Physical activity (PA) may modify the genetic effects that give rise to increased risk of obesity. To identify adiposity loci whose effects are modified by PA, we performed genome-wide interaction meta-analyses of BMI and BMI-adjusted waist circumference and waist-hip ratio from up to 200,452 adults of European (n = 180,423) or other ancestry (n = 20,029). We standardized PA by categorizing it into a dichotomous variable where, on average, 23% of participants were categorized as inactive and 77% as physically active. While we replicate the interaction with PA for the strongest known obesity-risk locus in the FTO gene, of which the effect is attenuated by ~30% in physically active individuals compared to inactive individuals, we do not identify additional loci that are sensitive to PA. In additional genome-wide meta-analyses adjusting for PA and interaction with PA, we identify 11 novel adiposity loci, suggesting that accounting for PA or other environmental factors that contribute to variation in adiposity may facilitate gene discovery.

Abstract Background The incorporation of annotated sequence information from multiple related species in commonly used databases (Ensembl, Flybase, Saccharomyces Genome Database, Wormbase, etc.) has increased dramatically over the last few years. This influx of information has provided a considerable amount of raw material for evaluation of evolutionary relationships. To aid in the process, we have developed JCoDA (Java Codon Delimited Alignment) as a simple-to-use visualization tool for the detection of site specific and regional positive/negative evolutionary selection amongst homologous coding sequences. Results JCoDA accepts user-inputted unaligned or pre-aligned coding sequences, performs a codon-delimited alignment using ClustalW, and determines the dN/dS calculations using PAML (Phylogenetic Analysis Using Maximum Likelihood, yn00 and codeml) in order to identify regions and sites under evolutionary selection. The JCoDA package includes a graphical interface for Phylip (Phylogeny Inference Package) to generate phylogenetic trees, manages formatting of all required file types, and streamlines passage of information between underlying programs. The raw data are output to user configurable graphs with sliding window options for straightforward visualization of pairwise or gene family comparisons. Additionally, codon-delimited alignments are output in a variety of common formats and all dN/dS calculations can be output in comma-separated value (CSV) format for downstream analysis. To illustrate the types of analyses that are facilitated by JCoDA, we have taken advantage of the well studied sex determination pathway in nematodes as well as the extensive sequence information available to identify genes under positive selection, examples of regional positive selection, and differences in selection based on the role of genes in the sex determination pathway. Conclusions JCoDA is a configurable, open source, user-friendly visualization tool for performing evolutionary analysis on homologous coding sequences. JCoDA can be used to rapidly screen for genes and regions of genes under selection using PAML. It can be freely downloaded at http://www.tcnj.edu/~nayaklab/jcoda.

Design: Prospective, randomized, double-blind, controlled, parallel group clinical trial. Setting: Two urban emergency departments. Participants: Twenty consecutive patients who presented to the ED with acute gout. Interventions: Each patient was randomly assigned to receive in the ED (1) 60 mg of IM ketorolac and oral placebo or (2) 50 mg of oral indomethacin and IM placebo. The patients rated the intensity of their pain on a Wong-Baker pain scale (which runs from 0 to 5) before treatment and 30, 60, 90, and 120 minutes after treatment. All the patients were discharged with instructions to take oral indomethacin and to complete pain score cards at home at 6, 12, and 24 hours. Results: The 10 patients in each group were similar with regard to age, sex, race, and initial mean pain score. - Abstract
Leptin is an adipocyte-secreted hormone, the circulating levels of which correlate closely with overall adiposity. Although rare mutations in the leptin (LEP) gene are well known to cause leptin deficiency and severe obesity, no common loci regulating circulating leptin levels have been uncovered. Therefore, we performed a genome-wide association study (GWAS) of circulating leptin levels from 32,161 individuals and followed up loci reaching P <10(-6) in 19,979 additional individuals. We identify five loci robustly associated (P <5 x 10(-8)) with leptin levels in/near LEP, SLC32A1, GCKR, CCNL1 and FTO. Although the association of the FTO obesity locus with leptin levels is abolished by adjustment for BMI, associations of the four other loci are independent of adiposity. The GCKR locus was found associated with multiple metabolic traits in previous GWAS and the CCNL1 locus with birth weight. Knockdown experiments in mouse adipose tissue explants show convincing evidence for adipogenin, a regulator of adipocyte differentiation, as the novel causal gene in the SLC32A1 locus influencing leptin levels. Our findings provide novel insights into the regulation of leptin production by adipose tissue and open new avenues for examining the influence of variation in leptin levels on adiposity and metabolic health.

