9. Deep learning based automatic segmentation of lumbosacral nerves on non-contrast CT for radiographic evaluation: a pilot study

Abstract

Background and objective: Combined evaluation of lumbosacral structures (e.g. nerves, bone) on multimodal radiographic images is routinely conducted prior to spinal surgery and interventional procedures. Generally, magnetic resonance imaging is conducted to differentiate nerves, while computed tomography (CT) is used to observe bony structures. The aim of this study is to investigate the feasibility of automatically segmenting lumbosacral structures (e.g. nerves & bone) on non-contrast CT with deep learning. Methods: a total of 50 cases with spinal CT were manually labeled for lumbosacral nerves and bone with Slicer 4.8. The ratio of training: validation: testing is 32:8:10. A 3D-Unet is adopted to build the model SPINECT for automatically segmenting lumbosacral structures. Pixel accuracy, IoU, and Dice score are used to assess the segmentation performance of lumbosacral structures. Results: the testing results reveals successful segmentation of lumbosacral bone and nerve on CT. The average pixel accuracy is 0.940 for bone and 0.918 for nerve. The average IoU is 0.897 for bone and 0.827 for nerve. The dice score is 0.945 for bone and 0.905 for nerve. Conclusions: this pilot study indicated that automatic segmenting lumbosacral structures (nerves and bone) on non-contrast CT is feasible and may have utility for planning and navigating spinal interventions and surgery., Comment: 11 pages, 5 figures

Clinical outcomes collected for analysis include operative time, hospital stay time, Visual Analog Scale, and Oswestry Disability Index before and after the surgery, the global outcome based on the Macnab outcome criteria.Results: Seventeen males and nineteen females with a mean age of 73.72 ± 7.15 were included in this study. Radicular pain was the main complaint of all the patients with the least symptom duration of two months. Radiological findings showed that 80.6% of the patients with multilevel disc herniation, 16.7% received lumbar fusion surgery before, and 8.3% with degenerative scoliosis. Abstract

Peripheral nerve injuries often result in severe personal and social burden, and even with surgical treatment, patients continue to have poor clinical outcomes. Over the past two decades, electrical stimulation has been shown to promote axonal regeneration and alleviate refractory neuropathic pain. The aim of this study was to analyse this field using a bibliometric approach. Literature was searched through Web of Science Core Collection (WOSCC) for the years 2002–2023. Literature analysis included: (1) Describing publication trends in the field. (2) Exploring collaborative network relationships. (3) Finding research advances and research hotspots in the field. (4) Summarizing research trends in the field. With the number of studies in this field still increasing, a total of 693 publications were included in the analysis. This field of research is interdisciplinary in nature. Research hotspots include peripheral nerve regeneration, the treatment of neuropathic pain, materials for nerve injury repair, and the restoration of sensory function in patients with peripheral nerve injury. Correspondingly, the development of nerve conduits and systems for peripheral nerve electrical stimulation, clinical trials of peripheral nerve electrical stimulation, and tactile recovery and movement for amputees have shown significant promise as future research trends in this field. Abstract

Background: In this study, an apple foam net packing machine is designed to improve the efficiency of apple foam net packing, reduce the intensity of manual work, and meet the market demand for diverse forms of apple packing. Methods: Based on the analysis of apple shape characteristics, a secondary posture adjustment method for apples with a vertical posture adjustment mechanism and a side posture adjustment mechanism is proposed. Based on the observation of human hand-set nets and the analysis of the mechanical characteristics of nets, a dual-system net opening method combining a net slackening system and a net opening system is proposed. Results: The experiment shows that: the apple foam net sleeve packing machine can reach 99% of the apple's posture adjustment rate, the net opening rate can reach 99%, and the packing rate can reach 98.10%. The packing speed is 10 s/pcs, and the packing efficiency is 355–365 pcs/h. The length of the fused mesh net sleeve is mainly distributed between 138 and 143 mm, which accounts for approximately 80% of the total number of samples. Conclusions: The apples packed by this machine are neatly packaged according to specifications. It can improve the efficiency of apple foam net sleeve packing and replace manual work.
