In the medical care of partial and full-thickness wounds, autologous skin grafting is still the gold standard of dermal replacement. In contrast to spontaneous reepithelializing of superficial wounds, deep dermal wounds often lead to disturbing scarring, with cosmetically or functionally unsatisfactory results. However, modern wound dressings offer promising approaches to surface reconstruction. Against the background of our future aim to develop an innovative skin substitute, we investigated the behavior of two established dermal substitutes, a crosslinked and a non-crosslinked collagen biomatrix. The products were applied topically on a total of 18 full-thickness skin defects paravertebrally on the back of female Göttingen Minipigs—six control wounds remained untreated. The evaluation was carried out planimetrically (wound closure time) and histologically (neoepidermal cell number and epidermis thickness). Both treatment groups demonstrated significantly faster reepithelialization than the controls. The histologic examination verified the highest epidermal thickness in the crosslinked biomatrix-treated wounds, whereas the non-crosslinked biomatrix-treated wounds showed a higher cell density. Our data presented a positive influence on epidermal regeneration with the chosen dermis substitutes even without additional skin transplantation and, thus, without additional donor site morbidity. Therefore, it can be stated that the single biomatrix application might be used in a clinical routine with small wounds, which needs to be investigated further in a clinical setting to determine the size and depths of a suitable wound bed. Nevertheless, currently available products cannot solely achieve wound healing that is equal to or superior to autologous tissue. Thus, the overarching aim still is the development of an innovative skin substitute to manage surface reconstruction without additional skin grafting.

Este trabajo se analizó el comportamiento térmico de las envolventes de edificios en el Nordeste Argentino (NEA), cuyos sistemas constructivos emplean materiales prefabricados elaborados en serie y mano de obra especializada, atendiendo particularmente al impacto de los puentes tér micos. El objetivo fue diagnosticar y desarrollar criterios de optimización del desempeño ener gético de estas envolventes. Se estudiaron, para ello, los tipos y subtipos de construcciones no conven cionales y se seleccionaron muestras de aquellos más empleados en el NEA. Éstas fueron evaluadas con los procedimientos de cálculo establecidos por las nor mas IRAM, de la serie 11.600, así como también mediante simulac iones realizadas con el software THERM. Los valores que pudieron obtenerse con ambos procedimientos presentaron discrepancias promedio del 2,6%. Con el software es posible obtener un coeficiente ponderado de transmitancia que incorpora muro opaco y puente tér mico y, con la graficación del flujo de calor a través del cerramiento, reconocer fácilmente los puntos más críticos, para concentrar la s acciones de optimización en ellos. This research analyzed the thermal performance of building envelopes in Northeast Argentina (NEA), whose construction systems use mass-produced, prefabricated materials and specialized workers. Particular attention was paid to the impact of ther mal bridges. The objective was to diagnose and develop criteria to optimize the energy performance of these envelopes. To this end, the types and subtypes of non-conventional buildings were studied and samples of those most used in NEA were selected. These were evaluated using the calculation procedures established by the 11,600 series of the IRAM standar ds, as well as by simulations perfor med with THERM software. The values that were obtained with both procedures presented average discrepancies of 2.6%. W ith the software it is possible to obtain a weighted coefficient of transmittance that incorporates opaque walls and thermal bridges and, by graphing the heat flow through the element, easily recognize the most critical points, to thus concentrate optimiza tion actions. A 61-year old male was presented at our Burn Centre with a severe burn injury affecting 95% TBSA second to third degree. On admission, a mechanical debridement, an escharotomy and a tracheotomy were performed. Due to acute respiratory and cardiac failure, no surgical intervention could be performed in the next few days. Thus, a fractional use of enzymatic debridement with Nexobrid was performed to a TBSA of 54% starting on Day 5: Day 5: upper limbs (20% TBSA); Day 7: lower legs and feet (17% TBSA); Day 8: upper legs (17% TBSA). Despite successful enzymatic debridement, the patient died on Day 8 due to acute lung and heart failure.This Case report reveals that a delayed and fractional application of Nexobrid to a TBSA of more than 15% is possible.
