Abstract

Progressive supranuclear palsy (PSP) is a degenerative brain disorder in which the patient develops many neurological symptoms including dementia and unstable posture. The term 'supranuclear palsy' refers to the fact that the paralysis results from lesions above the level of brainstem nerve nuclei. Pathological examination reveals damage in many parts of the brain. Abnormal findings such as neurofibrillary tangles, loss of brain cells, and gliosis are found in the globus pallidus, subthalamic nucleus, and other parts of the brain. Abnormalities of eye movements are also part of the syndrome of PSP, and some impairments get worse as the PSP progresses. Since much of the control of eye movements is centered in a part of the brain called the pontine reticular formation, this region was singled out for a detailed anatomical study. The brains of three patients with PSP and four control patients were examined. The brain regions were stained using immunocytochemical techniques, and antibodies that are specific for the enzyme choline acetyltransferase (ChAT). Two nuclei, or clusters of nerve cell bodies, were identified that were abnormal when compared with the control tissues. The nucleus papilloformis and the nucleus pontis centralis caudalis both stained with the ChAT antibodies. However, there were 46 and 60 percent fewer neurons than normal in the nucleus papilloformis and the nucleus pontis centralis caudalis, respectively. Since ChAT is regarded as a reliable indicator of nerve systems using the neurotransmitter substance acetylcholine, these results would indicate that cholinergic systems that serve eye movements are profoundly affected by the PSP disease process. However, it is not clear from the present study whether all nerve cells in this region are affected by PSP or whether the cholinergic systems are preferentially singled out for damage. Abstract

International audience; Recently, several reports underlined the usefulness of brain MRI for the diagnosis ofCreutzfeldt-Jakob dis ease. FLAIR sequence and diffusion-weighted imaging (DWI) areconsidered as high sensitive sequences to detect signal alteration of the cortex and thedeep grey matter. Recent advances in therapeutic approach of patients with prion diseaseshave emphasized the need for earlier diagnostic markers that would authorize the onset oftreatment before massive and irreversible lesions of the brain have occurred.Consequently, we designed a radio -clinical study using a multimodality MRIstandardized procedure that aimed to estimate differential sensitivity of FLAIR, DWI andMR spectroscopy for the diagnosis of human TSE. Here we report a case of familial fatalinsomnia with the D178N-129M mutation. FLAIR and diffusion-weighted sequenceswere normal in the whole brain notably in both thalami. However, spectroscopic studyshowed a striking increase of the peak of myo-inositol (mI) and of the mI/NAA ratio inthe thalamus when compared to the other studied brain regions of the patient (frontalisocortex, lenticular nucleus and cerebellar vermis) and to the thalami of control cases (n= 10). This metabolite pattern is indicating of gliosis. Because the MRI study wasperformed only two days before death, we were able to strictly correlate the spectroscopicdata with the neuropathological lesions (including the severity of astrogliosis andmicroglial activation) observed in the thalamus. From this observation, we can concludethat 1) MR spectroscopy can detect prion-related lesions even when other sequencesappear normal 2) spectroscopic metabolite pattern well correlates with theneuropathological one.

12. Microglia, amyloid and dementia in alzheimer disease. A correlative study

Abstract

To elucidate the role of microglia in Alzheimer's disease, a clinicopathological study was performed involving 26 cases, the mental status of which had been studied pre mortem by the Blessed test score (BTS). We measured the volume density of CD 68 immunoreactive (IR) microglia, congophilic plaques and Abeta deposits, and the numerical density of neurofibrillary tangles (NFT) in a sample of Area 9 (middle frontal gyrus). Dementia was significantly correlated only with the volume density of Abeta deposits and the numerical density of NFT. The volume densities of microglia and congophilic plaques were strongly correlated. With the intellectual status used as a time scale, IR microglia and amyloid deposits appeared almost simultaneously at an early stage in the pathological cascade and decreased, whereas Abeta and NFT were still accumulating. The intellectual deficit seemed to be more significantly related to the latter two lesions than to the microglia-amyloid complex, that was visible at an earlier stage (around BTS = 15)

Abstract

Sporadic Creutzfeldt-Jakob disease (sCJD) cases are currently subclassified according to the methionine/valine polymorphism at codon 129 of the PRNP gene and the proteinase K (PK) digested abnormal prion protein (PrPres) identified on Western blotting (type 1 or type 2). These biochemically distinct PrPres types have been considered to represent potential distinct prion strains. However, since cases of CJD show co-occurrence of type 1 and type 2 PrPres in the brain, the basis of this classification system and its relationship to agent strain are under discussion. Different brain areas from 41 sCJD and 12 iatrogenic CJD (iCJD) cases were investigated, using Western blotting for PrPres and two other biochemical assays reflecting the behaviour of the disease-associated form of the prion protein (PrPSc) under variable PK digestion conditions. In 30% of cases, both type 1 and type 2 PrPres were identified. Despite this, the other two biochemical assays found that PrPSc from an individual patient demonstrated uniform biochemical properties. Moreover, in sCJD, four distinct biochemical PrPSc subgroups were identified that correlated with the current sCJD clinico-pathological classification. In iCJD, four similar biochemical clusters were observed, but these did not correlate to any particular PRNP 129 polymorphism or western blot PrPres pattern. The identification of four different PrPSc biochemical subgroups in sCJD and iCJD, irrespective of the PRNP polymorphism at codon 129 and the PrPres isoform provides an alternative biochemical definition of PrPSc diversity and new insight in the perception of Human TSE agents variability

Abstract

A retroviral element (multiple sclerosis-associated retrovirus, MSRV) defining a family of genetically inherited endogenous retroviruses (human endogenous retrovirus type W, HERV-W) has been characterized in cell cultures from patients with multiple sclerosis. Recently, MSRV retroviral particles or the envelope recombinant protein were shown to display superantigen activity in vitro , but no animal model has yet been set up for studying the pathogenicity of this retrovirus. In the present study, the pathogenicity of different sources of MSRV retroviral particles has been evaluated in a hybrid animal model: severe combined immunodeficiency (SCID) mice grafted with human lymphocytes and injected intraperitoneally with MSRV virion or mock controls. MSRV-injected mice presented with acute neurological symptoms and died within 5 to 10 days post injection. Necropsy revealed disseminated and major brain hemorrhages, whereas control animals did not show abnormalities ( P < .001). In ill animals, reverse transcriptase-polymerase chain reaction (RT-PCR) analyses showed circulating MSRV RNA in serum, whereas overexpression of proinflammatory cytokines such as tumor necrosis factor (TNF)-α and interferon (IFN)-γ was evidenced in spleen RNA. Neuropathological examination confirmed that hemorrhages occurred prior to death in multifocal areas of brain parenchyma and meninges. Further series addressed the question of immune-mediated pathogenicity, by inoculating virion to SCID mice grafted with total and T lymphocyte-depleted cells in parallel: dramatic and statistically significant reduction in the number of affected mice was observed in T-depleted series ( P < .001). This in vivo study suggests that MSRV retroviral particles from MS cultures have potent immunopathogenic properties mediated by T cells compatible with the previously reported superantigen activity in vitro , which appear to be mediated by an overexpression of proinflammatory cytokines.
