Introduction: Formal nursing leaders play an important role in promoting and sustaining evidence-based practice (EBP) in the clinical context, particularly by creating a conducive environment for change and increasing clinicians' awareness of the positive results of EBP. However, nursing leaders encounter both barriers and facilitators in their work; therefore, it is important to understand their perceptions on this topic. Objective: The objective of this study was to understand the perceptions of formal nursing leaders about barriers and facilitators for EBP. Methods: A descriptive exploratory study with a qualitative approach was conducted. A convenience sample was obtained, consisting of formal nursing leaders from three Portuguese health care institutions. Data were collected through semi-structured interviews. Content analysis was conducted using MAXQDA Analytic Pro 2022 software. Results: Seventeen formal nursing leaders were interviewed. Five categories of EBP barriers were identified: (1) scarce resources and inefficient resource management; (2) non-conducive organizational culture; (3) distance between academia and the clinical context; (4) demotivation; and (5) resistance to change. Seven categories of EBP facilitators were identified: (1) availability of resources and efficient resource management; (2) conducive organizational culture; (3) partnerships between academia and the clinical context; (4) motivation and commitment; (5) leadership; (6) organization that regulates professional practice; and (7) multidisciplinary meetings. Conclusions: This study identified barriers and facilitators for EBP through the perceptions of formal nursing leaders. Collaborative multidisciplinary efforts by leaders, direct care professionals, academics, and researchers should be conducted to overcome barriers and strengthen facilitators for EBP. ABSTRACT Objective: to describe the perception of health professionals and managers in the prison system regarding the continuity of breastfeeding care for lactating women deprived of liberty. Method: descriptive-exploratory research, qualitative approach, developed in a prison unit in Rio de Janeiro from December 2022 to January 2023. Interviewees: five health professionals and two managers. Content analysis was performed using Bardin’s method with the assistance of Atlas.ti software, which has advanced resources to assist the researcher in organizing, analyzing and interpreting data. Results: the actions that promote the continuity of breastfeeding care were: educational actions, with professionals as a support network, and the exchange of knowledge between professionals and lactating women. However, there are actions that weaken the continuity of care, favoring early weaning: introduction of food before six months, lack of guidance from family members, unplanned mother-baby separation and lack of a healthcare network within the prison system. Conclusion: professionals and managers perceive that there is promotion of continuity of breastfeeding care in prison. However, there are hindering elements that contribute to the discontinuity of breastfeeding. A challenge then arises to guarantee the equitable and comprehensive healthcare of lactating women and babies.

Abstract Background The first waves of the COVID-19 pandemic had a negative impact on health systems and health professionals, due to the high number of cases and a lack of preparation. The aim of this study was to understand how nurses working in hospital units and in intensive care perceived the performance of nurse managers and senior hospital management during the first two waves of the pandemic. Methods The phenomenological approach proposed by Giorgi was used to investigate perceptions of the performance of nurse managers and senior hospital management during the first two waves of the COVID-19 pandemic in Spain. Fourteen clinical nurses who worked on the front line in inpatient units or intensive care units of the Health Services of Extremadura and Madrid in the first (March–April 2020) and second (October–November 2020) waves of the COVID-19 pandemic participated in this study. The data was collected through semi-structured interviews, following a script of themes, in a theoretical sample of nurses who were worked during the pandemic. Results Two main themes emerged from the analysis of the data: (1) perceptions about the performance of nurse managers and senior hospital managers during the first and second waves of the pandemic (health system failure; belief that senior hospital management professionals could have managed the pandemic better; recognizing the efforts of middle management (nursing supervisors); insufficient institutional support) and (2) strategies employed by nurses to compensate for the weaknesses in pandemic management. Conclusions The clinical nurses perceived that the nurse managers demonstrated better management of the pandemic than the hospital’s senior management, which they attribute to their proximity, empathy, accessibility, and ability to mediate between them and the senior management. The nurses also believe that the senior management of the hospitals was to blame for organisational failures and the poor management of the pandemic.

