15. A Literature Review of Nutrition Knowledge Measurement Items Used in Persons Living with Spinal Cord Injuries and Disorders.

Abstract

To identify and describe diet and nutrition knowledge measurement tools/items used among adults with spinal cord injuries and disorders (SCI/D). Conducted a literature search for publications measuring diet/nutrition knowledge using items or tools in persons with SCI/D. Eligible articles were published in 1992-2022, in English, and assessed SCI/D participants' nutrition knowledge. Articles were reviewed for study design, questionnaire validation, sample characteristics, item content, and whether nutrition knowledge was general or SCI/D-specific. Of 48 articles examined, five were included in the review. None of the nutrition knowledge questionnaires were validated in persons with SCI/D. Only one study assessed nutrition knowledge in nonathletes with chronic SCI/D, and this study excluded individuals with tetraplegia. The other studies either solely recruited athletes (n = 2; one included participants with amputation) or individuals with acute SCI/D (n = 2). Common item topics included calories/energy, sources of micronutrients and macronutrients and their effects on the body, and nutrition literacy. One questionnaire included non-SCI/D-specific nutrition items for persons with disabilities. Only one (unvalidated) questionnaire contained SCI-specific nutrition knowledge items, which only assessed whether more energy, fiber, and water are needed following acute SCI/D. Studies that assess nutrition knowledge among persons with SCI/D are rare and are typically observational or cross-sectional, with limited information on important SCI/D subgroups. Items relevant to unique needs of persons with SCI/D are sparse. A validated questionnaire that measures knowledge of SCI-specific nutrition, that is relevant to this population, and that can be parceled out by subgroup is needed. [ABSTRACT FROM AUTHOR]

Abstract

Disability is a common component of human diversity, yet people with disabilities (PWD) are underrepresented in the workplace despite known benefits to PWD and companies alike. There is an urgent need to promote inclusion of PWD in the workplace. The purpose of this study was to explore employer-perceived challenges and strategies for employing, retaining, and promoting people with physical disabilities (PWPD) and to describe the policies, supports, and job accommodations they provide to PWPD. We administered a cross-sectional, online survey to employers in the Midwestern United States (n = 53). Employers had an average of 8.5 years of experience in their job roles and represented diverse industries and company sizes. Employers provide a range of job accommodations; however, many reported lacking comprehensive disability policies such as training managers on how to support PWPD, providing employee resource groups for PWPD, and having a specific job accommodation fund. Major challenges reported by employers were related to lacking competencies in supporting employees with physical disabilities. However, employers endorsed the utility of strategies to address these challenges, including providing education and training and increasing buy-in from leadership. Study findings inform employers on ways to best support PWPD and improve diversity, equity, and inclusion in the workplace. [ABSTRACT FROM AUTHOR]

29. Near real-time patient experience feedback with data relay to providers: a systematic review of its effectiveness.

Abstract

Near Real-Time Feedback (NRTF) on the patient's experience with care, coupled with data relay to providers, can inform quality-of-care improvements, including at the point of care. The objective is to systematically review contemporary literature on the impact of the use of NRTF and data relay to providers on standardized patient experience measures. Six scientific databases and five specialty journals were searched supplemented by snowballing search strategies, according to the registered study protocol. Eligibility included studies in English (2015–2023) assessing the impact of NRTF and data relay on standardized patient-reported experience measures as a primary outcome. Eligibility and quality appraisals were performed by two independent reviewers. An expert former patient (Patient and Family Advisory Council and communication sciences background) helped interpret the results. Eight papers met review eligibility criteria, including three randomized controlled trials (RCTs) and one non-randomized study. Three of these studies involved in-person NRTF prior to data relay (patient-level data for immediate corrective action or aggregated and peer-compared) and led to significantly better results in all or some of the experience measures. In turn, a kiosk-based NRTF achieved no better experience results. The remaining studies were pre-post designs with mixed or neutral results and greater risks of bias. In-person NRTF on the patient experience followed by rapid data relay to their providers, either patient-level or provider-level as peer-compared, can improve the patient experience of care. Reviewed kiosk-based or self-reported approaches combined with data relay were not effective. Further research should determine which approach (e.g. who conducts the in-person NRTF) will provide better, more efficient improvements and under which circumstances. [ABSTRACT FROM AUTHOR]

44. Spasticity Predicts Motor Recovery for Patients with Subacute Motor Complete Spinal Cord Injury.

Abstract

Objective: A motor complete spinal cord injury (SCI) results in the loss of voluntary motor control below the point of injury. Some of these patients can regain partial motor function through inpatient rehabilitation; however, there is currently no biomarker to easily identify which patients have this potential. Evidence indicates that spasticity could be that marker. Patients with motor complete SCI who exhibit spasticity show preservation of descending motor pathways, the pathways necessary for motor signals to be carried from the brain to the target muscle. We hypothesized that the presence of spasticity predicts motor recovery after subacute motor complete SCI. Methods: Spasticity (Modified Ashworth Scale and pendulum test) and descending connectivity (motor evoked potentials) were tested in the rectus femoris muscle in patients with subacute motor complete (n = 36) and motor incomplete (n = 30) SCI. Motor recovery was assessed by using the International Standards for Neurological Classification of Spinal Cord Injury and the American Spinal Injury Association Impairment Scale (AIS). All measurements were taken at admission and discharge from inpatient rehabilitation. Results: We found that motor complete SCI patients with spasticity improved in motor scores and showed AIS conversion to either motor or sensory incomplete. Conversely, patients without spasticity showed no changes in motor scores and AIS conversion. In incomplete SCI patients, motor scores improved and AIS conversion occurred regardless of spasticity. Interpretation: These findings suggest that spasticity represents an easy‐to‐use clinical outcome that might help to predict motor recovery after severe SCI.
