Se observaron 625 citologías gástricas por cepillado tomadas mediante endoscopia a pacientes adultos con gastritis crónica y/o úlcera gástrica, con el objetivo de diagnosticar Helicobacter pylori. En un paciente con úlcera gástrica la prueba de la ureasa fue positiva y la citología resultó negativa de H. pylori. Se observaron otras bacterias espiraladas que solo se habían visto en la literatura, fueron medidas y fotografiadas. Se informó Gastrospirillum hominis. El paciente recibió tratamiento con Q-ulcer, metronidazol, tetraciclina y metoclopramida. A los 2 meses de finalizado se le repitió la endoscopia y había desaparecido la úlcera, se informó entonces gastritis crónica ligeramente agudizada. No existía reflujo biliar. La ureasa fue negativa y las bacterias descritas en la muestra anterior habían desaparecido.632 gastric brush cytologies taken by endoscopy from adult patients with chronic gastritis and/or gastric ulcer in order to diagnose Helicobacter pylori were observed. In a patient with gastric ulcer, the urease test was positive, whereas the H. pylori cytology was negative. Other spiral bacteria that had been only found in literature were detected, measured and photographed. Gastropirillum hominis was reported. The patient received treatment with Q-ulcer, metronidazole, tetracycline and metroclopramide. 2 months later, the endoscopy was repeated and ulcer had disappeared. A slightly more acute chronic gastritis was reported then. There was no biliary reflux. Urease was negative and the bacteria described in the previous sample were not found.

El presente trabajo es una revisión de los anexos existentes entre el Helicobacter pylori y la gastritis crónica, la úlcera gastroduodenal, el cáncer gástrico, el linfoma de células B y algunos aspectos relacionados con la epidemiología y el tratamiento de esta infección. Se sugiere la posibilidad de que modelos de infección semejantes se asocien a otras enfermedades así como el hecho de que ésta, procedente de Europa, se haya introducido en América como consecuencia del encuentro de estas 2 culturasA review is made on the links existing between Helicobacter pylori and chronic gastritis, gastroduodenal ulcer, stomach cancer, and B-cell lymphoma, as well as on some aspects connected with the epidemiology and treatment of this infection. It is suggested the possibility that similar models of infection be associated to other diseases, and that this infection original from Europe had been introduced in America as a result of the encounter between these two cultures

La infección por Helicobacter pylori (Hp), principal etiología de la gastritis crónica, úlcera duodenal y adenocarcinoma gástrico, ha sido motivo de estudios, muy controversiales, de muchos investigadores, sobre su papel en el desarrollo de enfermedad por reflujo gastroesofágico (ERGE) que es favorecida por la infección, según algunos y que es en sí provocadora, según otros. Los hallazgos manométricos reportados incluyen desde hipertonía del EEI con relajaciones transitorias espontáneas (RTEEI) y severos trastornos motores (TM) acompañantes, hasta todo lo contrario. Con este trabajo pretendemos demostrar la existencia de trastornos manométricos específicos en pacientes Hp+ comparándolos con pacientes Hp-. Se les realizó endoscopia (esofagitis en 20 Hp+ y 12 Hp-), manometría y Phmetría esofágica (a un subgrupo) a 76 pacientes sin tratamiento, de ambos sexos, con edad X = 46,3 años, 48 con test de ureasa positivo y 28 con test negativo. En los Hp+, la pr fue elevada significativamente, x = 41mm (vs. 14,5 mmHg) (p < 0,02); 63 % eran hipertónicos y 37 %, normotónicos, con alta incidencia de RTEEI (75 %) (p < 0,02) y marcada asimetría esfinteriana (77 %) (p < 0,02). Los TM fueron de moderados a severos en 89 % (p < 0,05). En el grupo Hp-, el EEI mostró presión normal-baja: X = 17,3 mmHg (39,3 % hipotónicos y 61 % normotónicos). Las RTEEI sólo en 25 % y la simetría normal en 75 %. Se halló predominio de los TM ligeros a moderados, para 61,2 %. Se concluyó que los pacientes Hp+ sí presentaron alteraciones manométricas significativas, con EEI hipertónico, incompetente por las RTEEI y por ser totalmente asimétrico, características que favorecen al RGE y con trastornos motores más severos que los pacientes no infectados por el H. pylori.The Helicobacter pylori (Hp) infection, main etiolgy of chronic gastritis, duodenal ulcer and gastric adenocarcinoma, has been object of very controversial studies conducted by many researchers on its role in the development of gastroesophageal reflux disease (GERD) that is favored by infection according to some authors, and that provokes it according to others. The manometric findings reported include hypertony of the EEI with spontenous temporary relaxations (STREI) and severe accompanying motor disorders (MD) up to all the contrary. This paper intends to show the existence of specific manometric disorders in Hp positive patients compared with Hp negative patients. Endoscopy (esophagitis in 20 Hp positive and 12 Hp negative),manometry, and esophageal Ph-metry (to a subgroup) were performed in 76 patients with no treatment of both sexes aged ^x = 46.3, 48, with positive urease test and 28 with negative test. Pr was significantly elevated in the Hp+, x = 41mm (vs. 14,5 mmHg) (p < 0.02); 63 % were hypetonic and 37 % normotonic, with a high incidence of STREI (75 %) (p < 0.02) and a marked sphincteral asymmetry (77 %) (p < 0.02). The MD were from moderate to severe in 89 % (p < 0.05). In the Hp negative group, the EEI showed normal-low pressure: x^=17.3 mmHg (39.3 % hypertonic and 61 % normotonic). The STREI just in 25 % and the normal assymetry in 75 %. It was found a predominance of the mild to moderate MD, accounting for 61.2 %. It was concluded that the Hp positive patients presented significant manometric alterations, with hypertonic EEI, incompetent for the STREI and for being totally asymmetric, characteristics that favor the GER and with more severe motor disorders than the non-infected H. pylori patients.

Se realizó un estudio observacional, descriptivo, prospectivo con todos los pacientes que habían recibido un trasplante renal en el Instituto de Nefrología desde el 2002 hasta el 2004. Se les realizó gastroduodenoscopia con prueba de ureasa para diagnóstico de Helicobacter pylori y alteraciones asociadas en el primer mes del trasplante y se repitió el estudio a los 6 meses y al año. Se encontró una prevalencia de la infección de 34,1 por cada 100 pacientes. Se halló con mayor frecuencia entre los 40 y 49 años de edad. Se encontraron como alteraciones digestivas asociadas a la infección de forma significativa: la gastritis eritematosa y la úlcera duodenal. Se comprobó que todos los pacientes tratados lograron curar la infección y solo uno con alteración digestiva asociada no resolvió con el tratamiento.A prospective, descriptive and observational study was conducted on all the renal transplant recipients at the Institute of Nephrology from 2002 to 2004. Gastroduodenoscopy in addition to an urease test were performed to diagnose Helicobacter pylori and associated alterations in the first month of transplantation, and then they were repeated six and 12 months afterwards. The infection prevalence rate was 34,1 per 100 patients. It was more frequent in the 40-49 years age group. The most significant infection-associated digestive disorders were erythematous gastritis and duodenal ulcer. It was demonstrated that all the treated patients managed to get over the infection and only one patient with associated digestive alteration did not suceed.