After 15 years of promulgating the national policy, many health units still have not implemented the Integrative and Complementary Practices. The reasons for this departure from Primary Health Care in the Metropolitan Region of Goiania-GO, Brazil, remain unknown. Thus, this study aims to understand how managers perceive the difficulties of implementing and implementing Integrative and Complementary Health Practices in Primary Health Care. This qualitative, descriptive, and exploratory study was carried out with 24 managers of basic health units through semi-structured interviews, transcribed and analyzed according to Bardin's content structure. Managers attribute the absence of Integrative and Complementary Practices in Primary Health Care to factors such as work overload, lack of support from the health secretariats, prejudice, inadequate infrastructure, lack of financial resources, and insufficient inservice training. Therefore, we conclude that Integrative and Complementary Practices are absent from Primary Health Care, mainly due to structural and organizational barriers. These results may imply the resolution of these obstacles so that Integrative and Complementary Practices can be offered to users of the Unified Health System more equitably. [ABSTRACT FROM AUTHOR]

This article analyzes the perception of municipal health managers about the process of regionalization of health in the State of Rio Grande do Sul. It has a qualitative nature, employing semi-structured interviews with eight municipal health managers, chosen intentionally. In the theoretical approach, the concept of Government Triangle was used, matching it with the categories of thematic analysis. In the government project on the regionalization of health, the means and objectives identified were the Pact for Health, financing, solidarity, access, health consortia, models of supply and contracting of health actions and services. In the capacity of government in the Health Care Networks and the instances of agreement, the ways of conducting, maneuvering and overcoming that emerged from the analysis were the Regional Intermanagers Commission, the decentralization of health services and intersectoriality. As for management and health governance, the interviewees identified the manager not having time and/or not prioritizing the planning and management processes, managers as political figures and without preparation, the technical staff without specific training, the care gaps and the fixation of professionals. Objective: To investigate the strategies Brazilian health managers employ to supply dependent elderly home care. Methods: This is qualitative hermeneutics-based research with a comprehensive perspective on elderly home care. Sixteen managers working in Primary Health Care and specific elderly healthcare programs from the five Brazilian regions participated in this study. Data were collected from June to September 2019 with semi-structured interviews, and later systematized and consolidated into two themes in light of hermeneutics. Results: Managers articulated health education practices, mapped vulnerabilities, implemented care flows and protocols, and organized community support and health promotion groups, besides integrative and complementary practices to consolidate and improve elderly home care. Conclusion: Multiple strategies in dependent elderly home care stand out, such as individual and collective health education actions and work management processes, in which the manager plays a crucial role in developing services for the elderly. Abstract Background Tobacco control should be a higher public health priority in Japan. Some workplaces provide smoking cessation support and connect employees to effective smoking cessation treatments such as outpatient clinics. However, tobacco control measures have not been sufficiently implemented in Japan, especially in small- and medium-sized enterprises (SMEs), where resources are limited. Organizational commitment and consistent leadership are crucial to facilitate implementation, but research on whether supporting organizational leaders leads to health behavior changes among employees is limited. Methods This hybrid type II cluster randomized effectiveness implementation trial (eSMART-TC) aims to examine the effects of interactive assistance for SME management on health and implementation outcomes. We will provide interactive assistance to employers and health managers for 6 months, aiming to promote the utilization of reimbursed smoking cessation treatments by public health insurance and to implement smoke-free workplaces. The intervention will consist of three strategies: supporting employees through campaigns, tailored ongoing facilitation, and ensuring executive engagement and support. The primary health and implementation outcomes will be salivary cotinine-validated 7-day point-prevalence abstinence rate, and the adoption of two recommended measures (promoting utilization of smoking cessation treatment and implementing smoke-free workplaces) 6 months after the initial session, respectively. Other outcomes for implementation (e.g., penetration of smoking cessation clinic visits), health (e.g., salivary cotinine-validated 7-day point-prevalence abstinence rate at 12 months), and process (e.g., adherence and potential moderating factors) will be collected via questionnaires, interviews, logbooks, and interventionists’ notes at 6 and 12 months. An economic analysis will be undertaken to assess the cost-effectiveness of the implementation interventions at 12 months. Discussion This will be the first cluster randomized controlled trial to evaluate the effectiveness of an implementation intervention with interactive assistance for employers and health managers in SMEs on smoking cessation and implementation of evidence-based tobacco control measures in SMEs. The findings of this