Abstract

Background: The Swedish National Quality Registries (NQRs) contain individual-level health care data for specific patient populations, or patients receiving specific interventions. Approximately 90% of the 105 Swedish NQRs include any patient-reported outcome measure, with EQ-5D being the most common. As there has been no general overview of EQ-5D data within the NQRs, this study fills a knowledge gap by reporting how the data are collected, presented, and used at different levels of the Swedish health care system. Abstract

Background. The relationship among the presence of human papillomavirus (HPV) in tumor cells, DNA ploidy, and the prognosis of squamous cell carcinoma of the uterine cervix was studied. Methods. HPV 16 was detected using the polymerase chain reaction on paraffin-embedded material from 69 patients with Stage IB and IIA carcinoma of the uterine cervix. The presence or absence of HPV was related to age, survival, and ploidy status as measured by DNA flow cytometry. All patients were treated by radical surgery. Results. Thirty-four patients had HPV 16, and 35 did not. The mean age of the patients differed statistically significantly between the HPV-positive group (51.1 years) and the HPV-negative group (45.1 years, P = 0.015). No difference was found in the mean DNA index (1.21 versus 1.22, P = 0.85) or 5-year survival rate (85% versus 86%, P 0.87) between the two groups. Conclusions. The high prevalence of HPV 16 in cervical cancer, which appeared to be correlated with age (in combination with its presence in diploid and aneuploid tumors), indicated the important role of HPV 16 in the evolution of cervical cancer. However, using stepwise Cox regression analysis, the presence of HPV 16 had no additional prognostic value over lymph node metastases findings. Cancer 1993; 71:397-401.

Abstract

Tumor stage refers to how far the growth of a tumor has progressed. Is it small? Has it invaded the local area? Has it spread to distant parts of the body? Tumor grade, on the other hand, refers to the histological (cellular) appearance of the tumor. A low-grade tumor has the appearance of differentiated tissue, while high-grade tumors have a clearly abnormal and malignant appearance. Ovarian cancers, unfortunately, are rarely seen in the early stages. However, there is some suggestion that the prognosis for ovarian cancers that are both early stage and highly differentiated is excellent. Indeed, some have advanced the notion that, once the cancer is surgically removed, no adjuvant therapy is required; the patient need only be watched for signs of recurrence. In an effort to explore this theory, 107 patients with apparent early stage, highly differentiated ovarian cancers were enrolled in a study; examinations revealed complicating factors in 21 patients, leaving 86 women for further review. Of these 86, 2 patients were excluded because no pathologic specimens were available and 17 were excluded because the original pathologic evaluation was incorrect, and the cancers were actually of higher grade. It was also found that staging of the cancer was inaccurate in 43 of the 67 remaining cases, and accurate in only 24. At the end of five years, all 24 patients who were accurately diagnosed as having both highly differentiated and early stage ovarian cancer were alive and free of cancer. Of the other 43, 88 percent remained alive without evidence of disease. All five patients who developed recurrent cancer failed to respond to secondary therapy and rapidly died. The results indicate that patients with early stage ovarian cancers (such as stages Ia, Ib, Ic, and IIa) which are highly-differentiated, have a high probability of survival and do not seem to require additional therapy. The findings also suggest, however, that much variation can occur in the staging and grading of tumor specimens, even by experienced professionals. Since in the case of ovarian cancer, therapeutic decisions rely upon the accuracy of the pathologic examination, the reliability of the analysis must be improved. Abstract

The objective of this study was to assess the effectiveness of sacrospinous ligament fixation of the uterus as a primary treatment of uterovaginal prolapse. In this observational study, 133 women underwent a sacrospinous hysteropexy. Data were obtained from their medical records, and standardized questionnaires about urogenital symptoms and quality of life were used. All women were invited for gynecological examination, using the Pelvic Organ Prolapse Quantification score. Ninety-nine women responded by returning the questionnaire (mean age, 59.2 and follow-up time, 22.5 months); 60 of these women underwent gynecologic examination. Eighty-four percent of women were highly satisfied about the outcome of the procedure. Serious complications were rare. The recurrence rate of descensus uteri that needed surgical treatment was 2.3%. The recurrence of cystoceles after surgery was 35%, but there were no differences in urogenital symptoms between women with or without a cystocele.

Abstract

Background: There is much debate on the necessity of regionalization of ovarian cancer care. We investigated the association between hospital type and survival of patients with ovarian cancer in The Netherlands. Methods: A retrospective, population-based cohort study was performed on all Dutch patients diagnosed with ovarian cancer from January 1, 1996, through December 31, 2003. We used data from the Netherlands Cancer Registry that were linked to mortality data from the Statistics Netherlands database to obtain the date and cause of death. Five-year relative survival ratios, defined as the ratio of the observed survival in the patient population to the expected survival of women in the general population with the same age, were determined for the total population and for groups stratified by tumor stage and/or hospital type. The association between hospital type and disease-specific survival was analyzed by use of multivariable Cox regression analyses. Results: We analyzed data from 8621 women with epithelial ovarian cancer, of whom 3482 (40%) were treated in general hospitals, 3510 (41%) were treated in semispecialized hospitals, and 1557 (18%) were treated in specialized hospitals. Five-year relative overall survival ratios of patients treated in general, semispecialized, and specialized hospitals were 38.0% (95% confidence interval [CI] = 36.0% to 39.9%), 39.4% (95% CI = 37.5% to 41.4%), and 40.3% (95% CI = 37.4% to 43.1%), respectively; median survival of patients aged 50-75 years was 36 months (interquartile range [IQR] = 13 to >54 months), 37 months (IQR = 14 to >54 months), and 38 months (IQR = 15 to >55 months), respectively. Age and cancer stage were associated with the relationship between hospital type and ovarian cancer-specific survival but histologic tumor type, grade, year of diagnosis, and socioeconomic status were not.
