The increased use of illicit drugs continues to lead to the discovery of various unexpected pathologies. This 7-month-old infant died suddenly at home. Pulmonary artery fibrinoid necrosis, diffuse fatty liver changes, and skin rash were the main histologic postmortem findings. Postmortem urine contained traces of methamphetamine. Methamphetamine was smoked by the parents. Fibrinoid necrosis has been described with inhaling methamphetamine and can result in fibrinoid angiitis such as in this case. Although this did not result in pulmonary hemorrhage or could be directly related to death, it does suggest that pulmonary artery fibrinoid necrosis may develop with passive inhalation of methamphetamines. Nevus is a benign melanocytic tumor which occurs anywhere in the skin and is considered to be the most common type of skin tumor. Albeit rare, its manifestation over the external auditory canal has been reported. Herein, we present a case of a previously healthy middle-aged female who presented with insidious onset of left-sided aural fullness and tinnitus. Otoscopic finding revealed a peculiar-looking blackish polypoidal mass occupying left ear canal. Histopathological examination of the excised polypoidal mass turned out to be intradermal melanocytic nevi. Melanocytic nevi in the ear canal may presents as polypoidal mass in a long-standing case. Any suspicious mass over the external auditory canal warrants excision along with histopathological examination of the excised mass as to exclude the possibility of malignancy. Constitutional symptoms are the most common clinical manifestation of the systemic disease. When there is the presence of cervical lymphadenopathy concurrently, systemic infection or malignancy has to be taken into consideration. At the paediatric or adolescent age group, tuberculosis and lymphoma are the common diseases that associated with these symptoms. We should consider patient's age group, risk factors and the disease presentation for the diagnosis.