trial targeting management in SMEs have the potential to accelerate the implementation of evidence-based smoking cessation methods as well as abstinence rates among employees in SMEs across Japan. This article aims to analyse the COVID-19 coping strategies adopted by municipal health managers in two regions. A mixed method study of the sequential explanatory type was carried out with municipal health managers and primary care coordinators. The quantitative stage was developed with the application of an instrument to 42 managers to identify coping actions and associations with demographic and epidemiological data of COVID-19 in the municipalities. The results aimed at the definition of 15 participants and the collection of data in the qualitative stage, enabling approximation of the meta inferences of the study. Of the actions implemented, there were immediate actions to monitor cases, organize new care flows and encourage the population to adopt nonpharmacological measures. Regional articulation was the device that allowed for expansion and municipal autonomy for testing, reducing contamination and deaths among citizens. Municipal coping strategies that focused on the surveillance of cases and expansion of testing showed positive outcomes in terms of the number of infections and deaths from COVID-19. Abstract Background Using management competency-based frameworks to guide developing and delivering training and formal education to managers has been increasingly recognized as a key strategy in building management capacity. Hence, interest in identifying and confirming the competency requirements in various contexts have been witnessed. Therefore, learnings from how competency studies were designed and conducted, how competencies were identified, and strategies in ensuring success in competency identification are of great value to researchers planning and conducting competency studies in their own country. Methods A scoping review was conducted guided by the Arksey and O’Malley framework and reported according to the PRISMA Extension for Scoping Reviews (PRISMA-ScR). All papers that published empirical studies aiming at identifying and assessing manager’s competencies at the peer-reviewed journals were identified from Web of sciences, PubMed, Scopus and Emerald Management between 2000 and 2021. In order to maximize learning, studies focusing on health and non-health sectors are all included. Results In total, 186 studies were included in the review including slightly more than half of the studies conducted in health sector (54.5%). 60% of the studies focused on mid to senior level managers. Surveys and Interviews were the two most commonly used methods either solely or as part of the mix-method in the studies. Half of the studies used mixed methods approach (51.1%). Large proportion of the papers failed to include all information that is necessary to contribute to learning and improvement in future study design. Based on the results of the scoping review a four steps framework was developed that can guide designing and implementing management competency studies in specific country vs. sector context and to ensure benefits of the studies are maximised. Conclusion The review confirmed the increasing trend in investing in management competency studies and that the management competency identification and development process varied substantially, in the choice of methods and processes. The identification of missing information in majority of the published studies calls for the development of more rigorous guidelines for the peer-review process of journal publications. The proposed framework of improving the quality and impact of the future management competency study provides clear guidance to management competency identification and development that promotes the functional alignment of methods and strategies with intended uses and contexts.

Objective: To analyze the perspectives of health managers about the violence experienced by people with disabilities living in a rural context. Material and method: A descriptive-exploratory study with a qualitative approach, developed in nine municipalities in the north/northwest region of the State of Rio Grande do Sul, in which 18 health managers participated. Results: The first category reveals the conception of violence by participants, which is characterized by maltreatment, abandonment, neglect and lack of care. The second category addresses the weaknesses in the conduct of actions and the ways to qualify health work, associated with health planning, professional training and work through the Health Care Network Conclusions: Faced with the conceptions, it was evidenced that violence remains invisible in the rural context. A pandemic is the spread of an infectious agent or disease over a wide area, between countries, continents, or the whole world. Crises are unexpected events that require an immediate response from the organization. In this study, it is aimed to examine the perspectives of health managers on crisis management and solution proposals during the pandemic period. Within the scope of the study, in-depth interviews were conducted with seven health managers working in the management part of a public hospital in Istanbul, and the qualitative data obtained were divided into various categories and analyzed by content analysis method. It has been determined that health managers provide uninterrupted communication in crisis management, examine events on the spot, share problems with the crisis center team and develop solutions, and they do not experience great difficulties during crisis management. In today's ever-changing environment, health managers need to focus on promoting the health of populations. Therefore, they need a specialized understanding of epidemiology, as a requirement for population-based management. Epidemiology is acknowledged as an important component of undergraduate health management education, providing the knowledge and skills that health managers need in their professional lives. This cross-sectional descriptive study aims to assess the epidemiology courses in health management undergraduate programs in Turkey. The current curricula of 75 health management undergraduate programs, listed on the Program Atlas of the Higher Education Council, are reviewed through universities' official websites to profile the characteristics of epidemiology courses. Descriptive statistics was expressed as mean ± standard deviation for continuous variables, frequency and percentage values were used for categorical variables. 