregulatory function by suppressing T helper (Th)17-related immune responses, which has been significantly associated with MS pathogenesis. However, this immunoregulatory function was reported to be decreased in MS patients. CD39 expression, and its corresponding immunological function, on Tregs is determined by the genetic polymorphism of the CD39 gene (at position rs_10748643), where its expression may be low (AA-genotype), medium (AG-genotype) or high (GG-genotype). Hence, CD39 genetic polymorphism may significantly determine CD39+ Treg function and reconstitution in MS patients and/or influence the immunological shift in patients following AHSCT. Three novel aspects regarding Treg-associated tolerance regeneration in MS patients following AHSCT are presented in this thesis: 1) although there is no direct clinical correlation, this is the first study investigating the role of CD39 genetic polymorphism in immune reconstitution, cytokine evolution, and Treg function following AHSCT in MS patients (Chapters 3, 4, and 6); 2) significant and long-term reductions in Th17- and Th1-related cytokines post-AHSCT further support the ongoing hypothesis that AHSCT induces an anti-inflammatory shift in MS patients (Chapter 4), and 3) to the best of this author’s knowledge, this thesis was the first to report that MS patients without clinical relapse post-AHSCT exhibit significant improvements in CD4+ Treg suppressive function at 24 months post-AHSCT (Chapter 6) and consistent suppression of Th17 cytokines at all timepoints up to 24 months post-AHSCT (Chapter 4). Future studies must address the exact immunoregulatory pathway that is regenerated in Tregs following AHSCT in MS patients.

Abstract

Background: Autologous haematopoietic stem cell transplantation (AHSCT) has been explored as a therapeutic intervention in multiple sclerosis (MS) over the last two decades; however, prospective clinical trials of the most common myeloablative conditioning regimen, BEAM, are limited. Furthermore, patient selection, optimal chemotherapeutic regimen and immunological changes associated with disease response require ongoing exploration. We present the outcomes, safety and immune reconstitution (IR) of patients with active, treatment refractory MS.Methods: This study was a single-centre, phase II clinical trial of AHSCT for patients with active relapsing remitting (RRMS) and secondary progressive MS (SPMS). Patients underwent AHSCT using BEAM (carmustine, etoposide, cytarabine, melphalan)+antithymocyte globulin chemotherapeutic regimen.Outcomes: The primary outcome was event-free survival (EFS); defined as no clinical or radiological relapses and no disability progression. Multiparameter flow cytometry was performed for evaluation of post-transplant IR in both MS and lymphoma patients receiving the same chemotherapy regimen.Results: Thirty-five patients (20 RRMS, 15 SPMS) completed AHSCT, with a median follow-up of 36 months (range 12-66). The median Expanded Disability Status Scores (EDSS) was 6 (2-7) and patients had failed a median of 4 (2-7) disease modifying therapies. 66% failed treatment with natalizumab. EFS at 3 years was 60%, (70% RRMS). Sustained improvement in EDSS was seen in 15 (44%) of patients. There was no treatment-related mortality. A sustained rise in CD39+ T regulatory cells, immunosuppressive CD56hi natural killer cells and ablation of proinflammatory mucosal-associated invariant T cells was seen for 12 months following AHSCT in patients with MS. These changes did not occur in patients with lymphoma receiving the same chemotherapy for AHSCT.Conclusions: The EFS in our MS cohort is significantly greater than other high-efficacy immunosuppressive therapies and similar to other AHSCT studies despite a more heavily pretreated cohort.Trial Registration Number: ACTRN12613000339752.