Cardiac tamponade is a life threatening condition that results from accumulation of intrapericardial fluid or air, leading to impaired cardiac filling and consequent reduction of cardiac output. There are many causes, best categorised as non-traumatic and traumatic. The diagnosis is most commonly made on echocardiographic evidence of effusion together with the clinical findings. Clinical findings, however, are non-specific and often unreliable especially early in the process of cardiac tamponade. A high index of suspicion must therefore be maintained. Definitive treatment consists of pericardiocentesis or surgical drainage, the choice of which will depend upon the urgency of the situation, the aetiology and the available facilities and personnel. - Abstract
The application of magnetohydrodynamic (MHD) devices to re-entry vehicle control requires estimation of the values of certain critical parameters along typical re-entry trajectories. A map giving the values of th interaction parameter, magnetic Reynolds number, Hall coefficient, ion slip parameter and ratio of magnetic to aerodynamic pressure is presented and evaluated. These parameters allow one to assess the types of interaction possible between magnetic fields and the flow. Flow calculations are presented of the interaction between the magnetic field due to either wires or coils below the surface and an ionized gas flow. Initially channel flow calculation were made to simulate the flow between a surface and a shock wave, and later more realistic geometries were considered. The force on the wire or coil and the resulting change in pressure and surface heating on the wall are calculated. The possibility of finding practical applications such as the magnetic flap, for these interactions between magnetic fields and ionized gas flow depends strongly on the weights necessary to produce the magnetic fields required by the calculations. The development of superconducting coils makes the production of strong magnetic fields for relatively small weights possible. Estimates are given for the weights of superconducting coils required to produce t e needed field strength. (Author)

Physical activity (PA) may modify the genetic effects that give rise to increased risk of obesity. To identify adiposity loci whose effects are modified by PA, we performed genome-wide interaction meta-analyses of BMI and BMI-adjusted waist circumference and waist-hip ratio from up to 200,452 adults of European (n = 180,423) or other ancestry (n = 20,029). We standardized PA by categorizing it into a dichotomous variable where, on average, 23% of participants were categorized as inactive and 77% as physically active. While we replicate the interaction with PA for the strongest known obesity-risk locus in the FTO gene, of which the effect is attenuated by ~30% in physically active individuals compared to inactive individuals, we do not identify additional loci that are sensitive to PA. In additional genome-wide meta-analyses adjusting for PA and interaction with PA, we identify 11 novel adiposity loci, suggesting that accounting for PA or other environmental factors that contribute to variation in adiposity may facilitate gene discovery.

Since the events of September 11th 2001, the United States Special Operations Command (USSOCOM) has taken the lead in fighting the Global War on Terror. In spite of this new charter, however, USSOCOM continues to conduct operations with antiquated command and control (C2) doctrine and structures. This failure to adapt has greatly hindered the ability of SOF to prosecute the war on terrorism with the necessary efficiency, flexibility and agility. When a Joint Task Force (JTF) is formed, the Theater Special Operations Command (TSOC) is normally tasked to form a joint special operations task force, or JSOTF, to command and control all special operations forces (SOF) in the JTF. A Joint Force Commander forms this organization on an ad hoc basis, usually cobbled together from the TSOC staff, the regionally oriented Special Forces Group, Naval Special Warfare Task Unit, or some other entity. Consequently, JSOTF staff members face a steep “learning curve” upon the headquarters’ formation, since in all likelihood they will not have trained or worked together before. This ensuing lack of efficiency and cohesion, at least during the initial stages of the JSOTF’s formation, causes unnecessary friction and turbulence during the conduct of operations. A likely solution to SOF’s command and control dilemma is a standing JSOTF (SJSOTF) headquarters. By forming these headquarters on a permanent basis, U.S. SOF would possess a cohesive, well-trained C2 node free from the organizational and administrative problems associated with ad hoc structures. This monograph explores an alternative way of providing a JSOTF headquarters to the Joint Force Commander. Beginning with a history of joint SOF doctrine, the monograph outlines the evolution of special operations command and control and the subsequent need for a standing operational-level headquarters. Having identified this void in capability, the monograph then presents a model for a SJSOTF headquarters possessing a full-time complement