60 (80.0%) programs have epidemiology in their curricula. Yet the characteristics of these courses, such as their core/elective status, the semesters they are conducted, the weekly contact hours and ECTS credits, and the topics covered in the syllabus vary across programs. The findings reveal that there are variations regarding how epidemiology courses are conducted across health management undergraduate programs in Turkey. Apparently, this poses a challenge against all efforts to ensure the uniformity of the minimum core competencies among all health management program graduates; and although the Health Management National Core Education Program has been a great starting point towards standardization, there are further steps to be taken to improve the health management undergraduate curricula in Turkey. Abstract Background There is an immense difference between large companies and small and medium-sized enterprises (SMEs) in implementation of evidence-based interventions (EBIs). Previous literature reveals various barriers that SMEs face during implementation, such as a lack of time, accessibility, and resources. However, few studies have comprehensively examined those influential factors at multi-levels. This study aims to identify the factors influencing the implementation of non-communicable disease prevention activities (tobacco, alcohol, diet, physical activity, and health check-up) in SMEs using Consolidated Framework for Implementation Research (CFIR). Methods We conducted 15 semi-structured interviews with health managers and/or employers in 15 enterprises with less than 300 employees, and four focus groups among public health nurses/nutritionists of health insurers who support SMEs in four prefectures across Japan. A qualitative content analysis by a deductive directed approach was performed. After coding the interview transcript text into the CFIR framework constructs by two independent researchers, the coding results were compared and revised in each enterprise until an agreement was reached. Results Of the 39 CFIR constructs, 25 were facilitative and 7 were inhibitory for workplace health promotion implementation in SMEs, which were across individual, internal, and external levels. In particular, the leadership engagement of employers in implementing the workplace health promotion activities was identified as a fundamental factor which may influence other facilitators, including “access to knowledge and information,” “relative priority,” “learning climate,” at organizational level, and “self-efficacy” at the health manager level. The main barrier was the beliefs held by the employer/manager that “health management is one’s own responsibility.” Conclusions Multi-level factors influencing the implementation of non-communicable diseases prevention activities in SMEs were identified. In resource-poor settings, strong endorsement and support, and positive feedback from employers would be important for health managers and employees to be highly motivated and promote or participate in health promotion. Future studies are needed to develop context-specific strategies based on identified barriers and facilitative factors, and empirically evaluate them, which would contribute to narrowing the differences in worksite health promotion implementation by company size.

The role of the municipal manager is primordial in the planning, monitoring and evaluation of health actions in their municipality, among them the management of obesity, so this article aims to describe the perspective of primary care managers of the nine municipalities of the health regions of the state of Amazonas on the Obesity Care Line. This is a cross-sectional, census, descriptive and quali-quantitative study. The average age of the managers was 38 years, majority female (57.1%), and brown (71.4%). By the criteria used for the scoring of the analysis of obesity care by managers, the municipalities presented classification between "critical" and "incipient", that is, all presented absence of factors that influence obesity care. The themes mentioned by the managers reinforce the results, being them: Challenges in the Care Line Flow; Lack of Infrastructure of services and actions for the Management of Obesity and Work Process. It's primordial to reassess the model of assistance practiced, emphasizing the care and autonomy of the subjects, the team role, the public management, the particularities of each location to understand the multicausalities of the data and assist in the development of specific actions aimed at tackling obesity. Objective. To identify evidence in the literature on the response of managers regarding the organization of global healthcare systems to face the COVID-19 pandemic. Method. For this scoping review, searches were performed in 11 databases accessible through the Virtual Health Library. Eleven studies, published in 2019 and 2020, were selected based on the following guiding question: "How did managers act to organize healthcare systems to face the coronavirus pandemic?". The results were organized in terms of the categories outlined in the Pan-American Health Organization's Framework for the response of Integrated