Purpose: The main objective of the paper is to assess to what extent the health care needs of the retirees in Poland are satisfied and what role the private health insurance has and might have in satisfying these needs. Design/methodology/approach: We use the data from Household Budget Survey (HBS) conducted yearly by Central Statistical Office as well as own database created on the outcomes of the survey carried out in 2018 on the representative sample of Polish seniors aged 65+ for whose households’ the state old-age pension is the primary source of income. Findings: Health care expenditures, according to the HBS database, accounted for almost 9,5% of total expenditures in the years 2005-2016. Differentiation of these expenses is also observed depending on the sex, age of the main respondent and place of residence. In the households of people at the age of 85 and more, they established as much as 12,9% of total spending. It was also found that only 8,9% of retirees’ households had supplementary health insurance and most of them rarely use paid health-care services. At the same time, it should be noted that respondents most often indicated free access to medicines and supplementary health care services (rehabilitation) as the most valuable supplement to the health services system in Poland. Practical implications: The analysis of health expenditures of households of retirees can become a basis for building strategies for companies that offer health care services and for financial institutions (insurance companies) that can prepare long-term health insurance products on this basis. Originality value: The study showed the importance of spending on health by asking about the direction of spending a sudden cash inflow of 226.50 Euro (1000 PLN). It was found that the oldest seniors, those with lower education, widows and those from small towns were the most likely to spend the extra money on health, indicating a wide variation in demand for medical services., peer-reviewed

OBJECTIVES. This study explored how vaccination coverage is associated with not being insured and with insurance type among children who are insured and to show how these associations are modified by race/ethnicity. METHODS. We determined whether 8324 children sampled in the National Immunization Survey in 2001 and 2002 were covered by private insurance only, Medicaid/State Children's Health Insurance Program, or another insurance type or were uninsured at the time of the National Immunization Survey interview or were uninsured at some time before the interview. Children were up to date if, by the date of the interview, their vaccination providers had administered [greater than or equal to] 4 doses of diphtheria-tetanus toxoids-acellular pertussis vaccine, [greater than or equal to] 3 doses of polio vaccine, [greater than or equal to] 1 dose of measles-mumps-rubella vaccine, [greater than or equal to] 3 doses of Haemophilus influenzae type b vaccine, and [greater than or equal to] 3 doses of hepatitis B vaccine. To evaluate the association between insurance type and breaks in insurance with timely completion of the recommended vaccination schedule soon after 19 months of age, we restricted our analyses to children 19 to 24 months of age. RESULTS. Nationally, 12.6 [+ or -] 1.6% of all children 19 to 24 months of age were uninsured at some time. Children who were uninsured at the time of the National Immunization Survey interview had significantly lower vaccination coverage than did children with Medicaid/State Children's Health Insurance Program coverage or children with private insurance only (52.6% vs 70.0% and 75.6%). Children who had never been insured and children who were insured but had a break in insurance coverage in the 12 months immediately preceding the National Immunization Survey interview had significantly lower vaccination coverage than did children who had been insured continuously (47.4% and 64.8% vs 73.5%). CONCLUSIONS. Approximately 1 of 8 children were uninsured at some time, and those children were at greater risk of not being vaccinated on time as recommended. Objective. To assess the impact of state laws requiring or encouraging employers to establish 'section 125' cafeteria plans that shelter employees' premium contributions from tax. Data Sources. Available descriptive statistics, 65 key-informant interviews, and relevant documents in study states and nationally, 2008-2009. Study Design. Case studies were conducted in Indiana, Massachusetts, and Missouri--three states adopting laws in 2007. Descriptive quantitative information came from insurers, regulators, and surveys of employers. In each state, 15-17 semistructured but open ended interviews were conducted with insurance agents, insurers, government officials, and third-party administration firms, and 29 informed sources were interviewed from a national perspective or other states. Key informants were selected based on their known or reported experience, in a 'snowball' fashion until saturation was reached. Interview notes were coded for systematic analysis. Finally, relevant rulings, brochures, instructions, marketing materials, and other documents were collected and analyzed. Findings. Despite the potential for substantial cost savings, use of section 125 plans to purchase individual insurance remained low in these states after 1 or 2 years. Absent a mandate, few employers were strongly motivated to offer these plans in order to retain an adequate workforce, and uncertainty about federal legality deterred doing so. For smaller employers, benefits to owners did not outweigh administrative complexities. Nevertheless, few downsides were found to states mandating or encouraging these plans. In particular, there is little evidence that many employers dropped group coverage as a result. Conclusions. Section 125 plans remain a limited tool for states to reduce the inequitable tax treatment of individually purchased insurance, but a complete remedy requires reform of federal tax law. Key Words.
