10. Association of Dental Caries, Periodontal, and Nutritional Status with Body Mass Index among Incarcerated Women in Central Jail, Karachi, Pakistan.

Objective: To determine the association body mass index and the status of dental decay and gum health in women serving prison sentences. Methods: A cross-sectional investigation took place at the Central Jail for Women, Pakistan, spanning from January 2021 to February 2022. The study included all incarcerated women and those awaiting trial aged between 18 and 65 years, while individuals who declined participation were excluded. Convenience sampling was employed, and each participant underwent examinations for dental caries and periodontal status. Sample size was calculated by considering the total number of women in prison to be 205, keeping the margin of error as 5%, and a confidence level of 95%, a sample size of 134 was calculated. Oral health was measured through DMFT index. Body mass indexes were recorded by measuring the height and weight of the participants. Furthermore, nutritional habits including height, weight, source of food and frequency of food were also documented. Oral health conditions were evaluated in 131 women currently serving prison sentences. Results: About 23.7% (n=31) were overweight and 6.1% were obese. Total 71.7% (n=94) of women had at least one decayed tooth, 63 (48.1%) had at least one missing tooth, and 7 (5.3%) had at least one filled tooth. The logistic regression analysis indicated a significant association between being overweight and the DMFT score, with an Exponentiation of B of 1.19 and a 95% confidence interval ranging from 1.03 to 1.38. The associated p-value was 0.021. Conclusion: It was found that high BMI was significantly associated with higher DMFT score. The results of this study drives the attention of clinicians and researchers towards the unidentified health needs of incarcerated women. Among other medical conditions, sensitization to oral hygiene and oral health must also be prioritized. Background and Objectives: Adequate access to oral and dental health services in childhood can reduce long-term complications in the following years of life. The objective of this study was to determine the barriers of access to children’s oral and dental health services from the point of view of mothers referring to health centers in Qom city. Methods: In this cross-sectional study, the statistical population included 325 mothers referred to health centers. Data were collected using a questionnaire consisted of items, including age, educational level, job, and barriers of access to oral health services. Data were analyzed using descriptive statistical indicators and logistic regression test. Results: In this study, lack of insurance coverage for dentistry costs (59.7%) had the highest frequency in barriers of access to dental health services, followed by child's fear of dentistry (53.2%) and high costs of dental services (49.8%). There was no significant relationship between mother's job and barriers of access to dental health services, but the chance of barriers of access to dental health services increased 1.60 times with father’s employment in government jobs. Also, the results showed that the chance of barriers of access to dental health services increased 3.60 times with residence in Pardisan region, on the other hand, the chance of access to the services, was improved up to 52% with residence in Tohid region. Conclusion: Expansion of insurance coverage of oral and dental health services and increase of public centers providing dental services can be eliminate the major part of barriers of access to these services. In addition, the proportional distribution of these services in different regions of the city can be effective in easy and low-cost access.

To assess whether increasing enrollment in State Children's Health Insurance Programs (S-CHIPs) has an impact on the number of office physicians participating in Medicaid and the extent of their participation. Effects are measured for a freestanding S-CHIP program with an open provider panel and an S-CHIP program that uses the state's Medicaid provider panel. Children's Medicaid claims data for primary care services were used to measure physician participation in the program; census and enrollment data were used to describe market area characteristics. This is a time series study of communities in two states, measuring physician Medicaid participation quarterly between 1998 and 2001, controlling for changes in community characteristics and children's program enrollment as well as other factors by quarter. Office physician participation is measured by practice site. Claims data are aggregated to the level of the community and reflect the number of limited practice sites, the ratio of Medicaid office sites to the number of primary care physicians in the community as reported by the American Medical Association (AMA), and the mean number of Medicaid office visits made to physician sites in the community in the quarter. In Alabama, the state with a freestanding S-CHIP program, there is little association between increased S-CHIP enrollment and physician participation in Medicaid. In Georgia, where the same provider network serves both programs, increases in S-CHIP enrollment are associated with a decline in office-based physician participation in Medicaid in urban areas. Linkage of S-CHIP and Medicaid programs through the use of the same provider network, in the absence of market conditions that encourage the expansion of the network, can lead to a negative impact on access for Medicaid enrollees. "The objective of this dissertation is to examine the fundamental geographic patterns regarding the spatial distribution of specific Aging in Place (AIP) and Aging in Community (AIC) opportunities in the United States by county, focusing on the characteristics of the workforce that provide these services. Secondly, this research aims to determine what the key determinants or predictor variables that best explain this spatial distribution. This examination will investigate the geographical link between Facility-Based Services (FBS) and Home and Community-Based Services (HCBS) and their relationship to the older adult population. It is hypothesized that certain socio-economic and demographic variables that gauge culture, social capital, and prosperity will be instrumental in describing the geographic inventory of the Long-Term Care workforce and that specific establishments and initiatives dedicated to the Continuum of Long-Term Care are disproportionately located and clustered in these geographic areas. Specifically, it is hypothesized that the geography of Facility-Based and Home and Community-Based Services will vary, and access to Long-Term Care will be unequal. The analysis revealed that Long-Term Care employment and Continuum of Care establishments are not evenly distributed throughout the United States, greatly influencing an individual's ability to Age in Community. Urban counties, with higher population densities and elevated levels of social capital provide more HCBS and innovative Long-Term Care (LTC) options. "The purpose of this study was to understand the perceptions and experiences of illness among American Indians (AI) in southeastern NC and to describe their decision making processes when accessing and using health care services. Most AIs in North Carolina live in rural areas, where chronic illnesses are a growing concern. Illnesses such as cardiovascular diseases and diabetes have crippled this population physically and emotionally and have resulted in undue financial hardship for AIs. Focused ethnography methodology was used to guide the study and the Social-Ecological Model (SEM) aided in the analysis of the data. AI gatekeepers recruited eighteen participants from rural areas to participant in one of four focus groups or one of three semi-structured interviews. After data analysis, confirmation of findings was received from the participants. These participants describe illness as having a medical diagnosis and the experience of being ill as having signs and symptoms, for example, breathing difficulty, pain, bleeding, inability to attend social functions, and the inability to be active. Many of these participants reported seeking care for primary, secondary and tertiary prevention. Factors that influenced their decision making involved all of the SEM levels. The two most significant factors that influenced their decision making to seek health care were adequate insurance and a relationship with the provider. With the exception of an emergency, without these two factors, there was a delay in seeking health care."--Abstract from author supplied metadata