Health Service Delivery Networks to COVID-19: governance; model of care; organizational and management; financial allocations. Results. Managers in countries that invested and articulated actions in the categories of the Reference Framework, with coordination of care at the primary healthcare level, achieved better outcomes in coping with the COVID-19 pandemic. Conclusions. Situating the coordination of healthcare systems at the primary care level, preparing managers and ensuring the continued allocation of financial resources to healthcare are important factors to secure a satisfactory response to crises such as the covid-19 pandemic. During the Covid-19 pandemic, the smoker treatment service was impacted. This article evaluated the impact of the pandemic on the service in the SUS. This is an ecological descriptive observational study using the National Tobacco Control Program (PNCT) database for the years 2019, 2020 and 2021, from five states (PA, PB, GO, RJ and RS) for the second four months. Relative differences were estimated in the number of units that offered the service and in the number of consultations for clinical evaluation, first and fourth sessions. For the years 2019 and 2020, there was a percentage difference of -51.9% in the number of units that offered the service in all states studied. For the years 2019 and 2021, the relative difference for the health units was -20.96% and, for the clinical evaluation, the first and fourth sessions were: -54.19%, -55.07% and -61 .31%, respectively. These differences were greater for the capitals when compared to the other municipalities. Despite the negative impact, especially in the first year of the pandemic, the study showed that the services did not interrupt their activities and showed some recovery in 2021. Objective: to identify documentary evidence on permanent education actions for health managers arising from the information, decision and action needs triggered by the COVID-19 pandemic. Method: Scoping Review according to Joanna Briggs Institute, between December 2019 and July 2022. Results: 11 studies were selected, classified based on the categories proposed in the Health workforce policy and management in the context of the COVID-19 pandemic response, published by the World Health Organization/Pan American Health Organization. The most observed category was Interventions to support health professionals at the individual level. No studies were found with interventions focused on the organizational environment. Final considerations: there was a positive impact of permanent education actions on the improvement of the well-being of health professionals, on the adequate facing of clinical challenges and on the optimization of the care provided to patients during the pandemic. Objectives The purpose of this study is to investigate health managers' exposure to workplace violence (WPV) at construction sites and to review the statistical characteristics of experts in the safety and health fields using a gender-based analysis. Methods A preliminary survey of health managers at construction sites was conducted from March to April 2020 via the Health Managers Council in Construction Industry online community (https://band.us/band/62438722). Available national statistical data of safety and health personnel were reviewed. Results A total of 85 health managers (13 men, 72 women) responded to a preliminary survey. Health managers reported little physical violence (5.9%), while inappropriate work orders (83.5%) and sexual harassment (52.9%) were high, and health managers' experience of sexual harassment was significantly higher among women than men (P < 0.01). The professional safety and health field in Korea have a large gender gap, with few women. In particular, the proportion of women in the safety field was lower than in the health field. Conclusions Young female health managers in the male-dominated construction industry are more vulnerable to WPV, including sexual harassment. To reduce the risk of WPV exposure and gender discrimination among female health managers, it is necessary to increase regular employment and provide more practical WPV prevention education for all construction employees. The article aimed to understand the view of managers of Primary Health Care Units about the competencies needed to exercise this function. This is qualitative research, conducted in the network of primary health care in Maceió, Alagoas, Brazil, with the participation of eight managers. Semi-structured interviews were performed, and for the treatment of the material, we used the Reflexive Thematic Analysis. The analysis resulted in the themes resulted: area of training, territorialization, work processes, and flows, emotional preparation, leadership, active posture, empathic availability, and professional self-realization. It was considered that the competencies valued by the participants fit the emphasis of the accountability model and incentives for performance. In the field of primary health care, this suggests a retraction toward the implementation of comprehensive health care practices adopted as a political guideline since the consolidation of the Unified Health System, and a reorientation of the route that goes back to the model of care centered on the individual and procedures, limiting care to biomedical care—a phenomenon that has been increasing in recent years in Brazil. Municipal health secretaries are strategic actors in the construction of the Brazilian national health system - SUS, actively participating in the system's governance. The comparative analysis of the profile of municipal secretaries over three decades (1996, 2006, and 2017) was carried out using the database of three cross-sectional surveys carried out at the national level with structured questionnaires aimed at all municipal health secretaries in the country. The increase