Objective To investigate the effect of care bundles based on the concept of enhanced recovery after surgery in perioperative nursing of patients undergoing laparoscopic cholecystectomy. Methods A total of 260 cases of laparoscopic cholecystectomy were randomly divided into observation group (n=130, received routine nursing care) and control group (n=130, received care bundles based on the concept of enhanced recovery). Postoperative recovery condition, incidence of complications and postoperative pain were compared between the two groups. Results At 2, 6 and 24 h after surgery, the Visual Analogue Scale of pain in the observation group were lower than those in the control group, with a significant difference(P＜0. 05). The time to first postoperative exhaustion, time to off-bed activities and hospital stays in the observation group were shorter than those in the control group (P＜0. 05). The incidence of vomiting, incision infection, and bile leakage in the control group was significantly higher than that in the observation group, with a significant difference (P＜0. 05). Conclusion The application of care bundles based on the concept of enhanced recovery after surgery are conducive to the postoperative recovery of patients undergoing laparoscopic cholecystectomy, as well as the improvement of postoperative quality of life. Objective To investigate the application effect of establishment of chief nurse position in training of newly recruited nurses. Methods The chief nurse position was established for the duty management of newly recruited nurses. Totally 45 newly recruited nurses who participated in the clinical practice after establishment of chief nurse position were included as the study, another 45 newly recruited nurses who received routine clinical teaching and practice were included as the control group. The nurses’ job satisfaction, anxiety states and turnover intention were measured and compared between two groups. Results In job satisfaction assessment, scores of Work-Family Balance, Relationship with colleagues, control and responsibility; Being Praised and Recognized and Responsibility Assignment items in the study group were higher than those in the control group respectively(P＜0. 05). The Self-rating Anxiety Scale in the study group was lower than that in the control group(P＜0. 05). The rate of turnover intention in the study group was lower than that in the control group(P＜0. 05). Conclusion The establishment of chief nurse position, by increasing job security, is effective to improve the nurses’ confidence and quality of nursing practice, reduce the fear of working independently and turnover intention, enhance the nursing safety in nursing practice. (目的 探讨科室总护士岗位设立在新入职护士值班管理中的应用效果。方法 科室通过设立总护士岗位加强新入职护士值班管理, 采用目的抽样法将本科室2020年7月—2022年7月设立总护士岗位以来培养的45名新入职护士作为研究组, 选取医院其他科室2020年7月—2022年7月以传统带教方式培养的新入职护士45名作为对照组。采用护士工作满意度量表、焦虑自评量表(SAS)、离职意愿量表进行对比分析。结果 度评价条目8个条目中, 工作与家庭平衡、与同事关系、控制与责任、被称赞与被认可和职责分配条目研究组得分高于对照组, 差异有统计学意义(P＜0. 05)。研究组SAS量表评分低于对照组(P＜0. 01)。研究组离职意愿比例低于对照组(P＜0. 05)。结论 设立科室总护士岗位能够增加新入职护士安全感, 提高了护士工作的自信心和护理工作质量, 降低新入职护士对独立值班的恐惧, 降低新入职护士的离职率, 提高独立值班中的护理安全。)

Background Functional dyspepsia (FD) after ischemic stroke belongs to the category of post-stroke gastrointestinal dysfunction, which is prevalent in middle-aged and elderly patients. Stroke patients may differ from the general population in the influencing factors of FD due to their own somatic and psychological changes. Objective To investigate the associated factors of the severity of functional dyspepsia after ischemic stroke. Methods A total of 180 middle and old aged patients with functional dyspepsia after ischemic stroke were selected from March 2017 to July 2018 in the Department of Acupuncture of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine. A self-designed dyspepsia symptom score scale was used to evaluate the severity of FD during hospitalization. The patients were divided into mild FD group (score≤7, n=115) and moderate-severe FD group (score>7, n=65) according to the dyspepsia symptom score. Patient data were collected through the medical record system, and multivariate Logistic regression were used to explore the influencing factors of functional dyspepsia severity after ischemic stroke. Results The FD symptom scores of 180 patients was in the range of 3-12 points, with an average of (7.2±1.7) points. Among them, 115 patients (63.9%) had symptom score≤7 points, and 65 patients (36.1%) had symptom score>7 points. With the development of deep learning technology, convolutional neural networks have made great progress in the field of image segmentation. However, for complex scenes and multi-scale target images, the existing technologies are still unable to achieve effective image segmentation. In view of this, an image segmentation model based on residual connection and feature fusion is proposed. The model makes comprehensive use of the deep feature extraction ability of residual connections and the multi-scale feature integration ability of feature fusion. In order to solve the problem of background complexity and information loss in traditional image segmentation, experiments were carried out on two publicly available data sets. The results showed that in the ISPRS Vaihingen dataset and the Caltech UCSD Birds200 dataset, when the model completed the 56th and 84th iterations, respectively, the average