This observational cohort study investigates treatment adherence and real-life clinical outcomes within the first year of treatment.Patients and methods: A total of 708 treatment-naïve patients (466 nAMD, 134 DME, and 108 BRVO) were included. Patients were followed with a PRN treatment protocol with three intravitreal injections (IVIs) and a series of 3 monthly injections in case of persistent or recurrent disease activity, as determined by monthly follow-up exams including optical coherence tomographies. Occurrence of gaps of >56 days between treatments or follow-up (nonadherence [NA]) and the reasons for NA (patient- or center-associated) as well as disease activity within the first 12 months of treatment were analyzed. Visual acuity (VA) as well as numbers and dates of optical coherence tomography and IVI were extracted from medical records.Results: NA occurred significantly more often in patients with DME (44%) than nAMD (32%) or BRVO (25%, p0.05).Conclusion: NA to treatment and follow-up regimens is a common problem in the management of patients with AMD and DME and limits clinical treatment outcomes under real-life conditions. Abstract

Formal verification (FV) is considered by many to be complicated and to require considerable mathematical knowledge for successful application. We have developed a methodology in which we have added formal verification to the verification process without requiring any knowledge of formal verification languages. We use only finite-state machine notation, which is familiar and intuitive to designers. Another problem associated with formal verification is state-space explosion. If that occurs, no result is returned; our method switches to random simulation after one hour without results, and no effort is lost. We have compared FV against random simulation with respect to development time, and our results indicate that FV is at least as fast as random simulation. FV is superior in terms of verification quality, however, because it is exhaustive.

Abstract

BACKGROUND: Clostridium difficile is the most common cause of infectious diarrhea in hospitalized patients. Recurrences are common after antibiotic therapy. Actoxumab and bezlotoxumab are human monoclonal antibodies against C. difficile toxins A and B, respectively.METHODS: We conducted two double-blind, randomized, placebo-controlled, phase 3 trials, MODIFY I and MODIFY II, involving 2655 adults receiving oral standard-of-care antibiotics for primary or recurrent C. difficile infection. Participants received an infusion of bezlotoxumab (10 mg per kilogram of body weight), actoxumab plus bezlotoxumab (10 mg per kilogram each), or placebo; actoxumab alone (10 mg per kilogram) was given in MODIFY I but discontinued after a planned interim analysis. The primary end point was recurrent infection (new episode after initial clinical cure) within 12 weeks after infusion in the modified intention-to-treat population.RESULTS: In both trials, the rate of recurrent C. difficile infection was significantly lower with bezlotoxumab alone than with placebo (MODIFY I: 17% [67 of 386] vs. 28% [109 of 395]; adjusted difference, -10.1 percentage points; 95% confidence interval [CI], -15.9 to -4.3; PCONCLUSIONS: Among participants receiving antibiotic treatment for primary or recurrent C. difficile infection, bezlotoxumab was associated with a substantially lower rate of recurrent infection than placebo and had a safety profile similar to that of placebo. Abstract

Vermicomposting, or using worms along with bacteria and fungi to recycle agricultural and organic wastes into nutrient-rich bio-fertilizer, has a variety of uses including protecting plants from disease. Through semi-structured in-person interviews, vermicompost practitioners across Kermanshah Province of Iran were surveyed to determine the strengths, weaknesses, opportunities, and threats (SWOT) of this emerging technology. The results provide useful implications for agricultural policymakers in general, and, in particular, for farmers who are seeking diversified sources of income. For both vermicompost practitioners and academicians alike, the SWOT analysis methodology combined with Analytic Network Process (ANP) analysis has implications for other types of agribusiness. Abstract

Since 1986, the library faculty of the McGoogan Library of Medicine at the University of Nebraska Medical Center (UNMC) has participated in small group activities during the week-long orientation for first-year medical students. This involvement paved the way for library faculty members to act as facilitators for small groups of medical students within the new problem-based learning (PBL) curriculum introduced in 1992 by the College of Medicine. The UNMC curriculum consists of traditional PBL groups as well as Integrated Clinical Experience (ICE) small groups. The ICE groups provide opportunities for discussion of the social and behavioral issues that arise in medicine, with the majority of the sessions designed to give students interviewing practice with simulated patients. The ICE small groups meet once a week with either one or two facilitators. Several library faculty members act as facilitators for ICE groups. As a result of this involvement, librarian contacts with College of Medicine faculty have grown in number and depth, there has been a corresponding increase in related activities with the first- and second-year medical students. Participation in ICE groups has caused some difficulties with respect to library work schedules, but it has been immensely rewarding and enriching in terms of professional growth. This paper describes the UNMC curriculum, the evolution and extent of the librarians' involvement, and the future involvement, ramifications, and challenges envisioned for McGoogan faculty and their medical library colleagues.