in female participation, greater diversity in ethnic and racial terms, greater professional qualification of managers, and the renewal of the generational profile, reiterates the advance of the democratization of these management spaces. Introduction: We hypothesized that there are gaps in knowledge about the managers of the Unified Health System (SUS) and that the term manager is used to designate different actors. Objective: To map the scientific production and synthesize the evidence on SUS managers from a scoping review. Method: Scoping review guided by PRISMA-ScR. The 'population, concept, context' approach was followed to formulate the research question: What are the characteristics of publications and the main scientific evidence on SUS managers? Studies were searched in scientific literature bases from the combination of 53 terms. Selection was performed by reading titles, abstracts, and methods, followed by full analytical reading. The data were submitted to the usual techniques of descriptive statistical analysis and content analysis. Results: 51 publications were analyzed. There was a predominance of qualitative studies carried out with municipal managers in the South and Southeast regions. Evidence was found on the profile, attributions, and attributes of the manager and, to a lesser extent, on the perceptions of managers about their role. Conclusions: There were different uses of the term manager, a reduced number of studies that take SUS managers as the focus of analysis, and scarcity of evidence on state and federal managers. The scoping review proved to be a useful and rigorous method for mapping evidence and identifying gaps in knowledge. [ABSTRACT FROM AUTHOR]

If we look around, we can easily notice that the world would not function without communication. How could we express our thoughts and what we want unless we say it, show it and touch it? Therefore, the business world could not function without interpersonal communication either, especially in the health institutions. The beginning of the research was based on the hypothesis that communication had to be open for the system to function, and employees satisfied with the working atmosphere and the relationship with the facility management. It was assumed that verbal communication was the most common form of communication, which often resulted in conflicts among health professionals. The research showed that the respondents were aware of the importance of creating effective communication, both within the same health care facility and outside it, especially conversation, assuming that they can convey the most important information in the shortest possible time. The data on formal communication at workplace indicated that employees believe that communication with their colleagues did not have to be formal, because in that way collective work would be more productive and the working atmosphere more pleasant. It is very important for every health care institution to invest enough funds in the activities, such as the health service promotion. Objective: Present the experience of a rapid response service to support decision-making in health systems., Methodology: Description of the processes and results of a service that produces rapid reviews and evidence maps to support decision-making under the National Health Promotion Policy, as well as the authors' perception of the work process., Results: The rapid response service started in 2020. By December 2023, 54 rapid reviews and five evidence maps had been produced, covering nine health promotion topics. These products were developed in 14 stages by a team made up of a coordinator, supervisors, proofreaders, and a librarian. The development of rapid responses involved a knowledge translation process, with continuous interactions between the requesting teams and production teams. Objective: to describe the technical-assistance arrangements developed within the scope of work management in the COVID-19 pandemic care network, from the managers' perspective. Method: a qualitative research study, of the incorporated single case type, conducted with 23 managers of a Health Care Network. The analysis was applied in two thematic coding cycles, with the aid of the ATLAS. ti software. Results: the arrangements were analyzed in categories related to health care; management; incorporation of technologies; implementation of a field hospital; and retrospective analysis of the experiences as a whole. There was emphasis on the implementation of care flows, virtual health bulletins, Telemonitoring, chatbots, use of applications, and implementation of field hospitals and of basic urgency services within the scope of the Basic Health Units. Hyperjudicialization in the system was identified; as well as weaknesses in information management, intersectoriality and technical-political leadership at the national level; the role of nurses in management positions and for coping with the pandemic. Conclusion: despite the health services' unpreparedness to face the pandemic, the actors' resilience promoted dynamism and technical-assistance arrangements in the context of management and humanized care. The objective was to characterize the profile of the managers of the Basic Health Units (BHU) with teams of the Family Health Strategy in Maceió, Alagoas, Brazil. This is a descriptive cross-sectional study carried out with 31 BHU managers located in eight health districts. Sociodemographic data were collected. This data is related to gender; age; type of employment; training area; experience in the area of management and time spent in the role of manager. It was identified that the managers are, for the most part, females (67.7 %); aged between 30 and 39 years (35.5 %); with higher education in the area of applied social sciences (42.9 %); with work experience in commissioned functions (93.6 %); with experience