Within the past decade, advances in medical technology, the desires and complex care needs of an ageing population, and innovative care delivery models have initiated a shift from providing care in hospitals to outpatient settings. And more recently, the acceleration and amplification of these factors is pushing healthcare options even further from the traditional inpatient and outpatient settings towards acute and subacute care in the home. This has led the medical community to look toward providing more tools and methods of care that patients can access safely right from home and the designers to think as the homes of the future will be flexible to support both an array of devices to provide a healthcare delivery and the humanization and personalization of the domestic space. The paper identifies criteria for the flexible design of the physical environment (including the home, equipment, furniture, etc.) that support and facilitate safety, comfort, and healing, in relation to the various patient populations, at their own physical and psychosocial needs, at the range of equipment/technology (from chronic to acute care), at the caregiving and daily living activities.

This paper presents an ongoing research to define the framework of a computational design approach based on the idea of spatial analysis and spatial synthesis to implement multi-criteria evaluations and provide evidence of the performance of the design alternatives in the specific case of home adaptation for healthcare at home. The European health systems place among the priority objectives the strengthening of the provision of healthcare at home to guarantee the aging in place of elderly people and to limit, at the same time, the unnecessary use of resources. Therefore, existing homes must provide adequate safety, comfort, and accessibility features to ensure a high quality of life for the care receivers and facilitate the caregivers' tasks. To address the complexity of the requirements to be met, we propose a spatial decision support system (SDSS) to implement multi-criteria assessments to ergonomic design problems at a spatial scale of apartment homes. The system is intended to streamline and assist designers and homeowners in planning interventions for home adaptations for healthcare. Such design problems can be formulated as decision problems with costs and benefits modeled within constraints of validity and quality criteria/objectives. Concerning the specific field of study, the system evaluates the degree of compliance with the accessibility and visibility quality criteria of each design alternative. The reiteration of the evaluation mechanism allows for the classification and supports the selection of satisfactory technical solutions identified with an in-formed and well-balanced trade-off between the relevant quality criteria.

Amaç:Bu araştırma, Samsun il Merkezinde evde sağlık hizmeti alan ve almayan hastalara bakım vericilerin bakım vermedeki yeterlilik düzeyleri ile bakım yüklerinin karşılaştırılması amacıyla yapılmıştır.Yöntem:Bu çalışma 111 evde sağlık hizmeti alan ve 111 evde sağlık hizmeti almayan hastalara bakım vericiler olmak üzere, toplam 222 evde bakım verici üzerinde gerçekleştirilmiştir. Veri toplama araçları olarak; araştırmacılar tarafından geliştirilmiş ‘’Tanıtıcı Özellikler Bilgi Formu’’ ve ‘’Zarit Bakım Yükü Ölçeği’’ kullanılmıştır. Verilerin değerlendirilmesinde; Yüzde, sayı, Mann-Whitney U testi, Kruskal Wallis H testi, Pearsen Ki-Kare testi uygulanmıştır. Kullanılan tüm istatistiksel testler % 95 güven seviyesinde sınanmıştır.Bulgular:Evde Sağlık Hizmeti alan hastalara bakım vericilerin bakım vermedeki yeterlilik düzeyi ortalaması % 42.5 iken Evde Sağlık Hizmeti almayan hastalara bakım vericilerin puan ortalaması %35.8 bulunmuştur. Evde bakım vericilerin, hastalarının Evde Sağlık Hizmeti alıp almama durumuna göre yeterlilik düzeyleri arasındaki ilişkinin incelenmesi sonucu anlamlı bir fark olduğu görülmüştür (p=0.000, Aim:This research was done with the purpose of comparing caregivers' efficiency level on giving healthcare and burden of care they give to the patients both those who receives healthcare at home and those who don't, in the city center of Samsun Province.Method:This research was carried out on 222 caregivers which 111 caregivers of patients receiving health care at home and 111 caregivers for patients who do not receive health care at home.Developed by researchers as data collecting tools; 'Introductory Features Data Form' and 'Zarit Caregiver Burden Scale' was used. To Evaluate data ;percentage ,numbers ,Mann-Whitney U Test, Kruskal Wallis H Test, Pearson Chi-Square Test was applied. All the statistical tests that used in this research was tested with 95% confidence level.Results:It was found out that caregivers ,who give care at home, scored average point of 42.5% on efficiency level on care giving ,while caregiver ,who don't give care at home, scored only 38.8%.It was found that there was a significant difference on efficiency level of the caregivers when consider some patients received healthcare at home or some didn't(p=0.000

The issue of access to heamthcare is crucial in our modern society. One obvious effect of the augmentation of healthcare services demand is the increasing occupancy rates in hospitals. The main difference between the provision of care at the hospital and at home is as follows: the patient is at hospital and all the resources necessary for its treatment are in the same place, while in the care delivered at home, resources must be moved to the bedside. There are several challenges in order to achieve this change. In this thesis we address three important issues in the design of structures of home care. First, the location of structures minimizing logistics costs, where we develop three models with different features such as traveling costs of resources, changes in demand over time and evolution of freelance resources. These models allow us to provide robust location over time while ensuring maximum coverage and minimizing costs. The second issue is the choice of activities, epidemiological and spatial coverage, taking into account different types of activities and resources, permissions to serve some pathologies and coverage. Two models developed allow us to show the effects on the demand allocation and resources planning induced by changes in the costs of freelance professionals and authorization to serve some pathologies. The third problem is the dimensioning of resources with demand uncertainty (volume, epidemiological and geographical) and the proposed model takes into account the underlying problem of moving resources using an estimate of the routes performed.; La question de l'accès au soin est cruciale dans notre société moderne. Un effet évident de la demande accrue de services de santé est l'augmentation du taux d'occupation dans les hôpitaux. La principale différence entre la dispensation de soins à l'hôpital et au domicile est la suivante: le patient doit se déplacer et toutes les ressources nécessaires à son traitement se trouvent dans le même endroit, tandis que dans les soins délivrés au domicile, les ressources doivent être déplacées au chevet du patient. Il existe plusieurs défis afin de pouvoir réaliser ce changement. Dans cette thèse nous traitons trois problèmes importants dans la conception des structures de soins à domicile. D’abord, la localisation des structures en minimisant les coûts logistiques, où nous développons trois modèles incluant différentes caractéristiques comme du système de santé comme les coûts liés aux déplacements des ressources, la variation de la demande dans le temps et l’existence et évolution des ressource libérales. Ces modèles nous permettent de proposer des localisations robustes dans le temps tout en assurant une couverture maximale et en minimisant les coûts. La deuxième problématique consiste au choix des activités et couverture épidémiologique et spatiale en tenant compte différentes activités et types de ressources, les autorisations pour réaliser les pathologies et la couverture. Deux modèles développés nous ont permis montrer les effets sur l’affectation de la demande et le dimensionnement de ressources induits par changements dans les coûts des libéraux, salaires et d’autorisation de servir la demande. Le troisième problème et celui du dimensionnement de ressources avec incertitudes de demande (volume, épidémiologique et géographique) et le modèle proposé tient compte du problème sous-jacent de déplacement des ressources à l’aide d’une estimation de la tournée réalisée.

In this article, we propose a smart system for elderly people with chronic heart diseases, especially the heart failure patients, for managing and giving feedback on their drinking activities at home. We present the prototype system including both the hardware and software implementation for tangible interaction and visual feedback.