accuracy of FRes-MFDNN was the highest, which was 97.89% and 98.24%, respectively. In the ISPRS Vaihingen dataset and the Caltech UCSD Birds200 dataset, when the system model ran to 0.20s and 0.26s, the F1 value of the FRes-MFDNN method was the largest, and the F1 value approached 100% infinitely. The FRes-MFDNN segmented four images in the ISPRS Vaihingen dataset, and the segmentation accuracy of images 1, 2, 3 and 4 were 91.44%, 92.12%, 94.02% and 91.41%, respectively. In practical applications, the MSRF-Net method, LBN-AA-SPN method, ARG-Otsu method, and FRes-MFDNN were used to segment unlabeled bird images. The results showed that the FRes-MFDNN was more complete in details, and the overall effect was significantly better than the other three models. Meanwhile, in ordinary scene images, although there was a certain degree of noise and occlusion, the model still accurately recognized and segmented the main bird images. The results show that compared with the traditional model, after FRes-MFDNN segmentation, the completeness, detail, and spatial continuity of pixels have been significantly improved, making it more suitable for complex scenes. Hyperuricemia, a major worldwide burden on public hygiene, is closely connected with dietary habits. However, few studies have evaluated the association of dietary diversity with hyperuricemia. To preliminarily reveal the status of a diversified diet in preventing hyperuricemia based on a neighborhood-based, massive-scale cohort in China, a total of 43,493 participants aged 20–74 years old, with no history of hyperuricemia at baseline, were enrolled in the research from April 2016 to December 2019. The Dietary Diversity Score (DDS) was utilized to evaluate the dietary variety and split the participants into the low-, medium-, and high-DDS groups. Information on participants was connected to regional health information systems that acquired data on hyperuricemia instances up to 28 February 2023. Hazard ratios (HRs) and 95% confidence intervals (CIs) were computed by Cox proportional hazards models. Restricted cubic splines (RCS) were implemented to analyze dose–response correlation. A total of 1460 individuals with newly diagnosed hyperuricemia were observed over a median follow-up period of 5.59 years. Compared to the low-DDS group, HRs for the medium- and high-DDS groups were 0.87 (95% CI 0.76–0.99) and 0.80 (95% CI 0.70–0.91) in the fully adjusted model, respectively. The risk of hyperuricemia incidence was reduced by 5% for each 1 unit of DDS increase. A linear correlation of DDS with hyperuricemia emerged and further revealed that the intake of 8–10 broad categories of food could decrease the incidence of hyperuricemia. Our results validate the dietary principle of "food diversification" recommended in guidelines. Chickpeas (Cicer arietinum L.) are an important legume crop known for their rich nutrient content, including proteins, carbohydrates, and minerals. Thus, they are enjoyed by people worldwide. In recent years, the production scale of chickpeas has been growing gradually. The planting area of chickpeas represents roughly 35–36% of the total planting area, and the output of the beans is roughly 47–48%. However, the growth and development process of chickpeas is limited by a number of factors, including high temperature, drought, salt stress, and so forth. In particular, high temperatures can reduce the germination rate, photosynthesis, seed setting rate, and filling rate of chickpeas, restricting seed germination, plant growth, and reproductive growth. These changes lead to a decrease in the yield and quality of the crop. Heat shock proteins (HSPs) are small proteins that play an important role in plant defense against abiotic stress. Therefore, in the present study, HSP20 gene family members were identified based on the whole-genome data of chickpeas, and their chromosomal positions, evolutionary relationships, promoter cis-acting elements, and tissue-specific expression patterns were predicted. Subsequently, qRT-PCR was used to detect and analyze the expression characteristics of HSP20 genes under different temperature stress conditions. Ultimately, we identified twenty-one HSP20 genes distributed on seven chromosomes, and their gene family members were found to be relatively conserved, belonging to ten subfamilies. We also found that CaHSP20 promoter regions have many cis-acting elements related to growth and development, hormones, and stress responses. In addition, under high-temperature stress, the relative expression of CaHSP20-17, CaHSP20-20, CaHSP20-7, CaHSP20-3, and CaHSP20-12 increased hundreds or even thousands of times as the temperature increased from 25 °C to 42 °C. Among them, excluding CaHSP20-5, the other five genes all contain 1-2 ABA cis-regulatory elements. This finding indicates that CaHSP20s are involved in the growth and development of chickpeas under heat stress, and the mechanisms of their responses to high-temperature stress may be related to hormone regulation. The results of the present study lay the foundation for exploring HSP20 gene family resources and the molecular mechanisms of heat resistance in chickpeas. Our results can also provide a theoretical basis for breeding high-temperature-resistant chickpea varieties and provide valuable information for the sustainable development of the global chickpea industry.