in management without being in health (38.7 %) and with less than one year in the exercise of the function (29 %). It is observed that the municipality complies with the recommendations of the National Primary Care Policy –all BHU have a manager. as well as that most of professionals had the minimum requirements to perform the function. Nevertheless, there was a high number of managers with unstable and insecure employment contracts and exercising the management function without experience in the health care area. Both, the immediate definition of specific criteria for selection and the degree of professionalization are considered necessary, as well as the prioritization of employment contracts that ensure the rights of the worker and a greater stability in function for these professionals. Amaç: Bu çalışma, sağlıkta dönüşüm programının çatı uygulaması olan kamu hastane birlik yapılanmasının hastane hizmetleri üzerindeki değişim etkisini yönetici ve çalışan görüşleri üzerinden mukayeseli araştırmak olup 2017 yılında sistemde yapılan değişikliği bu veriler üzerinden tartışmaktır. Yöntem: Araştırmada verilerin toplanmasında dörtlü Likert yapıda bir anket formu kullanılmıştır. Bostan ve arkadaşları (2012) tarafından sağlıkta dönüşüm programının hastane hizmetleri üzerindeki değişim etkisini ölçmek üzere geliştirdikleri ölçek bu çalışmaya uyarlanarak kullanılmıştır. Üç kamu hastaneleri birliği yapılanmasına bağlı yedi hastaneden 325 yönetici ve sağlık çalışanı katılımıyla veriler toplanmıştır. Ölçeğin güvenirlik ve geçerliliği test edildikten sonra veriler tanımlayıcı istatistiksel yöntemler ile bağımsız örneklerde t testi kullanılarak analiz edilmiştir.Bulgular: Uygulama sonuçlarına göre, sağlık yöneticileri ve sağlık çalışanları kamu hastane birlikleri uygulamasıyla kamu hastanelerinde meydana gelen yönetsel alandaki değişimin olumlu yönde (2,75/2,73) gerçekleştiğini, ortaya çıkan sayısal değişimlerin artış yönünde (2,52/2,53) olduğunu, genel olarak hizmet bileşenlerinin öneminin (3,09/2,86) artığını ifade etmişlerdir. Ayrıca, hem sağlık yöneticileri hem de sağlık çalışanlarına göre kamu hastanelerindeki sağlık çalışanı açısından değişimin olmadığını (2,31/2,28) ve kamu hastaneleri hakkında kamuoyundan duyduğu görüşlerin olumlu (2,81/2,72) olduğu görülmüştür.Sonuç: Araştırma bulgularına göre; kamu hastane birlikleri uygulamasının sağlık çalışanı ve hastane yöneticilerinin ifadelerine göre, önemli ölçüde benimsendiği, olumlu yönde değişimin sağlandığı, hasta memnuniyetinde artış olduğu, etkin ve yerinden yönetimin öne çıktığı anlaşılmıştır. Bununla birlikte, sağlık çalışanlarının dönüşümden yeterli pay alamadıkları söylenebilir.

Social participation can occur through institutionalized channels, from conferences and municipal health councils or non-institutionalized, performed in other social spaces for the exercise of democracy in the State and in decision making on public policy proposals. Investigate the involvement of the community in the planning, monitoring and control of the activities of health in the municipalities of the 28th Health Region of Rio Grande do Sul. Qualitive research with health managers of eleven municipalities of a region of Southern Brazil. The data were collected through semi-structured interviews and analyzed using content analysis. For the participants, the social control occurs at institutionalized spaces, especially in the municipal health councils. The majority of managers assigned to municipal health council as a supervisory body and linked the lack of interest of the population to lack of knowledge. The purpose of the study is to investigate which sources and how competition intelligence (CI) is collected in hospitals, and what are the environmental factors affecting the competitive intelligence activities. And it consists of presenting information on what kind of purposes hospitals use competitive intelligence and a description of how and in what way C.I found a place in the organizational structure in the hospitals that constitute the cases of the study. Multiple case analysis and theoretical sampling logic were used in the study. Through the purpose of the study, according to the data of the health statistics yearbook published by the Ministry of Health in 2017, the Marmara Region, where the population and the number of hospitals are the highest, was selected as the research area. Seven of the hospitals in this region were reached and semi-structured interviews were conducted with eight managers using qualitative research method. The questions were prepared according to the focal points determined to inquire about the hospitals' competitive intelligence activities. The categories developed in this framework and the relationships between these categories were examined with qualitative content analysis. As a result of the study, it was determined that the hospitals that constitute the cases of the study did not have a special competition intelligence unit. It was concluded that the competitive intelligence process took place unofficially and that environmental factors have a significant impact on intelligence practices. In this context, it has been found that competitive intelligence is generally used in tactical decisionmaking activities and that there are sector-specific competition conditions that hospitals should pay attention to in their activities. Another important outcome of the study is that tactical decision-making and environmental features, depending on short-term thinking among the cases, are not yet fully established in the institutional process of the concept of C.I in hospitals. Although there is a certain template in the minds of executives related to C.I, the lack of institutionalization of the concept causes a dilemma regarding C.I among the cases and this dilemma increases the clinging of cases to short-term tactical decisions rather than