The second edition of this book explores different aspects of the relationship between hypertension and brain damage, providing readers with up-to-date information that will be of interest to clinicians and researchers alike. In recent years, numerous studies have been published that have added new evidence on different aspects of the disease. New recommendations and the latest hypertension guidelines on stroke and dementia will be described in this edition, which will include not only updated content but also new chapters on aspects that have recently emerged in relation to hypertension and brain damage. After an introductory chapter on the epidemiology and burden of neurological disease induced by hypertension, the significance of various comorbidities that represent risk factors for brain damage in the context of hypertension is discussed. The primary aim in the surgical management of heart disease is an accurate diagnosis. This is achieved by a thorough clinical examination combined with the appropriate special investigations. For convenience, we think in terms of congenital and acquired conditions although the distinction is not always clear. Congenital lesions can usually be subdivided according to the presence or absence of cyanosis. There is also an important intermediate group of conditions which may develop cyanosis on exer tion or exercise. In the group of acyanotic lesions it is convenient to divide cases into those with and without a left-to-right shunt. This subdivision is made mainly on the radio graphic appearances of the lung fields. The following simple classification will be found helpful in narrowing the diagnostic possibilities in an individual case - Surgical Classification of Heart Disease Cyanotic Congenital Intermittent Heart Disease Cyanosis { Obstructive Lesions Acyanotic { L-R Shunts (lung fields) Endocardium (valves) Myocardium Pericardium Major blood vessels Such a classification cannot hope to be comprehensive but the main groups are covered including the majority of surgically treatable conditions. The major vessels leaving and entering the heart can conveniently be considered in relation to heart disease since coarctation and ductus arteriosus have long since been re garded as congenital cardiac surgical problems.

Die kardiovaskuläre Mortalität in der alten Bundesrepublik zeigte während des letzten Jahrzehnts für die Männer eine geringgradige Abnahme, für die Frauen jedoch eine Zunahme. Vordergründig könnte man meinen, daß somit der Gipfelpunkt kardiovas kulärer Erkrankungen erreicht sei. Diese Erkrankungen stellen nach wie vor 50% aller Todesfälle in der Bundesrepublik dar. Aus der jüngsten Erhebung des Augsburger Infarktregisters wissen wir, daß diese Abnahme nicht einhergeht mit einer Optimierung kardiovaskulärer Risikofaktoren. Dies steht in Übereinstimmung mit den Erhebungen im Rahmen des sog. nationalen Untersuchungssurveys der Deutschen Herz-Kreislauf Präventionsstudie (DHP), welche an 200 Erhebungspunkten in der alten Bundesrepu blik im Abstand von 3 Jahren erhoben wurden, um einen möglichen säkularen Trend zu erfassen. Trotz der Intensivierung von präventiven Maßnahmen ist es in Überein stimmung mit den Daten aus dem Augsburger Infarktregister zu einer Zunahme der Häufigkeit der Risikofaktoren gekommen, insbesondere zu einem Anstieg des Serum cholesterins und des Körpergewichts, zu praktisch keiner Veränderung beim Rauchver halten und nicht zu einer Abnahme der Hypertonie, sondern lediglich zu einer verbes serten Therapie der Hypertonie. Auf der anderen Seite ist ein starker Anstieg der speziellen therapeutischen Maßnahmen wie Bypassoperation, Ballondilatation und Herztransplantation in der Bundesrepublik festzustellen. 1990 hatte die Zahl der Dila tationen, wie schon 1989, mit 32. 459 wiederum die Zahl der Bypassoperationen (1990: 26. 159) in der Bundesrepublik übertroffen. Man muß somit davon ausgehen, daß die geringfügige Abnahme der Mortalität und der Morbidität, wie sie im Augsburger Infarktregister festgestellt wurde,Folge einer Ver besserung des kurativen Systems ist.

Die einzelnen Beiträge unterziehen sowohl Neuentwicklungen als auch Etabliertes auf dem Sektor Diagnostik und Therapie in beiden Disziplinen einer kritischen Würdigung hinsichtlich ihres Nutzens und Fortschrittes für die Lebensqualität und Prognose der Patienten. Es wird ersichtlich, wie wichtig eine enge Kooperation zwischen Kardiologen und Herzchirurgen ist, um die durch den Fortschritt neu definierten Zielsetzungen verwirklichen zu können. Angesichts der Kostensteigerungen im Gesundheitssystem und andererseits knapper werdender finanzieller Resourcen, ist ein weiterer Schwerpunkt auch den sozioökonomischen Bedingungen der universitären High-Tech-Medizin gewidmet. So wird in den Beiträgen das derzeit medizinisch Machbare auch immer einer Kosten-Nutzen-+Analyse unterzogen.

Posttraumatic stump formation and replantation of the severed limb are both reconstructive plastic operations which may lead to the improvement or destruction of a patient's lifestyle. For the primary attending surgeon, the decision whether to undertake such an operation depends on the patient's clinical condition, the operational circumstances, the psychological and social aspects and, last but not least, on the surgeon's own abilities.