long-term thinking. The study aims to determine the managerial resourcefulness of health managers and to measure whether there is any difference between male and female managers working in health institutions and organizations in terms of managerial resourcefulness. The Managerial Resourcefulness Scale developed by Şahin (2009) is used to collect data, and parametric and nonparametric difference analysis methods are used in the study. In the study, convenience sampling method is utilized. In the scope of field research, a survey is applied to 111 top managers and department managers working in health institutions and organizations in Isparta province. As a result of the research, the managerial resourcefulness levels of health managers are found to be medium level. It is also observed that variables such as education level and length of service in management make a significant difference on the level of managerial resourcefulness of health managers. Aim: Bring to discussion spiritual leadership and workplace spirituality on a health care system that faces constant challenges and seeks constant adaptations, as a way to guarantee nurses' well‐being and quality of care. Background: The work environment has shown to have impact on staff well‐being. Workplace spirituality relates to sense of belonging, motivation and commitment. A spiritual leadership is fundamental to develop workplace spirituality. Evaluation: Starting from literature, a reflection on the theme was carried out based on the results of the development of spiritual leadership and workplace spirituality in health care institutions, in professionals and in patients. Key issues: Nurses have spiritual needs which need to be also addressed in order to promote the sense of identification with the institutions' vision and goals. A relation between spiritual leadership, workplace spirituality and subjective well‐being is often found in literature, and this is critical evidence towards new management and leadership dynamics and models in health care institutions that should integrate workplace spirituality. Conclusion: Nursing leaders are responsible for workplace spirituality facilitation. Nursing leadership and workplace spirituality seem both an answer and way to the change of health institutions management paradigm, but more studies are needed to inform this change in practice. Implications for Nursing Management: Workplace spirituality must be promoted in all health care institutions, aiming the humanization of care and teams.Nursing leaders must have spiritual competences and must include the promotion of workplace spirituality in daily agenda as a foundational area in management.The health care institutions' managers should consider the best leaders who should facilitate workplace spirituality. ABSTRACT Objectives: to analyze the perception and conflict management strategies used by nurses in the management of people in Portuguese health services. Methods: descriptive, correlational study, carried out in Portuguese health services, with an intentional non-probabilistic sample, totaling 95 nurse managers. A questionnaire and Conflict Management Scale were used, analyzing the variables of managerial activities and conflict management, with the aid of software. Results: it was identified that 60% of the managers, report having to mediate conflicts daily, and the majority report adopting dialogue in conduct. However, through the Kruskal-Wallis test, it was shown that enforcement strategies in conflict management prevail (p = 0.008), with collaborative ones being more restricted to monthly intervals (p = 0.049). Conclusions: managers perceive the importance of collaboration in the mediation of conflicts, however, in their daily lives; they tend to maintain imposing behaviors, signaling for a little transformational leadership style.

Abstract Background Strong management skills are key to improving performance of health systems. Action learning, a technique to develop management skills, has been used successfully with health managers but not usually among lower level managers or in low and middle-income countries (LMICs). Methods This study uses a qualitative approach to explore experiences, successes and challenges of using an action learning approach to improve skills of managers in neonatal units in KwaZulu-Natal (KZN), South Africa. Eight action learning groups were convened with neonatal unit managers from all 39 district hospitals in KZN, each group had 4–6 participants. Meetings were conducted by a facilitator trained in action learning techniques, and groups met a minimum of ten times over a one-year period. After completion of the intervention, 14 in-depth interviews were conducted with purposively selected action learning participants. Data was transcribed and analysed using framework analysis. Results Neonatal unit managers found that action learning generated a sense of empowerment in their abilities, trust and confidence among participants was nurtured, problem solving and critical thinking skills were developed, and a continuous support system was created. The action learning process led to several positive changes in neonatal units, which enhanced the quality of care for patients. A number of challenges were also identified, mainly relating to administrative issues such as the provision of a skilled facilitator, permission to attend action learning meetings and logistical issues, including transport and other financial implications. Conclusions This paper illustrates that action learning can be an effective and practical method to support public health workers to manage their health units despite the challenges associated with the method. Time, energy and financial resources used to facilitate action learning for this cadre of health workers is rewarded by improved skills of managers and better quality of care for patients.
